Abstract
Description
毒素産生性グラム陽性菌であるクロストリジウム ディフィシルは、広域性抗生物質による治療のために正常腸管内菌叢が抑制されている患者の腸管に侵入する。細菌毒素は、大腸(即ち、結腸)上皮に対する様々な程度の傷害を引き起こし、軽度の下痢から重度の大腸炎にまで及ぶ疾病のスペクトルを引き起こす。抗生物質治療がC.ディフィシル疾患の開始を誘導するため、それに関連する症候群は、抗生物質関連性の下痢症および大腸炎と呼ばれる(LaMont,Bacterial Infections of the Colon,Textbook of Gastroenterology,second edition,1897-1903,1995)。

本発明は、ヒトのような哺乳動物におけるC.ディフィシル疾患の予防および治療のための方法および組成物を提供する。その方法は、C.ディフィシル・トキソイドに対する抗体(例えば、毒素中和ポリクローナル免疫グロブリン)、C.ディフィシル・トキソイドそれ自体、またはそれらの組み合わせの、経皮(例えば、筋肉内、静脈内、または腹腔内)投与を含む、受動および能動の免疫感作アプローチを含む。例えば、本発明は、トキソイドAおよび/またはトキソイドBを含むワクチンの経皮投与(例えば、筋肉内)による再発性C.ディフィシル関連下痢症(CDAD)の予防および/または治療の方法を含む。本発明は、C.ディフィシル・トキソイド、C.ディフィシル・トキソイドを含有しているワクチン組成物、C.ディフィシル毒素中和ポリクローナル免疫グロブリンを作製する方法、実質的に精製されたC.ディフィシル毒素中和ポリクローナル免疫グロブリン、およびC.ディフィシル毒素中和ポリクローナル免疫グロブリンのドナーを同定する方法を含む。これらの方法および組成物について、以下、さらに記載する。
概要
C.ディフィシル毒素Aおよび毒素Bは、培養ビン(10〜20L)において培養されたC.ディフィシルの嫌気培養物において産生される。C.ディフィシルのマスター(Master)セル・バンクおよびワーキング(working)セル・バンクが、C.ディフィシルATCC43255株からATCCにおいて調製された凍結乾燥リサーチ(research)セル・バンクから製造された。ワクチン作製のため、増殖培地に懸濁した透析サックで培養されたC.ディフィシル培養物により、毒素を産生させる。多数のサック培養物をプールし、生存可能なC.ディフィシルおよび胞子を、遠心分離、それに続くサブミクロン濾過により除去する。得られた濾液を濃縮し、透析濾過(diafiltered)し、4℃で60%飽和硫酸アンモニウムにより毒素を沈殿させ、ペレットを凍結保管する。硫酸アンモニウム・ペレットを、リン酸緩衝液に再溶解させ、S-300セファクリル・サイズ排除カラムに適用する。毒素Aおよび毒素Bを含有しているピークを収集し濃縮する(2:1の毒素A毒素B比で、50〜60%毒素)。次いで、毒素調製物を、4.25mg/mlリジンを含有している溶液中で、4℃〜6℃で、4.25mg/mlホルムアルデヒドにより、18日間不活化する。不活化の後、安定剤として使用するため、ホルムアルデヒド濃度を透析濾過により0.016%にまで低下させる。最終産物を、2.5mg/mlの濃度で、0.6mlの充填容量でガラス・バイアルに充填する。
リサーチ・シード(research seed)は、基準株ATCC43255のアンプルを使用して、標準的な方法により、ATCCによる契約の下で調製され凍結乾燥された。オキソイド・リインフォースド・クロストリジアル培地(Oxoid Reinforced Clostridial Medium)(RCM)が、シード・ストックを培養するために使用された(Oxoid Ltd.,Hampshire,England)。培地中のウシ由来材料は、オーストラリア、ニュージーランド、オランダ、および米国で、ヒト消費用に使用される健康な動物から入手された。培養物は、保存剤として5%デキストランおよびトレハロースを使用して、RCM中で安定化された。
