Abstract
Description
発明についての論考を簡明化するために、以下の定義を提供する。従ってそれらは発明を限定するものとして解釈されるべきでなく、発明の範囲は付随の請求項および下記の詳細な説明によって規定される。
心血管疾患は、世界的に、第一の死亡原因である。アテローム性動脈硬化は心血管疾患の主要な原因であり、アテローム性動脈硬化発生を助長する生活スタイルが全世界に拡がっているため、心血管疾患は将来においても第一の死亡原因であり続けるであろうと予測される。従って、心血管疾患に苦しむ個体に最善の治療を提供することは、その個体にとってだけではなく、さらに多数のそういった患者を処置することになるであろう医療機関にとっても、関心の対象である。
YKL-40のレベルの統計学的な増加は、生存に関する指標であり、ここで記述されるように、心臓疾患を患う個体の分類および/または監視において使用し得る。ある個体においてYKL-40レベルが上昇したかどうかは、測定された値を参照レベルに相関させることにより断定し得る。参照レベルの計算の基礎を形成する個体群は、様々な年齢の健康な個体の群であってもよいし、特定の年齢の群であってもよい。健康な個体とは、サンプリングの時点で心臓関連の疾患または障害を持つと診断されていない個体である。
本発明は、YKL-40レベルから測定される生存の予後に基いて個体を監視することに関連する。測定されたYKL-40レベルに基いて個体を監視することは、個体の一般的な健康状態の指標として、および/または施された治療の有効性の指標として、使用し得る。当該個体または患者は、心血管疾患または心血管障害など、疾患または障害を患っている場合があり得る。具体的には、当該個体または患者は冠動脈疾患を患っている場合があり得る。
心血管疾患という用語は、心臓または血管(動脈および静脈)に関わる疾患の種類を指す。この用語は、心血管系に影響を与えるあらゆる疾患、および心血管疾患の結果として生ずる事項を指し、本明細書においてそのように用いられるが、アテローム性動脈硬化に関して使用されることが一般的である。多くの種類の心血管疾患が存在し、それは以下のものを含む:急性冠症候群、急性心筋梗塞(AMI/STEMI/ST上昇)、心筋梗塞(心臓発作)、不安定狭心症/UAP/非ST上昇心筋梗塞、動脈瘤、狭心症、アテローム性動脈硬化、冠動脈疾患(CAD)、虚血性心疾患、虚血心筋、不整脈、心房細動、心不整脈、心室頻拍、心室細動、心臓死および心臓突然死、心筋症、うっ血性心不全、心不全、弛緩性および収縮期性心室機能不全、拡張型心筋症、血圧上昇(高血圧)、肥大型心筋症、心弁膜疾患、僧帽弁逸脱、僧帽弁逆流および/または狭窄、大動脈弁逆流および/または狭窄、心筋炎ならびに静脈血栓塞栓症。これらの全てが本発明に関係する。
アテローム性動脈硬化は動脈血管に影響を及ぼす疾患である。それは動脈の壁における慢性的な炎症反応であり、リポタンパク質の堆積によるところが大きい。それは動脈中に複数のプラークが形成することにより引き起こされる。アテローム性動脈硬化は二つの主だった問題を引き起こす。第一に、アテローム性のプラークは、動脈拡大により長期間代償されるものの、やがてはプラーク破綻および動脈の狭窄を起こすに至り、それによって、当該動脈とつながる器官への血液供給が不十分となる。代償性の動脈拡大が過剰ならば、動脈瘤という結果をもたらす。これらの合併症は慢性的であり、ゆっくりと進行し、累積的である。最も一般的には、柔らかいプラークが突然破綻し、血栓の形成を引き起こし、それが急速に(5分以内に)血流の遅速化または停止をもたらし、その動脈に血液を供給されていた組織が死ぬことに至る。この破滅的な現象が梗塞である。最も一般的に認識されるシナリオの一つは、冠動脈の冠動脈血栓症であり、これは心筋梗塞(心臓発作)を引き起こす。非常に進行した疾患におけるもう一つの一般的なシナリオは、脚への血液供給が不十分になることによる跛行であり、典型的にはこれは、狭窄症および血栓により狭まった動脈瘤セグメントの両方の組み合わせによる。アテローム性動脈硬化は全身に及ぶプロセスであるため、同様の現象は、脳、腸管、腎臓、脚などにつながる動脈においても起こる。
