Description
本明細書では本発明を単に例示し図面を参照して説明する。特に詳細に図面を参照して、示されている詳細が例示として本発明の好ましい実施態様を例示考察することだけを目的としており、本発明の原理や概念の側面の最も有用でかつ容易に理解される説明であると考えられるものを提供するために提示していることを強調するものである。この点について、本発明を基本的に理解するのに必要である以上に詳細に本発明の構造の詳細は示さないが、図面について行う説明によって本発明のいくつもの形態を実施する方法は当業者には明らかになるであろう。
図1は、同系のアポトーシス性細胞による治療の後でのMRL/MpJ−Faslprマウスにおける血清中の抗一本鎖DNA抗体の減少を示すヒストグラムである。黒丸、ビヒクルのみにより免疫化された6週齢のMRL/lpr/lprマウスのコントロール群;白丸、同系のアポトーシス性細胞により免疫化された6週齢のMRL/lpr/lprマウスの実験群;黒三角、10週間の治療の後でのコントロール群;白三角、10週間の治療の後での実験群。

図3は、血清枯渇および支持体接着による単球のアポトーシス誘導を示す1組の蛍光活性化細胞分取(FACS)ドットプロットである。70パーセントを超える単球が12時間までにアネキシンV陽性かつPI陰性であり、これは早期アポトーシスを示した。二次的な壊死性細胞が、アネキシンV陽性かつヨウ化プロピジウム(PI)陽性の細胞によって示されるように、細胞の5パーセント未満であった。アポトーシスプロセスの特異性が、20mMのzVAD−fmkの存在下での顕著な阻害によってさらに示された。早期アポトーシス性細胞および二次的壊死性細胞の百分率がそれぞれのヒストグラムの中に示される。データは6つの異なる実験を表す。
図4は、単球の、懸濁+血清枯渇により誘導される死がアポトーシス性でないことを示す1組の蛍光活性化細胞分取(FACS)ドットプロットであり、壊死の特徴を示す。血清枯渇に加えて接触を妨げることにより、死の機構が切り換えられた。細胞数が徐々に減少したが、アネキシン陽性/PI陰性の割合は一定のままであり、かつ、低かった。細胞は直接にアネキシン陽性/PI陽性になり、20mMのzVAD−fmkは死の割合を低下させなかった(図示せず)。
図5a〜図5cは、懸濁+血清枯渇にさらされた単球による前炎症性サイトカイン/ケモカインのmRNAのデノボでの転写を示す。図5a〜図5bは、懸濁+血清枯渇にさらされた単球による前炎症性サイトカイン/ケモカインのmRNAのデノボでの転写を時間0および30分においてそれぞれ示す遺伝子発現アレイ分析である。座標(A2、B2)(これはIL−1βを表す)および座標(E3、F3)(これはIL−8を表す)は、視認される蛍光を時間ゼロで示さず、顕著な蛍光をアポトーシス誘導後30分で示す。基底レベルの何らかの増大がIL−6のcDNA(C3−D3)およびIL−1αのcDNA(E1−F1)について認められる。生存性細胞に関して存在し、死の誘導のときに大きく変化しなかった他のcDNAが、TGF−β1(A7−B7)、IL−2(C2−D2)およびTNF−α(A8−B8)である。MIF(E6−F6)はダウンレギュレーションを示す。蛍光を示さなかったこのメンブランにおける他のウエルは、(A1−B1)がG−CSFについてであり、(C1−D1)がGM−CSFについてであり、(E2−F2)がIL−4についてであり、(A3−B5)がIL−5についてであり、(A4−B4)がIL−10についてであり、(C4−D4)がIL−12αについてであり、(E4−F4)がIL−12βについてであり、(A5−B5)がIL−16についてであり、(C5−D5)がIL−17についてであり、(A6−B6)がLT−βについてであり、(C6−D6)がMCP−1についてであり、(C7−D7)がTGF−β2についてであり、(E7−F7)がTGF−β3についてであり、(C8−D8)がTNF−βについてである。陰性コントロールを表す座標が(G1−G2、PUC18)である:陽性コントロールとして(G3−G4、β−アクチン)および(G5−G6、G7−G8、E8−F8、GAPDH)。ケモカインのメンブランスクリーニングは、IL−8、MIP−1αおよびMIP−1βのアップレギュレーションを示しただけであった(示さず)。メンブランは、エオタキシン、フラクタルカイン、GROa/MGSA、HCC−4、MCP−3、SDF2、PF−4、MDC、HCC−1、I−309、I−TAC、リンホタクチン、MCP−1、MCP−4、MIG、MIP−2、MIP−3α、P10、SDF−1、RANTESを含有した。図5cは、代表的なサイトカインおよびケモカインのcDNAレベル(任意の単位で)の変化を細胞死誘導後の時間の関数として示すデータプロットである。IL−1β、IL−8およびMIP−1αのみがデノボで産生されることに留意すること。
