Description
よって、より侵襲的でなく、病理的動脈リモデリングを効率的に治療することを可能にする新規な手段を有することが必要であると考えられる
歯肉繊維芽細胞は、歯肉の軟結合組織中で遊走、接着及び増殖できる間葉細胞であり、機械的外力、細菌感染、pHの変動、温度などの多数の攻撃に付される歯肉組織の完全性を維持する(GOGLYら, Clin. Oral Invest. 1: 147〜152, 1997; GOGLYら, Biochem. Pharmacol. 56(11): 1447〜1454, 1998; EJEILら, J. Periodontol. 74(2): 188〜195, 2003)。
それらが付される環境条件に応じて、歯肉繊維芽細胞は表現型を変更でき、かつ増殖、遊走又は細胞外マトリックス成分の合成若しくは分解を介して歯肉組織環境からの刺激に応答することができる。
同様の知見が、歯肉繊維芽細胞と損傷を受けた動脈との共培養においても得られている。この器官培養型モデルは、歯肉繊維芽細胞の存在が、弾性ネットワークの保護を誘導することを示すことも可能にしている。
本発明の別の好ましい実施形態によると、上記の病変は、血管形成術後狭窄又は再狭窄である。
本発明を行うために用いることができる歯肉繊維芽細胞培養物は、当業者にそれ自体で知られる従来の技法により得ることができる(BARLOVATZ-MEIMONら, "culture de cellules animales" ["Animal cell culture"] p. 898, ill. Paris: INSERM, 2003)。
繊維芽細胞の標識
歯肉繊維芽細胞は、WILHEMら(Biomaterials 24: 1001〜1011, 2003)により記載されるようにして、磁赤鉄鉱のアニオン性ナノ粒子を用いて標識する。
歯肉繊維芽細胞(6つの異なる培養物)は、10%胎児ウシ血清(FCS)中で集密まで培養し、48時間後に、培養上清を除去し、細胞を血清フリー培地で培養する。
これらのナノ粒子での歯肉繊維芽細胞の標識の優良性を、パール(Perl)染色(プルシアンブルー)により制御する。ナノ粒子は吸収され、図1に示すように、歯肉繊維芽細胞のエンドソーム内に内部移行される。
D1において、標識された繊維芽細胞でMMP2及びIL-1βの産生の増加が観察され、この産生は、D3及びD5において通常のレベルに戻る。この一過性の増加は、おそらく、ナノ粒子の取り込みの後のストレスによるのだろう。一方、研究期間の間に、TGFβの産生の変化は観察されない。
標識された歯肉繊維芽細胞(FG)と平滑筋細胞(SMC)との共培養を、GILLERYら(Experinetia 45(1): 98〜101, 1989)に記載された方法に従ってコラーゲンゲル中で行う。
簡単に、細胞は、歯肉繊維芽細胞については歯肉サンプルから、そして平滑筋細胞については動脈中膜から得る。サンプル(歯肉及び動脈中膜)を、ペトリ皿でDMEM培地/20% FCS中の初代培養に付す。集密において、細胞をトリプシン処理し、DMEM培地/10% FCS中で再び培養に付す。この培地中で数世代を経過した後に、細胞をコラーゲンI中で(2 mlコラーゲン中に60000個の細胞) 3、7、14又は21日間培養する。ゲルを浸している培養培地(DMEM/10% FCS)は、毎週交換する。
同じ条件下で、歯肉繊維芽細胞培養と平滑筋細胞培養とを別々に、コントロールとして行う。
MMP9分泌は、免疫検出によりD3に見出され、光学顕微鏡(×160)により観察する。
MMP9は、繊維芽細胞において検出されない。これは、ゲル中のSMCの位置に対応する部位でのみ検出される。これらの結果は、以前の研究を裏付けるものであり、この研究によると繊維芽細胞はMMP9を発現しない(GOGLYら, 1998, 上記)。
ザイモグラフィによるMMP9及びMMP2の活性の評価
FG及び/又はSMCの培養のサンプル20μlを、50%グリセロール及び0.4%ブロモフェノールブルー含有1M Tris、pH 6.8中で1/2に希釈し、1 mg/mlのα-カゼイン(Sigma Chemical)を含有するSDS 10%ポリアクリルアミドゲルでの1時間の電気泳動に付す。ゲルを、蒸留水で希釈した2.5% Triton X-100中で洗浄し、次いで、100 mM Tris-HCl、5 mM CaCl2、0.005% Brij 35、0.001% NaN3、pH 7.4中で36時間、37℃にてインキュベートする。次いで、ゲルを0.25%クーマシーブルー(Biorad, ref. G 250) (50%メタノール、10%酢酸)で染色し、次いで適切に脱染する(40%エタノール、10%酢酸)。結果を図2に示す。

