Description
本明細書に記載される状態を治療するのに有用な治療薬は、核酸であることもある。これらの核酸は、CRH、ACTH、又はコルチソールの発現を直接的又は間接的に抑制することにより、HPA系を標的とする第一薬剤として作用しうるし、そしてこれらは、GABAを増加させることにより前頭前野を標的とする第二薬剤として作用しうる。第一薬剤と第二薬剤のいずれか又は両方が、交感神経系を抑制する第三薬剤と組み合わせて使用される場合、第三薬剤は、交感神経系における神経伝達物質又はその同属受容体の発現を抑制する核酸(例えば、βアドレナリン受容体の発現を抑制することができる核酸)でありうる。

出願人のモデルでは、ラットをコカイン自己投与及び食餌強化に対して交互に15分間づつアクセスさせた。薬剤及び組み合わせ剤の潜在的非特異的な運動失調効果を調節するために食餌を使用した。理想的な薬剤又は薬剤の組み合わせは、摂餌維持応答(food maintained responding)に影響することなくコカイン自己投与を低下させるものである。出願人が用いた他の前臨床モデルは、消失したコカイン探索の再発モデル(extinguished cocaine seeking model of relapse)の契機誘導性(cue induced)の回復である。このモデルでは、ラットはコカインを自己投与するように訓練され、そして消失した応答性を回復させる周囲環境における条件付け契機の能力が、再発の尺度として評価されそして解釈される。
ここに記載される実験は、コカイン自己投与を低減する点で有効であると同定された薬剤の組み合わせが、消失したコカイン探索行動を回復させる条件付け契機の能力を阻害するか否かを調べるように設計された。成体雄ウィスターラットに留置頚椎カテーテルをインプラントし、そして1週間に5日間行なわれる毎日2時間のセッションの間における定率の4回強化スケジュール(FR4)の下で、実験チャンバー内の応答レバーの一つ(つまり、「活性」又は「コカイン」レバー)を押すことにより、コカイン(0.25mg/kg/注入)を自己投与するように訓練された。各セッションの開始において、両方のレバーをチャンバーへと伸ばし、そしてコカインが利用できることを示すために活性レバーの上の刺激光を点けた。最初に、活性レバーを押すごとに、コカインの静脈内注入をもたらし、そして部屋の光と音の複合刺激(つまり、条件付け契機又は二次的強化因子)の同時の提示をもたらす。各注入に続き20秒間の中断がされた。活性レバー上の刺激光、及び部屋の光と音の複合刺激は、当該中断時間のあいだ消され、そして活性レバー上の光が中断時間が終わると点灯された。活性レバー上の応答が連続する2日間で20%未満しか変化しなかった場合に、応答要求をFR2へと増加させた。FR2スケジュール又は強化の下で同様の安定性が観測された場合に、応答要求を最終定率4に増加させた。FR4の強化スケジュールの下での安定応答についての基準は、当該スケジュールに最低10日間かけられることであり、当該基準は10%未満応答が変化した場合に少なくとも連続3日間で結論付けられる。不活性レバーの押し行動は計数されたが、全ての場合でプログラムされた結果をもたらさなかった。ひとたび安定なコカイン自己投与が観察された場合、ラットを消失状態(extinction)にかけた;当該状態において、ラットは行動チャンバーに入れられたが、「コカイン」(活性)レバーへの応答はプログラムされた結果をもたらすことがない。消失トレーニングは、応答がベースライン自己投与の20%未満に低減するまで続けられた。次に、回復試験が行なわれた。ラットを実験チャンバーにいれ、応答レバーをチャンバーに設置し、そして「活性」レバー上の刺激光を自己投与トレーニングの間と同様に点けた。強化の間に、活性レバーについての応答は、条件付け強化因子(つまり、自己投与の間にコカインと一緒に与えられた部屋の光と音の複合刺激)の5.6秒間の提示をもたらした。「不活性」レバーについての応答を計数したが、計画された結果をもたらさなかった。強化試験の間の「活性」レバーについての応答は、コカイン探索行動の指標としてとられた。各実験群は8〜10のラットからなった。
