Abstract
Description
米国特許法第119条(e)に準じて、本出願は、2010年10月20日出願の米国仮特許出願番号61/405044の優先権を主張する。当該出願は出典明記によってその全体が本明細書中に援用される。
本発明は、米国国立衛生研究所(NHLBI)から与えられた助成番号HL094414による政府の援助を受けてなされた。米国政府は、本発明において一定の権利を有する。
過去10年間で、心不全は心血管医学の中で突出した公衆衛生問題に急増している。心不全は世界中で23万人弱、およそ6万のアメリカ人を襲っている。しかしながら、心不全の病態生理学はまだよく理解されておらず、この疾患による死亡率および経済的負担は高いままである。心臓のバイオマーカーの開発は急性心不全の診断だけでなく心筋梗塞の診断を決定する際に役立っている。
本発明を説明する前に、本発明は記載されている特定の実施形態に限定されず、したがって、当然ながら異なっていてもよいことが理解されるべきである。本発明の範囲は、添付の特許請求の範囲によってのみ限定されるため、本明細書において使用される専門用語は、特定の実施形態を説明するためだけのものであり、限定的であることを意図しないこともまた理解されるべきである。
本明細書において使用される「予後」なる用語は、例えば不良な心臓健康状態を有する被検体、例えばCHF患者などの心臓疾患患者における生存の可能性などの、患者における疾患の特定の転帰の可能性の予測を指す。
「評価する」および「査定する」なる用語は交換可能に用いられ、対象の症状の診断および/または予後を容易にすることを指し、症状の重症度を評価することを含みうる。したがって、本開示内容においては「評価する」および「査定する」は、例えば、被検体の心臓健康状態を評価する、不良な臨床転帰(例えば、特定期間内での心臓健康状態の低下)の可能性を決定する、そしてある一定期間にわたる心臓症状のモニタリングを容易にするためにのBIN1タンパク質レベルの使用を包含しうる。例えば、BIN1タンパク質レベルを、心臓健康状態を改善する特定の治療投薬計画の有効性を評価するために用いることもできるし、一定期間にわたる患者の心臓症状の進行をモニタリング、予測または追跡する(すなわち、注目または観察する)ために用いることもできる。
本開示内容の方法は、被検体の体液中のBIN1タンパク質レベルが被検体の心臓の健康状態を評価するのに有用であり、急性心臓症状の診断だけでなく被検体の転帰不良のリスクの予測を容易にするという発見に基づいている。
ブリッジングインテグレーター1(BIN1)遺伝子は、腫瘍抑制因子の特徴を有するMyc相互作用タンパク質としてもともと同定された、核内サイトゾルタンパク質をコードする。BIN1はまた、アンフィフィシンII、アンフィフィシン様、およびボックス依存性MYC相互作用タンパク質1としても知られている。BIN1遺伝子の選択的スプライシングは、10個の異なるアイソフォームをコードする転写変異体をもたらす。BIN1のアイソフォームは、いたるところで発現されるものもあれば、組織特異的な発現を示すものもある。BIN1アイソフォーム1〜7はニューロンにおいて発現される。アイソフォーム8は筋肉特異的であるが、アイソフォーム9および10はいたるところに存在する。中枢神経系において発現されるアイソフォームは、シナプス小胞エンドサイトーシスに関与している可能性があり、ダイナニム、シナプトジャニン、エンドフィリンおよびクラスリンと相互作用する可能性がある。腫瘍細胞系において発現される異常型スプライス変異体もまた記載されている。
BIN1タンパク質レベルはBIN1ポリペプチドを検出することによってアッセイすることができる。BIN1ポリペプチドのレベルは、定量的なウェスタンブロット、免疫沈降、免疫吸着アッセイ等のような免疫アッセイによって検出することができる。