図6aは、前炎症性サイトカインのIL−1βが、懸濁+血清枯渇にさらされた単球によって産生されることを示すデータプロットである。コントロールPBMC(これは20%の単球を含有する)は懸濁+血清枯渇の後でIL−1β分泌の上昇を示す(白丸)。磁石により単離された単球(黒三角)は懸濁+血清枯渇の後でIL−1β分泌の一層より大きい上昇を示す(このことは、瀕死単球がIL−1βの供給源であることを示している)。他方で、懸濁ではなく、血清枯渇にさらされた、磁石により単離された単球(黒四角)(すなわち、接着させられ、懸濁状態に置かれなかった)は、IL−1β(または何らかの前炎症性サイトカイン)を分泌せず、寛容性を樹状細胞において誘導することができた(図示せず)。Bリンパ球およびTリンパ球(黒丸)ならびに多形核細胞(白丸)は、IL−1β分泌が、接着させられなかった単球(すなわち、懸濁および血清枯渇にさらされた単球)に特異的であることを示す。
図6bは、前炎症性サイトカイン/ケモカインのmRNAおよびタンパク質が、懸濁+血清枯渇にさらされた単球によってデノボで転写および翻訳されることを示すELISAデータのヒストグラムである。アクチノマシンDによる転写活性の阻害、および、シクロヘキシミドによる翻訳活性の阻害は、ELISAによってピコグラム/ml(pg/ml)単位で測定されたとき、IL−1βサイトカイン分泌における顕著な阻害を示す。血清枯渇の期間中に接着させられた単球は前炎症性サイトカインの産生を何ら示さなかった(示さず)。さらに、接着させられ、かつ、アポトーシスを誘導する他の方法(例えば、スタウロスポリン、シクロホスファミド、Fasリガンドなど)によって死ぬことが誘発された単球は常に、死のその非炎症性様式を保っていた(図示せず)。
図7aは、生存性単球、高熱誘導による壊死にさらされた単球、または、接着および血清枯渇によりアポトーシスすることが誘導されたアポトーシス性単球からではなく、懸濁誘導による死(および血清枯渇)にさらされた単球によるIL−1βの分泌を示すELISAデータのプロットである。IL−1βのレベル(pg/mlで示される)を、生存性細胞(黒丸)、高熱により壊死性にされた単球(白丸)、または、懸濁誘導による死にさらされた単球(黒三角)をインキュベーションした後、0時間、1時間、4時間および24時間でELISAによって測定した。結果は、懸濁誘導を受けた単球の前炎症性特徴が熱誘導による壊死(偶発的な細胞死)においては見られず、また、接着させられなかった単球の生来的性質であることを示す。
図7bは、懸濁−血清枯渇により誘導された死にさらされた単球からのIL−1β分泌(pg/mlで示される)を示し、また、懸濁(非接着)による単球の死がカスパーゼ3依存的またはカスパーゼ1依存的のいずれでもないことを明らかにするELISAデータのプロットである。懸濁誘導による死にさらされた単球(黒三角、20マイクロモル濃度のDMSOにおいて)によるIL−1βの分泌は、カスパーゼ1阻害剤のZ−WEHD(20マイクロモル濃度、黒丸)または汎カスパーゼ阻害剤のZVAD−fmk(20マイクロモル濃度、白丸)のいずれでも阻害されなかった。
図8a〜図8cは、単球のアポトーシスの期間中における前炎症性サイトカインの分泌がNFκB依存的でないことを示す。図8aは、単球のアポトーシスの期間中における前炎症性サイトカインの分泌がNFκB依存的でないことを示すウエスタン免疫ブロッティングアッセイの写真である。37kDaのIκBおよびリン酸化IκB(黒矢印)が示される。生存性単球(レーンaおよびレーンb)を、MG132(プロテアソーム阻害剤)ととともに(レーンa)、または、MG132を伴うことなく(レーンb)、1mg/mlのザイモサン(これは、成熟し、炎症を生じさせるようにマクロファージサー(macrophagesor)樹状細胞上のTOLL受容体2を誘発する)の存在下で2時間インキュベーションした。レーンcおよびレーンdは、アポトーシスをザイモサン後2時間(レーンc)および10分(レーンd)で[血清枯渇および接着回避(または懸濁)の方法によって]受けている単球である。認められ得るように、ザイモサンにさらされた生存性単球は、MG132の存在下(レーンa)では現れないIκBのリン酸化を示す(レーンb、黒矢印)。単球がアポトーシスを受けるとき、リン酸化が10分(レーンd)または2時間(レーンc)においては見られない。アポトーシス誘導後の5分、20分、30分、40分、60分および90分でのさらなるサンプル(図示せず)は、IκBのリン酸化を示さなかった(5回の実験を表す)。図8bは、MG132の存在下でのIL−1β分泌がわずかに上昇することを示す棒グラフである(3回の実験)。図8cは、MG132の存在下での転写活性を示すヒストグラムである(3回の実験を表す)。mRNAのレベルにおける増大倍数(黒棒)がMG132の存在下(白棒)で変化しないことに留意すること。