E: コラーゲンゲル中で共培養した600個の標識繊維芽細胞と60000個のSMCについてのMMP2及びMMP9活性。
全RNA (1又は2μg)を、D14のFG及びSMCの培養又は共培養から、MMP-CytoXpress Multiplex PCRキット(BioSource International)を用いて抽出する。得られたmRNAを、逆転写酵素を用いて逆転写し、次いで、PCRを、MMP9特異的プライマーを用いて次のようにして行う:95℃にて1分間の変性工程、94℃にて1分間の変性及び60℃にて4分間のハイブリダイゼーションを5サイクル、94℃にて1分間の変性及び68℃にて2.5分間のハイブリダイゼーションを35〜40サイクル、70℃にて10分間の最終工程、続いて20℃にて冷却。構成的に転写されるGAPDHをコントロールとして用いる。得られたPCR産物を、2%アガロースゲル電気泳動に付す。ゲルをUVの下で調べ、写真撮影をする。結果を図3に示す。

MMP9阻害剤(TIMP-1)は歯肉繊維芽細胞により発現され、MMP9の活性形を、不活性なMMP9/TIMP-1複合体を形成することにより阻害する。
D3、D7、D14及びD21 (培養日数)でのTIMP-1の発現により、細胞相互作用を研究する。
コラーゲンゲル中でのFGとSMCの共培養の上清の一定量(1 ml)を、DMEM培地中で100μlの最終容量にし、細胞破砕物を除去するために10000 gで遠心分離し、次いで、10μlの1M Tris HCl、150 mM NaCl、pH 7.5の溶液を加える。コントロールとして、FG単独又はSMC単独からの培養上清を、同じ条件下で調製した。サンプル(5μl)を、ニトロセルロースメンブレン(Biorad, ref. 1620115)上に三重に置く。メンブレンを、1%ブロッキング溶液(Boehringer, ref. 1096176)と周囲温度にて1時間インキュベートし、TBS-Tween (50 mM Tris、150 mM NaCl、0.1% Tween 20、pH 7.5)中で4×15分間リンスする。メンブレンを、ヤギ抗ヒト抗TIMP-1ポリクローナル一次抗体(1/500、R&D Systems, ref. AF970)と、周囲温度にて一晩インキュベートする。一次抗体は、TIMP-1の遊離形に特異的である。TBS-Tween中での洗浄後(4×15分)、ブロットをペルオキシダーゼ標識二次抗体(1/1000)と1時間インキュベートし、次いで、過酸化水素及びジアシルヒドラジド含有現像溶液(ルミノール, Boehringer, ref. 1500694)中に1分間入れ、数秒〜10分の期間、Kodak BIOMAX MR写真フィルムと接触させる。次いで、フィルムを現像して固定する。結果の直線性を確かめるために、複数の露光について調べる。結果を図4Aに示す。

これらの全ての結果から、FGと共培養されたSMCによるMMP9活性の減少は、該MMP9の合成が減少したことによるのではなく、その阻害剤であるTIMP-1の合成の増加、それにより不活性なMMP9/TIMP-1複合体の増加によることが明らかである。
細胞相互作用を、サイトカインTGFβのD3、D7、D14及びD21/D28 (培養日数)での発現により研究する。
TGFβ産生を、DuoSet (登録商標) ELISA Developmentキット(R&D Systems, ref. DY240)を製造業者の使用説明に従って用いるELISAにより評価した。D3、D7、D14及びD21でのELISAアッセイの結果を、図7に示す。

