Abstract
Description
本発明の実施の形態は、複合織移植片を有する埋め込み可能な移植装置を提供する。一実施の形態では、移植装置は、送達時に外形を小さくし、長手方向の軸に対して径方向に伸長するために形成された、小径織糸と弾性織糸との複合材料を備える、移植片本体を備える。
移植片は、任意の種類の好適な織り方からなってもよい。たとえば、移植片本体は、これに限られるものではないが、平織り、改良された平織り、バスケット織り、リップス織りまたは畝織り、綾織り(たとえばストレートツイル、リバースツイル、ヘリンボーンツイル)、改良された綾織り、朱子織り、二重織り(たとえば二重幅の管状二重織り、逆二重織り)、およびその他の関連する織り方などを含んでもよい。一実施の形態では、移植片本体は、1インチ(2.54cm)あたり150本のエンドと1インチ(2.54cm)あたり250本のピックとを有する平織りからなる。「エンド」とは一本の縦糸をいい、「セット」とは織布における1インチあたりの縦糸の数をいう。「ピック」とは一本の横糸をいい、「ピック数」とは織布における1インチ(2.54cm)あたりの横糸の数である。
移植片の材料は、織るために好適な任意の生体適合性材料からなってもよい。移植片の材料は、天然の、合成の、または加工されたものであってもよい。たとえば、生体適合性材料は、これに限られるものではないが、ポリ(エチテンテレフタレート)などのポリエステル、ポリテトラフロオロエチレン(PTFE)および延伸PTFEの繊維などのフッ化重合体、ならびにポリウレタンを含む。加えて、本来的には生体適合性でない材料は、この材料を生体適合性にするために、表面改質されてもよい。表面改質の例は、材料表面からの生体適合性重合体のグラフト重合、架橋生体適合性重合体での表面の被覆、生体適合性官能基での化学修飾、およびヘパリンまたは他の物質などの相溶化剤の固定化を含む。よって、最終の織物が生体適合性であるならば、移植片の本体を形成するために任意の繊維材料が使用されてもよい。
一以上のステントが、これらに限られるものではないが溶接、縫い合わせ、ボンディングおよび接着剤を含む、当該技術分野における当業者に公知の任意の方法によって、移植片本体に取り付けられまたは接着されてもよい。一実施の形態では、ステントが移植片本体に縫合されてもよい。概して、本発明の実施の形態に従った使用のためのステントは、典型的には、金属の(繊維または金属線を含む)細糸の間、区分の間または領域の間に、複数の隙間または開空間を備える。典型的な構造は、一つ以上の編まれた、織られたまたは編み組まれた金属製の細糸を備える開かれた網状の組織、接合可能な区分の相互に繋がった組織、一以上の金属製の細糸を備えるコイル状または螺旋状の構造、および、模様の付いた管状の金属製の薄板(たとえばレーザカットチューブ)を含む。
移植片本体は、特定の解剖学的分枝導管(たとえば、頚動脈、無名動脈、鎖骨下動脈、肋間動脈、上腸間膜動脈、腹腔動脈、腎動脈、腸骨動脈、内腸骨動脈、または内蔵の管)を有する大動脈の内部における正確な調整を容易にするために、X線造影用の標識が付けられてもよい。移植片本体の放射性不透過性の部分は、X線、超音波、核磁気共鳴画像法などを含む遠隔撮像法によって、または標識からのもしくは標識に対応する信号の検出によって、観察される。
本発明に従った補綴物は、たとえば固着手段、縫合、ホッチキス留め、焼き付け、ボンディング、糊付け、生体接着剤、もしくはその他の医療装置の管壁への接着、またはこれらの組合せなどの、補助的な取り付け手段を任意に含んでもよい。たとえば、移植片本体は、一つ以上の固着手段を用いて所定の位置に縫合されてもよい。
任意には、移植片本体は少なくとも一つの生体活性剤を含んでもよい。生体活性剤は、補綴物の任意の好適な部分に含まれてもよい。生体活性材料は、任意の好適な方法で補綴物に取り付けられてもよい。たとえば、生体活性剤が移植片本体の材料に噴霧されてもよく、ステントが生体活性剤に浸漬されてもよい。生体活性剤の型式、生体活性剤を含む補綴物の部分、および補綴物への生体活性剤の取り付け方法は、埋め込み時に所望の機能を果たすように選択されてもよい。たとえば、生体活性材料は、たとえば冠状動脈の血管形成、腎動脈の血管形成、頚動脈の外科手術、腎透析フィステルの狭窄、または血管移植片の狭窄などの、適応症を治療するために選択されてもよい。
移植装置は、体内の管への送達のために形成されてもよい。たとえば、本発明の実施の形態に従った移植片本体とステントとを備える補綴物は、カテーテルを主体にしたシステムなどの送達システムの一部である保持シースの内部に圧縮されて送達形状にされてもよい。送達されると、補綴物は、たとえばステントの内部からのバルーンの膨張により拡張されてもよい。補綴物が留置される前には、シース、糸、チューブ、もしくはその他の圧縮された補綴物の全部もしくは一部の周囲を拘束する材料を含む、任意の好適な手段によって、またはその他の方法によって、送達形状が維持されてもよい。