冠動脈疾患(CAD)は、西洋世界において最も一般的な形態の心臓疾患である。冠動脈疾患は、アテローム硬化性冠動脈疾患、冠動脈性心臓疾患(HAD)、アテローム硬化性冠動脈心疾患、アテローム硬化性心血管疾患、虚血性心臓疾患、およびアテローム硬化性心臓疾患とも呼ばれ、心筋に酸素と栄養を供給する動脈の壁にアテローム性のプラークが蓄積することの最終結果である。冠動脈性心臓疾患の症状および徴候は進行した状態の疾患においては認識されるが、冠動脈性心疾患の個体のほとんどは、疾患が進行して最初の症状(「突然」心発作であることがしばしばである)がついに現れるまでの数十年間、疾患の証拠となるものを示さない。何十年にも渡って進行した末に、これらのアテローム性プラークのあるものが破綻し、(血液凝固系の活性化と相まって)心筋への血流を制限し始めることになり得る。
急性冠症候群(ACS)は、徴候と症状の一群(通常は胸痛およびその他の組み合わせ)であり、心臓への血流が急激に減少すること(心虚血)の結果だと解釈される。これを起こす最も一般的な原因は、心外膜の冠動脈におけるアテローム硬化性プラークの破損である。急性冠症候群は、多くの場合、アテローム性動脈硬化によって冠状動脈にある程度の損傷があることを反映する。急性冠症候群のサブタイプには、不安定狭心症(UA、心筋の損傷を伴わない)、および、心筋の損傷を伴う、二つの形態の心筋梗塞(心臓発作)が含まれる。これらのタイプは、心電図(ECG/EKG)の表れ方に従って、非ST部分上昇心筋梗塞(NSTEMI)およびST部分上昇心筋梗塞(STEMI)と名づけられる。
虚血性心筋症(虚血性心臓疾患(IHD)としても知られ、虚血性心筋疾患、虚血性冠動脈疾患、虚血性心不全、虚血性心臓疾患、虚血性心不整脈、および虚血心筋と関連する)は、心筋への酸素輸送が不十分なことによる心臓の筋肉の虚弱化であり、冠動脈疾患(冠動脈のアテローム性動脈硬化)が最も一般的な原因である。貧血症および睡眠時無呼吸は虚血心筋に貢献し得る比較的一般的な状態であり、甲状腺機能亢進症は高拍出性心不全に伴う二次的現象である「相対的」虚血を引き起こし得る。虚血性心筋症の個体は典型的には心筋梗塞(心臓発作)の経歴を持つが、長期に渡る虚血は心筋に十分な損傷を与え、心筋梗塞が不在であっても臨床的に有意な心筋症を誘発し得る。典型的な症状例では、心筋梗塞の影響を受けた心臓の領域が、初めは壊死性となりながら死に、その後瘢痕組織に置き換わる(線維症)。この線維性の組織は無動である。それはもはや筋肉ではなく、心臓のポンプとしての機能に貢献することができない。もし心臓の無動領域が十分に大きければ、心臓のうち影響を受けた側(即ち、左側または右側)は心不全に陥り、この心不全が虚血性心筋症の機能上の帰着点である。安定虚血性心臓疾患の症状には狭心症、および許容可能な運動量の減少が含まれる。不安定IHDは、安静時における胸痛その他の症状、または急速に悪化する狭心症として現れる。IHDの診断は、心電図、血液検査(心臓性のマーカー)、心臓負荷試験、または冠血管造影図による。治療は、症状とリスクとによって、薬物を用いたもの、経皮的冠動脈形成術(血管形成術)または冠動脈バイパス手術(CABG)であり得る。多くの疾患が心筋症に帰着し得る。これらは、ヘモクロマトーシス、アミロイドーシス、糖尿病、甲状腺機能亢進症、リソソーム蓄積症、および筋ジストロフィーなどのような疾患を含む。
うっ血性心不全(Congestive heart failure) (CHF)、うっ血性心不全(congestive cardiac failure) (CCF)または単に心不全と呼ばれるものは、身体に十分な量の血液を満たす又は送り出すという心臓の能力を損なうような、あらゆる構造的または機能的な心臓障害により引き起こされ得る状態である。それは、心静止(asystole)として知られる「心拍の休止」と混同すべきではなく、また、正常な心機能の休止に続いて血行動態の崩壊ひいては死に至るところのものである心停止(cardiac arrest)とも混同すべきではない。最初の又はそれに続く評価において、全ての個体が容量過負荷(volume overload)を有するわけではないので、より古い用語である「うっ血性心不全」よりも「心不全」という用語の方が好ましい。心不全は、広く受け入れられた定義の欠如、および診断の難しさのため、診断されないことがしばしばあり、特に状態が「軽度」とみなされる場合にそのことが当てはまる。
