Abstract
【解決手段】(a)医療提供者へ医薬上許容される一酸化窒素ガスを提供する;および(b)医療提供者に先在性左心室機能障害を有するヒト患者を一酸化窒素の吸入を含む医学的処置から除外することは医学的処置に関連する有害事象もしくは重篤な有害事象のリスクを減少させる、左心室後負荷は、患者へのニトログリセリンもしくはカルシウムチャネルブロッカーを含む医薬投与形態を投与することによって最小限に抑える、または減少させることができる。左心室後負荷は、さらにまた大動脈内バルーンポンプを用いて最小限に抑える、または減少させることができる。

Claims (30)
- 一酸化窒素の吸入による治療を必要とする意図された患者集団において1つ以上の有害事象もしくは重篤な有害事象を減少させる方法であって、先在性左心室機能障害を有する患者をそのような治療から除外することを含んでなる方法。
- 前記患者が、20mmHgを超える肺毛細血管楔入圧をさらに有する、請求項1に記載の方法。
- 前記治療が、酸素の吸入をさらに含む、請求項1に記載の方法。
- 前記治療が、ベンチレーターを使用して送達される、請求項1に記載の方法。
- 先在性左心室機能障害を有する前記患者が、拡張期機能障害、高血圧性心筋症、収縮期機能障害、虚血性心筋症、ウイルス性心筋症、特発性心筋症、自己免疫疾患関連性心筋症、薬物関連性心筋症、毒素関連性心筋症、構造的心疾患、弁膜心疾患、先天性心疾患、特発性肺動脈性高血圧および肺高血圧心筋症、またはそれらの組み合わせから選択される状態の1つ以上を有する、請求項1に記載の方法。

- ヒト被験者において、一酸化窒素の吸入を含んでなる医学的処置に関連する1つ以上の有害事象もしくは重篤な有害事象のリスクを減少させる、または発生を防止する方法であって:
a.一酸化窒素の吸入治療に対して適格であるヒト被験者を同定すること;
b.前記患者が先在性左心室機能障害を有するかどうかを決定すること;および
c.前記患者が先在性左心室機能障害を有していない場合は前記医学的処置を投与することを含んでなり、
それにより前記医学的処置に関連する前記有害事象もしくは重篤な有害事象のリスクを減少させる、または発生を防止する方法。 - 前記患者が、20mmHgを超える肺毛細血管楔入圧をさらに示す、請求項9に記載の方法。
- 先在性左心室機能障害を有する前記患者が、拡張期機能障害、高血圧性心筋症、収縮期機能障害、虚血性心筋症、ウイルス性心筋症、特発性心筋症、自己免疫疾患関連性心筋症、薬物関連性心筋症に起因する副作用、毒素関連性心筋症に起因する副作用、構造的心疾患、弁膜心疾患、先天性心疾患、特発性肺動脈性高血圧、肺高血圧および心筋症、またはそれらの組み合わせから選択される1つ以上の状態を有する、請求項9または10に記載の方法。
- 前記医学的処置が、酸素の吸入をさらに含む、請求項9に記載の方法。
- 前記治療が、ベンチレーターを使用して送達される、請求項9に記載の方法。
- 前記患者が、小児である、請求項9に記載の方法。
- 前記患者が、成人である、請求項9に記載の方法。
- ヒト患者において、一酸化窒素の吸入を含んでなる医学的処置に関連する1つ以上の有害事象もしくは重篤な有害事象のリスクを減少させる、または発生を防止する方法であって:
a.医療提供者へ医薬上許容される一酸化窒素ガスを提供すること;および
b.前記医療提供者に先在性左心室機能障害を有するヒト患者を前記治療から除外することが前記医学的処置に関連する有害事象もしくは重篤な有害事象のリスクを減少させる、または発生を防止することを知らせることを含んでなる方法。 - 前記患者が、小児である、請求項16に記載の方法。
- 前記患者が、成人である、請求項16に記載の方法。
