Description

本発明は医療装置の分野に関し、より具体的には埋め込み型装置についての検出ベクトルに関する。
心臓内又は心臓付近に埋め込まれるパルス発生器及び電極を有するリードは、ある種の生命に関わる不整脈を改善し、又は心臓収縮を刺激するために使用されてきた。心臓を正常な調律に戻すために、電気エネルギが電極を介して心臓に適用される。通常、リードは心室又は心房に接して配置されるか、心室又は心房の内部又はその付近に配置され、リードの端子ピンが、皮下に埋め込まれたペースメーカ又は除細動器に取り付けられる。パルス発生器は、電極を利用して、ある種の心事故を示し得る信号を心臓から受信するように構成されている。
システムは、缶電極を有するパルス発生器及びパルス発生器に接続可能なリードを含み、前記リードは遠位コイル電極及び近位コイル電極を含み、少なくとも一方のコイル電極と缶電極との間に単極検出ベクトルが提供されるように、両方のコイル電極が缶電極から電気的に切り離される。

図1はパルス発生器150に接続されたリード100を示す。一実施形態において、リード100は心臓に除細動ショックエネルギを付与するように適合される。ある実施形態は、心臓にペーシングパルスを付与する。パルス発生器150は、患者の胸部又は他の所望の部位に外科的に形成されたポケットの中に埋め込まれ得る。一般に、パルス発生器150は信号解析、処理、及び制御を行うための電子部品を含む。パルス発生器150は、ケース又は缶151に収納されるバッテリ、キャパシタ、及び他の部品等の電源を含み得る。デバイスは、心室の細動、頻拍、及び徐脈を含む心不整脈に対応する心室除細動、電気的除細動、及び心臓のペーシングのための様々なエネルギレベル及びタイミングの電気ショック及びパルスを決定し付与するための処理及び評価を提供するマイクロプロセッサを含み得る。
一実施形態において、リード100は近位端部107から遠位端部109まで延びるとともに中間部111を有するリード本体105を含む。リード100は、コイル状の導体や他の導体等の1つ以上の導体を含み、パルス発生器150から、右心室20に埋め込まれるように構成された遠位除細動コイル電極120及び右心房22又は上大静脈24に埋め込まれるように構成された近位除細動コイル電極122のような1つ以上の電極にエネルギを伝導する。上大静脈及び右心房は心臓の上室部と称される。一実施形態において、リード100は心室の検知及びペーシングのための先端電極130及び遠位リング電極132を含み得る。いくつかの実施形態は心房の検知及びペーシングのための1つ以上の近位リング電極を含む。
リード100は、パルス発生器150に取り付けられるリード端子ピンをヘッダ位置に含み得る。このシステムは電極として作用するハウジング缶151を伴う単極システム、又は2つの電極120,122、又は電極130,132の間のパルスを伴う双極システムを含み得る。
本システムは上室性頻拍検知ベクトルを提供することを目的とする。本システムは、上室性頻拍(SVT)と心室頻拍(VT)とをより良く識別する技術を提供する。例えば、過去の手法では、近位コイル122は缶電極151に電気的に接続され、遠位コイル120から、缶151及び近位コイル122の両方を用いて頻拍検知が行われた。しかし、この構成ではSVTとVTとを識別することが困難であった。
一実施形態において、本システムは近位コイル電極122及び遠位コイル電極120から缶電極151を電気的に絶縁することで、上室性頻拍検知ベクトルを提供する。例えば、コイル電極122は缶電極151から電気的に絶縁され、近位コイル電極122から缶電極151までの単極検知ベクトルが存在し得る。一実施形態において、遠位コイル120から缶電極151までの単極検知ベクトルが存在し得る。一実施形態において、遠位コイル120から近位電極122までの検知ベクトルが存在し得る。これらの構成はSVTとVTとを識別するのに役立つ。
他の実施形態において、本システムは、近位電極122を缶151から取り外し、1つ以上の電極130,132を利用した検知を提供する。
本明細書において、検知ベクトルという用語は、パルス発生器内の検知チャネルによって使用される2つの電極の位置によって説明される。検知チャネルは2つの電極間に存在する電気信号を用いて心臓活動を検知する。それぞれの検知ベクトルが、心臓信号のそれぞれの態様を検知チャネルに提示する。これらのベクトルは従来よりも大きなSVT乃至VTの信号を生成するため、本実施形態において、検知ベクトルはSVTを検知するための改善された性能を有する。
したがって、様々な実施例では、コイル電極120,122の少なくとも一方と缶電極151との間に単極検知ベクトルが提供されるように、両方のコイル電極が缶電極151から電気的に切り離されている。例えば、検知ベクトルは遠位電極120と缶電極151との間に提供されるか、又は検知ベクトルは近位電極122と缶電極151との間に提供される。別の実施例において、第2の検知ベクトルが遠位コイル電極120と近位コイル電極122との間に提供され得る。
別の実施例において、パルス発生器は、単極検知ベクトルが遠位コイル電極120及び缶電極151と、近位電極122及び缶電極151との間で交互に生じるように構成され得る。一実施形態において、パルス発生器は、遠位コイル電極120と缶電極151との間の第1の単極検知ベクトル、及び近位電極122と缶電極151との間の第2の単極検知ベクトルが同時に発生するように構成される。
パルス発生器は、上述した単極検知ベクトルを用いてVTとSVTとを識別するように構成され得る。例えば、VTとSVTとを識別する一つの技術は、正常洞調律中のショックベクトル電気記録図とRV率ベクトル電気記録図との間の相関関係のテンプレートを用いることである。不整脈が検出されると、そのテンプレートが進行中の調律と比較される。そして、その調律が既存のテンプレートと一致する場合、その調律の発生源は上室であると考えられ、デバイスは治療を行わない――上室の調律は正常洞調律と同じ伝導経路を辿っているためにSVT中の2つのベクトルの相互関係が正常洞調律テンプレートと一致するのであろうと仮定され、治療が与えられ得る。
しかし、調律中のベクトルの相互関係がテンプレートと一致しない場合、その調律はVTである(すなわち、その調律は正常伝導経路を用いていない)と推定される。
さらなる実施形態は、本明細書において説明する新たなベクトルから作成された異なるテンプレートを用いてもよい。この新たなテンプレートは、上述したテンプレートを用いて問題の症状を識別する能力をさらに提供し得る。したがって、より多くのベクトルの組合せからのより多くの情報は、より高い識別能力を生み出すであろう。
一実施形態において、電極122は、埋め込み後に右心房22内又は上大静脈24内に配置されるようにリードに沿って配置される。
図2は、一実施形態に基づく上述のシステムの一部の概略図を示す。リードはコイル電極122とパルス発生器150内の電子装置201との間に接続された第1の導体202を含む。リードは遠位コイル電極120と電子装置201との間に接続された第2の導体203をさらに含む。缶電極151もまた電子装置201に接続される。電子装置201は、電極122及び120が缶電極151から電気的に絶縁されるようにする電気経路を含む。既述したように、電子装置201は様々な接続を自由に取り外し、又は接続することが可能であるように構成され得る。
上記の説明は例示を目的としており、限定的なものでないことが理解される。上記の説明を検討すれば、多くの他の実施形態が当業者に明らかであろう。したがって、本発明の範囲は添付の特許請求の範囲及び請求の範囲が享受する均等物の全範囲に関連して決定されるべきである。

