Description
クロスリンキングは、ポリマー材料を硬化させて生存する組織を安定化させるのに用いられる広範に普及された方法である。より具体的にはそのような医療環境において、コラーゲンクロスリンキング(CXL)および結合技術が、歯科、整形外科、および皮膚科において長年用いられてきた。
公知手順において、処置の間に実質内にどれ程のリボフラビンが存在するかに関する測定法は存在せず、角膜上皮における、または肝要な角膜輪部幹細胞を含む輪部における細胞損傷を、確実に予防する手段はない。
様々なレーザ装置を用いた角膜の熱処置が、屈折率補正のために角膜を再形成しうることは公知である。公知の熱処置の幾つかは、USAにおいてFDAに認可されたが、角膜が本来の形状へ自然に回帰するため、結局のところ全てが失敗に終わった。この回帰は、数ヶ月から2、3年の期間をかけて行われる可能性がある。この分野において活動的であることが知られた会社は、Refractec(Conductive Kerotoplasty − CK)、Avedro(Keraflex)としても公知のThermal Vision、Rodenstock(Diode Thermal Kerotoplasty − DTK(撤退))およびSunrise(Laser Thermal Kerotoplasty − LTK(撤退))である。しかし、熱処置の後、角膜を予測可能に再形成および安定化させて、長期成功率を上昇させることが、依然として求められている。
熱放射線の技術において、好ましい実施形態を、図6の模範的装置に示すが、この場合、CW赤外ファイバーレーザビームが、熱放射線源であり、前記熱放射線を眼組織上へ送達するために、予め選択されたパターンを発射するファイバーカップリングXYスキャナーを介して眼組織へ送達される。模範的装置のXYスキャナーを、図8に示す。XYスキャナーの重要な利点は、様々な目の状態に合わせて熱エネルギーパターンをカスタマイズするための柔軟性を有意に上昇させることである。そのようなスキャナーは、様々な目の状態(近視、乱視、緑内障、老眼、遠視、円錐角膜および拡張症)を処置するよう設計およびプログラムされた、高度に統合された(速度+能力+位置+深さ)精密なパターンを含むことが可能である。これらのパターンは、以下に詳述される通り、外科医による所定の処置手順の間、そして/またはフィードバックメカニズムの使用により、予めプログラムまたは調整させてもよい。図10に、そのようなパターンの例を示す。加えて、柔軟性が装置に組み込まれているため、装置に介入してそれを再指導し、新規な、または患者個人に合わせたパターン選択を実施するように、外科医に選択させることができる。この個人用設定の能力は、処置の利益および有効性を示す。
以下は、模範的装置の熱送達を任意の先行技術と区別する設計因子であり、そのような特徴の1つ以上が本発明の実践に必須である:
(1)角膜形状のカスタム圧平レンズ(例えば、平凹またはメニスカス形状で、厚さがおよそ1mm〜5mm、口径が4mm〜20mm)を、眼組織へ適合させる(レンズの例示および適用については図13参照);
(2)熱放射線源(例えば、CW赤外ファイバーレーザ)を適用する時間全体または時間の一部の間、眼組織表面および実質コラーゲン組織は、好ましくはサファイアコンタクトレンズにより、温度制御される。レンズ表面は、熱放射線処置が経過する間、0℃〜20℃、または8℃〜18℃、そして好ましくは約8〜11℃の温度を保持していてもよい。上皮および実質の近接性および熱伝導性のため、表面熱制御/冷却の効果で、実質内の温度を精密に制御させることができる。表面温度を、連続してモニタリングし、PC画面に表示させる。本発明は、この温度制御方法に限定されない。組織表面を加熱または冷却された固体、液体、または気体と直接熱接触させる他の方法、および熱放射線源の使用などによる間接的方法をはじめとし、他の温度制御方法が用いられる場合があり、本発明において用いてもよい。