図9a〜図9eは、単球のアポトーシスの期間中における前炎症性サイトカインの分泌がp38依存的であることを示す。図9aは、抗Fas阻害抗体(BD29またはZB4)の存在下での24時間後、単球のアポトーシスが、p38阻害剤(p38INH)またはp38および抗fas(ZB4)の存在下で見られるアポトーシスにおける著しい低下*(p<0.001)と比較して、ほんのわずかに低下したことを示す棒グラフである(BD29が示される)。図9bは、P38が、LPSにさらされたか、または、アポトーシスを受けるように誘導された単球において匹敵するレベルで発現することを示すウエスタン免疫ブロッティングアッセイである。図9cは、リン酸化p38がLPS刺激のときに一時的に増大するが、アポトーシスのときには持続した増大を示すウエスタン免疫ブロッティングアッセイである。JNKのリン酸化が見出されなかった(図示せず)。結果は6回の実験を表す。図9dは、(懸濁および血清枯渇によって誘導された)アポトーシス性単球によるIL−1β分泌が特異的なp38阻害剤(p38IN)によって完全に妨げられるが、p38コントロール(DMSO)においては妨げられないことを示す棒グラフである。阻害がJNK阻害剤(JNKIN)の存在下またはそのコントロール(LTAT)では見られない。図9eは、コントロール(DMSO)またはJNK阻害剤(JNKIN)においてではなく、p38阻害剤(p38IN)の存在下でのアポトーシス性単球からのIL−8分泌における顕著な低下を示す棒グラフである。

図12a〜図12dは、アポトーシス性単球の飲み込みが樹状細胞(DC)上の成熟化関連分子の発現をダウンレギュレーションすることを示すFACS分析である。接着したアポトーシス性単球(これは接着および血清枯渇によって作製された)をDiI(瀕死細胞を染色する親油性色素)により染色し、未成熟な樹状細胞(iDC)に4:1の比率で加えた。iDCはアポトーシス性細胞由来のDilを獲得し、LPSにさらされ(実線)、またはさらされなかった(黒線)。抗DR−FITC抗体(図12a〜図12b)および抗CD86−FITC抗体(図12c〜図12d)を使用する、アポトーシス性単球の存在下(図12bおよび図12d)または非存在下(図12aおよび図12c)で処理されたDCのFACS分析が示される。数字は、それぞれのヒストグラムにおいて存在するLPS処理されたiDCのメジアン蛍光を示す。イソタイプコントロールが点線の曲線として示される。アポトーシス性単球の存在下では、CD86およびDRの顕著なダウンレギュレーションが観測されることに留意すること(p<0.0001)。接着していない瀕死単球はDRおよびCD86をダウンレギュレーションせず、DRおよびCD86をアップレギュレーションさえする(図示せず)。類似する結果が、CD40を使用して得られた(図示せず)。これらの結果から、未成熟な樹状細胞(iDC)の成熟化が、接着したアポトーシス性単球によって阻害されることが明らかにされる。
同系アポトーシス性リンパ球の投与による自己免疫疾患(SLE)の治療
序論:全身性エリテマトーデスなどの自己免疫疾患には、満足すべき治療または最適な治療が存在しない数多くの非常に衰弱性および/または致死性の疾患が含まれる。そのような疾患を治療するための最適な方法は、標的化抗原を、個体の免疫系によるそのような抗原に対する寛容性を誘導するような方法で、そのような疾患に冒されている個体の免疫系に提示することであると考えられる。この目標を達成するための最適な方法は、自己のアポトーシス性細胞を用いることであり、これにより、当該技術分野における標準的な治療手段である、毒性の免疫抑制薬物を投与しなければならないことが未然に除かれるか、または、最小限に抑えられると考えられる。様々な取り組みが、先行技術では、自己の細胞を使用して、そのような治療的免疫寛容性を誘導するために提案されている一方で、そのような取り組みには、最適でない有効性、および/または、有効性をヒトにおいて実証することができないことを含めて、様々な欠点がある。本発明を実施に移しているとき、全身性自己免疫疾患の治療を可能にするように免疫寛容性を効果的に誘導する方法が、下記において記載されるように、予想以外にも発見され、それにより、先行技術の制限が克服された。
アポトーシスの誘導:MRL/MpJ−FaslprマウスおよびC3H−SnJマウスをJackson Laboratories(Bar Harbor、ME)から得た。胸腺細胞および脾臓細胞を標準的な方法論に従って4週齢〜8週齢のマウスから調製した。胸腺細胞または脾臓細胞のアポトーシスを、血清枯渇、1マイクロモル濃度のデキサメタゾンまたはガンマ線照射(66rad)のいずれかによって誘導した。アポトーシスを、アネキシン−FITC染色のフローサイトメトリー分析、DNAフラグメント化、および、フラグメント化DNAのヨウ化プロピジウム染色によって確認した。