C.ディフィシルのMCBは、リサーチ・シード・ストックの凍結乾燥バイアルを、RCM(ATCCにより使用されたのと同じロット)に再懸濁させ、インキュベートし、それに続いて、トリプトン(0.48%)-酵母抽出物(0.24%)-マンニトール(0.1%)(TYM)培地で2回増幅することにより、調製された。グリセロールを凍結保護剤として添加し、およそ1mlずつに分割したもの250個を急速凍結させ、液体窒素中で保管した。ワーキング・セル・バンクは、MCBのバイアルを接種原として使用して、同様の様式で調製された。
マスター・セル・バンクおよびワーキング・セル・バンクは、生存率、純度、同一性、および毒素発現に関して試験された。生存率は、固形培地および液体培地の両方における増殖により証明された。純度は、嫌気培養下でのグラム染色およびコロニー形態学、ならびに好気増殖の欠如により試験された。C.ディフィシルの同一性は、ガス・クロマトグラフィ脂肪酸分析、および臨床的な嫌気的同一性試験により証明された。毒素の発現および同一性は、透析サックでセル・バンクを培養し、交差免疫電気泳動により両毒素の発現に関して培養物を試験することにより確認された。毒素Aの発現および同一性は、ELISAによっても確認された。毒素Bの発現は、細胞毒性に関する試験および細胞毒性の特異的な中和により確認された。毒素発現は、ATCC43255標準と平行に測定され、比較可能であることが示された。
毒素は、窒素源(例えば、1〜100g/L、5〜20g/L、または12g/Lの濃度のトリプトン)、酵母抽出物(1〜100g/L、15〜35g/L、または24g/L)、リン酸緩衝液、炭素源(例えば、マンニトール(1〜50g/L、例えば、8g/L)、グルコース、グリセロール(1〜50g/L、例えば、4g/L)、またはマンニトール+グリセロール(例えば、上に示された量)を含有している培地に懸濁した、透析サック(13〜14,000分子量カットオフ)で培養されたC.ディフィシルの嫌気培養物において産生される。産生は、ワーキング・セル・バンクのバイアルを小さい静置培養物において増幅し、その培養物の一定分量を、透析サックに接種するために使用することにより、開始させられる。およそ5日間の37℃における増殖の後、サック内の材料を採集する。採集された産物を、増殖型の細胞および胞子を除去するため、遠心分離し、濾過する(0.5μm、続いて0.2μm)。濾液を洗浄し、濃縮し、硫酸アンモニウムにより沈殿させる。
培養単位は、2つのサイドアーム・ポートを有する8Lまたは16Lのスピナー・フラスコ、透析サック、およびリン酸緩衝液の1Lまたは2Lのフラスコからなる。最大25個の8Lまたは16Lの単位を、各産生ランのために接種する。培養単位は、スピナー・フラスコで培地を溶解させ、サイドアーム・ポート間に透析サックを吊るし、ポートの端にキャップをし、100mMリン酸緩衝液のフラスコを1つのポートへ取り付けることにより調製される。培地の滅菌および嫌気生活の作出のため、単位全体をオートクレーブ処理する。50℃未満にまで冷却した後、リン酸緩衝液をポンプで透析サックへと送り込み、単位を37℃で一夜平衡化し(その間に、増殖栄養素が透析サックへと拡散し、細菌増殖に適した条件が確立される)、その後、接種を行う。
ワーキング・セル・バンクのバイアルを解凍し、嫌気TYスターター培地(トリプトン(0.48%)および酵母抽出物(0.24%))50mlに接種するために使用する。フラスコを、14〜16時間、37℃の嫌気チャンバーに置く。接種原を含む適切な容量の希釈剤およそ2mlを、各透析サックに添加する。次いで、培養単位をインキュベーターに戻し、5日間放置する。不必要な撹拌を防止することにより、オートクレーブ処理の後、嫌気生活を維持する。
インキュベーションの後、培養単位をインキュベーターから取り出し、透析サックの内容物をポンプで送り出し、プールし、培養物の純度および同一性に関して試験する。遠心分離、それに続く、0.5μmプレフィルター、次いで0.2μm滅菌フィルターによる濾過により、生存可能なC.ディフィシル生物および胞子を除去する。濾液を、毒素Aおよび毒素Bの濃度、ならびに無菌性に関して試験し、30,000MWカットオフ中空糸カートリッジを用いた限外濾過により10倍に濃縮する。濾液を、25mMトリス、pH7.5により洗浄し、低分子量培地成分を減少させる。濾過された飽和硫酸アンモニウム溶液を、60%飽和の最終溶液が得られるよう、濃縮物に添加する。その溶液を4℃で48時間以上インキュベートし、遠心分離および上清の廃棄により、毒素を含有している沈殿物を採集する。硫酸アンモニウム・ペレットを、さらなる加工まで、−10℃以下で凍結保管する。