心停止は、心呼吸停止、心肺停止、または循環停止としても知られ、収縮期において心臓が効果的に収縮できないことによる、正常な血液循環の急激な休止である。「停止した」血液循環は、身体の全ての部分への酸素の輸送を妨げる。低酸素脳症、または脳への酸素供給の欠乏は、意識の消失および正常な呼吸の停止を引き起こす(しかし死戦期呼吸はそれでもまだ起こり得る)。心停止が5分間以上無処置であると、脳損傷が起こる可能性が高いが、低体温療法のような新しい治療法によりこの時間は伸びつつある。生存率および神経学的回復率を改善させるためには、即時に対処することが最重要である。心停止は、個体によっては十分に早く処置さえすれば持ち返すことができる可能性のある、医学的な緊急事態である。予期せぬ心停止が死に至った場合、これは心臓突然死と(SCD)と呼ばれ、あるいは冠状動脈突然死と名づけられる。心停止に対する主要な救急療法は、最終的な医学的処置が可能となるまでの間循環の援助を提供する心肺蘇生法(一般的にはCPRとして知られる)であり、これは心臓が示しているリズムによって変わるが、しばしば除細動を必要とする。
心不整脈は、心臓の電気的な活動が不規則であるか又は正常な状態よりも速く若しくは遅くなる、一群の状態のいずれかである。不整脈の内のあるものは生命を脅かす医学的緊急事態であり、心停止と突然死を引き起こし得る。他のものは、例えば異なった心拍動が自覚されること、すなわち動悸、のような不快な症状を引き起こし、患者を悩まし得る。あるものは至極軽症であり、正常とみなされることもあり得る。本発明に関係する一般的な心不整脈のリストは以下のものを含むがこれらに限定されない:心房調律、心房性期外収縮(PACs)、移動性心房ペースメーカー、多源性心房頻脈、上室性頻拍(SVT)、心房粗動、心房細動(Afib)、心室調律 、心室性期外収縮(PVC)、頻拍性心室調律、心室頻拍 (VT)、心室細動(VF)、多形性心室性頻拍、心室の過剰拍動、心房性心室性不整脈、房室結節性リエントリ性頻拍、房室回帰性頻脈、ウォルフ・パーキンソン・ホワイト症候群、ラウン・ギャノン・レバイン症候群、接合部不整脈、接合部調律、接合部頻拍、早発性接合部複合体(premature junctional complex)、心ブロック(AVブロックとしても知られる)、第1度心ブロック(PR延長としても知られる)、第2度心ブロック、タイプ1第2度心ブロック(モービッツI型またはヴェンケバッハ型としても知られる)、タイプ2第2度心ブロック(モービッツII型としても知られる)、第3度心ブロック(完全心ブロックとしても知られる)、および、三叉神経調律のような比較的稀な不整脈。
心血管疾患に罹患する危険性は、年齢、喫煙、高コレステロール血症(コレステロールのレベルが高いこと)、糖尿病および高血圧症(血圧が高いこと)に伴って増加する。上記のいずれかの指標を有する個体を、当該個体から採られた生体試料から測定されたYKL-40レベルに基いて監視および分類する手段を提供することは、本発明の一つの目的である。参照レベルは、本明細書、特に「参照レベル」という節に記載されたようないずれかのYKL-40レベルであり得る。さらに、YKL-40は長寿命の指標のバイオマーカーとして使用し得る。血清YKL-40のレベルがより低いほど、生存の予後がより良いこととなる。
YKL-40は、そのN末端の3アミノ酸であるチロシン(Y)、リジン(K)およびロイシン(L)、ならびに分子量が約40 kDaであることに基いてそう名付けられている(Johansen et al. 1992)。ヒトYKL-40の完全長のアミノ酸配列(SEQ ID NO: 2)およびそのコード配列(SEQ ID NO: 1)は、GenBankにおいて受入番号M80927として見出される。ヒトYKL-40は383アミノ酸長の単一のポリペプチド鎖を含み、系統学的に高度に保存されたヘパリン結合およびキチン結合血漿糖タンパク質の一つである。ヒトYKL-40と、いくつかの他の哺乳類からのホモログとの間の配列同一性は以下の通りである:ブタ(84%配列同一性)、ウシ(83%)、ヤギ(83%)、ヒツジ(83%)、モルモット、ラット(80%)およびマウス(73%)。YKL-40は「哺乳類キチナーゼ様タンパク質」の一員であるが、キチナーゼ活性は有しない。