Claims (33)
- 移植可能なハウジングと、
前記ハウジング内に配置されるコントローラと、
前記コントローラに接続され、患者の心臓の電気的活性化を感知すべく構成された心臓リードシステムと、
前記コントローラに通信可能に接続され、前記電気的活性化によって発生する機械的心臓活動を検出すべく構成されたセンサと、
を含み、
前記コントローラは、検出された電気的活性化と前記検出された機械的心臓活動との間の時差相関を決定すべく構成されシステムであって、
前記コントローラは更に、前記時差相関に基づいて患者に施される治療を制御すべく構成される、
前記システム。 - 前記センサは患者外部のセンサとして構成される請求項1に記載のシステム。
- 前記センサは移植可能なセンサとして構成される請求項1に記載のシステム。
- 前記センサは患者の心臓又は冠状血管中に移植されるべく構成された圧力センサを含む請求項1に記載のシステム。
- 前記センサは左室圧、右室圧、左房圧、右房圧、全身動脈圧又は肺動脈圧を感知すべく構成された圧力センサを含む請求項1に記載のシステム。
- 前記コントローラは、機械的心臓活動を左室圧、右室圧、左房圧、右房圧、全身動脈圧又は肺動脈圧における変化として検出すべく構成される請求項5に記載のシステム。
- 前記センサはインピーダンスセンサを含む請求項1に記載のシステム。
- 前記センサは移植可能なインピーダンスセンサを含む請求項1に記載のシステム。
- 前記センサは患者外部のインピーダンスセンサを含む請求項1に記載のシステム。
- 前記センサは、電気的活性化により発生する機械的心臓活動を示す心室インピーダンスにおける変化を、経胸壁的に又は胸郭内で検出すべく構成されたインピーダンスセンサを含む請求項1に記載のシステム。
- 前記心臓リードシステムは、冠静脈電極を含む第一のリードと、右心室(RV)電極及び右心房(RA)電極の一方又は両方を含む第二のリードとを含み、前記センサは、冠静脈電極とRV電極又はRA電極との間のインピーダンスにおける変化を感知すべく構成されたインピーダンスセンサを含む請求項1に記載のシステム。
- 前記センサは心音を検出すべく構成された音響センサを含む請求項1に記載のシステム。
- 前記センサは心音を検出すべく構成された音響センサを含み、前記音響センサは隔膜ベースの音響センサ、MEMSベースの音響センサ、光ファイバ音響センサ、圧電センサ、加速度計ベースの音響センサ又はマイクロホンを含む請求項1に記載のシステム。
- 前記センサは、S1心音とS2心音の一方又は両方を検出すべく構成された音響センサを含む請求項1に記載のシステム。
- 前記センサは心音を検出すべく構成されたセンサを含み、前記コントローラは心音の存在、特性及び発生頻度のうち1つ以上を検出すべく構成される請求項1に記載のシステム。
- 前記センサは、血流速度か血液灌流を感知すべく構成されたセンサを含む請求項1に記載のシステム。
- 前記センサは、心室の運動又は加速を感知すべく構成されたセンサを含む請求項1に記載のシステム。
- 前記コントローラは、患者の心臓への電気刺激パルスの供給の感知、及び患者の心臓の内因性減極の感知の一方又は両方を行うべく構成される請求項1に記載のシステム。
- 前記時差相関は、左心室のペーシングされた又は内因性電気的活性化と、心臓の全体的な機械的活性化の開始との間の時間間隔を定義する請求項1に記載のシステム。
- 前記コントローラは、前記検出された電気的活性化と前記検出された機械的心臓活動との間の時間間隔を縮小するための治療を施すべく構成される請求項1に記載のシステム。
- 前記治療は心臓ペーシング治療を含む請求項1に記載のシステム。
- 前記治療は両心室の心臓ペーシング治療を含む請求項1に記載のシステム。
- 前記治療は心臓再同調治療を含む請求項1に記載のシステム。
- 前記治療は細動除去治療を含む請求項1に記載のシステム。
- 前記治療は、患者の心臓の心室間のディシンクロニーを改善するための治療を含む請求項1に記載のシステム。
- 前記治療は、患者の心臓の心室内のディシンクロニーを改善するために治療を含む請求項1に記載のシステム。
- 前記治療は、心拡張期及び収縮期の機能障害の少なくとも一方を治療するための治療を含む請求項1に記載のシステム。
- 前記治療は、心拡張期及び収縮期のディシンクロニーの少なくとも一方を治療するための治療を含む請求項1に記載のシステム。
- 前記治療は心室リモデリングリバース治療を含む請求項1に記載のシステム。
- 前記システムは左心室補助装置を含み、前記治療は心室補助治療を含む請求項1に記載のシステム。
- 前記治療はドラッグデリバリ治療を含む請求項1に記載のシステム。
- 前記治療は神経刺激治療を含む請求項1に記載のシステム。
- 前記コントローラは、非捕捉を検出し、且つ時差相関に基づいた捕捉閾値アルゴリズムを開始すべく構成される請求項1に記載のシステム。