(3)カスタム吸引リングが、熱的手順の前、その間、そしてその後に、穏やかな手法で眼組織をサファイアレンズに圧平して、防御される最前方の膜(例えば上皮およびボーマン膜および結膜または血管)に対する精密な熱制御/冷却効果を可能にする。吸引リングは、より大きなレーザ位置合わせ精度および患者の衝動的運動へのより大きな抵抗性も提供する(真空吸引リングアセンブリを備えた模範的コーンを示した図7参照)。
先に示された設計因子に基づけば、模範的装置は、上皮およびボーマン膜(または強膜処置の間の結膜)を通した前方眼組織(例えば実質または強膜)への熱放射線の伝送が可能にしながら、これらの膜の破壊を最小限にする。その結果、そのような新規な制御熱放射線は、これらの保護された膜の下のコラーゲン線維に到達することができ、つまり上皮を有害作用、例えば、創傷治癒速度の低下、罹患、瘢痕および曇り形成または易感染性などから防御することができる。
眼組織内の表面下傷害を、以下の効果の1つまたは組み合わせを得られるように設計する:
1)熱傷害によりコラーゲン収縮を生成して、具体的なプログラムされた組織再形成を行わせる。具体的には再形成は、ジオプトリーの特定範囲の上昇または低下に対応する角膜の、それぞれ平坦化または急勾配化を含むことができる。例えばそのような再形成は、角膜を平坦化(5ジオプトリー以下または5ジオプトリーを超える)または急勾配化(5ジオプトリー以下または5ジオプトリーを超える)してもよい。加えて、高次収差(HOA)を誘発してもよい(例えば、2μmを超えるまで)。図3に、対応する断面および傷害位置を示す、近視矯正による例示的な角膜前面変化を示す。図10に、そのような近視処置のための屈折放射線エネルギー処置パターンの例を示す。
2)熱傷害を連続パターンで適用して、制御された定方向の組織移行(1mmを超える)を誘発することができる。
3)熱傷害により組織の弾性をモジュレートする(即ち、術前状態の90%を超えて眼組織を軟化させる)ことができる。
4)熱傷害により眼組織(コラーゲン線維)を収縮させてもよく、隣接する排水管、例えばシュレム管を強制的に開口させ、それにより眼圧(IOP)を低下させることができる。
模範的装置は、角膜の任意の部分を実時間(RT)でスキャンおよび表示することができる画像能力を提供する埋込み型実時間光干渉断層(OCT)エンジン(例えば、ThorLabs Callisto 930nm)を含む。「実時間」は、プロセスまたは事象が起こる実際の時間、および/または入力データをミリ秒以内もしくは許容しうる時間枠内で処理して、事実上直ちにフィードバックとして入手できるようなシステムに関連する実際の時間を指す。図4は、本発明によりヒトの角膜へ適用された熱放射線から得られた表面下傷害のOCT画像を示す。次の処置の暴露パターンをカスタマイズする抽出のために、熱レーザがエネルギーを蓄積させた各表面位置での傷害の混濁度および深さおよびサイズを、OCTにより迅速に採取する。そのようなスキャンからのデータを、非常に精密な組織的特徴を引き出す目的で、同時に分析することができる。より具体的にはこのOCTの特色は、以下の事柄を実施する特別な能力を提供する:
1)術前および術後の両方で、そしてパキメトリー測定なら術間でも、目の表面のトポグラフィーおよびパキメトリー(即ち、角膜の厚さを測定するプロセス)を示すマップを作成することができる。
2)術間の(RTにおける)目の瞳孔(中心)を追跡し、それにより処置の適切な心合わせ(中心化)を確実に行うことができ:図8に表わされた実施形態は、カメラを追跡する独立した瞳孔中心も示す;
3)術間にサイズ、表面下の深さおよび熱傷害の混濁度をモニタリングして、処置の正確性および安全性プロファイルの両方を向上させることができる。