免疫検出方法が、BIN1タンパク質のレベルを検出するために適し得る。したがって、BIN1タンパク質に特異的な、ポリクローナル抗血清およびモノクローナル抗体などの抗体または抗血清を、BIN1タンパク質レベルを評定するために使用することができる。抗体は、例えば、放射性標識、蛍光標識、ビオチンなどのハプテン標識または西洋わさびペルオキシダーゼもしくはアルカリホスファターゼなどの酵素による抗体自体の直接的な標識化によって検出することができる。あるいは、一次抗体に特異的な、抗血清、ポリクローナル抗血清またはモノクローナル抗体を含む標識された二次抗体と共役させて、標識されていない一次抗体が用いられてよい。ある特定の例において、患者の体液試料におけるBIN1タンパク質レベルは、正常な心臓を有する被検体の体液におけるBIN1タンパク質レベルと比較され得る。免疫検出プロトコールおよびキットは、当技術分野においてよく知られており、市販されている。
体液試料中のBIN1タンパク質のレベルを測定するための方法は、メソスケールディスカバリー(登録商標)(MSD)プラットフォームを用いるアッセイ様式で行ってよい。一般的に、電気化学発光標識にコンジュゲートする二次結合体部分を用い、MSDプラットフォームで二次結合体部分からの電気化学発光を検出する。
生物学的流体試料は、BIN1タンパク質が存在しうる任意の試料とすることができる。上記のように、哺乳動物、例えばヒトからの液体の生物学的試料は、BIN1タンパク質レベルを検出するためにアッセイすることができる。このような流体には、血液(例えば、全血またはその画分(例えば、血清、血漿))、心嚢液などの水性流体が含まれる。
被検体の体液中のBIN1タンパク質レベルを用いて、被検体の心臓健康状態の評価および/又は臨床転帰不良のリスクを決定することができる。BIN1タンパク質レベルはまた、急性心臓症状(例えば、心筋梗塞)の診断を容易にするために使用することができる。心臓の健康状態を評価することは、被検体、例えば臨床上心不全を有すると診断された被検体の、転帰不良のリスクを予測することを含み得る。
この方法は、被検体、例えば深刻な心臓事象などの急性心臓症状を有する個体、急性心臓症状を有さない個体、例えば急性でない心臓症状または明らかでない心臓症状を有する個体からの体液試料中のBIN1タンパク質レベルをアッセイすること含むことができる。
体液(例えば血液)中のBIN1タンパク質レベルは、心臓健康状態の診断の容易化および/または転帰の予測の容易化に有用である。正常BIN1タンパク質レベルと比較したときの被検体の体液(例えば血液)中のBIN1タンパク質レベルの低減は、被検体の心臓健康状態の衰退を示す。正常BIN1タンパク質レベルと比較したときの被検体の体液(例えば血液)中のBIN1タンパク質レベルの低減は、被検体の心臓健康状態の評価および/または転帰不良の可能性の予測を容易にすることができる。被検体は、明らかでない心臓症状または急性でない心臓症状を有する被検体、例えば深刻な心臓事象を有さない被検体であってよい。
被検体の体液試料におけるBIN1タンパク質レベルの測定は、深刻な心臓事象などの急性心臓症状の診断を容易にする際に有益である。一般に、正常BIN1タンパク質レベルと比較して体液中のBIN1タンパク質レベルの有意な増加は被検体での深刻な心臓事象を示す。ゆえに、体液のBIN1タンパク質レベルは、急性心臓症状を持たない被検体と、深刻な心臓事象などの急性心臓症状を有する被検体を区別するためのマーカーとして有益である。このようなアッセイは、深刻な心臓事象の症状を違ったふうに表す被検体における鑑別診断を容易にすることができる。したがって、深刻な心臓事象の症状を示す被検体において、深刻な心臓事象の発症は、被検体の体液中のBIN1タンパク質レベルが正常BIN1タンパク質レベルと比較して有意に増加しているか否かで診断することができる。一方、正常BIN1タンパク質レベルと比較してBIN1タンパク質レベルが有意に増加していなければ、被検体は深刻な心臓事象を持っていないことを示す。
一般に、本明細書中に記載される方法に適する被検体は、哺乳動物被検体、例えばイヌ、ウシ、ウマ、またはヒト被検体である。
本明細書中の方法を用いた評価に適する被検体には、急性心臓症状を持たない被検体、例えば心臓症状を明確には持たない被検体、急性ではなく慢性の心臓症状を持つ被検体などが含まれる。