TGFβ産生を、ドットブロットにより、マウス抗TGFβ1モノクローナル抗体(R&D Systems, ref. MAB 240)を用いて、TIMP-1の場合に上述したプロトコルを用いて評価した。D3、D7、D14及びD28でのドットブロットにより得られた結果を図8に示す。

皮膚繊維芽細胞(FD)又は外膜繊維芽細胞(FAs)と平滑筋細胞(SMC)との共培養を、コラーゲンゲル中で、歯肉繊維芽細胞について上記の実施例1で説明したものと同じプロトコルを用いて行う。
細胞は、皮膚繊維芽細胞については皮膚サンプルから、外膜繊維芽細胞については外膜サンプルから、及び平滑筋細胞については動脈中膜サンプルから得る。
細胞相互作用を、D7及びD14に、上記の実施例1で説明したようにザイモグラフィにより培養培地に分泌されたMMP2及びMMP9の活性を決定することにより調べる。並行して、20μlの組換えMMP9 (R&D Systems, ref. 911 MP)を、分析されるMMPの種類を確かめるために、同じ電気泳動に付す。結果を図9に示す。

H: コラーゲンゲル中で共培養した600個のSMCと600個のFAについてのMMP2及びMMP9活性。
コラーゲンゲル中の損傷を受けた動脈の三次元培養を、時間経過に伴う動脈リモデリングを分析するためのモデルを提供するために開発した。
5匹のニュージーランドホワイト種のウサギに、LAFONTら(Circ. Res. 76(6): 996〜1002, 1995; Circulation 100(10): 1109〜1115, 1999)により記載されるようにして、空気乾燥(air desiccation)と高コレステロール食餌の組み合わせにより、アテローム性動脈硬化損傷を誘導する。4週間後に、アテローム性動脈硬化損傷の存在を、2本の大腿動脈内部の動脈造影により確かめる。次いで医原性損傷を、血管形成バルーンカテーテルを用いて(6 atm、60秒間での3回の吹送)、左大腿動脈に発生させる。
各セグメントを、6 mlの培養培地(DMEM/20% FCS、通常の抗生物質及び抗真菌剤)、3.4 mlのラットI型コラーゲン(Jacques Boy Institut, REIMS, France)及び600μlのろ過済み0.1N NaOHを含む10 mlのエルレンマイヤーフラスコに入れ、溶液とともにホモジナイズする。最後に、混合物全体を、37℃/5% CO2のインキュベータ中の培養皿に移す。毎週、蒸発の埋め合わせのために培養培地1 mlを上清に加える。
5つの系列の動脈の培養上清を、-80℃で貯蔵する。
厚さ7μmの切片をミクロトームで切断し、3つの異なる特定のプロトコルを用いて染色する。
ヘマラン(Hemalaun)-エオシンプロトコル:再水和の後に、切片をヘマランで5分間被覆し、その後、水道水、次いでエオシンで1分間分染する。顕微鏡調製物を最後に蒸留水でリンスし、脱水して最後に集合させる。
カテキン(+)-フクシンプロトコル:部分的再水和(95°アルコールで止める)の後に、切片を暗所にて2.5時間、染色溶液に浸漬する。95°アルコール(2滴の塩酸を含む)で迅速にリンスした後に、脱水して最後に集合させる。
ヘマラン-エオシン染色切片を、コンピュータ(BIOCOM 200ステーション)に連結した顕微鏡で観察する。細胞の半自動計数は、核を区別することにより可能になる。結果は、単位表面積当たりの細胞数として与えられる。
内膜(AAP)及び新内膜(AAA)の増殖のうち、単位表面積当たりの細胞数は、時間の経過につれて著しく減少する。この減少は、D3とD7の間で非常に有意であり(p<0.001)、D7とD14の間で有意であり(p<0.01)、D14とD28の間で比較的有意である(p<0.05)。
コラーゲンネットワーク内では、単位表面積当たりの細胞数は増加する。この増加は、D3とD7の間で非常に有意であり(p<0.001)、D7とD14の間で有意である(p<0.01)。この増加は、D14とD28の間ではもはや有意ではない。閾値効果(threshold effect)がD28で干渉していると考えられる。
同じイメージデジタル化プロセスを用いて、中膜の厚さの測定及び内膜増殖(AAP)及び新内膜増殖(AAA)の厚さの測定を行う。これらの測定は、培養での動脈の内部の6つの異なる領域において、10〜12回行う。
これらの測定は、時間の経過に伴ういずれの著しい変化も示さない。
コラーゲンネットワーク成分
イメージデジタル化の後に、ソフトウェアは、コラーゲン成分の相対表面積を評価できる(シリウスレッドで染色された切片について、繊維状コラーゲンを明らかにするために偏光の下で観察された)。
コラーゲンネットワーク成分は、内膜増殖(AAP)及び新内膜増殖(AAA)において評価される。これらの2つの場合において、コラーゲンネットワークの密度の増加が、時間経過に伴って観察される。さらに、測定が偏光の下で観察される切片について行われるので、観察された増加は、コラーゲンネットワークの繊維状成分に関する。
イメージデジタル化の後に、ソフトウェアは、弾性ネットワークの相対表面積を評価できる(カテキン(+)-フクシンで染色された切片について)。
中膜内の弾性ネットワーク成分の相対表面積は、時間経過に伴って減少を示す。AAPの場合、減少は、D3とD14の間で有意であり(p<0.01)、D14とD28の間で非常に有意である。AAAの場合、減少は、D14とD28の間でのみ有意である(p<0.06)。しかし、初期のAAP (D3にて)は、AAAよりも著しい弾性成分を有する。しかし、D28において、弾性ネットワーク成分は、これらの2つの系列の動脈の間で実質的に等しい。
ヘマラン−エオシンで染色された切片を、UV下で電子顕微鏡により観察する。切片の写真を撮影し、写真をコンピュータにより調べた後に、2つの断片化ポイント間の弾性繊維の長さを測定する。この長さは、断片化の状態を反映する。繊維の長さが短いほど、断片化がより著しい。
AAAにおいて、培養第1日から、弾性膜は組織化されておらず、断片化されている。D3におけるこの断片化は、AAPについてD28に観察されたものと類似している。しかし、この断片化現象は、時間の経過とともに、AAPの場合と同様に著しく進行し続ける。このモデルにおいて、UV顕微鏡の結果は、損傷を受けた動脈の弾性繊維の長さがD3の466μmからD28の87μmに減少することを示す。
MMP-1、-2及び-3、並びにTIMP-1及び-2の分泌
AAA及びAAP動脈の器官培養型培養の培地中に分泌されたMMP1、MMP2、MMP3、TIMP-1及びTIMP-2の発現を、上記の実施例1に記載されたようにして、これらのMMP及びTIMPに指向されたヒト抗体(Valbiotech)を用いて、ドットブロットにより分析する。コントロールとして、これらのMMP及びTIMPの存在を、培養血清(FCS)においても調べる。結果を図10に示す。