インビトロのYKL-40発現は、正常なヒト単球においては不在であるが、マクロファージ分化の後期段階において活性化された単球および好中球により強く誘導され、血管平滑筋細胞、癌細胞および関節炎の軟骨細胞によって強く誘導される。インビボにおいては、YKL-40のmRNAおよびタンパク質は、アテローム硬化性プラークのような炎症を有する組織におけるマクロファージの亜集団、巨細胞関節炎(giant cell arthritis)を有する個体の関節炎性血管(arthritic vessels)、炎症を有する滑膜、サルコイド病変(sarcoid lesions)、および腫瘍周囲のマクロファージによって発現される。
本発明は、生体試料中のYKL-40レベルを測定することおよびそこで見出される値を一つ以上の参照レベルと比較することに基く生存の予後に基いて個体を分類および/または監視する方法を開示する。当該個体は健康な個体であってもよいし、上述のいずれかの心臓疾患を患う個体であってもよい。本発明に特に関連が深いのは、アテローム性動脈硬化により引き起こされる心臓疾患または障害である。
本発明のペプチドおよびポリヌクレオチドは、YKL-40の機能的誘導体(functional derivatives)、YKL-40ペプチド、およびそれらをコードするヌクレオチドを含む。「機能的誘導体」は、ある分子の「断片」、「バリアント」、「類似体」、または「化学的誘導体」を意味する。分子、例えば本発明のいずれかのDNA配列の「断片」は、その分子のあらゆるヌクレオチド・サブセットを含む。そのような分子の「バリアント」とは、当該分子の全体または断片に対して実質的に類似性を有する、自然に起こる分子のことを指す。分子の「類似体」とは、当該分子の全体または断片に対して実質的に類似性を有する、非天然分子のことを指す。
イムノアッセイの手順は、疾患を有する個体のYKL-40のレベルを、健康なヒトに見られ得る正常レベルおよび/または当該個体において測定されるバックグラウンドレベルから区別し得るように定量的でなければならない。検出可能な標識(直接的または間接的)を使用した固相上の競合およびサンドイッチ・アッセイが従って好ましい。当該標識が、抗体によるYKL-40抗原への結合を示す検出可能なシグナルを提供する。抗体または抗原は、検出可能なシグナルを提供するために当該技術分野で知られるあらゆる標識により標識化し得、それらは放射性同位体、酵素、蛍光分子、化学発光分子、生物発光分子および金コロイドを含む。既知のアッセイ手順の中では、ラジオイムノアッセイ(RIA)がその感度から最も好ましい。従って、放射性同位体は好ましい標識である。
さらに、生体試料中のYKL-40の存在を検出し定量化するためにイムノアッセイ以外の手段を利用し得ることは、当業者には理解されることである。例えば、YKL-40をコードするポリヌクレオチドを、当技術分野で知られる定量的ポリメラーゼ連鎖反応(PCR)プロトコールを使用して検出し得る。定量的PCRを行う上で好ましい方法は、一つ以上の塩基対の突然変異が導入されており標的YKL-40遺伝子テンプレートとは配列またはサイズが異なるようにしてある競合的テンプレートを使用して実行される、競合的PCR技術である。プライマーの内の一つはビオチン化されるか、または好ましくはアミノ化され、それにより、結果として生じるPCR産物の一方の鎖(普通はアンチセンス鎖)を、適切な反応物(reactant)が堅固に結合した固相支持体に、アミノ-カルボキシル、アミノ-アミノ、ビオチン-ストレプトアビジン、またはその他の適度に強固な結合を介して固定化することができる。最も好ましくは、当該PCR産物、固相支持体および反応物間の結合は共有結合であり、それにより結合は変性条件下でも確実に切断に耐えられることとなる。
YKL-40は加水分解性の酵素であると見られ、従って、YKL-40の基質が検出可能な形態に加水分解される酵素アッセイに基いて、機能的なYKL-40タンパク質またはタンパク質断片のレベルを測定し得る。
YKL-40を検出する一つの特定の方法は、迅速で定性的および/または定量的な試験の系が固形支持体上に備え付けられた、生体試料におけるYKL-40レベルを決定するための器具(device:機器、装置、器具、用具)に関わる。
