Abstract
Description
a)有限インパルス応答平均フィルタ(例えば、1分から2分の移動平均、又は「ボックスカー」フィルタ)、
b)データの変化率(微分)及び累加平均(積分)、並びにその現在値に基づくフィルタ、
c)一般的なFIR(有限インパルス応答)又はIIR(無限インパルス応答)デジタル・フィルタ、
d)「メジアン」フィルタなどの統計的フィルタ、又は
e)カルマン形式などの適応フィルタ。
a)これまでのアラーム・イベントの頻度及び持続期間を使用する適応フィルタ、
b)注入された薬物の合計量を使用する適応フィルタ、又は
c)血漿の推定及び/又は鎮痛性薬剤の効果部位の濃度(Cp、Ce)の推定を使用する適応フィルタであり、このような推定は、適切な薬物動力学(「PK」)モデルを使用することにより行われ、PK効果部位の推定は、フィルタの特性(極及び零点)を変更するために使用される。
Claims (27)
- 薬剤を送達するように構成されたPCA装置に対する制御を最適化するためのシステムであって、
前記薬剤の送達を求める要求信号を提供するための薬剤送達要求装置と、
生理学的データを表す生理学的信号を提供するように構成された生理学的装置と、
前記生理学的信号を受け取り、前記生理学的信号を処理し、且つ第1のルールに従って第1のアラーム信号を提供する第1の制御装置と、
前記第1のアラーム信号を伝える第1のアラーム装置と、
前記要求信号を受け取り、前記生理学的信号を受け取り、前記要求信号及び前記生理学的信号を処理し、且つ前記第1のルールとは異なる第2のルールに従って薬剤を送達するための前記処理により前記PCA装置のオペレーションを制御する、前記第1の制御装置とは別の第2の制御装置と
を備える、制御を最適化するためのシステム。 - 前記第2の制御装置が、前記PCA装置が薬剤を送達しない非送達モードに応じた前記処理に従って前記PCA装置を制御する、請求項1に記載の制御を最適化するためのシステム。
- 第2のアラーム信号に応じて第2のアラームを伝える第2のアラーム装置をさらに備え、前記第2の制御装置が、前記第2のルールの下で、前記処理に従って前記第2のアラーム信号を提供する、請求項2に記載の制御を最適化するためのシステム。
- 前記生理学的データ装置が、生理学的パラメータを測定し、且つ前記測定されたパラメータを表す前記生理学的信号を提供する生理学的モニタを備える、請求項1に記載の制御を最適化するためのシステム。
- 前記第2の制御装置が、前記PCA装置が薬剤を送達しない非送達モードに応じた前記処理に従って前記PCA装置を制御する、請求項4に記載の制御を最適化するためのシステム。
- 前記第2の制御装置が、前記第2のルールに従って第2のアラーム信号を提供し、また第2のアラーム装置が、前記第2のアラーム信号を伝える、請求項5に記載の制御を最適化するためのシステム。
- 生理学的パラメータに関係する事前に決定された患者特有のデータに関する生理学的パラメータのデータベースをさらに備え、
前記データベースが、事前に決定された患者特有の生理学的パラメータに関係するデータを表すデータ信号を提供し、
前記第2の制御装置が、前記要求信号を受け取り、前記生理学的信号を受け取り、前記データ信号を受け取り、且つ前記要求信号、前記生理学的信号、及び前記データ信号を処理し、さらに前記第2のルールに従って薬剤を送達するための前記処理に応じて前記PCA装置の前記オペレーションを制御する、請求項1に記載の制御を最適化するためのシステム。 - 前記生理学的データ装置がさらに、生理学的パラメータを測定し、且つ前記測定されたパラメータを表す前記生理学的信号を提供する生理学的モニタを備える、請求項7に記載の制御を最適化するためのシステム。
- 前記第2のルールを変更するための入力装置をさらに備える、請求項1に記載の制御を最適化するためのシステム。
- 前記第2の制御装置と接続された遠隔サーバをさらに備え、前記第2の制御装置が、前記サーバから、前記第2のルールに対する変更を受け取り、前記変更された第2のルールに従って処理する、請求項1に記載の制御を最適化するためのシステム。