E:TIMP-2発現。
- AAA培養物における濃度がAAP培養物のものよりも著しく高い、MMP-1 (A)及びMMP-3 (B)の発現の著しい増加。培養血清中にはMMP1及びMMP3のいずれも存在しない;
- MMP2発現の第1週(D7)の間の増加(C)、及び第2週(D14)の間の減少、次いでD28でその最大に到達する新たな増加。MMP2は、血管形成(AAA)の後においてAAP培養物よりもかなり多く発現されたままであるが、同じ現象が2種の動脈培養において観察される。さらに、MMP2濃度は、培養上清におけるよりも血清中で著しく低い;
- 最初の1週間のTIMP-2 (E)の発現の増加、その後、AAP培養物におけるよりもかなり低いレベルでの停滞。血管形成後では、TIMP-2発現のピークが第1週に観察され、その後、非常に高い濃度で停滞する。さらに、TIMP-2は、AAP培養物におけるよりも血管形成後により高く発現されたままである。TIMP-2は、培養血清中に存在しない。
AAA 及びAAP動脈の器官培養型培養の培地中に分泌されるMMP2及びMMP9の発現を分析する。
70μlの動脈(AAP及びAAA)器官培養型培養上清を、50%グリセロール及び0.4%ブロモフェノールブルーを含有する1M Tris、pH 6.8中で3/5に希釈し、1 mg/mlのブタ皮膚ゼラチン(Sigma, ref. G2500)を含有するSDS 10%ポリアクリルアミドゲル中で、Laemmliバッファー中で4℃にて、濃縮ゲルで80ボルト及び分離ゲルで180ボルトの電気泳動に付す。移動の後に、ゲルを2.5% Triton X-100中で洗浄し(周囲温度にて30分間、2回)、次いで生理食塩緩衝溶液中で37℃にて48時間インキュベーションする。次いで、ゲルを0.25%クーマシーブルー(Biorad, ref. R250)で染色し、脱染する。酵素活性のあるバンドが、図11に示すように半透明となって現れる。
この結果は、D3とD28の間のMMP2及びMMP9の酵素活性の段階的及び著しい増加を示し、これは、AAP培養物におけるよりも血管形成後(AAA)により著しいことが明らかである。
ドットブロットは、上記の実施例1に記載したようにして、MMP9に指向されたヒト抗体(Valbiotech)を用いて行う。コントロールとして、MMPの存在を培養血清(FCS)中でも調べる。結果を図12に示す。
結果は、AAP培養物及びAAA培養物の両方の場合に、D3とD28の間でMMP9が非常に著しく増加することを示す(1〜4の比で)。さらに、D14から、AAA培養物によるMMP9発現は、AAP培養物のものよりも著しく多い。培養血清は少量のMMP9を含有するが、この量は、動脈器官培養型培養の上清中で測定された濃度よりも著しく低いままである。
サイトカインIL-1β、IL-4、IL-6及びTGFβを、動脈培養の上清において、D3、D7、D14及びD28に、上記の実施例1に記載されるようにしてELISAにより、Quantikine (登録商標) (R&D Systems, ref. DLB50)キット及びDuoSet (登録商標) ELISA Development (R&D Systems, ref. DY204, ref. DY206 and DY240)キットをそれぞれ用いて定量する。得られた平均値をヒストグラムの形に変換し、統計的に分析する。結果を図13に示す。