ペレットを、室温で、100mMリン酸緩衝液、pH7.4と混合することにより解凍する。可溶化された毒素を、遠心分離により清浄化し、0.45μmフィルタを使用して濾過する。次いで、清浄化された材料を、セファクリルS-300ハイ・リゾリューション(High Resolution)(Pharmacia Biotechnology)ゲル濾過カラムにおいて分画する。典型的なクロマトグラフィ・プロファイルを図1に示す。毒素ピークを収集し、5.0±0.5mg/mlに濃縮する。収集は、クロマトグラムの視覚的検査により決定されるような、毒素ピークの上昇期から開始し、下降期の変曲点まで継続する。
ホルムアルデヒドによる毒素不活化のための最適条件を確立するため、拡張的な研究を実施した。生物学的活性の損失をモニタリングするため、これらの研究は、C.ディフィシル毒素の生物学的活性の高感度の指標となるIMR90組織培養系を利用した(Torres et al.,supra)。研究されたパラメータには、完全なトキソイド化(toxoiding)を容易にするために設計された、ホルムアルデヒドおよび毒素の濃度、緩衝液の組成、pH、時間、温度、ならびに添加されるL-リジンの効果が含まれた(表1)。
サイズ排除クロマトグラフィの後、ホルムアルデヒド処理の前に、部分精製された毒素調製物を特徴決定するための研究を行った。毒素Aおよび毒素Bは、トリス−グリシン還元SDS-PAGEにおいて十分に分離されない。しかしながら、全毒素(毒素Aおよび毒素B)は、クーマシー染色されたトリス−グリシン還元SDS-PAGEゲルの濃度測定スキャニングによって推定され得る。全毒素は、全タンパク質の50〜60%を占める。これらの還元ゲルのイムノブロットは、一つの大きな抗毒素A反応性バンド、ならびに抗毒素B反応性の一つの大きなバンドおよびいくつかの小さいバンドを示す。
本発明者らは、市販の20mg/ml酸化アルミニウムを含有している無菌Rehydragel LV(登録商標)(Reheis,Inc.,Berkeley Heights,New Jersey)から、トキソイド吸着のためのミョウバンを調製する。この材料を、まず、pH7.4の50mMリン酸緩衝液、100mM NaCl、100mg/mlホルムアルデヒドにより、3mg/ml酸化アルミニウムにまで希釈する。希釈されたミョウバンを、クラス100条件下で、グレー・ブチル・ゴム栓を有する無菌の発熱性物質なしの10ml容量ガラス・バイアルに無菌的に充填する。
上述のように、C.ディフィシル免疫グロブリン・ドナーは、C.ディフィシル・トキソイド・ワクチンの経皮投与によって作製され得る。予防接種のための好ましい候補は、既にC.ディフィシル・トキソイド中和抗体を有している対象である。これらのドナーは、毒素に曝されたことがあり、有用な力価に達するために、より少ない追加刺激を必要とするであろう。本発明者らは、静脈内免疫グロブリンの9つの商業的なロットを試験し、それらが極めて低いレベルのC.ディフィシル毒素Aおよび毒素Bに対する中和抗体を含有していることを見出した。これらの調製物中の抗毒素の力価は、いずれの毒素に対しても<1:50であり、毒素Aより毒素Bに対する力価の方が高い。本発明者らは、Nevadaのセンターからの100人の専門家血漿ドナーの調査も実施した。その結果は、これらの個体の2%および13%が、それぞれ抗毒素Aおよび抗毒素Bの中和抗体を有していたが、力価は極めて低かったことを示している。これらのデータは、C.ディフィシル治療用の過免疫ヒト抗毒素を調製する目的のため、未刺激の血清陽性血漿ドナーからの血漿を選択することは効果的でないであろうこと、および、治療用のヒト免疫グロブリンを作製するためには、トキソイド・ワクチンによる免疫感作によりドナーを刺激する必要があることを示している。
