Abstract
【解決手段】自動レート調整ペーシング治療を送出することが可能な埋め込み可能心臓刺激デバイスは、心筋被刺激性の指示に応答する調整可能な上方レート限界値を有し、ペースメーカ、ペースメーカ/カーディオバータ/ディフィブリレータ等として具現することができ、心筋被刺激性の指標として不整脈の検出に応答して、上方レート限界値を調整する。調整された上方レート限界値は、「長期」ペーシング治療として以前に定義された任意のペーシング治療の送出中に、最高許容可能ペーシングレートとして適用される。

Claims (10)
- 最高許容可能ペーシングレートを制御する方法であって、
心筋被刺激性の指標として不整脈を検出すること、
前記検出された不整脈に応答して、上方ペーシングレート限界値をプログラムされている上方レート限界値より低いレートに調整すること、及び
選択されたペーシング治療の送出中に、前記調整された上方レート限界値を前記最高許容可能ペーシングレートとして適用すること
を含む、最高許容可能ペーシングレートを制御する方法。 - 前記選択されたペーシング治療は、長期ペーシング治療として以前に分類されたペーシング治療である、請求項1に記載の最高許容可能ペーシングレートを制御する方法。
- 前記ペーシング治療は、少なくともほぼ8つの連続した心周期にわたってペーシングパルスを送出するペーシング治療である、請求項1に記載の最高許容可能ペーシングレートを制御する方法。
- 前記不整脈を検出することは、心室頻脈を検出することを含む、請求項1に記載の最高許容可能ペーシングレートを制御する方法。
- 前記上方ペーシングレート限界値を調整することは、履歴心拍数データを使用することを含む、請求項1に記載の最高許容可能ペーシングレートを制御する方法。
- 前記上方ペーシングレート限界値を調整することは、履歴虚血検出データを使用することを含む、請求項1に記載の最高許容可能ペーシングレートを制御する方法。
- 前記調整された上方レート限界値が有効である持続期間にタイマをセットすること、及び、該タイマの終了時に、前記選択されたペーシング治療について、前記プログラムされている上方レート限界値を前記最高許容可能ペーシングレートとして回復させることをさらに含む、請求項1に記載の最高許容可能ペーシングレートを制御する方法。
- 前記調整された上方レート限界値が有効である持続期間に前記タイマをセットすることは、履歴虚血エピソード持続期間データを使用することを含む、請求項7に記載の最高許容可能ペーシングレートを制御する方法。
- 前記調整された上方レート限界値が有効である持続期間に前記タイマをセットすることは、安静時心拍数が、或る活動期間後に戻るのに必要とされる時間に相当するデータを使用することを含む、請求項7に記載の最高許容可能ペーシングレートを制御する方法。
- 埋め込み可能心臓刺激デバイスであって、
不整脈検出モジュールと、
前記不整脈検出モジュールによる不整脈の検出に応答して、調整された上方ペーシングレート限界値を設定するための、前記不整脈検出モジュールに応答するペーシングレート制御モジュールと、
前記ペーシングレート制御モジュールに応答して刺激パルスを送出するパルス発生器と
を備える、埋め込み可能心臓刺激デバイス。