- D7でのIL-1βの分泌のピーク、その後の突然の減少(A)。この現象は、AAP培養物におけるよりも、血管形成後に4倍大きい;
- IL-1βのものに比較してより遅い(B)が、AAP/AAA比はIL-1βのものに非常に類似するIL-6分泌のピーク。IL-1βとIL-6の間の機能的協調が、それらの分泌プロフィールの関係により再び確認される;
- D3での同じ値から始まってのAAP培養における(レベル:3)IL-4レベルの著しい減少(C)、及び血管形成後のIL-4レベルにおける割合的に同じ増加(レベル×3);
- AAP培養物における(レベル:2)TGFβ分泌の非常に著しい増加(D)、しかし血管形成後の非常に本質的な増加(D3及びD14の間で係数20)。
D3及びD28でのAAA及びAAP培養の厚さ7μmの切片を再水和し(2回のトルエン浴、次いで100°及び95°のアルコール溶液)、次いで、酢酸中で希釈した(1/10) 0.2%ペプシン(Sigma)を用いて2分間処理する。顕微鏡用調製物を0.1M PBS、pH 7.2中に10分間、次いでPBS-1%グリシン中に30分間リンスする。2回のリンスを1×PBS中で行う。
次いで、切片を、ブロッキング溶液(1% BSA、PBS 0.05% Tween及び10%ウマ血清)で30分間、37℃にて被覆する。
ブロッキング溶液を除去し、1% BSA及びPBS-0.05% Tweenを含有する溶液で希釈した一次抗体:1/50の抗MMP3又は抗MMP9 [Oncogene (Merk Eurolab)]で置き換える。切片を、一次抗体で一晩、周囲温度にて被覆する。
最後の現像は、各切片に対してDAB (ジアミノベンジジン)を用いて行う。
切片を、2回の連続するアルコール浴(95°及び100°)、続いて2回のトルエン浴を用いて最終的に脱水する。顕微鏡調製物は、DEPEX (GURR (登録商標))を用いて標本にする。
動脈と歯肉繊維芽細胞との共培養の取得
上記の実施例3に記載されるようにして得られたウサギからの損傷動脈の切片(アテローム動脈硬化性ウサギAAP)を、歯肉繊維芽細胞の存在下で、次の条件下で培養する:動脈/FG共培養のために犠牲にする1ヶ月前にウサギから採取した100000の自己歯肉繊維芽細胞に、コラーゲンゲルを混合してポリマー化し、2 mm (約3 mg)の動脈断片をゲルの中間部に堆積させる。培養方法は、上記の実施例1及び3に記載されたものである。
D3とD21 (培養日数)の間の細胞相互作用を、動脈及びFGの共培養培地中に分泌されるMMP9及びTIMP-1の発現を介して調べる。コントロールとして、これらの酵素の発現を、上記の実施例1及び3にそれぞれ記載されるものと同じ条件下で行われるFG単独の培養、及び動脈器官培養型培養中でも分析する。
D14及びD21において、動脈とFGとの共培養からの上清中に分泌されたMMP2及びMMP9の発現を、ゼラチンザイモグラフィにより分析する。結果を図14に示す。
T: 組換えMMP9 (R&D Systems, ref. 911 MP)及び組換えMMP2 (R&D Systems, ref. 902 MP)の10μlのスタンダード(100μg/ml);
A: コラーゲンゲル中の100000個のFGの細胞培養;