マウスの能動免疫感作
8匹の雌Swiss Websterマウスの群を、1週間間隔で2回、ミョウバン吸着トキソイド・ワクチンにより腹腔内(IP)免疫感作した。トキソイドは、ヒト製剤を模倣するためにミョウバンに吸着させられた(アルミニウム1mg当たりタンパク質0.144mgの比率)。用量は、吸着ワクチンの一連の10倍希釈で投与された。動物は、免疫原性の比較のための4つの異なるトキソイド・ロットを投薬された:1つは研究用ロット(ロット27-33)、3つは、臨床的産物の作製の方法に従い製造されたワクチン・ロット(ロット133、135、および144)であった。2回目の免疫感作の1週間後、血清を、ELISAにより、全抗体に関して試験し、細胞毒性中和により、毒素Aおよび毒素Bに対する抗体に関して試験した。細胞毒性中和アッセイにおいては、毒素(10×MC50)を、37℃で1時間、2倍抗体希釈物と共にインキュベートし、次いで、IMR90細胞の単層培養物へと接種する。中和力価は、球状化から細胞の50%を防御する抗体の最も高い希釈率として表される。
ハムスターは、毒素Aおよび毒素Bの作用に対して高度に感受性であるため、この種は、C.ディフィシル感染の優れたモデルを提供する。C.ディフィシルの存在下でのクリンダマイシンの単回投与の後、ハムスターは、劇症性の下痢および出血性の盲腸炎により2〜3日以内に死亡する。トキソイド・ワクチンにより筋肉内免疫感作されたハムスターは、その後C.ディフィシルをチャレンジされた場合、死亡および下痢から防御される。この免疫感作の経路は、血清抗体(IgG)を誘導するが、検出可能な粘膜の抗体を誘導せず、このことは、高力価のIgGが、C.ディフィシルにより引き起こされる腸疾患から防御し得ることを示している。
2つの動物モデルにおける実験は、受動免疫感作の治療効力を確認する。毒素中和抗体調製物による処置は、致死的なチャレンジからマウスを防御する。上記のクリンダマイシン・チャレンジ・モデルを利用して、毒素中和抗体を与えられたハムスターは、死亡および下痢から防御されることが示された。下痢を発症しているハムスターは、毒素中和抗体の非経口投与により治癒され得る。受動的に投与された抗体の防御治療活性は用量依存性であり、抗体含有調製物により処置された動物において達成される受動血清中和抗体レベルと相関する。
毒素中和抗体の防御力を試験するため、マウスに、腹腔内(IP)経路により、毒素Aおよび毒素Bの両方に対する抗体を含有している過免疫マウス腹水を与え、次いで、毒素Aまたは毒素Bをチャレンジした。100μl、10μl、または1μlの用量のプールされた腹水をマウスに単回投与し、血清中の受動的に入手された中和抗体のレベルを決定するため、毎日採血をした。次いで、動物に、毒素A(5×LD50 IP)または毒素B(5×LD50 IV)をチャレンジし、7日間モニタリングした。毒素Aおよび毒素Bの直接投与は、マウスを死亡させる。100μl用量の腹水は、毒素Aによる致死から動物の60%を防御し、毒素Bによる致死から動物の80%を防御した。マウスは、抗体投与後、最長23日間、毒素Aチャレンジから防御された。このレベルの致死からの防御を入手するためには、毒素注射の30分以内に抗体を投与しなければならなかった。実験室における測定は、生存していた動物が、全て、腹水の注入の結果として、≧200の相反血清中和力価を有していたことを示した。毒素中和抗体の半減期は、この研究の条件下では、2週間であると推定された。
雌ハムスターの群に、過免疫マウスの腹水をIPで単回投与した。段階的な用量が投与された(1匹当たり6、2、0.6、または0.2mlの腹水)。非免疫マウスからの腹水を、陰性対照とした。動物は、腹水を受容した翌日に受動血清抗体レベルを測定するために採血され、2日後にクリンダマイシンをチャレンジされた。チャレンジ後の3週間、生存、下痢、および体重減少に関して、動物をモニタリングした(図5)。死亡からの防御は、3つの最も高い用量群において達成され、死亡および下痢の両方からの防御は、最高用量の抗体を受容した動物において見られた(図6)。中和抗体のレベルは、死亡および下痢からの防御と相関していた。完全に防御された動物は、およそ800の相反血清中和抗体を有し、最小有効力価は200であった。この研究におけるハムスター血清中の中和抗体の半減期は、14日であると推定された。
