Abstract
Description

特定の態様において、喘息のための療法的措置は、(1)患者、例えば喘息の症状を示す患者を、(a)唾液中の好中球レベル上昇、(b)痰または唾液中の微生物の存在、および/または(c)NTHiに特異的な抗体に関して試験して、そして(2)3つのパラメーターの1つ、2つ、またはすべてに関して陽性の試験結果を示す患者に、本出願のワクチンを投与する工程を含む。特定の態様において、投与するワクチンは、1つ、2つまたはそれより多くの本出願のNTHi株および/または本出願のNTHi株の免疫原性分画を含む。

ベータ−ラクタマーゼ遺伝子の欠如は、本出願のNTHi株の、場合による特徴である。ベータ−ラクタマーゼは、いくつかの細菌によって産生される酵素であり、そしてペニシリン、セファロスポリン、セファマイシン、エルタペネムおよびカルバペネムのようなベータ−ラクタム抗生物質に対する耐性の原因となる。ベータ−ラクタム抗生物質は、典型的には、広い範囲のグラム陽性およびグラム陰性細菌を治療するのに用いられる。ベータ−ラクタマーゼ発現は、抗生物質耐性を生じうるため、ベータ−ラクタマーゼ遺伝子の存在は、NTHiなどの死菌病原体の製造および投与中には、一般的に好ましくない。製造または患者における不都合な事象中に必要となった場合に、ベータ−ラクタマーゼ遺伝子が欠如していると、生物をベータ−ラクタム抗生物質で制御することが可能になる。
カプセルおよび錠剤の代替物として、経口ワクチンは、液体型、例えば、溶液、シロップまたは懸濁物であってもよいし、あるいは水または他の適切なビヒクルで使用前に再構成するための薬剤製品として提示されてもよい。懸濁剤(例えばソルビトールシロップ、セルロース誘導体または水素化食用脂);乳化剤(例えばレシチンまたはアカシア);非水性ビヒクル(例えばアーモンド油、油性エステル、または分画化植物油);および保存剤(例えばメチルもしくはプロピル−p−ヒドロキシベンゾエートまたはソルビン酸)などの薬学的に許容されうる添加剤を用いた慣用的手段によって、こうした液体調製物を調製してもよい。
また、ワクチンを吸入または注射による投与のために配合してもよい。

ワクチンおよび第二の療法剤を投与する潜在的に相乗性の効果があるため、こうした剤を、一方または両方の剤を投与した際に、喘息またはCOPDを治療するのに有効でない量で投与してもよい。
長期制御薬剤の例には、限定されるわけではないが、(1)吸入コルチコステロイド、例えばフルチカゾン(例えばFlovent DiskusTM)、ブデソニド(例えばPulmicortTM)、トリアムシノロン(例えばAzmacortTM)、フルニソリド(例えばAerobidTM)、およびベクロメタゾン(例えばQvarTM);(2)長期作用ベータ−2アゴニスト(LABA)、例えばサルメテロール(例えばSerevent DiskusTM)およびホルモテロール(例えばForadil AerolizerTM、OxisTM、PerformistTMおよびBrovanaTM);(3)長期作用ムスカリン性アンタゴニスト、例えばチオトロピウム(例えばSpirivaTM)およびイプラトロピウム(例えばAtroventTM);(4)ロイコトリエン修飾剤、例えばモンテルカスト(例えばSingulairTM)、ザフィルルカスト(例えばAccolateTM)およびジロートン(例えばZyflo CRTM);(5)マスト細胞阻害剤、例えばクロモリン(例えばIntalTM)およびネドクロミル(例えばTiladeTM);(7)テオフィリンが含まれる。
1.1 背景
株ID164、165、167および168を含むいくつかのNTHi単離体を、オーストラリア・ニューカッスルのJohn Hunter病院の呼吸器クリニックに通う20人の患者から収集した。該単離体および別の研究単離体(NTHi−166株)を、増殖条件、生物型、血清型、Bカプセル遺伝子の存在およびベータ・ラクタマーゼ遺伝子の存在に関して性質決定した。結果を以下の表1に示す:

1.2.1 微生物学的同定
インフルエンザ菌同定:増殖にはXおよびV因子が必要である。分類不能同定であり(すなわちいかなるタイプのカプセルも持たない)、Bカプセルの発現がなく、そしてBカプセルの遺伝子がなく、β−ラクタマーゼがない。
ヘモフィルス試験(HTM)補充剤を補充したOxoidTMトリプトン大豆ブロス(TSB)中でよく増殖する。HTM補充剤を含むOxioidTM veggitoneブロス中でもよく増殖する。

のいずれか100mLを含有する250mLコニカルフラスコ中でインキュベーションした。
これらのデータによって、死菌NTHi−164が防御性免疫反応を刺激する能力を有することが立証される。本出願者らは、この能力が、細菌抗原の発現パターンおよび/または天然アジュバントの存在、および/またはパイエル板内に取り込まれる能力(粒子サイズ、そしてまたおそらく接着タンパク質に関連するようである)に関連すると考えている。