本実施例において示される研究では、SLE様疾患の古典的動物モデルの1つ、すなわち、MRL/MpJ−Faslprマウスモデルが、疾患の病理発生に対する、同系のアポトーシス性リンパ球の投与の影響を分析するために使用された。MRL/MpJ−Faslprマウスモデルは、Fas(免疫系のアポトーシスおよび誘導された細胞死の活性化を媒介する受容体)における変異のためにSLE様疾患を発症する。SLE患者では、MRL/MpJ−Faslprマウスと同様に、自己抗体および腎臓疾患の発生が最も特異的な病態生理学的パラメーターであるので、それらのパラメーターを、アポトーシス性細胞の投与の後、MRL/MpJ−Faslprマウスにおいて評価した。
エクスビボで懸濁された単球は死(一種のプログラム化された壊死または加速されたカスパーゼ非依存的アポトーシス)を受け、前炎症性媒介因子を産生し、これに対して、支持体接着性のエクスビボ単球は、前炎症性媒介因子の産生を伴うことなくアポトーシスを受ける:先行技術によるアフェレーシス法を改善する方法
序論:移植片対宿主病(GVHD)などの免疫/血液学的疾患には、著しい死亡率および罹患率に関連し、かつ、満足すべき治療/最適な治療が得られていない非常に多数の疾患が含まれる。非常に多数の場合において、そのような疾患を治療するための最適な方法では、アフェレーシス法を行うことが伴う。典型的には、アフェレーシス法は、血液を個体から取り出すこと、血液を分画物に分離し、特定の分画物の治療的治療を行うこと、望ましくない病理学的分画物を除くこと、および、残りを個体に戻すこと、または、所望の分画物を集めることを伴い、望ましくない副作用および/または最適でない有効性が付随する場合がある。従って、アフェレーシスを行うための潜在的に最適な方法では、アフェレーシスを行うための最適な方法およびデバイスの設計を可能にするように、血液成分に対するアフェレーシス法の有害な影響を特定することが伴う。本発明を実施に移しているとき、下記において記載されるように、典型的にはアフェレーシス法の期間中に生じるような、そのエクスビボ懸濁から生じる単球の壊死の誘導および単球による有害な前炎症性媒介因子の付随した分泌が予想外にも発見され、これは、前炎症性媒介因子の分泌の非存在下で単球のアポトーシスを誘導することが驚くべきことに見出された血清枯渇および支持体接着性条件とは対照的であった。そのため、下記に記載される実験結果より、先行技術の制限が、単球を血清枯渇/支持体接着性条件にさらすことを伴うアフェレーシス法では、先行技術によるアフェレーシス法に固有的な有害な前炎症性作用を防止することができることを初めて教示することによって克服される。
細胞の単離および培養:ヒト単核細胞を密度勾配遠心分離によってヘパリン添加の末梢血から単離した。単離された単核細胞を、単球を磁石ビーズ分離(Miltenyi Biotec.、Auburn、CA、米国)によりCD14+の分画物として陽性選択することによって、また、B細胞をCD22+の分画物として陽性選択することによって、また、T細胞をCD14−/CD22−の分画物として陰性選択することによって、単球集団、B細胞集団およびT細胞集団に分離した。純度は、単球については95パーセントを超え、B細胞については95パーセントを超え、T細胞については88パーセントを超えていた。多形核細胞(好中球)を、Plasmateril(GmbH、Bad Homburg、ドイツ)との末梢血のインキュベーションの後で得られた上部分画物の密度勾配遠心分離によって調製した。必要なときには、ペレット中の赤血球を低浸透圧条件のもとで溶血した。抗CD15磁石ビーズを用いて、好中球を、95パーセントを超える純度に精製した。あるいは、単球の単離を、下記で記載されるように、接着によるアポトーシス誘導と同時に行った。
CD14+の単球を単離し、支持体接着+血清枯渇、または、懸濁+血清枯渇のいずれかに供した。驚くべきことに、支持体接着および血清枯渇は細胞のアポトーシスをもたらし(図3)、細胞数の減少が最初の10時間はなく、これに対して、懸濁+血清枯渇は非常に迅速な死をもたらし、細胞数の50パーセントの減少が4時間で達した(図4)。懸濁された単球はほとんど、ヨウ化プロピジウムについて陽性に染色されず、また、懸濁の開始から、細胞のほとんどは、一定の割合でアネキシン−Vおよびヨウ化プロピジウムの両方について陽性であることが見出され(図4)、低いレベルの低二倍体染色を示し(図示せず)、また、細胞数の急激な低下を示した。