Abstract
Description
Claims (33)
- 患者の脊髄分節の屈曲を制御する方法であって、該方法は、
該脊髄分節の中の1対の隣接する椎骨に脊椎固定手技を行うことと、
拘束デバイスを該患者の中に埋め込むことであって、該埋め込むステップは、該拘束デバイスを該脊髄分節と連結することを含む、ことと、
該拘束デバイスが該固定された脊髄分節の屈曲に抵抗する力を提供するように、該拘束デバイスの長さまたは張力を調整することと
を含み、該拘束デバイスは、該固定された脊髄分節またはそれに隣接する組織によって負担される荷重を修正する、方法。 - 前記拘束デバイスは、上部テザー部分と、下部テザー部分と、その間に連結されるコンプライアンス部材とを備え、前記連結するステップは、該上部テザー部分を上棘突起と係合し、該下部テザー部分を下棘突起または仙骨と係合することを含む、請求項1に記載の方法。
- 前記長さまたは張力は、所望の値に調整される、請求項1に記載の方法。
- 前記長さまたは張力は、前記固定が前記患者の自然な前弯曲線と一致する位置において形成することを促すように調整される、請求項1に記載の方法。
- 前記脊椎固定手技を行う前記ステップは、前記隣接する椎骨の後方または側方移植のうちの少なくとも1つを含む、請求項1に記載の方法。
- 前記脊椎固定手技を行う前記ステップは、前記隣接する椎骨の後側方移植を含む、請求項1に記載の方法。
- 前記脊椎固定手技を行う前記ステップは、前記1対の隣接する椎骨の間の椎間板腔での椎間移植を含む、請求項1に記載の方法。
- 椎間移植は、骨移植材料を上棘突起および下棘突起に適用することを含む、請求項7に記載の方法。
- 前記脊椎固定手技を行う前記ステップは、第1の補綴を患者に埋め込むことを含み、該第1の補綴は、前記脊髄分節の少なくとも一部分と係合され、前記拘束デバイスは、該第1の補綴またはそれに隣接する組織によって負担される荷重を修正する、請求項1に記載の方法。
- 前記第1の補綴を埋め込むことは、前記1対の隣接する椎骨の間に椎間デバイスを設置することを含み、該椎間デバイスは、関節固定手技の進展中に該1対の隣接する椎骨の間の整列および距離を維持するように構成される、請求項9に記載の方法。
- 前記椎間デバイスは、椎体間固定ケージを備える、請求項10に記載の方法。
- 前記第1の補綴を埋め込むことは、前記1対の隣接する椎骨の間に骨移植材料を設置することを含む、請求項9に記載の方法。
- 前記第1の補綴を埋め込むことは、前記1対の隣接する椎骨の間に椎体間固定ケージを設置することをさらに含み、該固定ケージは、関節固定手技の進展中に該1対の隣接する椎骨の間の整列および距離を維持するように構成される、請求項12に記載の方法。
- 前記骨移植材料は、同種移植片、自家移植片、合成移植片、および異種移植片から成る群より選択される、請求項12に記載の方法。
- 前記第1の補綴または前記拘束デバイスは、前記脊髄分節の中の組織を修正するように適合される治療薬を備える、請求項9に記載の方法。
- 前記治療薬は、骨形成タンパク質を備える、請求項15に記載の方法。
- 前記拘束デバイスを埋め込むことは、前記上棘突起の下面と前記下棘突起または前記仙骨の上面との間に延在する棘間領域の中に補綴を直接埋め込むことなく、該拘束デバイスを該上棘突起および該下棘突起または仙骨と係合することを含む、請求項2に記載の方法。
- 前記拘束デバイスを埋め込む前記ステップは、
前記上棘突起の上面より上位で貫通を形成するように、棘間靱帯を穿刺することと、
該貫通を通して前記上部テザー部分を前進させることと
を含む、請求項2に記載の方法。 - 前記拘束デバイスを埋め込む前記ステップは、
前記上棘突起と隣接する棘突起との間の間隙を通して、前記上部テザー部分を前進させるステップを含み、該間隙は、該上棘突起と隣接する棘突起との間からの棘間靱帯の外科的除去によって作成される、請求項2に記載の方法。 - 前記拘束デバイスを埋め込む前記ステップは、
前記下棘突起の下面より下位において貫通を形成するように、棘間靱帯を穿刺することと、
該貫通を通して前記下部テザー部分を前進させることと
を含む、請求項2に記載の方法。 - 前記拘束デバイスを埋め込む前記ステップは、
前記下棘突起と隣接する棘突起または仙骨との間の間隙を通して、前記下部テザー部分を前進させることを含み、該間隙は、該下棘突起と隣接する棘突起または仙骨との間からの棘間靱帯の外科的除去によって作成される、請求項2に記載の方法。 - 前記拘束デバイスの長さまたは張力を調整することは、前記脊髄分節の治療中、および該脊髄分節の治癒中または治癒後に、該長さまたは張力を複数回調整することを含む、請求項1に記載の方法。
- 前記調整するステップは、前記長さまたは前記張力を経皮的に調整することを含む、請求項22に記載の方法。
- 患者の脊髄分節の屈曲を制御するシステムであって、該システムは、

- 前記第1の補綴は、2つの隣接する椎骨の間に配置される骨移植材料を備え、該骨移植材料は、前記脊髄分節の中において該2つの隣接する椎骨の固定を促進するように適合される、請求項25に記載のシステム。
- 前記骨移植材料は、同種移植片、自家移植片、合成移植片、および異種移植片から成る群より選択される、請求項28に記載のシステム。
- 前記拘束デバイスは、前記上棘突起および前記下棘突起または仙骨と係合され、該上棘突起の下面と該下棘突起または仙骨の上面との間に直接延在する棘間領域は、補綴を含まない状態のままである、請求項24に記載のシステム。
- 前記第1の補綴または前記拘束デバイスは、前記脊髄分節の中の組織を修正するように適合される治療薬を備える、請求項25に記載のシステム。
- 前記治療薬は、骨形成タンパク質を備える、請求項31に記載のシステム。
- 前記拘束デバイスの前記長さまたは張力は、経皮的に調整可能である、請求項24に記載のシステム。