第0日に、腸管腔に直接送達される単回用量の3x107、3x108、または3x109のホルマリンで殺したNTHi−164(本明細書においてHI−164と称する)、またはその後、第14日の1回の経口用量または第14日および第21日の2回の経口用量によって、ラットを免疫した(すなわち、ラットは1回、2回または3回の用量を投与された)。第28日、気管内への滴下によって、ブースト用量(腸用量をブーストする能力を持つが、それ自体に対する防御免疫を提供する能力を持たない)を肺に送達した。各免疫措置に関する対照ラットをプラセボ(PBS)で免疫した。第35日、5x108生菌NTHi−164の気管内滴下によってラットを感染させた。4時間後、サンプル採取のため、ラットを屠殺した。肺を洗浄して、気管支肺胞洗浄液(BAL)を提供し、そして肺をPBS中でホモジナイズして肺ホモジネート(LH)を提供した。BALおよびLHの連続希釈を調製し、そして各希釈の試料をチョコレート寒天プレート上にプレーティングした。5%CO2の大気中、37℃でプレートを一晩培養した。プレート上のコロニーを計数し、そしてBALおよびLH中のNTHi−164数(コロニー形成単位、またはCFU)を計算した。肺中の総NTHi−164 CFUを決定した。ワクチン免疫動物およびプラセボ免疫動物に関する肺中の平均CFUを計算し、そして各ワクチン接種措置によって提供される細菌クリアランスを適切な対照群と比較して計算した。結果を図2に示す。用量が3x108または3x109である場合、1以上の用量を投与された動物において、優れた細菌クリアランスが得られた。用量が3x107である場合、2または3の用量を投与された動物において、優れた細菌クリアランスが得られた。
これらの実験によって、NTHi−164がラットと同様にマウスを防御する能力を立証する。これらの実験はまた、ワクチンが、感染に最も感受性であるマウス株において、最も防御性であることを立証する。
本実施例は、ホルマリンで殺したNTHI−164での免疫によって、広い交差防御が提供されることを立証する。これは、主要OMP P2の保存が欠如していることが、ヒトにおける交差防御の見かけの欠如の原因であると捉えていたSethiおよびMurphy(SethyおよびMurphy, 2002, Clinical Microbiology Reviews 14(2):336−363)によっては予測されなかったであろう。
本実施例は、死菌NTHi−164でのワクチン接種が、細菌感染に対する肺防御において重要なNTHi特異的T細胞の誘導を導くことを立証する。

本研究は、ヒト血液中のインフルエンザ菌特異的T細胞が、NTHI−164を認識し、そしてこれに反応してIFN−γなどのサイトカインを分泌可能であることを示唆する。IFN−γはまた、防御機構にも関与し、これにより、IFN−γは食作用細胞(マクロファージおよび多形核細胞)をより活発に貪食し、そして細菌を殺すように活性化する。

プラセボ対照安全性および免疫原性研究に採用した被験体を、特異的免疫反応誘導に関して調べた。図4は、研究経過に渡って、ワクチンおよびプラセボ治療群で生じた特異的Tリンパ球誘導を示す。プラセボ患者群の血中の特異的Tリンパ球は研究期間に渡って増加した(図5A、5B)。この反応は、この期間中にインフルエンザ菌細菌への曝露によって誘導された可能性が最も高い。活性錠剤治療を受けた患者において、特異的リンパ球の誘導が増進され、天然誘導免疫反応がブーストされることが示唆された。in vitroでのPHAに対する反応増進もまた、HI−164OV群由来の血液リンパ球で観察された(図5C)。
HI−164OV治療群における唾液リゾチームの減少が観察され、炎症の減少が示唆される(図6)。
血清IgG、および喉の含嗽剤中でNTHiが検出された来診の回数の間の関係に関して、上記と同じ被験体を調べた。第1回および第6回の来診の間の血清NTHi特異的IgGの平均(±SEM)対数変化を、インフルエンザ菌が含嗽剤中で検出された来診の数(来診時0〜1回または来診時2〜4回のいずれかとして分類する)に対してプロットした(図7A)。プラセボ群では、高レベルの血清NTHi特異的IgGは、来診時2回以上での生菌NTHi検出と関連した。活性HI−164OV錠剤で治療した被験体では、こうした関連は明らかではなかった。基準からの血清IgGの変化中央値のプロットを、第2回〜第7回の来診すべてに関して、図7Bに示す。ここでも再び、プラセボ治療群における血清NTHI特異的IgGの増加およびHI−164OV治療群における血清NTHi特異的IgGの減少が立証される。このデータは、上気道の細菌が下気道に進入し、そして血清においてインフルエンザ菌特異的IgGを誘導するのが、活性治療によって防止されると解釈された。インフルエンザ菌が含嗽剤中で検出された来診回数が0〜1回または2〜6回であった被験体に関する、血清NTHi特異的IgGにおけるベースライン(第1回の来診)からの変化中央値を図7Bに示す。