- 前記第2の制御装置が、前記第2のサーバから、さらなる患者特有のデータを受け取り、前記患者特有のデータ、前記要求信号、前記生理学的信号を処理し、且つ前記変更された第2のルールに従って薬剤を送達するための前記処理により前記PCA装置の前記オペレーションを制御する、請求項10に記載の制御を最適化するためのシステム。
- 前記第2の制御装置が、前記PCA装置が薬剤を送達しない非送達モードに応じた前記処理に従って前記PCA装置を制御する、請求項11に記載の制御を最適化するためのシステム。
- 前記第2の制御装置が、移動平均フィルタ、変化率フィルタ及び累加平均フィルタ、インパルス応答フィルタ、統計フィルタ、適応フィルタ、並びに薬物動力学モデルのうちの1つ又は複数のものにより前記生理学的信号をフィルタにかける、請求項1に記載の制御を最適化するためのシステム。
- フィルタのパラメータが、生理学的パラメータに関係する履歴データにより適応される、請求項13に記載の制御を最適化するためのシステム。
- フィルタのパラメータが、薬剤注入に関係する随伴データにより適応される、請求項14に記載の制御を最適化するためのシステム。
- 薬剤を送達するように構成されたPCA装置に対する制御を最適化するための方法であって、
前記薬剤の送達を求める薬剤要求信号を受け取るステップと、
生理学的データを表す生理学的信号を受け取るステップと、
第1の制御装置を用いて前記生理学的信号を処理し、且つ第1のルールに従って第1のアラーム信号を提供するステップと、
前記第1のアラーム信号を伝えるステップと、
前記第1の制御装置とは別の第2の制御装置を用いて前記要求信号及び前記生理学的信号を処理し、且つ前記第1のルールとは異なる第2のルールに従って薬剤を送達するための前記処理により前記PCA装置のオペレーションを制御するステップと
を含む、制御を最適化するための方法。 - 前記PCA装置の前記オペレーションを制御する前記ステップが、前記PCA装置が薬剤を送達しない非送達モードに応じた前記処理により前記PCA装置を制御するステップを含む、請求項16に記載の制御を最適化するための方法。
- 前記第2のルールの下で、前記処理に従って第2のアラーム信号を提供するステップをさらに含む、請求項16に記載の制御を最適化するための方法。
- 生理学的データを表す生理学的信号を受け取る前記ステップが、生理学的パラメータを測定し、且つ前記測定されたパラメータを表す生理学的信号を提供するステップを含む、請求項16に記載の制御を最適化するための方法。

- 生理学的パラメータに関係する事前に決定された患者特有のデータに関する生理学的パラメータのデータベースから、前記第2の制御装置により生理学的データを表す生理学的データ信号を受け取るステップと、
前記第2の制御装置により、前記データ信号、前記要求信号、前記生理学的信号を処理し、且つ前記第2のルールによる前記処理に応じて前記PCA装置の前記オペレーションを制御するステップと
をさらに含む、請求項16に記載の制御を最適化するための方法。 - 生理学的データを表す生理学的信号を受け取る前記ステップが、生理学的パラメータを測定し、且つ前記測定されたパラメータを表す生理学的信号を提供するステップを含む、請求項22に記載の制御を最適化するための方法。
- 前記第2の制御装置に接続された入力装置を用いることにより、前記第2のルールを変更するステップをさらに含む、請求項16に記載の制御を最適化するための方法。
- 前記第2のルールを変更する前記ステップが、遠隔サーバにより前記第2のルールを変更するステップを含み、且つ
前記第2の制御装置により処理する前記ステップが、前記変更された第2のルールに従って処理するステップを含む、請求項24に記載の制御を最適化するための方法。 - 前記第2のサーバから、さらなる患者特有のデータを受け取り、且つ前記第2の制御装置により、前記患者特有のデータ、前記要求信号、前記生理学的信号を処理し、さらに前記変更された第2のルールに従って薬剤を送達するための前記処理により前記PCA装置の前記オペレーションを制御するステップをさらに含む、請求項25に記載の制御を最適化するための方法。
- 前記PCA装置を制御する前記ステップが、前記PCA装置が薬剤を送達しない非送達モードに応じた前記処理に従って前記PCA装置を制御するステップを含む、請求項26に記載の制御を最適化するための方法。