抗生物質関連下痢症のハムスター・モデルは、C.ディフィシルに対する予防戦略の評価に有用なものである。しかしながら、ハムスターにおけるC.ディフィシル疾患は極めて重度であり、クリンダマイシン・チャレンジの後、迅速に、急性盲腸炎および死亡が起こる。感染の重度は、死亡からは防御するが下痢からは防御しないよう設計された、予定された量の毒素Aおよび毒素Bに対する中和抗体の投与によって低下させることができる。死亡を予防し、下痢は予防しない用量を、用量範囲実験において確定した。動物が下痢を有している期間中、付加的な中和抗体は、下痢を消散することができた。非免疫動物からの腹水を与えられた動物は、下痢に罹患し続け、最終的に死亡した。治療された動物は、処置後24時間以内に下痢から回復し、再発しなかった。実験の設計および下痢の結果を、図7および8に示す。この実験は、C.ディフィシル関連下痢症を治療するために毒素中和抗体が使用され得ることを示す。回復は迅速であった。
毒素Aおよび毒素Bの両方に対する中和抗体が、本発明のトキソイド・ワクチンによる免疫感作の後にアカゲザルにおいて誘導された。3匹の動物の群に、ミョウバン吸着ワクチンまたはプラセボのいずれかを含む液体ワクチンを与えた。研究は、非ヒト霊長類において高力価の中和抗体を誘起するワクチンの能力を証明するために設計された。プラセボ対照は、主として、安全性比較のため含まれた。動物は、ミョウバン吸着ワクチン(110μg)またはプラセボを含む溶液を5回受容した。ワクチンは、臀部に筋肉内経路により、0.5mlの容量で、0日目、8日目、29日目、65日目、および118日目に投与された。免疫応答および臨床病状をモニタリングした(図9)。5回分が与えられた後、有害な病状または感受性は認められなかった。免疫感作された動物は、全て、結合抗体および中和抗体の両方により応答した。数回のワクチン投薬が、有意な中和抗体を誘導するために必要とされ;65日目および118日目における追加投薬は中和抗体レベルをさらに高めた。ミョウバン吸着ワクチンは、いくつかの動物において、より迅速な、より高い応答を誘導した(図10)。研究は、免疫グロブリン調製物へと加工するのに適している、誘導レベルの中和ワクチンにより誘導された抗体の実現可能性を示し、初回刺激された動物における追加投薬の、高力価の防御抗体を誘発する能力を証明した。この実験は、多数回のトキソイドの追加投薬による過免疫感作が、非ヒト霊長類において安全であることも証明した。
下記の研究においては、トキソイドAおよびBを含有している非経口C.ディフィシル・ワクチンが、再発性C.ディフィシル関連下痢症(CDAD)の多数回のエピソードの履歴を有する3人のヒト対象に投与された。対象は、8週間にわたり4回の50μgのワクチンの筋肉内接種を受容した。2人の対象は、血清IgG抗毒素Aおよび抗毒素B抗体のレベルの増加を示し、両毒素に対する血清細胞毒素中和活性を示した。予防接種の後、3人の対象は全員、経口バンコマイシンによる治療を中止したが、CDADのさらなる再発はなかった。これは、C.ディフィシル・ワクチンの使用が、高いCDADのリスクを有する対象の治療において有効であることを示す。
Claims (17)
- ヒト患者におけるクロストリジウム ディフィシル(Clostridium difficile)感染を治療する方法であって、ヒト・クロストリジウム ディフィシル毒素中和ポリクローナル免疫グロブリンを該ヒト患者へ経皮投与することを含む、方法。
- クロストリジウム ディフィシル毒素中和ポリクローナル免疫グロブリンが、ヒト患者へ筋肉内投与、静脈内投与、または皮下投与される、請求項1記載の方法。
- 体重1kg当たり0.01〜100mgのクロストリジウム ディフィシル毒素中和免疫グロブリンがヒト患者へ投与される、請求項1記載の方法。
- クロストリジウムの毒素またはトキソイドをヒト患者へ経皮投与することをさらに含む、請求項1記載の方法。