過去2年間に1日あたり少なくとも10本のタバコを喫煙していたことに基づき、64人の被験体を採用し、そして二重盲検研究において、経口NTHi療法またはプラセボ治療群に割り当て、プラセボ対照二重盲検臨床研究を行った。被験体をプラセボ群および活性群にランダム化し、そして研究薬剤の3回のコースを月間隔で投与した。各コースは、3日間の1日あたり2錠剤からなった。活性錠剤は、各々、45mgのホルマリンで殺したNTHi(活性錠剤あたり1011死細菌と同等)を含有した。2週間ごとの7回の来診時に、血液、唾液、含嗽剤、喉スワブ、および鼻スワブ(微生物学的評価のため)を収集した。
驚くべきことに、冬期に渡るプラセボ治療群およびワクチン治療群における測定によって、どちらの群でもNTHiが検出され、この細菌にランダムに曝露されていることが示された。図8は、各来診時のプラセボ群の含嗽剤におけるNTHiの平均レベルを示す。
ワクチン有効性に関するマーカーとして血清IgG抗体を測定した。ワクチン治療群におけるIgG反応の見かけの欠如が見出され、一方、患者のプラセボ治療群は、血清IgGの増加を示した。理論によって制限されることなく、本出願者らは、プラセボ群で観察されたIgGの増加は、下気道に到達した感染細菌に対する免疫反応を反映しており、下気道では、抗原提示細胞によって細菌が取り込まれ、そして排出リンパ節に輸送されて、抗細菌IgG反応が誘導されると考えている。対照的に、ワクチン治療群においてこうした反応が欠如していることは、細菌の下気道への到達が本質的に防止されている(粘膜ワクチン特異的免疫反応によって)ことを示す。また、低頻度(来診時に0〜1回検出)または高頻度(来診時に2〜6回検出)でNTHiが上気道で検出された被験体におけるIgG反応を比較すると、プラセボ群ではIgGの増加が示されたが、活性(ワクチン)治療群では示されなかった。これによって、NTHiでの経口ワクチン接種後の血清IgG測定は、感染に対する曝露およびこれが粘膜免疫によってどの程度防止されたかを反映することが示唆される。唾液IgG反応は、血清中で見られたものを反映した。
軽度から中程度または中程度から重度の気道疾患を有する140人のヒト被験体を二重盲検プラセボ対照研究に採用して、喘鳴可逆性気道閉塞の回数および重症度に対する経口死菌分類不能インフルエンザ菌(NTHi)ワクチン、ならびに併用薬剤の使用の効果、ならびに気道中のNTHiおよび他の細菌の存在を評価した。
本研究は、臨床研究機構の監視の下に、4つの場所で行われた、多施設二重盲検プラセボ対照試験であった。これらの場所は:Hollywood Private Hospital、ウェスタンオーストラリア州パース(M.K. Tandon博士); Sir Charles Gairdner Hospital、ウェスタンオーストラリア州パース(M. Phillips博士); Royal Perth Hospital、ウェスタンオーストラリア州パース(G. Waterer准教授)およびUniversity of Newcastle、ニューサウスウェールズ州ニューカッスル(R. Clancy教授)であった。各施設の倫理委員会によって同一のプロトコルが認可され、そして研究はこれらの委員会の指針にしたがった。主な参加基準(admission criterion)は、連続2年間で1年あたり2回以上のエピソード歴であった(「エピソード」は「バックグラウンドレベルからの咳および膿性痰の増加」と定義された)。被験体を2つの群に層別化した:i)予測される正常値の50%以下のFEV1を持つ患者(38人の被験体)、およびii)予測される正常値の50%より大きいFEV1を持つ患者(102人の被験体)。この二番目の群には、COPDの患者、気管支拡張症の一次診断を受けた患者、および「正常」FEV1レベルを持つ患者が含まれた。<80%のFEVを持ち、そして気管支拡張症の一次診断を受けていない2つの研究群由来の患者は、COPDを有すると定義され、そしてこの分析の対象である。分析は、すべてのCOPD患者におけるHI−164OVでの治療を調べ、そして次いで、特に、中程度から重度のCOPD患者群における治療を調べる。

急性エピソードを「痰の体積および化膿の増加」とするプロトコル定義を用いると、総COPD群に関して、HI−164OV群では36のエピソードがあり、そしてプラセボ群では50のエピソードがあり、これらは毎月の期間で分析すると、0.76のリスク比(95%CI 0.50、1.15;P=0.19)を有した。3ヶ月間に渡って分析すると、事象率/100日で、2番目の3ヶ月の期間、活性群でより大きい利益があった(プラセボ:事象/100日−0.82(最初の3ヶ月)、0.74(第二の3ヶ月);活性:0.72および0.48。P値。期間効果0.68、および治療効果0.19)。急性エピソードのプロトコル定義(「痰の体積および化膿の増加」)を用いると、中程度から重度のCOPD群で、プラセボ群の29エピソードに比較して活性群では22エピソードがあり、0.84のリスク比(95%CI:0.48、1.49)であり、有意ではなかった。しかし、3ヶ月の期間による分析では、事象率/100日は、最初の3ヶ月に関して、活性群およびプラセボ群で0.82で同じであったが、第二の3ヶ月では、プラセボ群に比較して活性群で減少し(0.6対0.85)、事象の29%の減少に相当した。中程度から重度の疾患をもつものでは、活性治療群(6人の被験体)に比較した際、2〜4のエピソードを伴い、プラセボ群ではほぼ2倍多い患者(11人の被験体)であり、再発減少に対する療法効果と一致した。総COPD群では、防御にはまったくまたはほとんど相違がないが、より重度の疾患を持つものでは、より高いレベルの防御(27%)が見られた。
