Abstract
本発明は、特徴付けが必要とされる被検者の組織機能を特徴付ける方法に関する。該方法は、関心のある組織空間の内部で定義されるボクセル(voxel)に対して画像化技術を実施することを含み、被検者は少なくとも2つの異なる分圧の常磁性ガスを含むガスを吸入する期間中、画像データが発生する。コンパートメントモデル・アルゴリズムが、ボクセルで発生した画像データに適用され、組織の代謝機能に関する情報を提供する。

本発明の実施形態について、下記の例および図面を参照しつつ一例として説明する。
本実施例は、OE−MRIスキャンデータが多数の被検者から収集された検査について説明する。このデータは、本発明に従って使用するのに適した性質のものである。本発明によれば、コンパートメントモデルがこのデータにフィットされ、組織機能に関連したパラメータ(例えば、Kox,Mox,Vb)を出力する。本発明に係るこうしたデータの使用の一例は、実施例2において提供している。
倫理的承認を得た。患者は、フィリップスInteraシステム(Philips Medical Systems, Best, Netherlands)において1.5テスラで撮像した。被検者は、医療空気(21%酸素)を吸入し、続いて100%酸素を、そして、第2段の医療空気を15L/分で非再呼吸回路を通してリザーバ付きマスクを用いて吸入した。初期のT1重み付けおよびT2重み付けした解剖学的手順を行って腫瘍を描写した。面内で≧3cmで、少なくとも3スライスに存在する病巣だけを含めた。
進行性固形癌を持つ5人の患者を採用した(全て女性、平均年齢62.6才)。画像化手順は、全ての被検者にうまく許容された。全体として、7個の病巣を識別した。100%酸素の呼吸時に、各腫瘍につき0.0087〜0.0526s−1の平均ΔR1値を測定した。この変化は、5個の病巣において統計的に有意であった(図3と図5)。ΔR1は、1個の腫瘍のみ、医療空気に切り替えたときにベースラインに戻った(患者1 腫瘍2;p=0.02)。グループ解析は、酸素吸入時にΔR1の明らかな上昇を示し(p<0.001)、患者が医療空気の呼吸に戻った時、ベースライン値に向かうΔR1の無意味な減少を示した(p=0.117)(図4)。患者人口統計データおよび腫瘍の詳細を表1に集約している。酸素誘導ΔR1の大きさと腫瘍メジアンIAUCまたはKtransとの間に有意な相関は存在しなかった。
酸素過剰ガスの吸入によるR1マッピングでの画像コントラストは、動脈血漿および組織液での溶解分子酸素の常磁性効果に起因する。この研究は、人間の腫瘍での効果を説明した最初のものであり、進行性上皮性卵巣癌を持つ4人の患者において、被検者が医療空気から100%酸素に切り替えた時に有意なΔR1を報告している。胃腺癌を持つ患者では2つの肝転移において、緩慢で無意味なΔR1を検出した。動脈血流が信号変化に寄与する重要な要因になるであろうが、測定したΔR1は、IAUCおよびKtransの両方によって推定される腫瘍血流から独立しており、酸素誘導ΔR1は、酸素の運搬、拡散、代謝の複合測定になるであろうことを示唆する。一般に、該手順はうまく許容され、許容できる信号対ノイズ比を持つ測定可能な信号変化を出力した。これらの予備結果は、有望であり、腫瘍変調R1が酸素化状態についての新規なバイオマーカーを出力し得ることを示唆しており、更なる研究に値するものである。
本実施例は、コンパートメントモデルを、例えば、実施例1で生成されたようなOE−MRIデータにフィッティングすることについて説明する。例示のコンパートメントモデルの適用の結果は、DCE−MRIから生成される標準パラメータを参照して説明する。DCE−MRIは、実績のある診断および予後の能力を備えた、許容された医用画像取得手段(modality)である。
更なる被検者のスキャンから取得したデータは、第1実施例と関連して上述のスキャンに従って実施されたものであり、式(I)〜(V)と関連して、上述したコンパートメントモデルのフィットへの入力データとして使用した。