これらの実験は、食餌強化とコカイン自己投与の複数回の交互のスケジュール下のラット応答における静脈内コカイン自己投与についてのメチラポンとオキサゼパムの組み合わせの効果を決定するように設計された。当該結果を図1Aのグラフに示す。左側の1つめの棒グラフ(Ext)は、生理食塩水の注入のみをもたらす「活性」レバーに応答する場合の消失の結果を示す。2つ目の棒グラフ(Veh)は、治療薬についてビヒクル(0.9%生理食塩水中の5%Emulphor(登録商標))での前処理に続き自己投与されたコカイン注入回数を示す。「高用量Met」の棒グラフは、高用量のメチラポン(25〜175mg/kg、ip)で前処理後のコカインの注入回数を示し、一方「高用量OX」棒グラフは、高用量のオキサゼパム(5〜80mg/kg、ip)で前処理後の自己投与されたコカイン注入回数を示す。

ラットにおける消失したコカイン探索の契機誘導性の回復についてのメチラポン慢性投与の効果を決定するためにこれらの実験を設計した。出願人は、コカイン探索の再発モデルを用いた。薬物治療の組み合わせは、慢性の形でコカイン依存症に対して投与されるので、このモデルは重要な実験である。図11を参照すると、「SA」と付された棒グラフは、コカイン自己投与の間に「活性」レバーについてなされる応答回数を示す。「Ext」と付される棒グラフは、当該レバーに応答した場合に生理食塩水の注入のみをもたらす場合の消失の間に、「活性」レバーについての応答回数を示す。「VEH」と付された棒グラフは、ビヒクル(5%emulphorを含む0.9%生理食塩水)で前処理の後の回復試験の間に「活性」レバーに対する応答を示す。「メチラポン」と付された棒グラフは、メチラポンの慢性デリバリー(50mg/kg、ip、1日1回、14日間)の後の回復試験の間の「活性」レバーに対する応答回数を示す。図11に見てとれるように、メチラポンの慢性投与は、生理食塩水のみがデリバリーされ、そして消失の間に活性レバーが押される場合に見られるレベルにまでコカイン探索を低減した。これらのデーターにより、メチラポンは、慢性投与の後にコカイン探索の再発を阻害する点で有効性を保つということが示される。
ラットにおける消失したコカイン探索の契機誘導性の回復についてのCP-154,526及びオキサゼパムの組み合わせの効果を決定するためにこれらの実験を設計した。図12を参照すると、「自己投与」と付された棒グラフのセットは、コカイン自己投与の間に「活性」レバー及び第二「不活性」レバーについてなされた応答回数を示す。当該セットにおける最初の二つの棒グラフは、回復試験の間に治療薬としてビヒクル(5%emulphorを含む0.9%生理食塩水)を最終的に受けたラットの応答を示す。3番目及び4番目の棒グラフは、回復試験の間に薬物治療の組み合わせ(つまり、コカイン自己投与実験において決定された有効でない用量のCP-154,526及びオキサゼパムからなる注射)を最終的に受けたラットの応答を示す。ラットは、ビヒクル又は薬物治療の組み合わせを1回だけ注射された。当該注射は、回復試験のセッションの開始の30分前に行なわれた。自己投与及び消失の間の応答は、図12に示され、群間で応答の有意差がないことが示された。不活性レバーについての応答は、全ての場合でプログラムされた結果をもたらさなかった。「消失」と付された棒グラフの第二セットは、「活性」レバーについての応答が生理食塩水の注入のみをもたらす場合の消失の間に、「活性」及び「不活性」レバーに対する応答回数を示す。「回復」と付された棒グラフの三番目のセットは、薬物治療の組み合わせのデリバリー後の回復試験の間での「活性」及び「不活性」レバーについての応答回数を示す(つまり、CP-154,526とオキサゼパムの不活性用量からなる注射)。明らかに見られるように、CP-154,526とオキサゼパムからなる薬物治療の組み合わせは、コカイン探索(つまり、回復の間の活性レバーに対する応答)を、消失の間に活性レバーが押された際に生理食塩水のみがデリバリーされた場合に見られるレベルにまで低減させた。薬物治療の組み合わせが、摂餌維持応答に影響することなく消失レベルにまで回復(再発)を低下させた。このことにより、組み合わせ剤が、別の強化因子(つまり、摂餌)に対する応答又は意欲に影響を与えることなくコカインを探索する意欲を低減させた。