急性エピソードを伴う患者の比率は、「コルチコステロイド治療」エピソードの定義を用いて、より重度のエピソードのみを捉える場合を除いて、療法ではわずかしか減少しなかった。
エピソードの平均期間は、プラセボ群に比較して活性群でより少なかった(「全COPD」;17.3日に比較して14.7日;中程度から重度のCOPD 22.7日に比較して14.3日)が、この相違は中程度から重度の群においてのみ統計的有意性に到達した(P=0.01)。
COPDを有する被験体の全群および中程度から重度のCOPDを有するものの両方で、活性治療に続く抗生物質の処方コースの数および抗生物質治療の日数(増悪に対するもの)両方の有意な減少が見られた。どちらの型の分析に関しても、「全COPD」および中程度から重度のCOPD患者群の両方において、防御は統計的に有意であった。プラセボ群におけるよりも活性群において、抗生物質の3以上のコースを投与される患者はより少なく(「全COPD」に関しては14人の患者に対して5人、中程度から重度のCOPDに関しては11人の患者に対して2人)、再発エピソードに対する防御があることが裏付けられた。
COPDの増悪のための入院率は高く(以下の表8を参照されたい)、中程度から重度のCOPDのプラセボ群(20人の被験体)では7人の患者が10回入院した。より重度でないCOPDの被験体(17人の被験体)では、研究期間全体で、COPDの増悪のため、2人のみが入院した。「全COPD」におけるCOPDの増悪による入院に対する防御は61%であり、そして有意性は境界線であった(P=0.07)が、中程度から重度のCOPDを有する被験体では、より高い防御レベルが見出され(90%)、これは有意であった(P=0.01)。このより重度の群では、「すべての原因による入院」の有意な相違が検出された(P=0.04)が、非呼吸器事象による入院に対する独立の効果を同定するには、数が小さすぎた。
「急性事象」の痰試料の分析は、抗生物質治療、輸送の遅延および通知の遅れのため、ほとんど情報を提供しなかった。研究開始後の定期的な来診時に収集された標本は、培養が収集4時間未満で行われた場合に有効と見なされ、そして唾液が混入した標本(上皮細胞の存在によって検出)は排除された。「全COPD」のプラセボ群(37人の被験体)におけるこれらの「ルーチン」の試料に関して、患者の半数(19人の被験体)が陽性培養を有し、総数88の陽性培養が生じた。活性群(36人の被験体)に関する同等の数値は、13人の被験体で45の陽性培養であり、すなわちHI−164OV療法後、呼吸器病原体に関して陽性の痰培養を有する被験体はより少なく(31%少ない)、陽性痰培養の総数が、プラセボ群のもののほぼ半数であった。プラセボ群において、14人の被験体で、平均2.8回、NTHiが単離された。他の病原菌に関しては:カタル球菌が5人の被験体で(平均2.4回);肺炎球菌が6人の被験体で(平均1.7回);緑膿菌が11人の被験体で(平均2.3回)単離された。活性群では、NTHiが8人の被験体で(平均2.9回);カタル球菌が7人の被験体で(平均1.0回);肺炎球菌が4人の被験体で(平均1.5回);緑膿菌が3人の被験体で(平均3回)単離された。さらなる分析によって、プラセボ群では、陽性痰培養を有した21人の被験体が34回の急性エピソード(被験体あたり1.62回)を有した一方、陽性試料がなかった16人は16回の急性エピソード(被験体あたり1.0回)を有した。活性群では、陽性培養を有した14人が20回の急性エピソード(被験体あたり1.42回)を有し、そして陽性痰培養を有した22人が16回の急性エピソード(被験体あたり0.73回)を有した。NTHi単離体に関して、類似のパターンが示された(プラセボ:20回の急性エピソードを伴う14人の被験体(被験体あたり1.42回)で陽性培養、および30回の急性エピソードを有した23人(被験体あたり1.31)では増殖なし);活性:陽性培養を有した8人の被験体が10回のエピソードを有する(被験体あたり1.25回)一方、増殖がない28人は26回のエピソードを有した(被験体あたり0.93回)。(i)陽性痰培養は、急性臨床的エピソードを有するリスクがより高いことを予測し(被験体がプラセボ群または活性群にあるかどうかに関わらず)、(ii)活性群においては、培養陽性および培養陰性群の両方において、明らかな防御があり(培養陽性群ではおよそ12%−プラセボ群の1.62に対して、活性群の1.42の感染エピソード/個体値として計算;そして「非培養陽性」群ではおよそ27%−プラセボ群の1.0に対して、活性群の0.73の感染エピソード/個体値として計算);(iii)HI−164OV経口療法後には、陽性培養がより少ない、と結論づけられる。これらの「増殖がない」培養は、より少ない増悪と関連する(上記)。NTHi陽性痰の類似の分析によって、経口HI−164OV療法の防御効果に関してほぼ同一の転帰が提供された(データ未提示)。

