Abstract
Description
The present invention is equipped with an expandable / shrinkable balloon for preventing withdrawal at the distal end of the introduction tube, and is inserted into the body using an insertion assisting rod through which a guide wire can be inserted, so that liquid food or The present invention relates to a catheter and a catheter kit for introducing a nutrient or the like.
As a conventional technique, for example, there is a technique shown in Patent Document 1.
That is, in this document, a balloon is attached to the distal end portion of the introduction tube, which is the catheter body, to prevent it from coming out before the distal end of the catheter body when expanded, and the balloon is bonded without being folded back so as to overlap the introduction tube. Disclosed technology is disclosed.
However, in such a conventional technique, although the balloon is not folded back at the distal end portion of the introduction tube that is the catheter body, the balloon is provided as a whole, and the thickness of the portion to be inserted as a whole increases. In terms of form, the distal end portion is thicker than that of the introduction tube at the time of insertion, which makes it difficult to insert, is troublesome to handle, and may cause pain to the patient.
Therefore, the applicant of the present application solved this problem by the invention shown in Patent Document 2.
However, in the technique of Patent Document 2, when a catheter inserted from outside the body is inserted into an organ through a fistula such as a gastrostoma or an intestinal fistula, the distal end portion of the introduction tube extends from the fistula while minimizing stimulation to the organ. It is not always easy to insert into an organ without detachment.
For such a problem, there is a method in which a guide wire is inserted into the organ in advance, and the introduction tube is reliably inserted along the guide wire into the organ. A catheter that is equipped with a balloon to prevent it from slipping out at the distal end on the insertion side, and that the distal end on the insertion side of the introduction tube can be narrowed during insertion into an organ to ease pain and burden on the patient However, there has been no catheter that can be easily and reliably inserted using a guide wire, and that can ease pain and burden on the patient.
The present invention has been made in order to improve the insertion operation of a catheter having a balloon for preventing slipping off at the distal end portion of the introduction tube, and the balloon portion at the distal end of the introduction tube is thinned and inserted. To provide a catheter and a catheter kit that are easy and easy to operate, can be easily and reliably inserted into an organ along a guide wire, and can relieve pain and burden on a patient. It is aimed.
The gist of the present invention for achieving the object lies in the inventions of the following items. [1] The distal end of the introduction tube (20) is equipped with a balloon (50) that can be expanded and contracted to prevent it from coming out, and is inserted into the body using an insertion assisting rod (60) through which a guide wire (W) can be inserted. In the catheter (10) for introducing liquid food or nutrients into the organ to be inserted,
The introduction tube (20) includes a main lumen (21) into which the insertion auxiliary rod (60) is inserted when the catheter body (11) is inserted into the body, and a sub-conducting fluid to the balloon (50). Lumen (25),
The balloon (50) is in the shape of a rubber balloon, and is attached to the distal end (27) of the introduction tube (20) so as to extend from the distal end (27) first. ) Is formed before the lead-out port (28) at the tip of the lead-out pipe (51) and communicates with the lead-out port (28), and the tip (63) of the inserted auxiliary insertion rod (60) is formed. A receiving portion (500) that is received and held inside is provided on the outer peripheral portion (52),
The catheter (10), wherein the receiving part (500) is formed so that the outside and the inside are communicated by a guide wire passage (520) for passing the guide wire (W).
(10)。 [2] The catheter (10) according to item [1], wherein the receiving part (500) is formed integrally with the balloon (50).
(10)。 [3] The item [1] or [3], wherein the receiving portion (500) has a reinforcing portion (512) on at least a part of a surface that receives the distal end portion (63) of the insertion assisting rod (60). The catheter (10) according to 2].
[4] A catheter kit comprising a combination of the catheter (10) according to item [1] and an insertion assisting rod (60).
The present invention operates as follows.
The catheter (10) is for introducing a fluid such as a liquid food or a nutrient into the inside of the organ. The liquid food or nutrition is supplied from a fistula such as a gastrostoma or intestinal fistula into the organ through the introduction tube (20). Fluids such as agents are injected.
When the catheter (10) is inserted into the organ, the distal end side of the guide wire (W) is previously inserted into the organ from a fistula such as a gastrostoma or intestinal fistula. On the other hand, the catheter (10) contracts the rubber balloon-like balloon (50) provided at the distal end of the introduction tube (20).
The balloon (50) is like a rubber balloon that is deflated and deflated by removing fluid from the balloon (50) from the sub-lumen (25) attached to the main lumen (21) on the introduction tube (20). It becomes a state.
