Description
本出願は、引用により全体が本明細書中に組み込まれている、2006年10月31日に出願された米国特許仮出願第60/855,247号の優先権を主張するものである。本出願は、引用により全体が本明細書中に組み込まれている、2007年10月31日に出願された米国特許出願第11/933,247号の優先権も主張するものである。
本発明は、慢性の流体収集に関する病気、例えばうっ血性心不全の治療中に過剰な体液を除去するための医療装置に関する。より詳細には、本発明は、過剰な流体を第1の体腔から第2の体腔に移送するためのポンプ式経膀胱排液装置に関する。
様々な病状が体液の収集異常を引き起こす。こうした状態の1つがうっ血性心不全(CHF)であり、CHFにより心臓は、他の器官の酸素の必要を事実上満たすことができなくなる。CHFは、心臓への障害を受けた後、例えば心臓のポンピング動作が不全になる心筋梗塞後の成人に発生することが多い。CHFは、心機能が正常に近い状態でも、例えば塩類及び水分の貯留に起因する血液量の増加による、又は頻脈による高需要の状況下で起こることがある。こうした代償性変化が心機能に負荷を与えて、進行性CHFが引き起こされる。
本発明は、定期的又は必要に応じて、流体を第1の体腔例えば腹腔又は肺から除去し、その流体を第2の体腔例えば膀胱に移送するための装置及び使用方法に関する。本発明の原理によって構築された装置を使用することによって、感染の危険を回避し、過剰な流体を生成する腔を不必要に脱水することなく、特定の病気の治療に必要な程度に流体の除去を制限することができる。したがって、本発明は利益をもたらすものであり、CHF及び他の病的状態を治療する腹膜透析の欠点を回避し、また対象の体腔から流体を過剰に除去してしまう従来技術の装置の改善をもたらすものである。
本発明の実施態様の詳細な説明を本明細書に記載する。しかし、本発明を様々な形態で実施することができることが理解されるべきである。したがって、本明細書に開示した具体的な詳細は限定的ではなく、むしろ本発明を任意の詳細なシステム、構造、又は方法で実際に使用する方法を当業者に教示するための代表的な基礎であると解釈されたい。
Claims (25)
- 埋込型流体管理装置であって、
患者の身体の第1の腔と流体連通している近位端及びポンプと流体連通している遠位端を有する取込管と、
該ポンプと流体連通している近位端及び該患者の身体の第2の腔と流体連通している遠位端を有する流出管とを備え、
該取込管の遠位端が、該第1の体腔からリザーバ内に浸入すると膨張し、かつ該ポンプによる該流体の該リザーバからの除去で収縮するように構成されたリザーバに結合される、前記埋込型流体管理装置。 - 前記リザーバが外面を備え、該外面の少なくとも一部には配置された孔を有し、該孔が前記第1の腔から該リザーバへの前記流体の浸入をもたらす、請求項1記載の埋込型流体管理装置。
- 前記孔の少なくとも一部が、前記外面に対して凹んだ位置に配置される、請求項2記載の埋込型流体管理装置。
- 前記リザーバの外面の少なくとも一部に配置された複数の凹部をさらに備え、該凹部が、前記流体の前記第1の体腔から前記孔の少なくとも一部に向かう流れを促進する、請求項2記載の埋込型流体管理装置。
- 前記凹部の間の前記外面の少なくとも一部が、該表面内での組織の内植を助ける材料又はコーティングを含む、請求項4記載の埋込型流体管理装置。
- 前記孔が、前記流体の所定の成分が前記リザーバ内に浸入するのを阻止するように構成される、請求項2記載の埋込型流体管理装置。
- 前記リザーバが本質的に半球形状を有する、請求項1記載の埋込型流体管理装置。
- 前記取込管が、前記半球形状の本質的に平坦な部分から延びる、請求項7記載の埋込型流体管理装置。
- 前記取込管が、前記半球形状の本質的に湾曲した部分と該半球形状の本質的に平坦な部分を連結する表面から延びる、請求項7記載の埋込型流体管理装置。
- 前記リザーバが本質的に円筒形状を有する、請求項1記載の埋込型流体管理装置。
- 前記リザーバが、前記本質的に円筒形状の長手方向軸に沿って膨張し収縮するように構成される、請求項6記載の埋込型流体管理装置。
- 前記リザーバが支持構造を備える、請求項1記載の埋込型流体管理装置。
- 前記支持構造が、収縮後に前記リザーバの反跳をもたらし、該反跳が前記流体の除去後に該リザーバの膨張を促進する、請求項12記載の埋込型流体管理装置。
- 前記流出管から前記取込管への逆流を阻止する手段をさらに備える、請求項1記載の埋込型流体管理装置。
- 前記リザーバが収縮した状態の場合に、該リザーバが、前記ポンプの作動を阻止する脱水防止要素を備える、請求項1記載の埋込型流体管理装置。
- 前記リザーバが、前記装置の埋込み後に該リザーバを前記第1の腔内の所望の位置に配置させる重りを備える、請求項1記載の埋込型流体管理装置。
- 前記ポンプの作動が、前記リザーバ内、前記リザーバ付近、又は前記患者の身体の外部に配置されたセンサによって開始される、請求項1記載の埋込型流体管理装置。
- 前記ポンプの作動を、前記患者の身体の外部にあり、かつ該ポンプと無線で通信するように構成された装置によって開始又は停止することができる、請求項1記載の埋込型流体管理装置。
- 請求項1の埋込型流体管理装置であって、
前記リザーバが、互いに流体連通している第1のリザーバ要素及び第2のリザーバ要素を備え、
該第1及び第2のリザーバ要素のそれぞれが、流体を含み、かつ前記ポンプが作動すると収縮するように構成され、
環状面が該第1の要素を該第2の要素に結合し、該環状面が該第1の要素の一部及び該第2の要素の一部によって画定された溝様に構成され、
該環状面内の開口部が、該流体の該リザーバへの浸入を可能にする、前記埋込型流体管理装置。 - 前記第1及び第2の要素が、それぞれ互いに平行の外周を有する膨張可能な円板様に形付けられ、かつ前記環状面が該第1及び第2の要素に対して平行に配置される、請求項19記載の埋込型流体管理装置。
- 前記環状面が該環状面の外面上に放射状に配置された凹部を備え、かつ該凹部が前記流体の流れを前記開口部に向ける、請求項20記載の埋込型流体管理装置。
- 前記凹部が、前記第1の要素の一部及び前記第2の要素の一部上に対称に配置される、請求項21記載の埋込型流体管理装置。
- 前記開口部のそれぞれが、前記凹部の対の間の前記環状面上に配置される、請求項22記載の埋込型流体管理装置。
- 前記凹部が前記環状面の内面上のリブを画定し、該リブが前記リザーバの所定の膨張及び収縮の方向に有利に働く、請求項23記載の埋込型流体管理装置。
- 前記リブが前記第1の要素と前記第2の要素の間に連続的に延びる、請求項23記載の埋込型流体管理装置。