1.17 考察
COPDの自然経過における重要な事象である急性増悪は、現在の指針では、介入療法のための特別のターゲットと認識されている。増悪の発生および重症度に対するNTHiの新規単離体(NTHI−164)から調製した経口ワクチン(HI−164OV)での免疫療法の影響を決定するため、再発性増悪の病歴を有するCOPDの被験体を研究した。しかし、こうした被験体は、COPDを有するもの全体の20〜25%にしか相当せず、このため、本研究では、計画したよりもはるかに少ない登録者数しか得られなかった。この困難は、「痰の体積および化膿の増加」としての急性エピソードの古典的な記述定義を使用することによって、さらに困難となった。この定義は、急性エピソードが感染起源であることを反映するものの、COPDにおける気管支内炎症のバックグラウンド・レベルが多様であり、別個のエピソードの区別が不鮮明であることを考慮すると、不正確でありうる。定義によって課される制限にもかかわらず、この研究から、2つの結論が得られうる。第一に、COPDおよび再発性増悪を伴う被験体は、HI−164OVでの経口免疫療法から利益を得て、特にその重症度が減少する。第二に、中程度から重度のCOPDを有する被験体は、より軽度な疾患を持つ被験体よりも、大きな度合いで利益を得る。
気管内(IT)免疫および一団のNTHi単離体での急性呼吸器感染曝露のラットモデルにおいて、NTHI−164およびNTHI−166(NTHi289)をワクチンとして試験すると、異なるレベルの防御が生じた。感染曝露に用いた単離体は、NTHI−164、NTHi−165、NTHI−166、NTHI−167、NTHi−168、ATCC 3041、ATCC 43095、ATCC 35092およびATCC51997であった。
PBS群中の平均細菌
4時間の細菌クリアランス・パーセントは、ワクチンによって提供される防御の度合いの尺度となる。
中程度から重度の気道疾患を有する患者において、急性気管支炎のエピソードを防止する際の、ホルマリンで殺したNTHI−164を含有する、経口投与された死菌全細胞分類不能インフルエンザ菌(NTHi)ワクチン(本明細書において、「HI−164OV」と称する)の安全性および有効性を評価する、多施設二重盲検プラセボ対照前向き研究。

1.18.2 方法論
中程度から重度(予測される値の50%以下のFEV1)の気道疾患を有する患者における急性気管支炎のエピソードを防止する際の、経口投与された死菌全細胞NTHi HI−164OVの安全性および有効性を評価する、前向き二重盲検プラセボ対照平行群多施設研究。
研究サンプルサイズを、5%有意性レベルおよび80%検出力に基づいて計算し、群あたり50人の参加者のサンプルサイズは:
・プラセボ群において感染率を2と仮定して、およそ0.63の相対感染リスク(すなわち37%の相対リスク減少);
・28%の何らかの感染を経験している患者比率の絶対減少(プラセボ群の80%が何らかの感染を有すると仮定して);
・プラセボ群が2.4の処方率であると仮定して、0.67の処方抗生物質に関する相対リスク(すなわち33%の相対リスク減少);
・1.2日の感染期間の平均日数の相違(2日間の期間の標準偏差と仮定して)
の検出を可能にするであろう。
HI−164OV腸溶性コーティング錠剤: 45mgのホルマリン不活性化NTHi−164に加えて賦形剤を含有した。凍結乾燥活性物質をHI−164と称する。プラセボ腸溶性コーティング錠剤は賦形剤のみを含有した。プラセボおよびHI−164OVを2〜8℃で保存し、そして錠剤は約450mgの重量であった。
患者は、連続3ヶ月間、各月に連続3日間、朝食の30分前の空腹時に、コップ一杯の水で研究薬剤を経口摂取した(第1〜3日、第29〜31日、および第57〜59日)。研究期間はおよそ8ヶ月であった。
1.18.6 評価基準
主な有効性変数は:

・急性気管支炎エピソードを経験している治療中の患者の比率
・急性気管支炎エピソードの期間
・急性気管支炎エピソードを治療するための抗生物質コース数
・痰中のインフルエンザ菌、カタル菌、肺炎球菌およびシュードモナス属種の存在
・急性気管支炎エピソードの重症度
であった。
38人の患者の治療意図集団、すなわち研究薬剤を投与されたかどうかに関わらず、治療にランダム化されたすべての患者に対して統計分析を行った。すべての関連するデータを治療群によって要約した。分析は3つのセクションからなった:
・採用、および研究終了時の状態
・ベースライン特性
・一次および二次目的に対する治療効果の評価。