D3とD21の間の動脈と歯肉繊維芽細胞との共培養からの上清のドットブロット分析を、上記の実施例1に記載されるようにして、抗MMP9抗体及び抗TIMP-1抗体を用いて行う。MMP9分泌及びTIMP-1分泌の結果は、それぞれ図15 (A = ドットブロット、B = ドットブロットの定量)及び16 (A = ドットブロット、B = ドットブロットの定量)に示す。

D14の動脈と歯肉繊維芽細胞との共培養からのMMP9 mRNA及びTIMP-1 mRNAの抽出、並びにRT PCR分析を、上記の実施例1に記載されるようにして、MMP9に特異的なプライマー及びTIMP-1に特異的なプライマーを用いて行う。MMP9及びTIMP-1転写分析の結果を、それぞれ図17及び18に示す。

歯肉繊維芽細胞と3、7及び14日間共培養した損傷動脈に由来する7μmの切片を、オルセイン染色の後に顕微鏡(×40)で観察する。コントロールとして、上記の実施例3に記載のようにして単独で培養した動脈の切片を、同じ条件下で分析する。結果を図20に示す。

この研究の目的は、注射の実現可能性、及びパール染色により証明された繊維芽細胞のナノ粒子での標識の妥当性を決定することである。ウサギに注射されたヒト細胞は、パール染色された細胞が実際に注射された繊維芽細胞であるかをHLA免疫検出により視覚化することを可能にする。これは、ナノ粒子が死滅(dead) FGから逃れて中膜のSMCを標識できるからである。
ヒト歯肉繊維芽細胞の移植を、2つの異なるモデル:正常動脈モデル及び動脈瘤モデル(エラスターゼモデル)で、ウサギの動脈内にインビボで行う。
移植手順は、上記の実施例6に記載のものと同じである。結果を図21に示す。
A: 正常動脈モデル(×10及び×40)
B: 動脈瘤モデル(×5)。
歯肉繊維芽細胞を、実施例1に記載のようにして強磁性体のナノ粒子で単層培養において標識し、次いでトリプシン処理する。
これらを、種々の量で(105〜5×106細胞)ウサギの腸骨動脈内にINFILTRATORカテーテル(Boston Scientific)を用いて注射する(TEIGERら, Eur. Heart J. 18: abstract suppl. 500, 1997)。注射の24時間後にウサギを犠牲にし、注射の部位を視覚化する。結果を図22に示す。
Claims (6)
- 動脈リモデリング病変の治療用の細胞組成物を得るための歯肉繊維芽細胞の使用。
- 前記病変が動脈瘤であることを特徴とする請求項1に記載の使用。
- 前記病変が血管形成術後狭窄又は再狭窄であることを特徴とする請求項1に記載の使用。
- 前記病変が大動脈切開であることを特徴とする請求項1に記載の使用。
- 前記病変がアテローム性動脈硬化症であることを特徴とする請求項1に記載の使用。
- 前記繊維芽細胞が、治療を意図する個体から予め採取された歯肉組織に由来することを特徴とする請求項1〜5のいずれか1つに記載の使用。