本明細書中の方法を用いた評価に適する被検体には、急性心臓症状を有する被検体が含まれる。ここで用いられる「急性心臓(heartまたはcardiac)症状」なる用語は、心臓状態と関連する臨床症状を表し、その症状が急性でない心臓症状を有する被検体によって表される臨床症状よりも重症である被検体を指す。急性心臓症状を有する被検体は通常、心臓症状および定期的なモニタリングのために入院しているか、または時に頻回な外来臨床管理で管理されうる。「急性心臓症状」には、深刻な心臓事象、例えば、急性冠症候群、心筋梗塞、または急性劇症性心筋炎が含まれる。急性心臓症状を有する被検体は、急性心臓症状の他の血清学的な証拠を有し得、例えば、正常レベルより高いB型ナトリウム利尿ペプチド(BNP)を有する、正常レベルより高いクレアチンキナーゼ(CK)を有する、正常レベルより高いCK−MBを有する、または正常レベルより高いトロポニン(特に、心臓トロポニンTおよび心臓トロポニンI)を有する。場合によって、急性心臓症状を有する被検体は上記の徴候の2以上を有しうる。
被検体の体液試料中のBIN1タンパク質レベルは、心臓健康状態の評価を容易にする際に、転帰不良のリスクを予測する際に、また、深刻な心臓事象などの急性心臓症状を診断するために有益である。ある実施態様では、BIN1タンパク質レベルは、心臓健康状態の評価を容易にする際に、転帰不良のリスクを予測する際に、急性心臓症状を診断するために、心臓疾患の他の徴候と併せて用いられる。
急性でない心臓症状患者の体液試料中のBIN1タンパク質レベルの測定は、先に述べたように、患者に機能的分類、例えばニューヨーク心臓協会(NYHA)クラス1−4に割り当てるために用いられうる。この機能的分類は、心臓移植を受けるための患者への優先順位を決定するために用いられうる。急性でない心臓症状患者の体液試料中のBIN1タンパク質レベルの測定は、CHF治療への応答評価を容易にするため、および/または治療計画の変更を導くために用いられうる。
急性心臓症状を有することが疑われる被検体の体液試料中のBIN1タンパク質レベルを評価することは、深刻な心臓事象などの急性心臓症状の診断を容易にするのに有用である。急性心臓症状の診断には、急性心臓症状の既知のマーカー、例えばBNP、CK、CK−MB、および/またはトロポニンのタンパク質レベルの評価と併せて、上記のように、被検体の体液中のBIN1タンパク質レベルの評価が含まれうる。「〜と併せて」という表現には、BIN1タンパク質レベルと急性心臓症状の他のマーカーの連続的または同時の評価が含まれる。急性心臓症状を有することが疑われる被検体において急性心臓症状を診断すると、被検体の治療プロとコール(一または複数)の決定を容易にすることができる。
本開示の方法において使用するための材料は、よく知られている手順に従って製造されるキットの調製に適している。本開示はしたがって、臨床転帰の予測、治療の選択肢の決定、または治療に対する反応の予測などのためにBIN1のレベルを定量するためのBIN1結合部分(一または複数)でありうる作用物質を具備するキットを提供する。加えて、キットは、場合によって、識別のための記述もしくはラベルまたは本開示の方法におけるそれらの使用に関する使用説明書とともに試薬(一または複数)を具備してよい。キットは、例えば、作製済み(pre−fabricated)マイクロアレイ、緩衝液、抗体、酵素基質、本開示のものを含む、方法において利用される様々な試薬(典型的には、濃縮された形態で)の1つまたは複数をそれぞれ含む、容器(方法の自動化された実行において使用するのに適したマイクロタイタープレートを含む)を具備してよい。予後のおよび/または予測的な情報を推定または定量するために使用される数学アルゴリズムもまた、当然、キットの潜在的な構成要素である。
本開示の方法は、BIN1のレベルをアッセイした結果を要約した報告の作成に適している。ある実施形態では、報告は被検体の心臓健康状態の決定を含みうる。ある特定の実施形態では、報告は、CHF患者における心臓死亡リスクの決定を包含していてもよい。本明細書において記載される「報告」は、被検体の体液試料中のBIN1タンパク質レベルおよび/または心臓死亡リスクに関する対象とする情報を提供する報告要素を包含する電子文書または有形の文書である。報告は、完全にまたは部分的に電子的に作成されてもよく、例えば、電子ディスプレイ(例えば、コンピュータのモニター)上に提示される。報告はさらに、1)試験設備に関する情報;2)サービス提供者情報;3)患者データ;4)試料データ;5)a)適応;b)試験データを含む様々な情報を包含し得る解釈レポート、および6)別の特徴、のうち1つまたは複数を包含することができる。