YKL-40は、全死亡および心臓死についての独立したバイオマーカーであり、そのように使用することができる。しかしながら、YKL-40は、例えばC反応性タンパク質(CRP)、脳性ナトリウム利尿タンパク質(BNP)、インターロイキン、腫瘍壊死因子-アルファ、ホモシステイン、アミロイドAタンパク質、妊娠関連血漿タンパク質A、トロポニン、可溶性細胞間接着分子-1、可溶性UPAR、III型プロコラーゲンのアミノ末端プロペプチド(P-III-NP)、単球走化性タンパク質-1、フィブリンD-ダイマー、増殖分化因子15、虚血変性アルブミン、リポタンパク質関連ホスホリパーゼA2、マトリックスメタロプロテイナーゼ、ペントラキシン3、分泌型ホスホリパーゼA2グループIIA、細胞間接着分子-1、心臓型脂肪酸結合タンパク質(H-FABP)、ミオシン軽鎖-1 (MLC-1)、P-セレクチン、およびCKMBのような、他の既知のバイオマーカーと組み合わせて使用することもできる。前述のバイオマーカーの内、例えばUPARと可溶性UPAR、細胞間接着分子-1と可溶性細胞間接着分子-1、等のように、可溶性および不溶性の形態のタンパク質は両方とも本発明に関係する。上述のマーカーはいずれも、そのレベルを、例えば血液、血清、血漿、または組織試料などの生体試料において測定することができ、また、例えばイムノアッセイやPCRに基くアッセイまたは複数の種類のアッセイの組み合わせなどのような、あらゆる利用可能な手段によって測定し得る。
生体試料は、個体から採られる試料である。従って、生体試料は、以下のものから成る群から選択される試料であり得る:組織、骨、血液、血清、および血漿試料。本発明に特に関連があるのは、血液、血清および血漿の試料である。特定の疾患もしくは障害、または一般的な健康状態(いずれにおいてもYKL-40レベルの増加が生存についての予後となる)を分類または監視することに使用する上で、どのアッセイ試料源が最も適切かは、当業者ならば容易に決定することができる。
ここで言う個体とは、ある生物種の一個の構成員のことを指し、ここでは哺乳類種であることが好ましい。あらゆる哺乳類種が本発明の対象であるが、以下の種が特に重要である:マウス、ラット、モルモット、ハムスター、ウサギ、ネコ、イヌ、ブタ、ウシ、ウマ、ヒツジ、サル、およびヒト。本発明における個体は、最も好ましくはヒトである。本明細書において、個体は患者(patients)または被験者(subjects)とも呼ばれ得る。
本発明のもう一つの実施態様は、キットを含み、ここで当該キットは、少なくとも、個体から採られた生体試料中のYKL-40のレベルを評価するための要素と、そのやり方についての説明書とを含む。前記の要素は、例えばイムノアッセイのような、YKL-40レベルを検出する方法、または特異的にYKL-40検出をするイムノアッセイを行うために必要な複数の構成部分であり得る。当該キットは任意で、生体試料のYKL-40を検出しそのレベルを測定するためのPCRベースのアッセイを行うための要素を、追加としてまたは替わりとして、含み得る。当該キットはさらに、例えば注射器、バイアルその他のような、生体試料を採るための機器を含み得る。当該キットは、一回使用または繰り返し使用のために包装され得るし、その構成要素は、一度使用した後に捨てられるようなディスポーザブルのものであってもよいし、または、繰り返し使用できるような品質のものであってもよい。
2.6年の追跡調査期間における、心筋梗塞(MI)、心血管疾患性の死、および全ての原因の死までの時間についての、多変量モデルにおける3つのコックス分析から得られた、6つの血清YKL-40グループそれぞれについての、95% 信頼限界を伴ったハザード比(HRs)。介入指標(クラリスロマイシンまたはプラシーボ)および他の危険因子(性別、急性心筋梗塞の経歴、年齢(<60; ≧60歳)、喫煙状況、高血圧、および糖尿病)が共変量として含まれた。P値: * 0.01<P<0.05; ** 0.0005≦P<0.01;および ***P<0.0005。グループI<50%パーセンタイル: YKL-40<110μg/l; II 50%≦YKL-40<60%パーセンタイル: 110μg/l≦YKL-40<129μg/l; III 60%≦YKL-40<70%パーセンタイル: 129μg/l≦YKL-40<153μg/l; IV 70%≦YKL-40<80%パーセンタイル: 153μg/l≦YKL-40<19lμg/l; V 80%≦YKL-40<90%パーセンタイル: 191μg/l≦YKL-40<256μg/l; およびVI≧90%パーセンタイル YKL-40: 256μg/l≦YKL-40。

