Abstract
Description
本出願は、2010年9月20日に出願された米国特許出願第61/384,399号明細書の利益を主張し、その出願の開示内容は本明細書に完全に記載されているものとして参照により本明細書に組み込まれる。
本開示は、概して整形外科用インプラントに関し、特に、柔軟な(compliant)整形外科用インプラントに関する。
健康な関節の動きパターンは複雑であり、回転および並進を、時には単独で時には組み合わせて含んでいる。したがって、瞬間的な回転中心または回転軸は、その関節とともに移動する。典型的な従来の関節形成(arthoplasty)インプラントは、通常、完全な(intact)解剖学的構造の生理学的な動きをよりよく再現する複雑な動きを具現化することに機能が限定され、したがって、解剖学的荷重に対して正確に反応しない。
一実施形態によれば、整形外科用インプラントは中心軸に沿って圧縮可能である。整形外科用インプラントは、第1部材と、第2部材と、第1部材と第2部材との間に接続された柔軟な接合部とを備えている。柔軟な接合部は、筋かい状に接続される少なくとも一対のばね部材を備えている。ばね部材は、中心軸に沿って第1部材を第2部材から間隔を空けて配置する。
図1および図2を参照すると、柔軟な整形外科用インプラント20を、脊柱の腰椎または頸部において、または要求に応じて脊柱のいずれかの別の位置において生体椎間板に置き換わることができる、椎間板インプラントとして構成することができ、または、指関節、股関節、肘関節、肩関節、手首関節、膝関節等、要求に応じてあらゆる人工関節として構成することができる。インプラント20は、中心軸22に沿って垂直に延在し、第1の隣接する骨に当接するように構成された第1端部部材24等の第1部材と、中心軸22に沿って第1端部部材24から垂直方向に間隔が空けられて配置され、第2の隣接する骨に当接するように構成された第2端部部材26等の第2部材と、第1端部部材24および第2端部部材26の間に結合されるとともにそれらに接続され得る、柔軟な中間接合部32とを備え、第1端部部材24および第2端部部材26を、中心軸22を中心に回転するように構成することができ、それにより、中心軸22は、第1端部部材24および第2端部部材26に対する回転の軸を画定することができる。たとえば、第1端部部材24および第2端部部材26の一方または両方は、整形外科用インプラント20の中心軸22を中心に他方の端部部材に対して回転することができる。
Claims (27)
- 中心軸に沿って圧縮可能な柔軟な整形外科用インプラントであって、
第1部材と、
第2部材と、
前記第1部材と前記第2部材との間に接続された柔軟な接合部であって、筋かい状に接続される少なくとも一対のばね部材を備え、前記ばね部材が前記中心軸に沿って前記第1部材を前記第2部材から間隔を空けて配置する柔軟な接合部と、
を具備する柔軟な整形外科用インプラント。 - 少なくとも1つの支持材が、前記ばね部材の対を合わせて筋かい状に接続するように構成されている、請求項1に記載の柔軟な整形外科用インプラント。
- 前記支持材が、前記ばね部材の対の各々の間に接続されている、請求項2に記載の柔軟な整形外科用インプラント。
- 前記支持材が、断面積がおよそ4mm2およびおよそ6mm2の範囲内である、請求項2または3に記載の柔軟な整形外科用インプラント。
- 前記第1端部部材、前記第2端部部材および前記柔軟な接合部が、単一の一体構造を画定している、請求項1〜4のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記ばね部材の各々が、曲率半径が変化する弧を画定する、請求項1〜5のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記ばね部材の各々が、前記中心軸に向けられた頂点を備え、前記曲率半径が前記頂点において最小である、請求項6に記載の柔軟な整形外科用インプラント。
- 前記曲率半径が、およそ1mmおよびおよそ2mmの範囲内である、請求項7に記載の柔軟な整形外科用インプラント。
