Abstract
Description
本出願は、2007年12月6日に出願された米国仮出願第60/992,897号の利益を主張し、該仮出願は、その全体がすべての目的のために本明細書に参考として援用される。
本発明の様々な実施形態は、概して、医療デバイスならびに診断処置および治療処置の同時の送達に関する。より詳細には、本発明の複数の実施形態は、磁気共振画像法(MRI)などの医療手順中に移植可能リード線を磁界から保護する医療デバイスおよび方法に関する。
磁気共振画像法(MRI)は、核磁気共振技術を利用し、患者の体内の画像を描く非侵襲性の画像化方法である。典型的には、MRIシステムは、約0.2〜3テスラの磁界強度を有する磁気コイルを使用する。処置中、体組織は、磁界に垂直の平面において電磁エネルギのRFパルスに一時的に暴露される。これらのパルスから結果として生じる電磁エネルギは、組織内の励起した原子核の緩和特性を測定することによって、体組織を画像化するために用いられ得る。
磁気共振画像法(MRI)などの医療手順中に移植可能リード線を磁界から保護するためのシステムおよび方法が開示される。一部の実施形態は、概して近位部と、遠位部と、長さとを有するリード線を備えている医療デバイスに関する。一部の場合において、近位部は、パルス発生器(例えば、ペースメーカー、心臓除細動器、センサなどの監視デバイス、および/または類似のもの)に結合され得、遠位部は、電極を含み得、患者の心臓内に移植され得る。この構成において、例えば、リード線は、内部導体を通って心臓とパルス発生器との間で電気信号を伝えるように構成される。
図1は、患者の体内に移植されたリード線を備えている例示的医療デバイス12の概略図である。描かれた例示的実施形態において、医療デバイス12は、体内に移植されたパルス発生器を備えている。医療デバイスは、患者の心臓16に配置されたリード線14に結合され得る。心臓16は、右心房18と、右心室20と、左心房22と、左心室24とを含む。パルス発生器12は、典型的には、患者の胸部または腹部などの位置において皮下体内に移植されるが、但し、他の移植位置は可能である。
Vb=Vi Zbe/(Zbe+Zl)によって関係づけられ、ここで、Zcと並列でZbe=Zbである。
Claims (24)
- 近位部と、遠位部と、長さとを有するリード線を備えている医療デバイスであって、
該リード線は、らせん形状である内部導体を含み、
該内部導体のピッチは、該リード線の長さに沿って連続的に変化し、該リード線の長さに沿って複数の高インピーダンス点を形成し、該複数の高インピーダンス点は、それぞれ吸収されたエネルギの伝達が、磁気共振画像化デバイスのインタロゲーション周波数で該リード線の長さに沿って伝わることを妨げるように構成されている、医療デバイス。 - 前記内部導体は、1つ以上の外部シールド導体によって少なくとも部分的に放射状に囲まれる、請求項1に記載の医療デバイス。
- 前記リード線は、前記1つ以上の外部シールド導体を囲む絶縁材料の1つ以上の層をさらに含む、請求項2に記載の医療デバイス。
- 前記1つ以上の外部シールド導体は、連続的に変化するピッチを有する、請求項2に記載の医療デバイス。
- 前記内部導体は、前記1つ以上の外部シールド導体の内径より小さい外径を有し、該内部導体の該外径は、前記リード線の前記近位部および遠位部において該1つ以上の外部シールド導体の該内径まで増加する、請求項2に記載の医療デバイス。
- 前記内部導体のピッチにおける可変性は、シヌソイド関数、変形方形波関数、三角波関数、またはのこぎり波関数に従う、請求項1に記載の医療デバイス。
- 前記内部導体のピッチにおける可変性は、前記磁気共振画像法デバイスの前記インタロゲーション周波数の波長の1/4に等しい固定長の繰返しパターンに従う、請求項1に記載の医療デバイス。
- 前記内部導体は、5ミル〜50ミル、および5ミルと50ミルとを含む平均ピッチを有する、請求項1に記載の医療デバイス。
- 前記内部導体は、約5ミルより大きくないピッチを有する、請求項1に記載の医療デバイス。
- 前記リード線の長さは、40センチメートル〜90センチメートルである、請求項1に記載の医療デバイス。
- 前記内部導体のインピーダンスは、前記1つ以上の外部シールド導体のインピーダンスに類似する、請求項2に記載の医療デバイス。
- 前記リード線の前記遠位部は電極を含む、請求項1に記載の医療デバイス。
- 前記リード線は、該リード線の前記近位部および前記遠位部の近くに低インピーダンス絶縁体を含み、該低インピーダンス絶縁体は、電磁エネルギを該リード線の長さに隣接した体組織に沿って消散させ、該電磁エネルギの前記電極へのエネルギ伝達を減少させる、請求項12に記載の医療デバイス。
- 前記低インピーダンス絶縁体は、前記内部導体の比誘電率の5〜10倍の該誘電率を有する材料から作られる、請求項13に記載の医療デバイス。
- パルス発生器に結合された近位部と、患者の心臓内に移植された遠位部と、長さとを有するリード線と、
らせん状の形状を有する内部導体であって、1つ以上の周波数で該リード線の長さに沿って1つ以上の高インピーダンス点を形成する連続的に変化する内部導体ピッチを有する、内部導体と、
該内部導体の少なくとも一部分を放射状に囲む、らせん状の形状を有する外部導体とを備え、該リード線は、内部導体を通って該心臓と該パルス発生器との間に電気信号を伝えるように構成される、該外部導体は、該リード線の長さに沿って1つ以上の位置において変化する外部導体ピッチを有する、移植可能医療デバイス。 - 前記外部導体ピッチは、連続的に変化する、請求項15に記載の移植可能医療デバイス。
- 前記内部導体ピッチは、シヌソイド関数または変形方形波関数に従う、請求項15に記載の移植可能医療デバイス。
- 前記リード線の長さに沿って高インピーダンスを有し、断面に沿って低インピーダンスを有するシールドをさらに備えている、請求項15に記載の移植可能医療デバイス。
- 前記内部導体のインピーダンスは、前記外部導体のインピーダンスに実質的に類似する、請求項15に記載の移植可能医療デバイス。
- 前記パルス発生器は、ペースメーカーまたは心臓除細動器である、請求項15に記載の移植可能医療デバイス。
- 前記外部導体および前記内部導体の周囲に配置される絶縁材料の1つ以上の層をさらに備えている、請求項15に記載の移植可能医療デバイス。