124人のランダム化患者の採用目標は満たされず;実際にスクリーニングした患者の総数は42人であり、このうち38人がランダム化され、そして34人が研究を完了した。ランダム化された患者のうち、研究を完了しなかった患者が各群に2人おり、このうち各群1人の患者が不都合な事象のために中断した。ベースライン患者人口統計学は、HI−164OV群およびプラセボ群に関して、概して類似であった。集団の平均年齢は約70歳であった。女性よりも多くの男性が登録され、活性群およびプラセボ群に関して、それぞれ、83.3%および70.0%が男性、そして16.7%および30.0%が女性であった。すべての患者における平均FEV1は、0.95リットル/秒であり、そして活性群およびプラセボ群間で類似であった;活性群およびプラセボ群において、中央値はそれぞれ、1.00および0.85であった。両群で、喫煙したことがない患者に比較して、喫煙歴がある患者がより多い比率であると報告された(84.2%が喫煙ありに対して15.8%が喫煙したことがない)。患者は年間、平均51.36パックを喫煙し、そして中央値は年間、45.5パックであり;活性群およびプラセボ群に関して、それぞれ、中央値は年間45.0パックおよび46.0パックであった。スクリーニング時、すべての患者の15.8%(群間で類似の分布)がなお喫煙していた。
急性気管支炎のエピソード数: HI−164OV群において、18人の患者のうち13人で22のエピソードがあり、そしてプラセボ群において、20人の患者のうち13人で29のエピソードがあった。1ヶ月の期間を用いた急性気管支炎エピソードの分析によって、0.83のリスク比[95%CI 0.47、1.46]が示され、治療の利益があるが、統計的有意性は達成しないことが示された(p=0.520)。3ヶ月の期間を用いた分析によって、0.84のリスク比[95%CI 0.48、1.49]が示され、やはり治療群で利益があるが、統計的に有意でないことが示された(p=0.397)。しかし、研究の最初の3ヶ月では急性気管支炎事象は類似の率であった(各群に関して0.82事象/100患者日)一方、最後の数ヶ月には、プラセボ群では率は0.85であったが、HI−164OV群では0.60であった。これは、薬剤の3サイクルすべてが投与された後、この第二の期間ではHI−164OVが有益であることを示唆するが、これは有意ではなかった(回帰p値:期間0.894、治療0.397、相互作用0.533)。第二の期間でエピソード数がほぼ30%減少したのは、研究が晩秋に始まり、そして利益が明らかになる前に6〜8週間の遅滞期間があることが以前の研究から推測されるため、臨床的に関連している。
防御の代理粘膜免疫マーカー(血清におけるNTHi特異的抗体)の測定値:NTHi抗原に対する抗体をこの実施例研究の開始時および終了時に検出した。平均レベルまたは変動の度合い(大きなSEバーによって決定)のいずれに関しても、プラセボ群および活性群間で、有意な相違は見られず、これは、喫煙するが、その大部分が気管内コロニー形成の異なるレベルを反映する気道疾患に気付かなかった被験体のHI−H003研究において見られたとおりであった。中程度から重度のCOPDを有するものにおいて(HI−H002研究)、IgG抗NTHi抗体の変化の小さいが有意な相違があり(log10の0.15;p=0.004)、IgA抗NTHi抗体に関する変化に類似の方向の傾向があった(log10の0.09;p=0.08)。この有意な治療効果は、予測の>50%のFEV1を有するものでは見られなかった。機構は明らかでない。治療に対する抗体反応をより頻繁に監視すると、変化機構のより明らかな図が提供されうる。
不都合な事象:任意の一次器官別大分類においてAEを経験している患者の総比率は同様であった(HI−164OV群における18人(100%)対プラセボ群における19人(95%))。AEと報告されるものの大部分は軽度または中程度であり、そして研究治療と関連しているとは見なされなかった;研究治療に関連している可能性があると記載される事象を有した患者が、HI−164OV群において1人(中程度の胃腸炎のエピソードを有した患者03)およびプラセボ群において2人(第1回来診直後に中程度の発疹および体の火照りを有し、そしてそれ以上の研究薬剤の摂取を止めるよう助言された患者14;および軽度の吐き気を有した患者27)いた。研究治療に明確に関連すると見なされる事象はなかった。
1.20 材料および方法
1.20.1 全細菌抽出物の調製
方法I−チョコレート寒天プレート上で増殖させた細菌
チョコレート寒天プレートに、NTHI−164または他のインフルエンザ菌単離体の単一コロニーを接種した。プレートを37℃+5%CO2で一晩インキュベーションした。これによって、多量の増殖が生じた。滅菌10μlループを用いて、プレート表面から細菌を取り除き、そして微量遠心管のCelLyticB細胞溶解試薬0.5mL内に浸した(溶解緩衝液に浸したまま、2本の指の間でループをこすりつけて回転させることによって、細菌を落とした)。試験管を5分間静置して細菌が完全に溶解するのを可能にした。次いで、試験管を11,500gで5分間遠心分離して、いかなる不溶性細胞破片もペレットにした。次いで、滅菌微量遠心管内に上清を注意深く取り除き、そして必要になるまで−70℃で保存した。Pierce BCAタンパク質アッセイキットを用いたタンパク質概算アッセイを行うことによって、試料のタンパク質濃度を決定した。