本明細書に記載の方法およびシステムは、非常に多くの方法で実行することができる。特に興味深い一実施形態において、この方法は、通信インフラストラクチャー、例えば、インターネットの使用を伴う。いくつかの本発明の実施形態が、下記に論じられている。本発明は、様々な形態のハードウェア、ソフトウェア、ファームウェア、プロセッサ、またはそれらの組み合わせで実行されてもよいこともまた理解されるべきである。本明細書に記載の方法およびシステムは、ハードウェアとソフトウェアの組み合わせとして実行することができる。ソフトウェアは、プログラム記憶デバイスにおいてタンジブルに組み入れられているアプリケーションプログラム、またはユーザーのコンピュータ使用環境において(例えば、アプレットとして)およびレビュアーが関連付けられたリモートサイトに(例えば、サービス提供者の施設に)位置していてもよいレビュアーのコンピュータ使用環境において実行されるソフトウェアの異なる部分として実行することができる。いくつかの実施形態において、BIN1タンパク質レベルを使用してCHF患者における心臓死亡リスクを決定するステップは、リスクを計算するためのアルゴリズムを実行するようにプログラムされたコンピュータによって実施される。別の例において、対象方法は、コンピュータに、BIN1のレベルに基づいてCHF患者における心臓死亡リスクを計算するためのアルゴリズムを実行させるステップを包含する。
本開示はまた、コンピュータ使用環境において実行されるときに、本明細書において記載されている通りに心臓死亡リスクの計算のすべてまたは一部を行うためのアルゴリズムの実行を提供するプログラムが保存されている、コンピュータで読取り可能な記憶媒体(例えば、CD−ROM、メモリーキー、フラッシュメモリーカード、ディスケットなど)を想定する。コンピュータで読取り可能な媒体が本明細書に記載の方法を行うための完全なプログラムが入っている場合、プログラムは、出力を収集、解析、および作成するためのプログラム使用説明書を包含し、一般に、本明細書に記載の通りユーザーと情報交換するため、解析情報とともにそのデータを処理するため、そのユーザーのための独自の印刷媒体または電子的媒体を作成するための、コンピュータで読取り可能なコードデバイスを包含する。
BIN1 ELISAアッセイ。 市販のELISA試薬セット(BD Bioscience)を用いてBIN1 ELISAアッセイを設定した。簡単に言うと、96ウェルプレートに捕捉抗体マウス抗BIN1(1:1000、Sigma)を塗り広げ、4℃に終夜置いた。室温で1時間かけて希釈緩衝液にて洗浄およびブロッキングを行った後、標準物質と試料の両方を加え、回転させながら室温で1時間インキュベートし、BIN1を捕捉した。一連の洗浄の後、次いで一次ヤギ抗BIN1抗体(1:1000、Abeam)を加え、捕捉されたBIN1タンパク質を検出した。十分に洗浄した後、HRPコンジュゲートロバ抗ヤギIgG二次抗体(1:4000、Abeam)とともにインキュベートすることによって結合した一次ヤギ抗BIN1を検出した。最後に、洗浄した後、基質であるTMBを加えて1時間置き、停止緩衝液にて反応を止めた。マイクロプレート読み取り機(BioRad)を用いて各ウェルの405nm波形の吸光度をすぐに読み取った。
56人の心不全と診断された患者と28人の年齢が一致する健康個体(コントロール)において血清BIN1タンパク質レベルを測定した。UCSFの高度心不全および移植クリニックの心不全患者から血清試料を入手した。ELISAアッセイによってBIN1タンパク質レベルを測定し、図1Aおよび1Bに示す。図1Aは、BIN1タンパク質レベルとして測定したBIN1度数分布と特定のBIN1タンパク質レベルを示す心不全患者およびコントロールの割合を示す。図1Aを見ると、およそ9μg/ml付近に集中した血清BIN1タンパク質レベルを持つコントロール個体と比較して、より低いレベルの血清BIN1を持つ心不全患者の割合が多かった。図1Bは、コントロール個体と心不全患者の平均血清BIN1レベルを示す。
心不全患者において測定された血清BIN1タンパク質を、患者の年齢、性別、体重およびBMIに対してプロットした。年齢、性別、体重またはBMIと血清BIN1タンパク質レベルとの相関は明らかでなかった。
