Abstract
【解決手段】 身体キャビティを閉塞する脈管閉塞デバイスであって、当該デバイスのルーメン内に配置された内部エレメントを具えるデバイス。内部エレメントは、自然位でデバイスが配置された後に、生物学的反応を顕在化させることができる生物活性物質を具える薬物キャリアを具える、あるいは含んでいる。例えば、生物活性物質は、薬剤キャリアの一成分の一部であっても、薬剤キャリアによって吸収されても、あるは薬剤キャリア上の層として被覆されていても良い。

Description
本発明は、医療用デバイスに関し、特に内用の生物学的活性薬剤を伴う脈管閉塞デバイスに関する。
多くの臨床場面において、出血のコントロール、腫瘍への血液の供給の防止、動脈瘤、動静脈奇形、動静脈フィステル内での血流のブロックなど、様々な目的で血管を閉鎖することがある。
薬剤送出能力を有する脈管閉塞デバイスを提供する。一の実施例では、脈管閉塞デバイスは、コイルと、当該コイルのルーメン内に配置した薬剤キャリアを具える。薬剤キャリアは、身体内部に配置されたときに組織反応を顕在化させる生物活性物質または薬剤を具える。非限定的な例として、薬剤キャリアは細長形状を有し、球形、円錐形、板状、メッシュ、あるいはその他の注文に応じた形状をしている。薬剤キャリアは、生分解性の物質で作ることができ、この場合、薬剤キャリアの組成は、身体内に置かれたときに放出される生物活性物質あるいは薬剤を具える。薬剤キャリアは、非生分解性の物質でできていても良く、この場合、生物活性物質または薬剤を、薬剤キャリアの表面に被覆するか、あるいは薬剤キャリア内部に組み込むようにする。別の実施例では、薬剤キャリアは、生物活性剤を付着あるいは吸着する物質でできている。非限定的な例として、薬剤キャリアは、一またはそれ以上のポリマフィラメント、スポンジ、チューブ、クロス、あるいは生物活性剤を包含、吸収、吸着することができるその他の物質を具えることができる。この場合、薬剤キャリアは生物活性剤を送出するのに使用され、生物活性剤はターゲット部位に配置されると薬剤キャリアの外の周囲に分散される。この実施例の一つの利点は、コイルのルーメン内に薬剤キャリアを配置することによって、コイルを送出する間にコイルの外側が生物活性物質によって影響を受けないようになることである。
図1は、本発明の一実施例によって構成した脈管閉塞デバイス10を示す側面図である。脈管閉塞デバイス10には、コイル12によって搬送された薬剤キャリア14が設けられている。コイル12は、ワイヤなどのリニアエレメント16でできており、これは好ましくは円形の断面形状を有する。代替の実施例では、コイル12のリニアエレメント16は、矩形、三角形、その他のジオメトリック断面、あるいは不規則形状の断面を有していても良い。コイル12は、リニアエレメント16で形成された一またはそれ以上のループまたは巻線18を具える。ループ18は、内部に薬剤キャリア14を配置する中央ルーメン20を規定している。図に示す実施例では、脈管閉塞デバイス10は、全径または断面が、0.010ないし0.023インチの範囲にあることが好ましい。しかしながら、脈管閉塞デバイス10は、その他の径及び/又は断面を有していても良い。
Claims (12)
- 脈管閉塞デバイスにおいて:
ルーメンを有する閉塞部材と;
前記ルーメン内に配置された薬剤キャリアであって、身体内の生物学的反応を顕在化させる生物活性薬剤を具えるあるいは搬送する薬剤キャリアと;
を具えることを特徴とする脈管閉塞デバイス。 - 請求項2に記載の脈管閉塞デバイスにおいて、前記薬剤キャリアが前記閉塞部材に固着されていることを特徴とする脈管閉塞デバイス。
- 請求項1又は2に記載の脈管閉塞デバイスにおいて、前記生物活性薬剤がホモポリマ、コポリマ、あるいはこれらの組み合わせを具えることを特徴とする脈管閉塞デバイス。
- 請求項3に記載の脈管閉塞デバイスにおいて、前記生物活性薬剤が、ポリエステル、アクリル酸、ポリエーテル、ポリシロキサン、ポリウレタンおよびポリカーボネートのうちの一またはそれ以上を具えることを特徴とする脈管閉塞デバイス。
- 請求項1又は2に記載の脈管閉塞デバイスにおいて、前記薬剤キャリアと生物活性薬剤のうちの一方あるいは双方が、合成ポリマ、ポリサッカリド、およびたんぱく質のうちの一またはそれ以上を具えることを特徴とする脈管閉塞デバイス。
- 請求項5に記載の脈管閉塞デバイスにおいて、前記薬剤キャリアと生物活性薬剤のうちの一方あるいは双方が、ポリグリコール酸、ポリ乳酸、ポリカプロラクトーン、ポリヒドロキシアルカノエート、ポリジオキサノン、ポリ(トリメチレンカーボネート)、ポリ無水物、およびポリアミノ酸、およびこれらのコポリマのうちの一又はそれ以上を具えることを特徴とする脈管閉塞デバイス。
- 請求項6に記載の脈管閉塞デバイスにおいて、前記薬剤キャリアおよび生物活性薬剤の一方、あるいは双方が、ポリ(g−エチルグルタミン酸)、ポリ(DTHイミノカーボネート)、ポリ(ビスフェノールAイミノカーボネート)、及びポリアリーレート、およびこれらのコポリマのうちの一またはそれ以上を具えることを特徴とする脈管閉塞デバイス。
- 請求項1ないし7のいずれかに記載の脈管閉塞デバイスにおいて、前記薬剤キャリアが細長形状であることを特徴とする脈管閉塞デバイス。
- 請求項1ないし7のいずれかに記載の脈管閉塞デバイスにおいて、前記閉塞部材がコイルであることを特徴とする脈管閉塞デバイス。
- 請求項9に記載の脈管閉塞デバイスにおいて、前記薬剤キャリアがコイル形状を有することを特徴とする脈管閉塞デバイス。
- 請求項1ないし10のいずれかに記載の脈管閉塞デバイスにおいて、前記薬剤キャリアが前記生物活性物質を吸着または吸収する物質を具えることを特徴とする脈管閉塞デバイス。
- 請求項1ないし11のいずれかに記載の脈管閉塞デバイスにおいて、前記薬剤キャリアが前記ルーメン内を搬送される活性エレメントを具え、当該活性エレメントが身体内に配置されたときに膨張あるいは緊縮し、これによって身体キャビティ内で展開されたときに前記閉塞部材の形状を実質的に維持することを特徴とする脈管閉塞デバイス。