- 前記有害事象もしくは重篤な有害事象が、肺水腫、低血圧、心停止、心電図変化、低酸素血症、低酸素症および徐脈、またはそれらの組み合わせのうちの1つ以上である、請求項16に記載の方法。
- ヒト患者において、一酸化窒素の吸入を含んでなる医学的処置に関連する1つ以上の有害事象もしくは重篤な有害事象のリスクを減少させる、または発生を防止する方法であって:
a.医療提供者へ医薬上許容される一酸化窒素ガスを提供すること;および
b.前記医療提供者に先在性左心室機能障害を有するヒト患者は前記医学的処置に関連する有害事象もしくは重篤な有害事象の増加した比率を経験することを知らせることを含んでなる方法。 - 前記医療提供者に、20mmHgを超える肺毛細血管楔入圧を有するヒト患者における有害事象もしくは重篤な有害事象のリスクを知らせることをさらに含む、請求項20に記載の方法。
- 前記医療提供者に、先在性もしくは臨床的に重大な左心室機能障害を有するヒト患者においては吸入用一酸化窒素を使用することに関連するリスクが存在すること、およびそのようなリスクはケースバイケースで評価すべきであることを知らせることをさらに含む、請求項20に記載の方法。
- 前記医療提供者に、左心室機能障害を有するヒト患者においては吸入用一酸化窒素を使用することに関連するリスクが存在することを知らせることをさらに含む、請求項20に記載の方法。
- iNOによる治療を必要とする意図された患者集団において、1つ以上の有害事象もしくは重篤な有害事象を減少させる方法であって:
a.iNO治療に対して適格である患者を同定すること;
b.前記患者が先在性左心室機能障害を有するかどうかを同定するために前記患者を評価およびスクリーニングすること;および
c.先在性左心室機能障害を有すると同定された患者をiNO治療から除外することを含んでなる方法。 - 一酸化窒素の吸入を含んでなる医学的処置に関連する1つ以上の有害事象もしくは重篤な有害事象のリスクを減少させる、または発生を防止する方法であって:
a.一酸化窒素の吸入治療を受ける必要のある患者を同定すること;
b.前記患者が先在性左心室機能障害を有するかどうかを同定するために前記患者を評価およびスクリーニングすること;および
c.前記患者が先在性左心室機能障害を有していない場合に前記一酸化窒素の吸入を投与することを含んでなり、
それにより前記一酸化窒素の吸入治療に関連する前記有害事象もしくは重大な有害事象のリスクを減少させる、または発生を防止する方法。 - 先在性左心室機能障害を有する前記患者が、20mmHgを超える肺毛細血管楔入圧を示す、請求項24または25に記載の方法。
- 前記一酸化窒素の吸入による治療を必要とする意図された患者集団が、静止時に25mmHg超のPAPm、15mmHg以下のPCWP、および3u・m2超のPVRIを特徴とする特発性肺動脈性高血圧;静止時の25mmHg超のPAPmおよび3u・m2超のPVRIを特徴とする修復済みおよび未修復の肺高血圧を伴う先天性心疾患;静止時の25mmHg超のPAPmおよび3u・m2超のPVRIを特徴とする心筋症のうちの1つ以上を有するか;または前記患者が、急性肺血管拡張試験により肺血管反応性を評価するための右心カテーテル検査を受けることが予定されている、請求項1に記載の方法。
- 前記患者における肺水腫である有害事象もしくは重篤な有害事象の発生のリスクを最小限に抑える、または減少させるために左心室後負荷を減少させることをさらに含む、請求項1、9、16、20、24または25に記載の方法。
- 前記左心室後負荷を、前記患者へのニトログリセリンもしくはカルシウムチャネルブロッカーを含む医薬製剤を投与することによって最小限に抑える、または減少させる、請求項28に記載の方法。
- 前記左心室後負荷を、大動脈内バルーンポンプを用いて最小限に抑える、または減少させる、請求項28に記載の方法。