TSの模範的装置は、取り外し可能で滅菌可能なコーン状アセンブリを含む。図7に、コーン状アセンブリの構成部品を示す。以下の特色を、図7に示す:1)サファイアレンズ;2)サファイアレンズの温度を制御/設定する水路;3)アイ真空吸引リング;4)サファイアレンズのデフォッガー;5)アイイルミネーター;6)目の負荷/力センサー。サファイアレンズは、好ましいメニスカス形状で設計され(コンタクトレンズと同様、図13参照)、複数の機能を実施し:角膜に圧平し、それにより目の表面の熱制御として働き、同時に2.01ミクロンのレーザ放射線を効率的に伝送する。加えてサファイアレンズは、930nmのOCT放射線および可視放射線を伝送する。水路は、熱処置の間のサファイアレンズのためのプログラム可能な(即ち、プリセット可能な)温度(1〜40℃)を提供する熱交換器として働く。アイ吸引リングは、3回の処置相:a)約30秒間の角膜プリセット処置温度の安定化;b)約60秒以下のレーザ熱送達間の角膜熱制御;および)約15秒間の角膜冷却後、の間に目を適所に保持する。リングの吸引は、約300ミリバールの真空を利用することにより、患者の目から容易に取り外されるよう設計されている。サファイアレンズデフォッガーは、サファイアレンズ表面から水分を除去する。イルミネーターは、手術の間に処置眼の最適なカメラ視界を提供するために、ファイバーからコーン/サファイアレンズへ直接可視光を送達し、そのような光の強度は、最適な視界を実現するように調整することができる。負荷力センサーは、目の表面のコーンにより任意の過剰な圧力に関して外科医を警告する安全基準として働く。コーンの特色は、図5のシステムブロック図に示された通り制御され、制御ボタンを示す典型的なユーザーインターフェース処置PC画面は、図11に参照される。
模範的実施形態の光学システムは、4つの基本的機能を有する(図6および図8参照)。第一の機能は、眼組織の具体的領域内へ熱放射線を蓄積させる能力を有するXYスキャナーを含めることである。この制御されたエネルギー蓄積により、装置のユーザーは、所望の深さ、温度範囲および期間に眼組織(処置のターゲットとなった)における温度プロファイルを生成させることができる。この熱放射線の重要な態様(即ち、制御された熱伝導)は、それが上皮およびボーマン膜または内皮もしくは結膜(熱変性による混濁がその構造およびバリア保護層において可視である)の任意の破壊を著しく最小化することである。(光学システムの一部としてのXYスキャナーの実施形態を示す図8を参照)。XYスキャナーは、熱放射線を組織表面、好ましくは口径が200μm〜1mmの領域、より好ましくは口径が約600μmの領域に送達するのに、適宜小さな「ピクセル」サイズを可能にする。更に、好ましい実施形態において、XYスキャナーは、この領域を0.1〜10mm/秒の速度で組織を横切ってスウィープすることができ、一般には口径2〜20mmの範囲内の円形領域でスウィープする(円形および非円形パターンの実施例に関しては図10を参照)。そのようなパラメータは、処置をカスタマイズするために熱放射線源のエネルギー出力に基づいて調整される。模範的熱放射線源は、500mW〜2Wのエネルギーを送達するCW赤外ファイバーレーザ(およそ2013nm)である。電力出力、XYスウィープ速度、パスパターンおよびピクセル径の調整は、所定の組織表面領域内に送達されるエネルギーに影響を及ぼす。