In this state, the introduction tube (20) and the insertion assisting rod (60) of the catheter body (11) are inserted into the organ along the guide wire (W). The insertion assisting rod (60) is inserted into the introduction tube (20). ), And the tip (63) of the auxiliary insertion rod (60) is fitted to the receiving part (500) of the outer peripheral part (52) of the balloon (50), the guide wire (W) can be passed. It becomes extremely easy.
When the insertion assisting rod (60) is inserted into the introduction tube (20), the distal end portion (63) thereof comes out before the distal end portion (27) of the introduction tube (20). The receiving portion (500) of the outer peripheral portion (52) of the balloon (50) is received by the distal end portion (63) of the insertion assisting rod (60).
Subsequently, when the insertion assisting rod (60) is pushed forward, the balloon (50) is stretched so as to extend ahead of the distal end portion (27) of the introduction tube (20), resulting in a conical state with a thin distal end. When the degree to which the distal end portion (63) of the insertion assisting rod (60) protrudes ahead of the distal end portion (27) is increased, the balloon (50) is further pulled and becomes thinner.
Since the guide wire passage (520) through which the guide wire (W) passes from the inside to the outside is formed in the receiving portion (500), the end of the guide wire (W) is connected to the guide wire passage (520). ) Can be inserted. Since the inserted guide wire (W) enters the insertion auxiliary rod (60) from the tip (63) of the insertion auxiliary rod (60) in the receiving portion (500), the guide wire (W) is inserted into the insertion auxiliary rod. It can come out of the grip head (62) on the other end side of (60).
In this manner, the guide wire (W) is passed along the guide wire (W) in a state where the balloon (50) extends thinly ahead of the distal end portion (27) of the introduction tube (20) as described above. If the distal end (27) of the introduction tube (20) is inserted into the organ through the fistula of the gastrostomy or intestinal fistula, it can be inserted smoothly and gently, and it is easy to wear and does not place a burden on the patient. No worries about the occurrence.
After the distal end portion (27) of the catheter body (11) is inserted into the organ through the fistula of the gastrostoma or intestinal fistula in this way, the balloon (50) is expanded more greatly than the gastrostoma or intestinal fistula, Prevents the introduction tube (20) from coming off.
The fluid enters and exits the balloon (50) through the fluid conducting plug (31) provided on the catheter head (30) attached to the injection end of the introduction tube (20) of the catheter body (11). It is. In the catheter head (30), a communication path (32) is formed from the fluid conducting plug (31). This communication path (32) communicates with the end of the sub-lumen (25) that allows fluid to flow to the balloon (50). The fluid conducting plug (31) conducts the sub-lumen (25) to the outside only when the syringe barrel (I) is attached.
When the fluid in the balloon (50) is withdrawn, the balloon (50) can be crushed by sucking the fluid into the syringe barrel (I).
In the mounted state in which the catheter body (11) is inserted into the body, the balloon (50) inflated in a rubber balloon shape is positioned before the outlet (28) at the distal end of the introduction tube (20), and this outlet (28 ) Is led out from the lead-out pipe (51) through which the liquid food or nutrients etc. are communicated. Even if the inflated balloon (50) may touch the inner wall of the organ, the introduction tube (20) does not touch the inner wall of the organ directly. Since this balloon (50) is in the form of a soft rubber balloon, it has no adverse effect on the organ.
The catheter kit in which the catheter (10) and the insertion assisting rod (60) are combined is convenient because it can be used for medical treatment as it is, and is safe without operation failure.
According to the catheter and the catheter kit according to the present invention, the guide wire that has been passed through the tip portion in advance into the organ is passed through the insertion assisting rod via the wire passage of the receiving portion provided in the balloon. The introduction tube can be easily and surely inserted.
When the catheter is mounted, the balloon is stretched to extend beyond the distal end of the introduction tube, and the distal end of the introduction tube is deformed into a thin cone at the distal end from the fistula of the gastrostoma or intestinal fistula to the organ. Since it is inserted through the inside, it can be smoothly and gently inserted, is easier to wear and does not place a burden on the patient, and there is no worry about the cause of a medical accident.
A catheter kit that combines a catheter and an insertion assisting rod is convenient because it can be used for medical treatment as it is, and since it is an appropriate tool, it is safe without operation failure.

FIG. 5 shows a state in which the gripping
Next, the operation will be described.
The
The balloon-
As shown in FIG. 1, before the
The fluid enters and exits the
When the
In this state, if the
FIG. 5 shows a state in which the insertion
As described above, the
Inside the receiving
A state in which the end portion of the guide wire W extends from the gripping
When the
When the
When the
When the
