Abstract
Description
The present invention relates to a drug removal filter, a drug removal system, and a drug removal method for removing a drug from a blood product.
In recent years, in the field of blood transfusion, a technique for inactivating (inactivating) the ability of a pathogen or leukocyte contained in a blood product to proliferate before blood transfusion has become widespread mainly in Europe. Such an inactivation technique is roughly achieved by drug (inactivation drug) administration, light irradiation, or a combination thereof.
A pathogen is, for example, a virus, bacterium, protozoan, parasite, or other DNA or RNA. Due to inactivation of pathogens, transfusions such as infections caused by viruses such as HBV, HCV, HIV, HAV, HEV, HTLV, WNV, CMV, infections caused by bacteria, infections caused by protozoa such as Babesia, Chagas disease, and malaria Infections can be prevented. Even if the blood product contains an unknown pathogen species or a pathogen with a dilute concentration level that is difficult to detect, the risk of infection can be suppressed by inactivation. In addition, the inactivation of leukocytes can suppress side effects caused by blood transfusion caused by leukocytes. Side effects caused by blood transfusions caused by leukocytes include headache, nausea, chills, and non-hemolytic fever reaction as relatively minor ones. As serious ones, post-transfusion GVHD (Graph Versus Host Dissease) and so on.
As inactivation techniques against pathogens, in the case of plasma preparations, S / D method (Otapharma), UVC irradiation (Makopharma), a combination of methylene blue and visible light irradiation (Makopharma), amotosalen and UVA irradiation (Cerus) ), And a combination of riboflavin and UVB irradiation (Thermo BCT).
In the case of platelet preparations, a combination of amotosalen and UVA irradiation (Cerus) is used mainly in Europe. Further, in the case of erythrocyte preparations, there are those based on ethyleneimine multimers (Inactine, VITEX), acridine derivatives (CERUS), combinations of riboflavin and UVB irradiation (Termo BCT), dimethylmethylene blue (Japan Red Cross), etc. . Such pathogen inactivation technology itself has been developed.
When using an inactivating agent in these inactivating techniques, it is desirable to remove the used agent. Conventionally, for example, as shown in
However, the techniques of
Then, an object of this invention is to provide the chemical | medical agent removal filter, the chemical | medical agent removal system, and the chemical | medical agent removal method which can remove efficiently the chemical | medical agent which inactivates a blood formulation, suppressing hemolysis.
As a result of intensive studies to achieve the above object, the present inventors have designed a cylindrical device in which particulate activated carbon capable of adsorbing and removing a drug is accommodated in a container with a predetermined size. It was found that it is possible to provide an inactivated chemical removal filter that can remove a high removal rate. Furthermore, it has been found that when providing a blood product containing red blood cells, the activated carbon inactivating drug removal filter is in a wet state or can be hemolyzed by being moistened with an aqueous solution immediately before use. Based on the above findings, the inventors have arrived at the present invention.
That is, the drug removal filter of the present invention is a drug removal filter that removes a drug that inactivates an inactivation target contained in a blood product from the blood product, the inlet into which the blood product is introduced, the blood product And a particulate activated carbon that is housed in the tubular container and is capable of adsorbing and removing the drug in a wet state. The particulate activated carbon has a diameter D within the tubular container. The L / D ratio is 0.5 or more and 4.4 or less.
According to this drug removal filter, since the particulate activated carbon capable of adsorbing and removing the drug in a wet state is provided, hemolysis can be suppressed even when used for blood products containing red blood cells. Further, since the particulate activated carbon is housed in the cylindrical container at an L / D ratio of 0.5 or more and 4.4 or less, it is possible to obtain a high drug removal rate and suppress hemolysis. it can. Moreover, since the blood product is allowed to flow inside and is brought into contact with the particulate activated carbon by a flow method, the drug can be efficiently removed.
Further, the particulate activated carbon preferably has an average particle size of 0.3 mm or more and 2.0 mm or less and a standard deviation of 0.05 or more and 0.35 or less. According to this, a higher drug removal rate can be obtained and hemolysis can be suppressed.
Furthermore, it is preferable that the particulate activated carbon is coated with a hydrophilizing material. According to this, since the hydrophobic adhesive force between erythrocytes and particulate activated carbon is suppressed, the physical friction which the erythrocyte surface membrane receives at the time of filtration can be reduced, and hemolysis can be suppressed more.
The drug removal system of the present invention is the above-described drug removal filter, blood product storage means for storing a blood product, and a liquid supplied to the drug removal filter before the liquid to be treated in the blood product is supplied, And a wetting liquid storing means for storing a wetting liquid that contains less red blood cells than the liquid to be treated, and a collecting means for collecting the blood product from which the drug has been removed by the drug removal filter. Features.
According to this chemical removal system, the particulate activated carbon of the chemical removal filter can be in a wet state by supplying the wetting liquid to the chemical removal filter before supplying the liquid to be treated in the blood product. In other words, the liquid to be treated in the blood product can be supplied to the drug removal filter equipped with wet particulate activated carbon. As a result, the drug in the liquid to be treated is effectively removed while suppressing hemolysis. be able to.
A drug removal system according to the present invention includes the drug removal filter, a blood product storage unit that stores a blood product, and a recovery unit that collects the blood product from which the drug has been removed by the drug removal filter. The particulate activated carbon is characterized in that it is wetted by a wetting liquid that contains less red blood cells than the liquid to be treated in the blood product.
According to this drug removal system, the liquid to be treated in the blood product can be supplied to the drug removal filter including the particulate activated carbon that has been preliminarily filled with the liquid, and as a result, hemolysis can be performed. While suppressing, the chemical | medical agent in a to-be-processed liquid can be removed effectively.
The drug removal method of the present invention is a drug removal method using a drug removal filter for removing a drug that inactivates an inactivation target contained in a blood product from the blood product, and the drug removal filter is a blood product. A particulate activated carbon comprising: a cylindrical container having an inlet for introducing a blood product; and an outlet for discharging a blood product; and a particulate activated carbon housed in the cylindrical container and capable of adsorbing and removing a drug in a wet state. According to this method for removing a drug, before the drug removal step, the method includes a moistening step in which the particulate activated carbon is moistened with a moistening liquid that contains less red blood cells than the liquid to be treated. However, the chemical | medical agent in a to-be-processed liquid can be removed efficiently.