トリプトン大豆ブロス基剤およびHTM補充剤をOxoidから得た。
HTM補充剤を含むトリプトン大豆ブロスを調製するため、12.5gmのTSB粉末を測り取り、そして1L Schott瓶に入れた。次いで、2.5gの酵母エキスを測り取り、そして瓶に添加した。蒸留水(0.5L)を添加し、そして瓶を振盪して材料を溶解させた。次いで、培地を15分間オートクレーブした。培地を一晩、室温に冷却させた。第2日、滅菌技術を用いて、3mlシリンジおよび19G針を用い、HTM補充剤のバイアルに、2mLの滅菌蒸留水を添加した。これを回転させて固体を溶解し、そして室温で30分間静置した。滅菌シリンジおよび針を用いて、バイアルの内容物を培地瓶に添加した。Schott瓶の内容物を混合し、そして日付を記した。培地を最長1ヶ月、2〜8℃で保存した。チョコレート寒天プレートに、HI−164の単一コロニーを接種し、そして37℃+5%CO2で一晩インキュベーションした(これによって、多量の増殖が生じた)。第3日、100mLの調製TSB+HTM培地を250mL Erlenmeyerコニカルフラスコに測り入れ、そして37℃振盪インキュベーター中であらかじめ温めた。プレートからPBS内に細菌を採取し、光学密度を測定し、細菌濃度に対して、光学密度の先に調製したプロットを比較し、そして必要に応じて濃度を調整することによって、1x1010の生菌NTHI−164の調製物を調製した。ブロスに1.0x1010生菌NTHI−164(1mLの1010/mL)を接種して、1x108/mLの開始濃度を生じ、そして回転させることによって混合した。細菌懸濁物を37℃振盪インキュベーター中で11時間、一晩インキュベーションした。この時点で、細菌は最適増殖期にあり、そして関心対象のタンパク質を発現していた。これを決定するため、懸濁物の595nmでのOD読み取り値を30分ごとに決定した。ODが安定したとき、増殖曲線は静止期に到達していた。100μlの懸濁物を平底96ウェルマイクロタイタープレートのウェル内にピペッティングして、そして595nmの波長で読み取った。光学密度が0.5〜1.0の間であることが必要であった。懸濁物ODが1.0より高い場合、さらなる滅菌ブロスをフラスコに添加することによって調整した。調整したら、1.5mLの細菌懸濁物を11,500gで5分間遠心分離した。上清を取り除き、そしてペレットを0.4mLのCelLyticBに再懸濁した。これを短時間ボルテックスし、そして5〜10分間混合して、可溶性タンパク質の完全な抽出を確実にし、そして再び11,500gで5分間遠心分離して、いかなる不溶性細胞破片もペレットにした。上清を滅菌微量遠心管に注意深く取り除き、そして必要になるまで−70℃で保存した。Pierce BCAタンパク質アッセイキットを用いたタンパク質概算アッセイを行うことによって、試料のタンパク質濃度を決定した。
各抽出物由来の20マイクログラムのタンパク質を、再蒸留水で20μLに希釈し、そして10μLのSDS−Stop溶液を添加した。試料を、BIORAD正確分子量マーカーとともに、12%SDS−PAGEゲルのウェルに装填した。色素最先端がゲルの底に到達するまで、200Vで試料を泳動した。次いで、Sigma EZBlue染色剤でゲルを染色した。
100μgの各試料をアセトン沈殿させ、そして再水和緩衝液中で再懸濁した。7cm、3〜10のNL IPG片を最初の次元に用い、総フォーカシング時間は5時間および18750V時間であった。IPG片を12%1.5mm厚のSDS−PAGEゲルの上部に装填し、そしてアガロースで密封した。分子量アガロースプラグもまた、ゲルの上部に添加した。ゲルを50Vで10分間、そして次いで200Vで45分間泳動した。ゲルをクーマシーブルーで染色し、そして24時間脱染色した。次いで、ゲルをスキャンし、そしてスポットを検出した。
SDS−PAGEゲルからタンパク質バンドまたはスポットを切り出し、そして1.5ml試験管に入れた。50%メタノール/50mM重炭酸アンモニウムの3回交換を用いて、バンドを脱染色して、そして一晩風乾した。次いで、20mM重炭酸アンモニウム中に溶解した200〜400ngのトリプシンを用いて、バンドを再水和した。10分後、必要であれば、さらに5〜10μlの20mM重炭酸アンモニウムを各バンドに添加した。バンドを37℃で3時間インキュベーションした。次いで、バンドを浸して柔らかくし(macerated)、そして1μlの試料を取り除いた。次いで、試料をCHCAマトリックスと1:1で混合し、そしてMALDIスライド上にスポットした。
1.21.1 一次元ゲル