Claims (20)

  1. 缶電極を含むパルス発生器と、
    パルス発生器に接続可能であるとともに、遠位コイル電極及び近位コイル電極を有するリードとを含むシステムであって、
    少なくとも一方のコイル電極と缶電極との間に単極検知ベクトルが提供されるように、両方のコイル電極が缶電極から電気的に切り離されるシステム。
  2. 検知ベクトルが遠位電極と缶電極との間に提供される請求項1に記載のシステム。
  3. 検知ベクトルが近位電極と缶電極との間に提供される請求項1に記載のシステム。
  4. 遠位コイル電極と近位コイル電極との間に第2の検知ベクトルが提供される請求項1に記載のシステム。
  5. パルス発生器は、遠位コイル電極及び缶電極と、近位電極及び缶電極との間で単極検知ベクトルが交互に生じるように構成される請求項1に記載のシステム。
  6. パルス発生器は、遠位コイル電極と缶電極との間の第1の単極検知ベクトル及び近位電極と缶電極との間の第2の単極検知ベクトルが同時に発生するように構成される請求項1に記載のシステム。
  7. パルス発生器は、単極検知ベクトルを用いてVTとSVTとを識別するように構成される請求項1に記載のシステム。
  8. 缶電極を含むパルス発生器と、
    パルス発生器に接続可能であるとともに、遠位コイル電極及び近位コイル電極を有し、かつ先端ペーシング/検知電極及びリング電極をさらに有するリードとを含むシステムであって、
    遠位電極と缶電極との間に単極検知ベクトルが提供されるように、近位コイル電極及び遠位コイル電極が缶電極から電気的に切り離されるシステム。
  9. 近位電極と缶電極との間に第2の単極検知ベクトルが提供される請求項8に記載のシステム。
  10. 遠位コイル電極と近位コイル電極との間に第3の検知ベクトルが提供される請求項8に記載のシステム。
  11. リードは、近位コイル電極が右心房に埋め込み可能であり、かつ遠位コイル電極が右心室に埋め込み可能であるように構成される請求項8に記載のシステム。
  12. パルス発生器は、単極検知ベクトルを用いてVTとSVTとを識別するように構成される請求項8に記載のシステム。
  13. ハウジング缶電極を含むパルス発生器と、
    パルス発生器内に配置された電子装置と、
    ハウジング缶電極と電子装置との間の電気的接続とを含む装置であって、
    電子装置及び電気的接続は、リードがパルス発生器に接続されるとき、少なくとも一方のコイル電極と缶電極との間に単極検知ベクトルが提供されるように、リード上の遠位コイル電極が電気経路によってハウジング缶電極から電気的に切り離され、かつリード上の近位コイル電極がハウジング缶電極から電気的に切り離されるように構成されるシステム。
  14. パルス発生器は、遠位コイル電極と缶電極との間に単極検知ベクトルを提供するように構成される請求項13に記載の装置。
  15. パルス発生器は、近位電極と缶電極との間に単極検知ベクトルを提供するように構成される請求項13に記載の装置。
  16. 遠位コイル電極と近位コイル電極との間に第2の検知ベクトルが提供される請求項13に記載の装置。
  17. パルス発生器は、単極検知ベクトルを用いてVTとSVTとを識別するように構成される請求項13に記載の装置。
  18. 缶電極を含むパルス発生器を提供する工程と、
    パルス発生器にリードを接続する工程であって、リードは缶電極から電気的に絶縁された少なくとも1つの近位コイル電極を含み、かつリードは電気的に絶縁された1つの遠位コイル電極を含む工程と、
    近位コイル電極が右心房に配置され、かつ遠位コイル電極が右心室に配置されるようにリードを埋め込む工程と、
    少なくとも1つのコイル電極と缶電極との間の単極検知ベクトルを用いて心臓信号を検知する工程と
    を含む方法。
  19. 心臓信号を検知する工程が、SVT信号とVT信号とを識別する工程を含む請求項18に記載の方法。
  20. リードが1つ以上のペーシング及び検知電極の少なくとも一方を含む請求項18に記載の方法。