さらに、単球が懸濁および血清枯渇の方法による死の誘導にさらされたとき、汎カスパーゼ阻害剤のZvad−FMKによる阻害は細胞死を阻害しなかっただけでなく、実際には、細胞死を増大させた(図示せず)。まとめると、これらの結果から、懸濁誘導による死を受けている単球は、アポトーシスではなく、むしろ、壊死を受けることが示唆される。死のこのモードをさらに特徴づけるために、懸濁された単球における様々なサイトカインおよびケモカインのmRNA発現を調べ、これにより、前炎症性媒介因子のIL−1β、IL−8、MIP−1α、MIP−1β、IL−6およびIL−1αのmRNAの転写の著しいアップレギュレーションが明らかにされた(図5a〜図5bおよび図5c)。この転写活性が分泌タンパク質を産生させたかどうかを明らかにするために行われた試験では、懸濁を受けている好中球に対してではなく、単球に対して特異的であるIL−1β(図6a)、IL−8(320±64pg/ml)およびMIP−1α(320±64pg/ml)の高レベルの産生が明らかにされた。IL−4、IL−10、IFN−γおよびTGF−βのmRNAは検出されなかった。コントロールとして、試験を、IL−4、IL−10、IFN−γおよびTGF−βのタンパク質発現を検出するために行った(Quantikine、R&D Systems)。これらのうち、IL−10のみが、前炎症性サイトカインの分泌後、50ng/ml〜100ng/mlで分泌され、懸濁開始後24時間でピークに達することが見出された。他方で、表面接着および血清枯渇にさらされた、磁石により分離された単球はIL−1β(または何らかの前炎症性サイトカイン)を分泌せず(図6a)、寛容性を樹状細胞において誘導することができた(図示せず)。図6bに示されるように、転写阻害剤(アクチノマイシンD)または翻訳阻害剤(シクロヘキシド)による細胞の処理は、懸濁+血清枯渇にさらされた単球によるサイトカイン分泌を劇的に阻害した。他方で、血清枯渇の期間中に接着させられた単球は前炎症性サイトカインの分泌を何ら示さなかった(図示せず)。さらに、接着させられ、かつ、アポトーシスの他の誘導方法(例えば、スタウロスポリン、シクロホスファミドおよびFasリガンドなど)によって死ぬことが誘発された単球は常に、死の非炎症モードを保っていた(図示せず)。まとめると、これらの知見から、転写、翻訳および分泌の特異的な炎症誘発活性が、懸濁+血清枯渇にさらされた単球において開始されることが示唆される。懸濁を受けている単球に対するこれらの観察結果の特異性が、瀕死の好中球、瀕死のリンパ球(図6a)、アポトーシス性単球、または、高熱誘導による壊死にさらされた単球ではなく、懸濁誘導による死を受けている単球のみが前炎症性サイトカインを分泌するという事実によってさらに示された。アポトーシス性単球、生存性単球、および、懸濁により壊死性にされた単球の間でのIL−1β分泌の比較では、分泌が、懸濁誘導による死を受けている細胞に対して特異的であることが示された(図7a)。サイトカイン/ケモカインの分泌がカスパーゼ依存的な機構に関連したかどうかをさらに調べるために、アポトーシスを受けている単球を汎カスパーゼ阻害剤のZvad/fmkにさらした。驚くべきことに、単球の懸濁誘導による死は、ZVAD−fmkによる阻害がないことによって判断されるように、カスパーゼ依存的ではなかった(図7b)。さらに、アネキシン−Vおよびヨウ化プロピジウムによる染色では、アポトーシスによる死ではなく、むしろ、壊死による死が明瞭に示された(図4)。
現時点において開示された実験結果から、他の白血球のエクスビボでの懸濁ではなく、単球のエクスビボでの懸濁が予想外にも、迅速な壊死と、前炎症性媒介因子の付随した産生とを生じさ、これに対して、単球を、その支持体接着を容易にする条件にさらすことにより、そのアポトーシスが、前炎症性媒介因子の分泌の付随した非存在とともに生じることが初めて教示される。これらの新規で、予想外の発見は様々な方法で活用することができ、具体的には、単球のエクスビボでの懸濁および単球のその後の再注入を伴う、炎症関連疾患の治療で使用される最適でない先行技術によるアフェレーシス法(例えば、移植片対宿主病を治療するための体外フォトフェレーシスなど)を改善するために活用することができる。現時点において開示された結果から、そのような方法は、炎症関連疾患のために治療されている患者への前炎症性媒介因子の逆効果的な導入を本質的には伴うので、明らかに最適でないこと、および、そのような先行技術の欠点が、エクスビボで処理された単球を、懸濁の代わりに、支持体接着性の条件にさらすことによって回避され得ることが初めて教示される。
アポトーシス性リンパ球の治療的使用
