Abstract
Description
内部固定によって治療される骨折の回復は、骨内膜経路及び骨膜経路によって提供される血液供給に依存する。骨内膜経路及び骨膜経路は、骨折時だけでなく、後に治療によって損傷されるおそれがある。慣用のプレートは、骨にできるだけ密着しかつ骨に密にねじ留めされるように形成されているので、骨膜血液供給に対して特に高い危険性を生じる。このことは、骨膜、及び血液供給のために骨膜を必要とする骨を死なせる。死んだ骨は回復することができず、感染症の特に高い危険性を生じる。リモデリングによって骨は回復されることができるが、元の形状及び機械的特性にはめったに回復しない。
Allgowerに発行された米国特許第3552389号明細書は、骨折の内部固定における事実上のスタンダードになった、いわゆる圧迫プレート、すなわちダイナミック圧迫プレートを開示している。このプレートにおけるねじは、プレートと骨片との間の軸方向変位を生ぜしめ、骨折部における骨片の圧迫を生ぜしめるために使用されることができる。これらの、今や一般的なプレートの主な欠点は、プレートと骨との広範囲な接触、及びその結果としての骨膜血液供給の破壊である。
本発明は、上面と、下面と、上面から下面まで延びた、少なくとも2つの好適には円形の穴とを有する、骨折を治療するためのプレートに関し、前記穴が、実質的に平坦な下面における凹所によって包囲されている。
プレート100の断面が、図1に、骨に面した下面の標準投影図法で示されており、ねじ穴4に沿って切断した横断面A−Aと一緒に示されている。プレート100の上面2は下面3に対して平行であるのに対し、プレート1の側部は有利には湾曲及び傾斜させられており、これにより、プレートの上面をより狭幅にし、プレート上における柔軟な組織の閉鎖を容易にさせている。ねじ穴4は、プレートの下面3において凹所5によって包囲されている。凹所5の目的は、プレートとねじを包囲する骨との潜在的な接触を排除することである。プレート100の下面は実質的に平坦である。
Claims (19)
- 骨折治療用のプレート(100)において、上面(2)と、骨に面した下面(3)と、骨への取付けのための少なくとも2つの穴(4)とが設けられており、該穴が、前記上面(2)から前記下面(3)にまで延びており、前記穴が、骨に面した下面(3)において凹所(5)によって包囲されており、前記穴(4)のうちの少なくとも幾つかの穴と穴との間に横方向溝(9)が配置されていることを特徴とする、骨折治療用のプレート。
- 前記横方向溝(9)が、プレートの下面(3)と骨との潜在的な接触をさらに減じ、前記穴ごとに元の下面(3)の2つの小さな領域(10)のみをそのまま残している、請求項1記載のプレート。
- 横方向溝(9)が下面(3)において凹所(5)の間の領域に亘って延びている、請求項1又は2記載のプレート。
- 横方向溝が、下面(3)において凹所(5)の一部と少なくとも部分的に重なり合っている、請求項1から3までのいずれか1項記載のプレート。
- 横方向溝の深さが凹所(5)の深さと等しいか又は凹所の深さよりも大きい、請求項1から4までのいずれか1項記載のプレート。
- 前記凹所が円環状である、請求項1から5までのいずれか1項記載のプレート。
- プレートの幅(106)が前記穴(4)の直径(102)よりも2.5〜3.5倍大きく、前記凹所の直径(103)が前記穴(4)の直径(102)よりも1.5〜2倍大きい、請求項1から6までのいずれか1項記載のプレート。
- 凹所(5)の深さ(104)がプレートの厚さ(105)の4分の1である、請求項1から7までのいずれか1項記載のプレート。
- 上面(2)に隣接した上側区分(11)における穴(4)が円錐形である、請求項1から8までのいずれか1項記載のプレート。
- 穴(4)の円錐形の区分(11)が、30゜以上の、自己ロッキング範囲よりも上の円錐角(108)を有している、請求項9記載のプレート。
- 穴(4)の中間から下側までの区分(12)にねじ山が設けられている、請求項9又は10記載のプレート。
- 中間から下側までの区分における穴(4)の長手方向アスペクトに延びた角度解放切断部(13)が設けられている、請求項1から11までのいずれか1項記載のプレート。
- 前記プレートの形状が、前記穴(4)のうちの少なくとも幾つかの穴と穴との間に間隔を置かれた側部切断部(14)によってさらに変更されている、請求項1から12までのいずれか1項記載のプレート。
- 穴と穴との間における断面が、穴を通る断面よりも小さな抵抗モーメントを有している、請求項13記載のプレート。
- 請求項1から14までのいずれか1項記載のプレートと、少なくとも1つのねじ穴プラグ(40)とを含むことを特徴とする構造体。
- 請求項1から14までのいずれか1項記載のプレート又は請求項15記載の構造体と、少なくとも1つのロッキングねじ(20)とを含むことを特徴とする構造体。
- 請求項1から14までのいずれか1項記載のプレートか、又は請求項15又は16記載の構造体と、少なくとも1つの標準的な骨接合用ねじ(30)とを含むことを特徴とする構造体。
- 骨折治療用の装置を製造するための、請求項1から14までのいずれか1項記載のプレート又は請求項15から17までのいずれか1項記載の構造体の使用。
- 請求項1から14までのいずれか1項記載のプレート又は請求項15から17までのいずれか1項記載の構造体を、必要としている主体の折れた骨に対して適用することを含む、骨折の治療のための方法。