光学モデリング(ZEMAXソフトウェアを用いる)を本発明の実施形態に用いて、眼組織に到達する熱レーザビームが、有効な処置(サーマルリモデリング)を提供するのに必要な特徴全てを有することを確実にした。調整可能なサイズを有するレーザビームが、予め決定された位置で眼組織に到達して、異なるタイプの屈折率補正に関して所望のパターンを生成させることができる(均一性または品質を犠牲にすることなく)ことが、そのモデルで実証された。図12は、熱レーザビームのサイズおよび位置を制御する重要な光学成分を模写している。このビームが、口径1.2mmのファイバーコリメータ(ラベル1)を離れる。レンズ3枚の望遠鏡(ラベル2.1〜2.3)は、ビームの倍率を低下させ、眼組織の前で圧平レンズ(ラベル4)上へ処置スポットの焦点を合わせる。スポットのサイズは、中央のレンズ(ラベル2.2)を光軸に沿って移動させることにより調整することができる。

より高速で移動するレーザビームを用いて、処置時間を短縮した。レーザ出力を増加させずに温度を上昇させるために、サファイア温度を18℃に上昇させた。

眼組織を再形成するための熱放射線送達(先に説明)に加えて、本明細書の発明は、先に記載された熱放射線手順の前、その間、その後、またはそれらを任意に組み合わせて、角膜組織をクロスリンキングする適用を包含する。そのようなクロスリンキングは、熱的手順の前6ヶ月以内の任意の時間、または熱的手順の後6ヶ月以内の任意の時間に実施することができる。本発明の重要な特色は、本発明から有効な結果を得るための、熱処置とクロスリンキングとの組み合わせが存在することである。角膜コラーゲン組織の熱的再形成が、ほぼ本来の状態へ回帰することは、文献において公知である。しかし本明細書における本発明は、そのようなコラーゲン組織のクロスリンキングが実施されると、回帰速度が有意に低下または排除されることを企図する。クロスリンキングは、長年にわたり、コラーゲン組織に安定性/剛性を提供することができる。それは臨床的に検査されていないが、熱放射線再形成(TS)とクロスリンキングの組み合わせが、本明細書に先に言及された疾患および状態の有効な処置を提供しうると考えられる。本発明によれば、目のコラーゲン組織の光化学的(例えば、リボフラビンUVA)で最小限に侵襲性のクロスリンキングが、安定性を確保する目的で(即ち、剛直性/剛性を保持するために)、そして処置された組織の回帰を長期間にわたり、好ましくは2〜20年間にわたり、予防または緩やかに回帰させる目的で提供される。
本発明の高速クロスリンキング(RXL)テクノロジーは、現行のクロスリンキングプロセス(CXL)の欠点に取り組んでいる。本発明の一部として展開しうるRXLテクノロジーの幾つかとして、1)WO2011/019940に記載された、強化されたリボフラビン配合剤(重水素化水および/または薬理学的透過増強剤を含むものなど);2)マイクロニードルアレイ(MNA)の使用(参照により本明細書に組み入れられた米国特許出願第61/443,191号に記載)またはレーザ誘起応力波(LISW)の使用による角膜実質内への光増感剤(リボフラビン)の上皮保存的送達;3)米国特許出願第61/443,191号に記載された瞼の下に適合させるコンタクトレンズの形状のUVA送達マット;4)脈動および分割を用いたUVAエネルギーパターン;5)眼組織のガス補給(in−gassing)のための、酸素富化された周囲環境(例えば、アイマスクまたは酸素分配チューブ)の生成、が挙げられる。
1)非侵襲性;RXL手順は、角膜上皮の除去および/またはデブリドマンを必要とせず、術間または術後疼痛および視力回復の遅延を低減する;
2)速度;CXLでは眼球あたりおよそ1時間であるに対し、RXL手順は2つの眼球で15分未満が可能である;
3)精密性;本発明によるRXLは、(a)患者の、UVAに対するより良好な心合わせ;(b)透過フィードバックを用いた精密なタイミングでの自動リボフラビン送達、のためにより精密な処置が提供される;
4)均一性;UVAのRXLビームプロファイル(時間的および空間的)は、CXLガウシアンビームプロファイルに反して均一である。前者は、より正確で再現性のある均一なクロスリンキング結果を実現する;