アポトーシス性リンパ球は、アポトーシス性リンパ球が最も適切な方法で単離され、最も適切な条件で治療的に投与されるならば、また、他の細胞と混合されるならば、(血液からの白血球において自発的には生じない)制御された方法でのみ、免疫抑制的な、寛容化性で、抗炎症性の効果を有する。下記には、アポトーシス性リンパ球を様々な疾患状態の治療のために好適に得て、投与する方法が記載される。
アポトーシス性リンパ球の作製:
1.500ミリリットルまでの自己血液からの10億個までのPBMCの単離、または、白血球アフェレーシスによる100億個までのPBMCの単離。
2.リンパ球表面マーカーのリガンドに共役化された磁石ビーズを使用するPBMCからのリンパ球の単離、または、接着性リンパ球のサブトラクションによるPBMCからのリンパ球の単離。
3.下記の方法の1つ以上によるリンパ球のアポトーシスの誘導:
(i)血清枯渇により誘導されるアポトーシス;
(ii)照射(例えば、UVまたはガンマ線の照射など)を使用する照射誘導によるアポトーシス;
(ii)化学的に誘導されるアポトーシス(スタウロスポリン、シクロホスファミドおよび過酸化水素などの化合物を使用する);および
(vi)デス受容体リガンドにより誘導されるアポトーシス。
4.簡便な分離を使用する5億個までのアポトーシス性リンパ球の回収、および、白血球アフェレーシスを使用する50億個までのアポトーシス性リンパ球の回収。
5.下記経路のいずれかによるアポトーシス性リンパ球の治療的投与:非経口、静脈内、筋肉内、皮下、皮内および経口。
6.疾患または指示に従って手順を繰り返す。
下記に記載される細胞投薬量は70kgの患者について好適であり、体重に従って調節することができる。
1.1.1000万個〜50億個の細胞を、再狭窄を防止するためのステントまたは任意の他の血管内デバイスもしくは血管内手法などを伴う経皮的経管的冠状動脈形成術(PTCA)の24時間前および24時間後に投与する。必要に応じて、1000万個〜50億個の細胞を2週間後に投与する。
1.2.1000万個〜50億個の細胞を、梗塞サイズおよび再灌流傷害を軽減するために急性冠動脈事象のときに投与する。
1.3.1000万個〜50億個の細胞を、1.1.と類似するプロトコルで、ステントの何らかの血管埋め込みのときに投与する。
1.4.1000万個〜50億個の細胞を急性血栓症のときに投与する。
2.1.実質臓器拒絶の防止:1000万個〜50億個の細胞を実質臓器移植前の2時間〜24時間および実質臓器移植後の24時間で投与する(2時間〜24時間前および24時間後)。必要に応じて、1000万個〜50億個の細胞を2週間毎に投与する。
2.2.異種骨髄拒絶の防止:1000万個〜50億個の細胞を骨髄移植前の2時間〜24時間および骨髄移植後の24時間で投与する(2時間〜24時間前および24時間後)。必要に応じて、1000万個〜50億個の細胞を2週間毎に投与する。
2.3.GVHDの防止。
2.3.1.予防的治療:1000万個〜50億個の細胞を移植前の2時間〜24時間および骨髄移植後の24時間で投与する。必要に応じて、1000万個〜50億個の細胞を2週間毎に投与する。
2.3.1.顕在的GVHDについての治療:必要に応じて、1000万個〜50億個の細胞を2週間毎に投与する。
3.1.全身性エリテマトーデス(SLE)の治療:
3.1.1.活動性疾患の治療:必要に応じて、1000万個〜50億個の細胞を2週間毎に投与する。
3.1.2.発赤拡大の防止:必要に応じて、1000万個〜50億個の細胞を2週間〜4週間毎に投与する。
3.2.自己免疫疾患の治療:治療可能な自己免疫疾患には、リウマチ様関節炎、特発性多発性関節炎、多発性硬化症、炎症性腸疾患、強皮症、シェーグレン症候群、多発性筋炎または皮膚筋炎、全身性または局所性の脈管炎、セリアック病、ギラン・バレー症候群、重症筋無力症、I型糖尿病、抗リン脂質症候群、甲状腺炎、グレーヴズ病および乾癬が含まれる。活動性疾患を治療するために、または、発赤拡大を防止するために使用することができる。必要に応じて、1000万個〜50億個の細胞を2週間〜4週間毎に投与する。
4.1.慢性的な炎症性疾患または突発性の炎症関連疾患(例えば、家族性地中海熱(FMF)および他の周期的発熱疾患など)の治療:
4.1.1.発作時に、1000万個〜50億個の細胞を投与する。
4.1.2.発作に対する防止として、必要に応じて、1000万個〜50億個の細胞を2週間毎に投与する。
4.1.3.アミロイドーシスの防止:必要に応じて、1000万個〜50億個の細胞を2週間〜4週間毎に投与する。
アポトーシス性単球の治療的使用
アポトーシス性単球は、アポトーシス性単球が最も適切な方法で単離され、最も適切な条件で治療的に投与されるならば、免疫抑制的な、寛容化性で、抗炎症性の効果を有する。そうでない場合、アポトーシス性単球は前炎症性の壊死を受ける。下記には、アポトーシス性単球を様々な疾患状態の治療のために好適に得て、投与する方法が記載される。