変換したOE−MRIデータ(PO2)およびPaO2データは、式(V)にCTおよびCbとしてそれぞれ入力し、レーベンバーグ・マルカート(Levenberg Marquardt)非線形最小二乗フィッティングアルゴリズムを適用した。
本実施例において、発明者は、本発明の一態様に係るコンパートメントモデル・アルゴリズムを実施した。アルゴリズムは、Matlabスクリプト言語でソウトウエアとして実行した。発明者は、コンパクトディスクで配布するために、コンパートメントモデル・アルゴリズムを実行するMatlabスクリプトをパッケージ化した。
式(V)のコンパートメントモデルにおけるパラメータCb(血液コンパートメントでの酸素濃度)は、モデルへの入力であり、これは、実施例2において、血液中の分圧PaO2の推定から直接導出される。PaO2は、推定可能な血液のウォッシュイン時間およびウォッシュアウト時間(TOIF)の考慮により、被検者が呼吸する酸素の分圧から推定される。しかしながら、PaO2が直接測定が可能であれば、これらの推定は、Cbへの入力としてもはや必要でない。本実施例は、PaO2およびCbが、被検者の大動脈領域から走査したOE−MRIデータから直接測定可能であることの実証を提示する。本実施例から得られるデータは、実施例2のコンパートメントモデルのCbパラメータへの入力として用いられる推定PaO2データと容易に置換できることは理解されよう。さらに、PO2を測定する本例示の方法は、特に大動脈からのPO2測定での誤差を修正する観点から、実施例2の推定と上手く組み合わせが可能であることは理解されよう。
24人の被検者が、1.5テスラのダイナミックOE−MRIを受診した。14人のボランティア(7人の喫煙者(S)と7人の非喫煙者(NS))が、視野内に大動脈を有し、更なる解析のために選択した。全被検者からインフォームド・コンセントを得た。図8に示すように、44.5cm×44.5cmの視野で、15mm厚の冠状スライスを後方に位置決めした。この体積は、反転回復ターボ磁界エコーシーケンス(TR/TE 2.2/1.0ms、フリップ角5°、取得マトリクス128×256ゼロ充填256×256)を用いて画像化して、初期の非選択反転パルス(25回の反転回数を使用。143msの間隔で最短74ms)から反転を通じて画像を取得し、T1の測定を行った。取得は、18分間連続的に繰り返し、6秒の時間分解能でT1の測定を行った。ボランティアは、医療空気をハドソン(Hudson)マスクを介して最初3分間呼吸し、そして、マスクへの供給を100%酸素に切り替えた。更に9分間後、画像取得の残りのために、供給を空気に戻した。ガスは15L/分で供給した。
関心のある領域、即ち、大動脈を示す画像エリアは、42±20画素の平均(mean)を含む。平均ベースライン値および平坦域T1値は、Sに関して1300±200msと1200±100msであり、NSに関して1300±200msと1100±100msであった。図10Aと図10Bは、2つのグループでの各被験者について時間の関数としてプロットしたΔPO2を示すもので、5点移動平均を用いて平滑化している。太いラインは、全ボランティアについて平均時間コースを示す(平滑化なし)。平均の平坦域値は、Sに関して350±90mmHg、NSに関して430±40mmHg(p=0.049)であった。
少数のボランティアでは、平坦域ΔPO2値は、2つのグループ間で際どく有意な差を示し、Sは、NSよりも低い平坦域ΔPO2値を示した。Sにおける平坦域ΔPO2値の標準偏差は、NSの2倍であった。空気および100%酸素の呼吸時に、正常なボランティアにおけるPO2動脈血液ガス測定値に関する文献値は、予想ΔPO2は490±20mmHgであることを示唆しており(文献:Floyd TF, Clark JM, et al. J Appl Physiol 2003;95:2453-2461)、これは我々の所見と一致する。動脈血液ガスサンプリングとの直接比較は、測定を正当化するために有利であり、より多くの被検者についての研究は、SとNSと間の差に関して強力な結論を引き出すことが可能であろう。