5)エネルギー出力の増加:基本的な固定CXLの強度3mWcm2に比較して、RXLにおいてはUVA強度を9〜100mWcm2増加させる柔軟性が存在する;
6)エネルギー適合の増加:RXLビームは、UVAの放射照度がより高いため、脈動および分割することができ、それによりクロスリンキングをより効率的に奏功することができる;
7)強膜送達;RXLは、強膜送達用に構成させることができる(図10参照)、
というRXLプロセスの重要な利益を実現することができる。
現行のCXLプロセスにおいて過去に記述された通り、通常は、UVA照射前に30分間(眼球あたり)のリボフラビン目薬(3分毎)の利用により、角膜組織を予浸する。しかし本発明のRXLプロセスは、予浸時間を大幅に低減する。このRXLの発明は、クロスリンキングにとって十分なリボフラビンを、角膜実質へ迅速に(2〜15分以内で)送達するMNAまたはLISWの使用を企図する。
RXLには、実質におけるリボフラビンの必要な吸収および活性化が求められる。これらの吸収および活性化条件を実現するために、新規なリボフラビン配合剤が、RXLにおいて用いられる(温度の蛍光モニタリングの記載において先に言及されたものと同様)。この配合剤は、塩化ベンザルコニウム(BAC)などの薬理学的透過増強剤と、重水素化水(D2O)などの酸素ラジカルの有効期限延長剤(oxygen radical lifetime enhancers)とを含んでいてもよい。RXLと熱放射線との組み合わせの予期せぬ利益は、重水素化溶液が従来の溶液よりも組織表面に関してより大きな熱バリアとして働くことである。
クロスリンキングUVA送達相の間、溶解酸素は、活性酸素種(ROS)の生成により消費される。それは、クロスリンキング(即ち、タイプII光増感剤反応)を活性化するROSである。このため溶解酸素は、コラーゲン組織内での分子反応を生成して共有結合をもたらすのに不可欠である。溶解酸素が存在しない場合、そのプロセスは遮断される。重大なこととして、重水素化されたリボフラビン配合剤は、高濃度の溶解酸素のプレローディング(pre−loading)を可能にする新規な能力を有する。この能力により、ROSプロセスが減衰せずに進行することが保証される。
角膜実質内のコラーゲンを取り囲むリボフラビンへのUVA照射の適用は、低酸素を(UVA照射の強度に比例する割合で)誘発する。過度の低酸素(長期間のUVA照射により誘発されうる)は、内皮および角膜実質細胞の細胞毒性をまねく可能性があり、そのような過剰は、コラーゲン組織のクロスリンキングを増強しない。低酸素を予防するわずかな直接的手段の1つが、周囲の(または補助的に瓶詰めにされた)酸素を実質内へ拡散させることであるが、これは緩やかなプロセスである。30秒間〜2分間での処置には、周囲酸素が実質全体を拡散および横断する必要がある。
図9のチャートに、インビトロでの熱送達を用いたブタ眼組織の安定性(剛直性)を示す。熱傷害による誘発されうる上皮およびボーマン膜への損傷を予防することから、それは、カスタム圧平レンズを熱制御/冷却デバイスとして用いた表面熱制御/冷却の重要性を実証している。先行技術の方法と同様に、上皮およびボーマン層が熱放射線適用により損傷された、クロスリンキングされた角膜では、安定性は有意に低い。しかし、熱制御/冷却技術、例えばTSサファイアレンズを本発明による熱処置の間に用い、その後、RXLクロスリンキングを実施すると、80%までの剛直性を眼組織において達成しうることを、検査は示している。ところが、熱送達による表面熱制御/冷却が存在しなければ、剛直性因子のクロスリンキングが、10%以下の上昇となる。
以下の記載は、本発明を実行するための可能な一手順を表わす。患者のインフォームドコンセントが得られたこと、および器具を較正したこと、処置される患者の片眼または両眼が特定され、処置計画またはひな形が特定されたことを、仮定している。本発明は、この手順に限定されず、手順の変形例は本発明の範囲内である。