アポトーシス性単球の作製:
1.500ミリリットルまでの自己血液からの10億個までのPBMCの単離、または、白血球アフェレーシスによる100億個までのPBMCの単離。
2.抗CD14抗体共役化磁石ビーズ、支持体接着または遠心分離水簸のいずれかを使用するPBMCからの単球の単離。

2.3.移植片対宿主病(GVHD)の防止:
2.3.1.予防的治療:1000万個〜10億個の細胞を移植前の2時間〜24時間および骨髄移植後の24時間で投与する。必要に応じて、1000万個〜10億個の細胞を2週間毎に投与する。
2.3.1.顕在的GVHDについての治療:必要に応じて、1000万個〜10億個の細胞を2週間毎に投与する。
3.1.全身性エリテマトーデス(SLE)の治療:
3.1.1.活動性疾患の治療:必要に応じて、1000万個〜10億個の細胞を2週間毎に投与する。
3.1.2.発赤拡大の防止:必要に応じて、1000万個〜10億個の細胞を2週間〜4週間毎に投与する。
3.2.自己免疫疾患の治療:治療可能な自己免疫疾患には、リウマチ様関節炎、特発性多発性関節炎、多発性硬化症、炎症性腸疾患、強皮症、シェーグレン症候群、多発性筋炎または皮膚筋炎、全身性または局所性の脈管炎、セリアック病、ギラン・バレー症候群、重症筋無力症、I型糖尿病、抗リン脂質症候群、甲状腺炎、グレーヴズ病および乾癬が含まれる。活動性疾患を治療するために、または、発赤拡大を防止するために使用することができる。必要に応じて、1000万個〜10億個の細胞を2週間〜4週間毎に投与する。
4.1.慢性的な炎症性疾患または突発性の炎症関連疾患(例えば、家族性地中海熱(FMF)および他の周期的発熱疾患など)の治療:
4.1.1.発作時に、1000万個〜10億個の細胞を投与する。
4.1.2.発作に対する防止として、必要に応じて、1000万個〜10億個の細胞を2週間毎に投与する。
4.1.3.アミロイドーシスの防止:必要に応じて、1000万個〜10億個の細胞を2週間〜4週間毎に投与する。
アポトーシス性好中球の治療的使用
アポトーシス性好中球は、アポトーシス性好中球が最も適切な方法で単離され、最も適切な条件で治療的に投与されるならば、また、他の細胞と混合されるならば、(血液からの白血球において自発的には生じない)制御された方法でのみ、免疫抑制的な、寛容化性で、抗炎症性の効果を有する。好中球は、正しく投与されないならば、抗炎症性な、免疫抑制性の効果を阻害し得るプロテアーゼおよび他の内容物を含有する。下記には、アポトーシス性好中球を様々な疾患状態の治療のために好適に得て、投与する方法が記載される。
アポトーシス性好中球の作製:
1.500ミリリットルまでの自己血液からの10億個までの好中球の単離、または、好中球アフェレーシスもしくは白血球アフェレーシスによる100億個までの好中球の単離。
2.下記の方法の1つ以上による好中球のアポトーシスの誘導:
(i)血清枯渇により誘導されるアポトーシス;
(ii)照射(例えば、UVまたはガンマ線の照射など)を使用する照射誘導によるアポトーシス;
(ii)化学的に誘導されるアポトーシス(スタウロスポリン、シクロホスファミドおよび過酸化水素などの化合物を使用する);および
(vi)デス受容体リガンドにより誘導されるアポトーシス。
3.簡便な分離を使用する5億個までのアポトーシス性好中球の回収、および、白血球アフェレーシスを使用する50億個までのアポトーシス性好中球の回収。
4.下記経路のいずれかによるアポトーシス性好中球の治療的投与:非経口、静脈内、筋肉内、皮下、皮内および経口。
5.疾患または指示に従って手順を繰り返す。
下記に記載される細胞投薬量は70kgの患者について好適であり、体重に従って調節することができる。
1.1.1000万個〜50億個の細胞を、再狭窄を防止するためのステントまたは任意の他の血管内デバイスもしくは血管内手法などを伴う経皮的経管的冠状動脈形成術(PTCA)の24時間前および24時間後に投与する。必要に応じて、1000万個〜50億個の細胞を2週間後に投与する。

2.3.1.顕在的GVHDについての治療:必要に応じて、1000万個〜50億個の細胞を2週間毎に投与する。

本発明の接着したアポトーシス性単球は、未成熟な樹状細胞(IDC)のLPS誘導による成熟化を阻害することができる
未成熟な樹状細胞(iDC)はLPSまたは他のTOLL様受容体リガンドに対して劇的に応答し、免疫学的応答を開始させるために成熟化を受ける。LPSにより誘導されると、iDCによるDR(MHCクラスII)の発現が、CD86のような分子の共刺激と一緒にアップレギュレーションされる。
Claims (34)
