Claims (10)
- 心臓に心臓電気療法を実施する装置であって、
少なくとも1つの電極を備え、かつ、前記心臓の脈間構造内に位置決め可能であるように構成されたリードと、
前記心臓の心筋組織の酸素使用量を示す冠状静脈酸素飽和パラメータを測定するように構成された、前記リードに接続された冠状静脈血液酸素飽和センサと、
植込み可能なハウジングと、
前記ハウジング内に配置され、かつ、前記リードに結合されたパルス発生器と、
前記植込み可能なハウジング内に配置され、かつ、前記パルス発生器及び前記リードに結合されたプロセッサであって、測定された前記冠状静脈酸素飽和パラメータの変化を引き起こすように前記心臓に実施される心臓電気療法を調整して、心臓効率の改善を示す変化後の冠状静脈酸素飽和パラメータに基づいて、実施のために、前記調整された心臓電気療法を選択するように構成されたプロセッサと、
を備える装置。 - 心臓に心臓電気療法を実施する装置であって、
少なくとも1つの電極を備え、かつ、前記心臓の脈間構造内に位置決め可能であるように構成されたリードと、
前記心臓の心筋組織の酸素使用量を示す冠状静脈酸素飽和パラメータを測定するように構成された、前記リードに接続された冠状静脈血液酸素飽和センサと、
植込み可能なハウジングと、
前記ハウジング内に配置され、かつ、前記リードに結合されたパルス発生器と、
拍出量を示す1又は複数のパラメータを検出するように構成されたセンサと、
前記植込み可能なハウジング内に配置され、かつ、前記パルス発生器及び前記センサに結合されたプロセッサであって、前記拍出量を示す1又は複数のパラメータ及び前記パルス発生器によって得られた心拍数パラメータを使用して心拍出量を計算し、前記計算された心拍出量を使用して、前記測定された冠状静脈酸素飽和パラメータの外力仕事負荷による変化を補正するように構成されたプロセッサと、
を備える装置。 - 前記センサが、心臓内インピーダンスセンサである請求項2に記載の装置。
- 心臓に心臓電気療法を実施する装置であって、
少なくとも1つの電極を備え、かつ、前記心臓の脈間構造内に位置決め可能であるように構成されたリードと、
前記心臓の心筋組織の全体的な酸素使用量を示す冠状静脈酸素飽和パラメータを測定するように構成された、前記リードに接続された第1冠状静脈血液酸素飽和センサと、
当該左室の部位から血液が流れ出てくる冠状静脈内に設置することができ、局所冠状静脈酸素飽和パラメータを測定することができるように構成された、前記リードに接続された第2冠状静脈血液酸素飽和センサと、
植込み可能なハウジングと、
前記ハウジング内に配置され、かつ、前記リードに結合されたパルス発生器と、
前記植込み可能なハウジング内に配置され、かつ、前記パルス発生器及び前記第1及び第2冠状静脈血液酸素飽和センサに結合されたプロセッサであって、前記全体的な酸素飽和パラメータと前記局所的な酸素飽和パラメータの差に基づいて局所心臓効率を改善するように、心臓に実施される心臓電気療法を調整するように構成されたプロセッサと、
を備える装置。 - 前記プロセッサは、前記全体的な酸素飽和パラメータと前記局所的な酸素飽和パラメータの一方又は両方に基づいて、還流血液の酸素飽和を増大させるように、心臓に実施される心臓電気療法を調整するように構成されている請求項4記載の装置。
- 心臓に心臓電気療法を実施する装置であって、
少なくとも1つの電極を備え、かつ、前記心臓の脈間構造内に位置決め可能であるように構成されたリードと、
前記心臓の心筋組織の酸素使用量を示す冠状静脈酸素飽和パラメータを測定するように構成された、前記リードに接続された冠状静脈血液酸素飽和センサと、
外力仕事負荷を測定するように構成されたセンサと、
植込み可能なハウジングと、
前記ハウジング内に配置され、かつ、前記リードに結合されたパルス発生器と、
前記植込み可能なハウジング内に配置され、かつ、前記パルス発生器、前記リード及び前記センサに結合されたプロセッサであって、心臓電気療法に摂動が加えられる間の外力仕事負荷の変化をモニタし、酸素飽和変化の方向が心臓効率の改善を示していれば、前記選択された心臓電気療法を更に調整するように構成されたプロセッサと、
を備える装置。 - 前記プロセッサは、所定の外力仕事負荷に対する心筋酸素消費量を最小にするか、又は、心筋酸素消費量の変化が外力仕事負荷の変化によるものでないことを確実にするために、心臓に実施される心臓電気療法を調整するように構成されている請求項6記載の装置。
- 前記プロセッサは、酸素飽和度が最大化されるまで、心臓電気療法を滴定するように構成されている請求項1乃至7の何れか1項に記載の装置。
- 前記冠状静脈血液酸素飽和センサが、心筋毛細血管床から戻る血液の酸素含有量をモニタし、又は、静脈血管又は心臓の大心静脈内に配置されるように構成されている請求項1乃至8の何れか1項に記載の装置。
- 前記パルス発生器が、心臓のぺーシング療法又は再同期療法を実施するように構成されている請求項1乃至9の何れか1項に記載の装置。