調べられたイベントは:図5A) 不安定狭心症、図5B) MI、図5C) 心血管疾患性の死、および図5D) 全ての原因の死。
患者
患者は、以前に論文発表された、一日一回500 mgの経口的クラリスロマイシン(Klacid Uno(登録商標)、英国アボット)または対応するプラシーボで2週間処置された安定冠動脈疾患の患者についての、無作為化され、プラシーボのコントロールを伴い、複数のセンターが関与した(multicentre)、CLARICOR治験に含まれていた(Jespersen et al., 2006)。当該患者(18歳から85歳まで)は1993年から1999年までに心筋梗塞または狭心症(ICDコード 209-219)の診断を受け、1999年8月の時点で生存していた。4373人の患者が当該調査への参加条件を満たし、1999年10月5日から2000年4月15日の間に無作為化された。以前記述されたように(Jespersen et al., 2006)、心筋梗塞、狭心症、経皮的冠動脈形成術、または冠血管バイパス手術の経験があれば、参加条件を満たすとされた。4350人の患者が血液を提供し、4298人の患者についてはYKL-40を測定するための血清が得られた。無作為化の際に、患者は、過去の心筋梗塞、狭心症、経皮的冠動脈形成術、冠血管バイパス手術、動脈性高血圧、糖尿病、喫煙、および医学的治療についての情報を電子的記録に入力した。
追跡調査のための患者の来院は計画しなかった。死亡についての情報は、住民の生存状態について記録するデンマーク中央住民登録簿から得た。致死的(fatal)および非致死的来院についての情報は、病院へのすべての身体の受け入れ(somatic hospital admissions)のデータベースであるデンマーク国立病院登録簿から得た。これらの登録簿における登録率は100%である。これらの登録簿に基き、コーディネイト・センターが死亡証明書および病院記録の写しを収集し、追跡期間の最初の2.6年間は関連の可能性があるイベントをそれぞれ別個にイベント委員会に転送し、追跡期間の残りの6年間においては収集された全ての死亡証明書を転送した(別の文書に記載された通りである(Jespersen et al., 2006, Gluud et al., 2007))。
バイオマーカーについての本調査においては、無作為化の時点から再来院するまでの以下のことについて調べた: I. 死亡、心血管疾患性の死、非致死的MI、不安定狭心症、II. 非致死的MIまたは不安定狭心症、および、III. 非致死的MI、不安定狭心症、または死亡までの時間。心筋梗塞の診断をするためには、心筋酵素(クレアチンキナーゼ-アイソザイムMBまたはトロポニン)の増加、および、心電図における、心筋虚血または心筋梗塞を示す有意なSTの変化が必要条件とされた。酵素の大きな変化を伴わない、長期間続く胸痛または安静時の胸痛は、不安定狭心症として分類した。
本調査は、地方倫理委員会(KF 01-076/99)、デンマーク医学庁(2612-975)、およびデンマーク・データ保護庁(199-1200-174)の認可を受け、ヘルシンキ宣言に従って行われた。参加者は文書によるインフォームド・コンセントを提出した。
YKL-40の血清中濃度は、ストレプトアビジンでコーティングされたマイクロプレート・ウェル、ビオチン化されたFabモノクローナル・キャプチャー抗体、およびアルカリホスファターゼ標識ポリクローナル検出抗体を使用して、市販の2部位(two-site)サンドイッチ式酵素結合免疫吸着測定法アッセイ(ELISA) (Quidel社、カリフォルニア州サンディエゴ) (Harvey 1998)によって、繰り返しを伴って測定された。ELISAの収率(recovery)は102%である(Harvey et al.,1998、および個人的な観察)。検出限界は20μg/Lである(Harvey et al, 1998)。アッセイ内CVsは5.0% (平均YKL-40濃度40μg/L, n=40)、3.9% (平均104μg/L, n=40)および3.8% (平均155μg/L, n=40)である。アッセイ間CVsは5.3% (平均42μg/L, n=277)および6.3% (平均151μg/L, n=277(個人的な観察))である。