In this method, a blood removal product is introduced from the inlet of the drug removal filter and discharged from the outlet to remove the drug in the blood product. It is characterized by having .
According to this method for removing a drug, the drug removal filter is preliminarily wetted with particulate activated carbon by a wetting liquid that contains less red blood cells than the liquid to be treated of the blood product. By supplying, the drug in the blood product can be efficiently removed while suppressing hemolysis.
Furthermore, the wetting liquid is preferably a liquid separated from a liquid to be treated in a blood product, a physiological saline solution, a blood product storage solution, or an anticoagulant.
Hereinafter, a preferred embodiment of a drug removal filter according to the present invention will be described in detail with reference to the drawings.
[First Embodiment]
(Drug removal filter)
First, the
Here, “pathogen inactivation” refers to depriving pathogens such as viruses, bacteria, protozoa, and parasites of their ability to replicate or propagate. Further, the “pathogen inactivating drug” is a drug having a function of inactivating pathogens such as viruses, bacteria, protozoa, and parasites mixed in the blood product B. Further, inactivation treatment using a pathogen inactivating agent (pathogen inactivation treatment) is a treatment method in which a pathogen inactivating agent is added to blood product B and light is irradiated for a certain period of time as necessary. It is. Pathogen inactivating agents generate highly reactive compounds such as singlet oxygen, hydroxy radicals, and hydrogen peroxide by photosensitization, which can damage pathogen DNA and RNA, and can be used between pathogen DNA and RNA bases. It inactivates pathogens by generating covalent bonds and hindering pathogen DNA cleavage, RNA viral protein expression and self-replication behavior. Examples of the pathogen inactivating agent include acridine derivatives, thiazine derivatives, phenothiazine derivatives, pyridine derivatives, porphyrin derivatives, coumarin derivatives, pyrimidine derivatives, riboflavin, psoralen derivatives, ethyleneimine multimers, and the like.
The
On the side of the particulate activated
(Cylindrical column)
In addition, although it is cylindrical shape in this embodiment, it is not restricted to this. As long as it is a cylindrical container that can hold the particulate activated
(Particulate activated carbon)
The particulate activated
The particulate activated
Here, the L / D ratio is an index of the slenderness of the particulate activated
Although the particulate activated
The particulate activated
The particulate activated
例えば、poly(2−hydroxyethyl methacrylate)(PHEMA)、poly(N−isopropyl acrylamide)(PNIPAAm)、poly(N,N−dimethyl acrylamide)(PDMAAm)、poly(vinyl alcohol)(PVA)、poly(N−vinyl−2−pyrrolidone)(PVP)等が挙げられる。この中でも特に汎用性が高く、生産コストが抑えらえるpoly(2−hydroxyethyl methacrylate)(PHEMA)、poly(N−vinyl−2−pyrrolidone)(PVP)等の合成高分子を素材とする親水化ポリマーが好ましい。
The particulate activated

The
A device that allows blood product B to flow in like the
The volume of the
The
The liquid used for wetting the particulate activated
According to the
In addition, the
Moreover, since the standard deviation of the particle diameter of the particulate activated

The
The blood product B is, for example, whole blood, RCC (concentrated red blood cells), or the like. The

EXAMPLES Hereinafter, although an Example demonstrates this invention further in detail, this invention is not limited by a following example.
(Evaluation of drug removal rate)
Using the
The particle size of the particulate activated
Specifically, the particle size of the particulate activated
Here, the measurement number N is 100. And m is, N pieces of data x 1, x 2, which is the arithmetic mean of · · · x N.

The evaluation results of the drug removal rate are shown in Table 1.
(Evaluation of hemolysis)

Human blood was obtained as a result of blood collection at a ratio of adding 14 ml of CPD as an anticoagulant to 100 ml of human whole blood derived from healthy individuals. Human blood was stored at room temperature of 22 ° C. and used within 4 hours of collection.
A human blood filtration test was performed with the same flow rate and filtrate volume as in the evaluation of the drug removal rate. In the human blood filtration test, whole blood derived from a healthy person not containing a drug was flowed at a constant flow rate corresponding to a drop using a syringe pump, and hemolysis was evaluated.
Hemolysis was evaluated as follows. The supernatant was collected by centrifuging the blood before and after filtration at 3000 rpm for 15 minutes. Thereafter, the supernatant Hb was measured using a Plasma / Low Hb System apparatus from HemoCue and a dedicated cuvette Storage for HemoCue (registered trademark) Plasma / Low Hb Microcubettes.