Claims (25)
- 特徴付けが必要とされる被検者の組織機能を特徴付ける方法であって、
関心のある組織空間内で定義されるボクセルに対して画像化技術を実施することを含み、被検者が少なくとも2つの異なる分圧の常磁性ガスを含むガスを吸入する期間中に画像データが発生し、
さらに、コンパートメントモデル・アルゴリズムを、ボクセルで発生した画像データに適用して、組織の代謝機能に関する情報を提供することを含む方法。 - 画像化技術は、酸素造影磁気共鳴映像法(OE−MRI)であり、常磁性ガスは酸素である請求項1記載の方法。
- 組織は、マトリクス状のボクセルに分割され、OE−MRIデータは各ボクセルについて生成される請求項1または2記載の方法。
- 被検者が、最初に0%〜35%の酸素分圧を持つ第1ガスを吸入し、続いて45%〜100%の酸素分圧を持つ第2ガスを呼吸し、最後に第1ガスを再吸入するときに、OE−MRIデータが生成される。請求項2または3記載の方法。
- 第1ガスは空気であり、第2ガスは100%酸素である請求項4記載の方法。
- 組織は、腫瘍を含む請求項1〜5のいずれかに記載の方法。
- 画像化技術が時間とともに同じボクセルについて実施されることを確保するための画像レジストレーションを改善する手法が適用される請求項1〜6のいずれかに記載の方法。
- コンパートメントモデル・アルゴリズムは、運搬、拡散および代謝のレートについての生理学的パラメータをベースとした2−コンパートメントモデルである請求項1〜7のいずれかに記載の方法。
- コンパートメントモデル・アルゴリズムは、細胞および間質を含む第2コンパートメント(Ce)の複合酸素濃度を計算するようにした請求項8記載の方法。
- コンパートメントモデル・アルゴリズムは、MRI可視物質当たりの血液の分数体積(Vb)を計算するようにした請求項8または9記載の方法。
- コンパートメントモデル・アルゴリズムは、血管系の拡散能力(Kox)を計算するようにした請求項8〜10のいずれかに記載の方法。
- コンパートメントモデル・アルゴリズムは、代謝消費レート(Mox)を計算するようにした請求項8〜11のいずれかに記載の方法。
- コンパートメントモデル・アルゴリズムは、血液中の酸素濃度(Cb)を示す項を組み込むようにした請求項8〜12のいずれかに記載の方法。
- 血液中の酸素濃度(Cb)は、呼吸した酸素濃度PIO2から計算される請求項13記載の方法。
- Cbは、血液について推定したウォッシュイン時間またはウォッシュアウト時間(TOIF)に関して計算される請求項13記載の方法。
- Cbは、血液中の酸素濃度の測定値から決定される請求項13記載の方法。
- 血液中の酸素濃度の測定値は、OE−MRIデータから測定される請求項16記載の方法。
- OE−MRIデータは、大血管を含む、被検者のある領域から採られる請求項17記載の方法。
- アルゴリズムは、Ketyによって開発され、(文献:Kety, SS (1951) Pharmacological Reviews. 3: 1-41)に公開された方程式をベースとしている請求項8〜18のいずれかに記載の方法。

前記プロセッサ読み取り可能な命令は、前記プロセッサを制御し、請求項8〜21のいずれかに記載のアルゴリズムを、請求項1〜7のいずれかに記載の画像データに適用する命令を含むようにした装置。 - 請求項8〜21のいずれかに記載のアルゴリズムを、請求項1〜7のいずれかに記載の画像データに適用する方法をコンピュータに実行させるように構成された、コンピュータ読み取り可能なプログラムコードを搭載したキャリア媒体。