イベントまでの時間の分布は、カプラン-マイヤーの方法により計算し、ブレズロウの検定を使用して有意差について比較した。イベントまでの時間に対するの一つ以上の共変量の影響を分析するために、コックス分析を使用した。変数として、共変量、時間、および時間と共変量との相互作用を含む拡張されたコックス分析を、コックス・モデルの比例性の仮定(proportionality assumption)をテストするために使用した。後者のテストは、共変量の群のlog-log図の目視による評価により補完した。血清YKL-40の分布およびlog (YKL-40)の分布は、シャピロ・ウィルクW検定による評価ではガウス分布とは有意に異なったため、患者群間の血清YKL-40レベルの比較には、ノンパラメトリック検定(2つの群についてはマン・ホイットニー検定および2つを超える群についてはクラスカル・ウォリス検定)を使用した。当該分析はSPSSバージョン15.0を用いて行った。
コックス・モデルの、血清YKL-40とハザード比(HR)の対数との間の直線関係の前提は、log HRと血清YKL-40パーセンタイル・グループの血清YKL-40平均値との間の関係を調べることにより評価した。第1のグループを、HRについての標準として用いた。
変換されたYKL-40が、あるイベントまでの時間に対して有意に影響を有していた場合、既知の危険因子(年齢、性別、過去のMI、喫煙状況、高血圧、糖尿病)について調整して分析を繰り返した。当該影響がもしそれでも有意であったならば、選択された治療の指標を分析に含めた。これらの因子は次のようにして選択された:患者の半分を含む無作為サンプルを選定し、共変量として危険因子および医学的治療の全ての指標(ベータ遮断薬、Ace阻害薬、カルシウム拮抗薬、スタチン、マグニル(magnyl)、長時間持続硝酸薬、ジゴキシン、利尿薬(diuretica)、抗不整脈(anti-arrhytmica))を含む、対応データ素材の分析を行った。当該分析は、p=0.10で変数を除去する尤度比検定を用いた変数減少法(backward elimination)を含んだが、治療指標以外の全ての共変量が分析に残るように強制した。データ素材の残りの半分を使用して当該分析を繰り返した。選択された治療指標は、これら2つの分析の両方において保持されたものである。
被験者の人口統計学的および臨床的特徴と関連した血清YKL-40。
調査に含まれた安定冠動脈疾患を有する患者の人口統計学的データを図1に記す。データ入力時における平均年齢は65歳(30歳から85歳の範囲)であった。参加した4298人の患者の内、31%が女性であり、68 %が過去に急性心筋梗塞を経験していた。当該4298人の患者における血清YKL-40の中央値は110μg/l (20 から3047μg/lの範囲)であった。クラリスロマイシン処置の患者とプラシーボ処置の患者との間には血清YKL-40の差は見出されなかった。以前にMIを経験していた患者と経験していなかった患者との間では血清YKL-40に有意な差は無かった(中央値111μg/l、20から3047μg/lの範囲、に対して、106μg/l、20から2802μg/lの範囲)。
2.6年間の追跡期間において、330人の患者(7.6%)において新たな心臓性のイベントが起こり、それには115人の患者における不安定AP (2.6%)および219人におけるMI (5.0%)が含まれていた。
この調査は、安定CADを有する患者において、高い血清中YKL-40濃度が、MI、心血管疾患性の死、および全ての原因の死についての有意な予測因子であることを実証した。YKL-40レベルが最も高い患者においては、3倍を超えて増加したハザード比が見られた。これに加えて、高い血清YKL-40は、年齢の増加、高血圧、および糖尿病と連関していたが、性別、過去のMI、またはデータ入力時における喫煙状況とは連関していなかった。これらの変数に関して分析を補正することは、MI、心血管疾患性の死、および全ての原因の死についてのHRに対して大きなインパクトを与えなかった。