成体雄ウィスターラット(90〜120日齢)に常時性留置頚椎カテーテルをインプラントし、そして手術から回復させた。試験の日に、当該ラットは、コカイン注射の投与の30分前に、オキサゼパムとメチラポンの様々な組み合わせ剤(図14の表に記載される)又はビヒクル(5%emulphorを含む生理食塩水)で前処理された。オキサゼパム/メチラポンの組み合わせ剤は、当該組み合わせ剤が摂餌維持応答に影響することなく、消失したコカイン探索の契機誘導性回復を行なったか又はコカイン自己投与を低減したということを示す出願人の行なった行動試験から選択される。薬剤の組み合わせ又はビヒクル注射の30分後、ラットは、1時間の間2分ごとにコカイン(0.25mg/kg/注入)の静脈内注射を受けた。最後のコカイン注射の後に、コカイン及びその代謝物であるエクゴニンメチルエステル及びベンゾイルエクゴニンの分析のために血液をカテーテルから回収した。メチラポンとメチラポール、並びにオキサゼパムの濃度も測定した。全ての薬剤濃度を、GCMS法を用いて測定した。これらの試験の結果により、オキサゼパムとメチラポンの組み合わせが、コカイン又はその代謝物の血漿濃度に影響を与えなかったということが示された。これらの研究により、メチラポンとオキサゼパムがお互いの血漿濃度に影響を与えないことが示された。出願人がラットで観察した行動効果が、様々な薬剤の間で生じる薬物動態相互作用のため生じるものではないということがこれらのデーターにより示唆される。
強制水泳試験(FST)は、薬剤の抗うつ病効力を評価するための予測的妥当性を有する動物モデルである。対象は、学習性無力感を誘発させるために、逃れることのできない生命を脅かす状況へと直面させられる。学習性無力感を達成させるために、逃れることができず、そしてその中で浮かんでいるために泳いでいなくてはならない水を満たしたシリンダー内にラットを配置した。このラットがこの状況が絶望的なものであると認識した時点で、絶望した際の行動が現れ、そして逃げたり又は泳いだりする試みをするのではなくラットは動かなくなる。この無動姿勢の時間は、絶望と判定される行動である。オキサゼパム、ベンゾジアゼピン、及びメチラポン、コルチコステロン合成の11-β-ヒドロキシラーゼ阻害剤は、それぞれ抗不安及び抗うつ効果を有することが示されてきた。急性でそして慢性的に単独でそして一緒に投与されたオキサゼパム及びメチラポンの潜在的な抗うつ性質は、FSTを用いて雄ウィスターラットで評価された。薬剤(5又は10mg/kgのオキサゼパム、25又は50mg/kgメチラポン)のうちの一つ、又はその組み合わせを、試験の1日後と、試験の2日前(急性)又は1日目の試験開始前の14日間(慢性)に注射した。単独及び組み合わせでの薬剤の急性及び慢性投与は、FSTにおける無動性を低減するのに有効であり、この薬物治療が抗うつ活性を有するということを示唆する。

- 単位用量における前記第一薬剤の量又は前記第二薬剤の量が、同一の単位用量を必要とする患者に現在又は一般的に処方される当該第一薬剤又は当該第二薬剤の量よりも少ない、請求項1〜13のいずれか一項に記載の医薬組成物。
- 約5〜60mgのオキサゼパム及び約250〜1000mgのメチラポン(Metopirone(商標))を、単位投与形態中に含む医薬組成物。
- 交感神経系において活性を阻害する第三薬剤をさらに含む、請求項1〜15のいずれか一項に記載の医薬組成物。
- 前記第三薬剤が、βブロッカー又は他の抗不安化合物である、請求項16に記載の医薬組成物。