部分麻酔の滴下:プロパラカインまたはテトラカイン。予浸へ進める前に5分間待つ。
リボフラビンの予浸:マイクロニードルアレイまたはレーザ誘起応力波(LISW)を利用して、リボフラビン配合剤を実質内へ送達する。
適切な拡散のために、5分間の浸潤時間をあてた。
UVAステップ:目盛を定めた(calibrated)コンタクトレンズマットを、各眼内へ挿入して、PC画面上で10分間のカウントダウンを開始した。患者の瞼を閉じさせても、または一部閉じさせてもよい。
部分麻酔の滴下:プロパラカインまたはテトラカイン。心合わせおよびカップリングステップに進む前に5分間待つ。XYZ方向に調整可能な処置用コーンを「ホーム」ポジションに、そしてコーンレンズの温度を8℃に設定する。
反対側の目にパッチを当てる
心合わせおよびカップリング
(1)患者をTSシステムのベッドに仰臥させて、瞼に開瞼器を挿入する。
(2)処置される目がコーンから1cm以内になるように、ベッドの高さを調整する;2台のライブビデオカメラにより、視覚化の補助をPC画面上で提供する。
(3)患者の瞳孔を中心化するために、XYZ方向の制御によりコーンの位置を調整するが;瞳孔の中心をPC画面に表示して、外科医に処置中心を選択させることができる。
(4)吸引リングを作動させて、目を適所に固定する。
熱送達
(1)システムの力を圧平後30秒間控えて、角膜を冷却させる
(2)処置パターン/心合わせをPC画面上でプレレビューする
(3)タッチ画面の「Ready to Treat」を押すことにより、ステップ2番を確定する
(4)足元のスイッチを押して始動させ、処置の終了まで押し続ける(およそ60秒間)
(5)ステップ4番が完了した後、冷却するためにシステムの力を15秒間控える
(6)冷却した後、システムが吸引リングの真空を自動的に停止する
反対側の目を処置する:人工涙液を滴下して、処置する目にパッチを当て、反対側の目に先に記載された通り印をつけて心合わせし、カップリングして処置した。
UVA処置
(1)患者は、TS−RXLベッドに仰臥位の状態である
(2)局所麻酔1滴を両眼に滴下する
(3)UVAコーンをTS−RXLシステムの処置目的で挿入する
(4)予備のUVAイルミネーションを用いて、UVAコーンを各角膜中心に心合わせし圧平する
(5)心合わせおよび圧平を視覚カメラのフィードバックによりPC画面上で確認する
(6)ポンプを始動することによりRF送達を開始する
(7)10分間のUVA暴露を開始する
(8)システムがUVAを自動的に停止させる
Claims (26)
- 目の表面を照射して、前記目の表面の下の表面下領域内に傷害を誘発するための、水の高吸収範囲内で動作されるレーザと、
前記レーザにより照射される目の表面の温度を制御するように構成された目の表面の冷却メカニズムと、
目の表面および表面下領域を照射して、傷害内または傷害付近の目の表面の下で眼組織のクロスリンキングを誘発するように構成された紫外線源と、
を含む眼組織の処置のための装置。 - 目の表面の表面下への送達のための溶液供給源を更に含み、前記溶液が眼組織のクロスリンキングを促進する、請求項1に記載の装置。
- 前記溶液が、重水素化水のリボフラビン溶液である、請求項2に記載の装置。
- 前記レーザが、連続波赤外ファイバーレーザである、請求項1に記載の装置。
- 前記冷却メカニズムが、目の表面上で圧平するように構成されたサファイアレンズを含む、請求項1に記載の装置。
- 前記冷却メカニズムが、目の表面上で圧平するように構成された高度熱伝導性材料で製造されたレンズを含む、請求項1に記載の装置。
- 前記冷却メカニズムが、目の表面上で圧平するように構成されたレンズの温度を制御するための冷却水を輸送する水路を含む、請求項1に記載の装置。