- 病理学的免疫応答によって特徴づけられる疾患を治療するために特定される医薬品を製造するための、瀕死白血球または死白血球を含む細胞調製物の使用であって、前記瀕死白血球または死白血球は、生きている白血球を、表面に接着するように誘導することによって得られ、前記病理学的免疫応答を抑制することができる、使用。
- 病理学的免疫応答によって特徴づけられる疾患を治療するために特定される医薬品を製造するための、瀕死白血球または死白血球を含む細胞調製物の使用であって、前記瀕死白血球または死白血球は、IL−1β、IL−8、MIP−1α、MIP−1β、IL−6およびIL−1αからなる群から選択される前炎症性媒介因子を分泌せず、前記病理学的免疫応答を抑制することができる、使用。
- 前記瀕死白血球または死白血球は、生きている白血球を、インビトロ血清枯渇、ステロイドまたはステロイド誘導体による処理、照射、および、プロアポトーシス処理からなる群から選択される細胞破壊処理に供し、それにより前記瀕死白血球または死白血球を作製することによって得られる、請求項1または2に記載の使用。
- 前記病理学的免疫応答は少なくとも1つの抗原に対して向けられ、かつ、前記瀕死白血球または死白血球は前記少なくとも1つの抗原を含む、請求項1または2に記載の使用。
- 前記瀕死白血球または死白血球は対象に由来する、請求項1または2に記載の使用。
- 前記瀕死白血球または死白血球は、瀕死脾臓細胞または死脾臓細胞、および/または、瀕死胸腺細胞または死胸腺細胞を含む、請求項1または2に記載の使用。
- 前記瀕死白血球または死白血球は瀕死リンパ球または死リンパ球を含む、請求項1または2に記載の使用。
- 前記瀕死白血球または死白血球は瀕死単球または死単球を含む、請求項1または2に記載の使用。
- 前記瀕死白血球または死白血球は瀕死好中球または死好中球を含む、請求項1または2に記載の使用。
- 前記瀕死白血球または死白血球はアポトーシス性白血球を含む、請求項1または2に記載の使用。
- 疾患は全身性の自己免疫疾患である、請求項1または2に記載の使用。
- 疾患は抗体媒介による自己免疫疾患である、請求項1または2に記載の使用。
- 疾患は紅斑性狼瘡である、請求項1または2に記載の使用。
- 疾患は移植関連疾患である、請求項1または2に記載の使用。
- 疾患は移植片対宿主病である、請求項1または2に記載の使用。
- 前記医薬品は、対象の体重1キログラムあたり約2000万個〜約20億個の細胞の範囲から選択される前記瀕死白血球または死白血球の治療効果的な量を含む、請求項1または2に記載の使用。
- 前記医薬品は、対象の体重1キログラムあたり約400万個〜約20億個の細胞の範囲から選択される前記瀕死白血球または死白血球の数を含む前記細胞調製物の治療効果的な量を含む、請求項1または2に記載の使用。
- 白血球のアポトーシスを誘導し、それによりアポトーシス性白血球を得るための1つ以上のアポトーシス誘導チャンバーを含む、白血球のアポトーシスを誘導するためのデバイスであって、前記1つ以上のアポトーシス誘導チャンバーは、単球のアポトーシスを誘導するための第1のチャンバー、好中球のアポトーシスを誘導するための第2のチャンバー、および、リンパ球のアポトーシスを誘導するための第3のチャンバーからなる群から選択される、デバイス。
- 前記第1のチャンバーは、表面への単球の接着を高めるための表面を含む、請求項18に記載のデバイス。
- デバイスはさらに、単球のアポトーシスを誘導するための単球培地を含有するための第1のリザーバーを含む、請求項19に記載のデバイス。

前記表面接着性の単球を前記第1のチャンバーの前記表面から掻き取るための掻き取り機構
からなる群から選択される、請求項23に記載のデバイス。 - 前記1つ以上のアポトーシス誘導チャンバーは、
アポトーシスを誘導するための照射機構;
アポトーシスを誘導するための機械的機構;および、
アポトーシスを誘導するための化学的または生化学的な物質または環境
からなる群から選択されるアポトーシス誘導機構を含む、請求項18に記載のデバイス。 - 病理学的免疫応答によって特徴づけられる疾患を治療するためのデバイスであって、
(a)血液を対象からデバイスの中に送り、かつ、血液をデバイスから前記対象に戻すためのポンプ;
(b)循環している白血球を全血から分離するために前記ポンプと連絡している白血球分離器;および、
(c)前記白血球のアポトーシスを誘導し、それによりアポトーシス性白血球を得るために前記白血球分離器と連絡し、かつ、前記アポトーシス性白血球を対象に投与するために前記ポンプとさらに連絡している1つ以上のアポトーシス誘導チャンバー
を含む、デバイス。 - 前記アポトーシス誘導チャンバーは、単球のアポトーシスを誘導するための第1のチャンバーと、好中球のアポトーシスを誘導するための第2のチャンバーと、リンパ球のアポトーシスを誘導するための第3のチャンバーとを含む、請求項26に記載のデバイス。