試験した心不全患者では、左心室駆出分画率(LVEF)は心エコーによって得、侵襲性心カテーテル法の血行動態を行って肺動脈楔入圧(PCWP)を測定した。BIN1タンパク質レベルとこれら2つのパラメータには有意差はない(図4Aおよび4B)。PCWPは、血清マーカーBNPに反映される心臓内充填圧力の測定値であることを注記する。
図5Aおよび5Bは、血清BIN1が侵襲的に測定した心拍出量および心臓指数と直接相関することを示す。血清BIN1が低ければ低いほど、心拍出量が小さくなる。血清BIN1は心拍出量を測定する肺動脈カテーテルよりも侵襲性がはるかに低いので、有用なマーカーである。
UCSFの高度心不全および移植クリニックの心不全専門心臓専門医によってNYHA機能分類を決定した。NYHAスケールのクラスI、II、IIIおよびIVによって定義されるように患者機能状態によって割り振った後にBIN1をプロットした。NYHAスケールは、BIN1タンパク質レベルを測定するために血液試料を採取した時点の患者の臨床兆候から得た。クラスの増加は症状の悪化を示す。クラス4(またはIV)は安静時に症状を示す患者を包含する。これら患者での予後は極めて不良である。
BIN1タンパク質レベルを評価した心不全患者は5年間にわたって遡及研究にて追跡調査した。図7Aおよび7Bは、BIN1タンパク質レベルが低い患者において死亡率が高いことを示す。逆に、BIN1が高い患者はその同じ期間生存する可能性があったことを示す。これらデータは、心不全患者の血清に基づく予測にBIN1タンパク質レベルを有益であることを示す。5以上のBIN1を有する患者は、その後5年のうちに移植または左室補助機器などの他の先進治療の必要がないであろう。BIN1が低い患者はこれら治療を必要とするであろう。5のBIN1は正常の50%であることを注記する(図1Aおよび1B)。ゆえに、BIN1は全心不全集団を階層化しえる。
ARVCは、心室筋の局所性またはびまん性の線維脂肪性置換に特徴がある疾患であり、結果として再発性心室性不整脈および右および/または左心室機能障害が生じうる(Basso C. et al., Lancet 2009; 373: 1289-300)。この疾患は家族性であり、デスモソーム機能には様々な原因遺伝子が関与することが示されている(Sen-Chowdhry S., et al., J Cardiovasc Electrophysiol 2005; 16: 927-35)。疾患重症度および進行は、無症候性疾患から重度の心不全および難治性心室性不整脈への範囲となりうる(Hulot JS, et al., Circulation 2004; 1 10: 1879-84)。現在、ARVC患者での将来の不整脈負荷を予測すること、またはどの患者が進行性不整脈を発症するかを予測することは難しい。臨床進行リスクが高い個体を同定するための試験は非常に臨床上の利便性があるであろう。
患者。 試験は、カリフォルニア大学サンフランシスコ校(UCSF)の病院およびジョンズホプキンス大学によって承認された(Western IRB、臨床データはジョンズホプキンスプロトコールにより集めた)。すべての登録患者にはインフォームドコンセントを行った。ARVCと診断された31人の患者からおよそ7年間にわたって静脈血試料を採取した。7人の患者は異なる時点で2回採血した。血液試料を得、4℃で4000rpmで20分間遠心し、後のELISA分析のために血清を−80℃の冷凍庫に保存した。MRI、RV血管造影、右心カテーテル検査、心エコー、病理、ECG、平均加算ECG、ホルターモニター結果、家族歴や遺伝子型を含む各患者の対応する臨床データを、ARVC診断の確認のために記録した。

- 被検体の心臓健康状態を評価するための方法であって、
被検体から得られた体液試料中のBIN1タンパク質レベルを測定することと、
該BIN1タンパク質レベルを用いて被検体の心臓健康状態を評価することとを含み、
このとき正常なBIN1タンパク質レベルと比較したときの体液試料中のBIN1タンパク質レベルの減少が、心臓の健康状態不良と正の相関がある、方法。 - 被検体が心不全関連のリスク因子を有する個体である、請求項1に記載の方法。
- 被検体が慢性心不全患者である、請求項1に記載の方法。
- BIN1タンパク質レベルを用いて心不全の心臓機能分類を決定し、BIN1タンパク質レベルが心臓機能分類と相関している、請求項3に記載の方法。