- 各ばね部材が面に沿って延在し、前記面が中心軸を含む、請求項1〜8のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記柔軟な接合部が、各々が面を画定する3つ以上のばね部材を含み、前記3つ以上の面が等間隔の角度で配置されている、請求項1〜9のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記横断する支持材が、前記ばね部材の前記頂点の間に接続されている、請求項2〜4のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記第1部材と前記第2部材との間を前記中心軸に対して実質的に平行に延在する1つまたは複数の止め具をさらに具備する、請求項1〜11のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記止め具が、非圧縮状態で前記柔軟な接合部の高さより低い高さである、請求項12に記載の柔軟な整形外科用インプラント。
- 前記柔軟な接合部が、1つまたは複数の第1接触面を有し、前記1つまたは複数の止め具が、前記柔軟な接合部が前記止め具のうちの1つの高さまで圧縮された時に前記柔軟な接合部の前記第1接触面と接触するのに好適な1つまたは複数の第2接触面を有している、請求項12または13に記載の柔軟な整形外科用インプラント。
- 前記第1接触面および前記第2接触面が前記中心軸に対して平行な方向に沿って先細りになっており、それにより、前記第1接触面が、横方向並進、前記中心軸に対して角度をなしている枢動軸を中心とする枢動、および互いに対する前記第1部材および前記第2部材の前記中心軸を中心とする回転のうちの少なくとも1つを可能にするように、前記柔軟な整形外科用インプラントの非対称圧縮時に、前記中心軸に対して斜めに前記第2接触面の上を摺動することができる、請求項14に記載の柔軟な整形外科用インプラント。
- 前記第1接触面および前記第2接触面が、前記中心軸に沿って相補的に先細りになっている、請求項15に記載の柔軟な整形外科用インプラント。
- 前記止め具が、前記第1部材および前記第2部材のうちの1つと一体化している、請求項12〜16のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記柔軟な接合部が、弾性係数が10GPa〜120GPaの範囲である材料から作製されている、請求項1〜17のいずれか一項に記載の柔軟な整形外科用インプラント。
- 最大サイズが8mmを超える、請求項1〜18のいずれか一項に記載の柔軟な整形外科用インプラント。
- 前記最大サイズが12cm未満である、請求項19に記載の柔軟な整形外科用インプラント。
- 前記最大サイズが、前記柔軟な整形外科用インプラントの長さ、幅および高さのうちの1つまたは複数である、請求項19または20に記載の柔軟な整形外科用インプラント。
- 人工関節として構成されている、請求項1〜21のいずれか一項に記載の柔軟な整形外科用インプラント。
- 生体椎間板に置き換わるように構成された人工椎間板を含む、請求項1〜22のいずれか一項に記載の柔軟な整形外科用インプラント。
- 第1骨係合面を画定する第1端部部材と、
第2骨係合面を画定する第2部材と、
前記第1端部部材と前記第2端部部材との間に接続された少なくとも一対のばね部材を含む柔軟な接合部であって、前記第1端部部材および前記第2端部部材が中心軸に沿って間隔を空けて配置されており、前記ばね部材の対の各々の間に接続された少なくとも1つの補強部材をさらに備える柔軟な接合部と、
を具備する柔軟な整形外科用インプラント。 - 前記補強部材が、前記ばね部材の対と一体化している、請求項24に記載の柔軟な整形外科用インプラント。
- 請求項1〜24のいずれか一項に記載の少なくとも1つの柔軟な整形外科用インプラントと少なくとも一対の剛性要素とを具備し、前記柔軟な整形外科用インプラントが前記剛性要素の対の各々の間に接続されている、可撓性ロッド。
- シャフト、および長手方向ロッドを受け入れるように構成された横方向溝を備えるU字型頭部と、請求項1〜25のいずれか一項に記載の柔軟な整形外科用インプラントとを具備し、前記柔軟な整形外科用インプラントが前記シャフトと前記横方向溝との間に配置されている、椎弓根スクリュー。