- 目の表面が角膜表面であり、表面下領域が実質を含む、請求項1に記載の装置。
- 水の高吸収範囲のレーザ光を目の表面および前記目の表面の下の表面下領域へ誘導し、前記レーザ光が目の表面の形状を改良すること、
表面下領域、またはその付近の眼組織の照射表面の下の深さで、クロスリンキング剤を送達すること、ならびに
紫外線を目の表面へ誘導して、表面下領域またはその付近でコラーゲンクロスリンキングを促進して、目の表面の改良された形状を安定化させ、前記クロスリンキング剤がクロスリンキングを促進すること、
を含む、眼組織を処置する方法。 - 目の表面が、角膜表面である、請求項9に記載の方法。
- レーザ光により誘発される表面下領域のサーマルリモデリングが、目の表面の形状を改良する、請求項9に記載の方法。
- 目の表面を冷却して、表面領域のサーマルリモデリングを予防し、表面下領域の温度を制御することを更に含む、請求項9に記載の方法。
- 紫外線が、脈動および分割形式(pulsed and fractionated format)で送達されて、眼組織への外傷を最小限に抑え、眼組織の透過性を高める、請求項9に記載の方法。
- レーザ照射の間、レーザ照射の後、またはその両方で、眼組織の温度を0℃〜20℃の範囲内で制御することを更に含む、請求項9に記載の方法。
- レーザ光が、連続波赤外レーザからのものである、請求項9に記載の方法。
- 目の表面が角膜表面であり、レーザ光が上皮、ボーマン層、デスメ膜、および内皮膜における傷害を誘発せずに、実質における熱傷害を選択的に誘発する、請求項9に記載の方法。
- レーザ光が、100μm〜400μmの目の表面より下の深さの表面下領域内に熱傷害を選択的に誘発し、その熱傷害の上および下に熱傷害を誘発しない、請求項9に記載の方法。
- レーザ光が、表面下領域内の傷害を、0.2mm〜1mmの選択された輪状の幅で誘発する、請求項9に記載の方法。
- 目の表面を照射して、目の表面の下の表面下領域内に傷害を誘発するための、水の高吸収範囲内で動作されるレーザと、
前記目の表面上で圧平するレンズを含み、前記レーザにより照射される目の表面の温度を制御するように構成された、目の表面の冷却メカニズムと、
を含む眼組織の処置のための装置。 - 前記レーザが、連続波赤外ファイバーレーザである、請求項19に記載の装置。
- 前記圧平レンズが、サファイアレンズを含む、請求項19に記載の装置。
- 水の高吸収範囲内のレーザ光を、目の表面および前記目の表面の下の表面下領域へ誘導すること、ならびに
目の表面の温度を制御してコラーゲンの収縮を予防しながら、表面下領域内のコラーゲン収縮を可能にし、前記表面下領域内のコラーゲン収縮が目の表面の形状を改良すること、
を含む、眼組織を処置する方法。 - 前記目の表面が角膜表面であり、前記表面下領域が上皮およびボーマン層を含み、前記表面下領域が実質を含む、請求項22に記載の方法。
- 前記表面領域の温度が、照射の間、照射の後、またはその両方で、40℃未満の範囲で制御される、請求項22に記載の方法。
- 眼組織で、熱放射線およびOCT光を含む同軸性ビームを誘導し、前記熱放射線が眼組織を改良すること、
前記OCT光が眼組織改良の映像を提供すること、ならびに
OCT像に基づいて眼組織の処置のパラメータを調整すること、
を含む、眼組織を処置する方法。 - ダイクロイックミラーと、
前記ダイクロイックミラーに高度の透過性となる放射線を発光しうる熱放射線源と、
前記ダイクロイックミラーに反射性となる光を発光するように構成されたOCT光源と、
を含み、
前記ミラーを通して伝送される放射線と前記ミラーにより反射される光とが同軸性になるように、前記熱放射線源、OCT光源、およびミラーが配置されている、
眼組織を処置するシステム。