- 前記βブロッカーが、ソタロール(Betapace(商標))、イモロール(Blocadren(商標))、カルテオロール(Cartrol(商標))、カルベジロール(Coreg(商標))、ナドロール(Corgard(商標))、ナドール/ベンドロフルエタジド(Corzide(商標))、プロプラノロール(Inderal(登録商標))、プロプラノロール/HCTZ(Inderide(商標))、ベタキソロール(Kerlone(商標))、ペンブトロール(Levatol(商標))、メトプロロール(Lopressor(商標))、ラベタロール(Normodyne(商標))、アセブトロール(Sectral(登録商標))、アテノロール/HCTZ(Tenoretic(商標))、アテノロール(Tenormin(登録商標))、チモロール/HCTZ(Timolide(商標))、メトプロロール(Toprol(商標))、ラベタロール(Trandate(商標))、ピンドロール(Visken(商標))、ビソプロロール(Zebeta(商標))、ビソプロロール/HCTZ(Ziac(商標))、又はエスモロール(Brevibloc(登録商標))である、請求項17に記載の医薬組成物。
- 前記他の抗不安化合物は、選択的セロトニン再取り込み阻害剤(SSRI)である、請求項16に記載の医薬組成物。
- 前記SSRIが、シタロプラム(Celexa(登録商標))、エスシタロプラム・オキサレート(Lexapro(登録商標))、フルボキサミン(Luvox(登録商標))、パロキセチン(Paxil(登録商標))、フルオキセチン(Prozac(登録商標))、又はセルトラリン(Zoloft(登録商標))である、請求項19に記載の医薬組成物。
- 前記他の抗不安化合物が、アンジオテンシンII阻害剤である、請求項16に記載の医薬組成物。
- 前記アンジオテンシンII阻害剤が、カンデサルタン(Atacand(登録商標))、エプロサルタン(Teveten(登録商標))、イルベサルタン (Avapro(登録商標))、ロサルタン (Cozaar(登録商標))、テルミサルタン(Micardis(登録商標))、又はバルサルタン (Diovan(登録商標))である、請求項21に記載の医薬組成物。
- HPA系の異常活性に関連する障害を患う患者を治療する方法であって、当該方法が以下の:
(a)治療を必要とする患者を同定し;そして
(b)当該患者に治療有効量の請求項1〜22のいずれか一項に記載の組成物を投与する
を含む、前記方法。 - 前記障害が、依存症、不安症、統合失調症、又はうつ病を含む、請求項23に記載の方法。
- 前記障害が、物質への依存症である、請求項23又は24に記載の方法。
- 前記物質が化学刺激物質、オピエート、ニコチン、アルコール、疼痛緩和処方薬、又は天然植物由来の薬剤である、請求項25に記載の方法。
- 前記化学刺激薬が、コカイン、アンフェタミン、メタンフェタミン、メチルフェニデート、或いはそれらのアナログである、請求項26に記載の方法。
- 前記オピエートが、ヘロイン、コデイン、ヒドロコドン、又はそれらのアナログである、請求項26に記載の方法。
- 前記天然植物由来薬剤がマリファナである、請求項26に記載の方法。
- 前記障害が、行為に対する依存症である、請求項23又は24に記載の方法。
- 前記行為が、ギャンブル又は性的行為への従事である、請求項30に記載の方法。
- 前記行為が、過食である、請求項30に記載の方法。
- 前記不安症が、パニック障害、強迫性障害(OCD)、外傷後心的ストレス障害(PTSD)、社会不安障害、全般性不安障害を伴う、請求項23又は24に記載の方法。
- 前記うつ病が、大うつ病障害又は気分変調を伴い、双極性うつ病であり、又は医学的状態又は物質乱用を伴う、請求項24に記載の方法。
- 前記障害が、摂食障害又は睡眠障害である、請求項23に記載の方法。
- 前記障害が、注意欠陥障害である、請求項23に記載の方法。
- 更年期障害又はHPA系の活性と関連する月経周期の不所望な症状を患う患者を治療する方法であって、当該方法が、以下の:
(a)治療を必要とする患者を同定し;そして
(b)当該患者に、治療有効量の請求項1〜22のいずれか一項に記載の組成物を投与すること
を含む、前記方法。 - 前記症状が、不安症、うつ病、又は睡眠困難である、請求項37に記載の方法。