HI−164およびHI−166の抽出物を一次元ゲル上で泳動した。図14の左パネルにゲル写真を示す。図14の右パネルにバンド同一性を示す。外膜タンパク質2は、NTHI−164ゲル中、2本のバンドとして、そしてNTHI−166ゲル中、1本のバンドとして現れる。これは、2つの細菌単離体におけるこのタンパク質の相違を示し、そして単離体の防御能の相違を説明しうる。NTHI−164中に存在する2つの同定されていないタンパク質(MW 27および35)は、NTHI−166には存在せず、そしてこれらのタンパク質はまた、HI−164によって提供される優れた防御に寄与しうる。
プレート上で増殖させた細菌単離体の抽出物から、2次元ゲルを調製した。2Dゲルを以下の図16A〜16Dに示す。2Dゲルは、HI−164ゲルに2つのスポットが存在するように、外膜タンパク質P2において、HI−164(図16A)およびHI−166(図13B)間での相違を示す。HI−164外膜タンパク質調製物(すなわち抽出された外膜タンパク質)の2Dゲル(図16C)は、この調製物中のタンパク質が、主にOMP2およびOMP26であったことを示す。
一次元ゲルから配列決定したバンドの同一性を図14の右のパネルに示す。2Dゲルから配列決定した特定のスポットの同一性を以下の表11A〜11Dに示す。
Claims (39)
- (a)死菌インフルエンザ菌(Haemophilus influenzae)集団または死菌インフルエンザ菌集団の膜分画であって、前記インフルエンザ菌が以下の特性:
(i)分類不能である;
(ii)生物型Iである;
(iii)好気性条件下で増殖する;
(iv)Bカプセル遺伝子を欠く;および
(v)β−ラクタマーゼ遺伝子を欠く
の任意の2以上の組み合わせによって性質決定される、前記集団
および
(b)薬学的に許容されうるキャリアー
を含む、ワクチン。 - 胃防御コーティングを有する、請求項1のワクチン。
- カプセル、錠剤、または腸溶性コーティング顆粒の形である、請求項2のワクチン。
- 前記錠剤が、前記集団または前記膜分画を含有するコア、および前記コアを取り巻く腸溶性コーティングを含む、請求項3のワクチン。
- 前記コアおよび前記腸溶性コーティングの間にサブ腸溶性(subenteric)コーティングをさらに含む、請求項4のワクチン。
- 最外層としてフィルムコーティングをさらに含む、請求項4のワクチン。
- 前記コアがラクトースを含む、請求項4のワクチン。
- 前記コアがセルロースまたはセルロース誘導体を含む、請求項4のワクチン。

- 単一細菌ワクチンである、請求項1〜20のいずれかのワクチン。
- 多重細菌ワクチンである、請求項1〜20のいずれかのワクチン。
- インフルエンザ菌が、国立計量研究所(「NMI」)に寄託されそして寄託番号第V08/021002号を割り当てられているような単離体NTHi−164、またはNMIに寄託されそして寄託番号第V08/021003号を割り当てられているような単離体NTHi−167である、請求項1〜20のいずれか一項のワクチン。
- ワクチンの単位用量が、前記インフルエンザ菌の約108〜約1013の死菌コロニー形成単位または前記インフルエンザ菌の約108〜約1013の死菌コロニー形成単位の膜分画を含む、請求項1〜23のいずれか一項記載のワクチン。
- 慢性粘膜疾患患者を治療する方法であって、請求項1〜23のいずれか一項記載のワクチンの有効量を前記患者に投与する工程を含む、前記方法。
- 前記有効量が、連続2〜5日間、毎日投与される前記ワクチンの1〜3単位用量であり、各単位用量が、前記インフルエンザ菌の約108〜約1013の死菌コロニー形成単位または前記インフルエンザ菌の約108〜約1013の死菌コロニー形成単位の膜分画を含む、請求項25の方法。
- 3〜5週間間隔後、前記投与を反復する工程をさらに含む、請求項26の方法。
- 前記投与を2回反復し、各投与を、先行する投与後、3〜5週間の期間で反復する、請求項27の方法。
- 前記慢性粘膜疾患が慢性閉塞性肺疾患または嚢胞性線維症関連疾患である、請求項25の方法。
- 前記慢性閉塞性肺疾患が慢性気管支炎である、請求項29の方法。
- 前記慢性閉塞性肺疾患が肺気腫である、請求項29の方法。
- 前記慢性閉塞性肺疾患が中程度から重度である、請求項29の方法。
- 喘息を治療する方法であって、治療する必要がある患者に、請求項1〜23のいずれか一項記載のワクチンの有効量を投与する工程を含む、前記方法。
- 前記有効量が、連続2〜5日間、毎日投与される前記ワクチンの1〜3単位用量であり、各単位用量が、前記インフルエンザ菌の約108〜約1013の死菌コロニー形成単位または前記インフルエンザ菌の約108〜約1013の死菌コロニー形成単位の膜分画を含む、請求項33の方法。
- 3〜5週間間隔後、前記投与を反復する工程をさらに含む、請求項34の方法。
- 前記投与を2回反復し、各投与を、先行する投与後、3〜5週間の期間で反復する、請求項35の方法。
- 喘息が内因性喘息である、請求項33の方法。
