Abstract
Description
肝細胞癌(HCC)は世界的に見て最も患者数の多い癌の1つである。しかし、この疾患を治癒し得る治療法は存在しない。これはおそらく、この疾患の発症および進行の過程で、癌細胞の特徴が順次変化することが原因している。特に、癌の進行は、腫瘍細胞の分化段階と関連している場合が多い。癌細胞のそのような変化を診断および管理することによって、癌治療はより効果的になると考えられる。本発明では、50のC型肝炎ウイルス(HCV)関連HCC組織および11の非腫瘍(非癌性および前癌)肝組織由来の、約11,000個の遺伝子を示すオリゴヌクレオチドマイクロアレイにより、発現がHCCの発症および進行と相関する遺伝子を同定する。
本発明では、ヒト肝細胞癌(HCC)組織、非腫瘍(非癌性および前癌)肝組織およびHCV感染を有するHCCを解析に使用する。HCVおよび/またはHBV感染の有無は、抗HCV抗体および抗HBV抗体に対する免疫反応性、またはPCRによるHCVおよび/またはHBVゲノムの増幅のいずれかによって判定することができる。HCCの分化段階は組織病理学的検査によって判定することができ、HCCは高分化型HCC(G1)、中分化型HCC(G2)、および低分化型HCC(G3)に分類される。非腫瘍肝組織は、良性または転移性肝腫瘍を切除した患者から採取し得る。HCVが感染していない肝組織は非癌性肝臓(L0)に分類され、HCVが感染している肝組織は前癌肝臓(L1)に分類される。手術時に肝組織を切除した後、組織を液体窒素またはドライアイスを含むアセトン中で直ちに凍結し、使用時まで-70℃〜-80℃で保存することが好ましい。組織は、O.C.T.化合物(Sakura-Seiki、日本、東京、カタログ番号4583)に包埋してもしなくてもよい。

1997年5月から2000年8月の間に山口大学医学部付属病院において、50名の患者がHCCの外科治療を受けた。手術前に、すべての患者から書面によるインフォームドコンセントを得た。研究手順は、山口大学医学部のヒト対象の利用に関する院内倫理委員会によって承認された。50名の患者はすべて、HCV抗体(HCVAb)について血清陽性であり、B型肝炎ウイルス表面抗原(HBsAg)について血清陰性であった。術後、全症例においてHCCの組織病理学的診断を行った。この組織病理学的検査によって、7名の患者が高分化型HCC(G1)を有し、35名が中分化型HCC(G2)を有し、残りの8名が低分化型HCC(G3)を有することが判明した。国際対癌連合のTNM分類に従って、臨床病理学的要因を決定した。フィッシャーの直接確率検定、スチューデントのt検定、およびマンホイットニーのU検定を用いて、3段階、G1、G2、およびG3 HCC間の臨床病理学的特徴の相違を明らかにした。P<0.05を有意と見なした。
組織学的検査から、本研究に登録された50例のHCV関連HCCのうち、7例が高分化型HCC(G1)であり、35例が中分化型HCC(G2)であり、残りの8例が低分化型HCC(G3)であることが明らかとなった(表2)。G2およびG3 HCCの腫瘍の大きさは、G1 HCCの腫瘍の大きさよりも有意に大きかった(マンホイットニーのU検定により、それぞれp=0.0007およびp=0.028)。G2およびG3 HCCにおける血管侵襲(vessel involvement)の発生率は、G1 HCCにおける発生率よりも有意に高かった(フィッシャーの直接確率検定によりp=0.038)。G1からG3への脱分化と平行して、腫瘍期はより進行していた(フィッシャーの直接確率検定によりp=0.066)。このように、本研究に登録されたG1、G2、およびG3 HCCの各種類は、Kojiroによって提唱される脱分化に対応するする特徴、すなわち腫瘍の大きさ、転移能、および腫瘍期を示した(Kojiro, M. Pathological evolution of early hepatocellular carcinoma, Oncology 62, 43-47(2002))。
組織片(約125 mm3)をTRIZOL(Life Technologies、米国、ゲイサーズバーグ、カタログ番号15596-018)、またはSepasol-RNAI(Nacalai tesque、日本、京都、カタログ番号306-55)に懸濁し、ポリトロン(Polytron)(Kinematica、スイス、リッタウ)を用いて(最高速度で5秒間)2回ホモジナイズした。クロロホルムを加えた後、組織ホモジネートを15,000×gで10分間遠心分離し、RNAを含む水相を回収した。全細胞RNAをイソプロピルアルコールで沈殿させ、70%エタノールで1回洗浄し、DEPC処理水(Life Technologies、米国、ゲイサーズバーグ、カタログ番号10813-012)に懸濁した。DNase I(Life Technologies、米国、ゲイサーズバーグ、カタログ番号18068-015)1.5単位で処理した後、RNAをTRIZOL/クロロホルムで再抽出し、エタノールで沈殿させ、DEPC処理水に溶解した。その後、RNeasyミニキット(QIAGEN、ドイツ、ヒルデン、カタログ番号74104)を用いて、製造業者の取扱説明書に従って低分子量ヌクレオチドを除去した。全RNAの質は、アガロースゲル電気泳動後の28Sおよび18SリボソームRNAの比から判定した。精製した全RNAは、使用時まで70%エタノール溶液中で-80℃にて保存した。
cDNAは、reverse SuperScript Choiceシステム(Life Technologies、米国、ゲイサーズバーグ、カタログ番号18090-019)を用いて、製造業者の取扱説明書に従って合成した。精製した全RNA 5μgを、T7プロモーターの配列を含むオリゴ-dTプライマー(Sawaday Technology、日本、東京)およびSuperScript II逆転写酵素200単位とハイブリダイズさせ、42℃で1時間インキュベートした。得られたcDNAをフェノール/クロロホルムで抽出し、Phase Lock Gel(商標) Light(Eppendorf、ドイツ、ハンブルグ、カタログ番号0032 005.101)で精製した。
神経膠腫患者由来のヒト原発腫瘍の遺伝子発現を、高密度オリゴヌクレオチドマイクロアレイ(U95Aアレイ、Affymetrix、米国、サンタクララ、カタログ番号510137)によって調べた(Lipshutz, R.L. et al. High density synthetic oligonucleotide arrays, Nat. Genet. 21, 20-24(1999))。チップ上のオリゴヌクレオチドとハイブリダイズさせるため、cRNAを、40 mM Tris(Sigma、米国、セントルイス、カタログ番号T1503)-酢酸(Wako、日本、大阪、カタログ番号017-00256)(pH 8.1)、100 mM酢酸カリウム(Wako、日本、大阪、カタログ番号160-03175)、および30 mM酢酸マグネシウム(Wako、日本、大阪、カタログ番号130-00095)を含む緩衝液中で95℃で35分間処理し断片化した。ハイブリダイゼーションは、0.1 M 2-(N-モルフォリノ)エタンスルホン酸(MES)(Sigma、米国、セントルイス、カタログ番号M-3885)(pH 6.7)、1 M NaCl(Nacalai Tesque、日本、京都、カタログ番号313-20)、0.01%ポリオキシレン(10)オクチルフェニルエーテル(Wako、日本、大阪、カタログ番号168-11805)、ニシン精子DNA(Promega、米国、マディソン、カタログ番号D181B)20μg、アセチル化ウシ血清アルブミン(Sigma、米国、セントルイス、カタログ番号B-8894)100μg、断片化cRNA 10μg、およびビオチン化対照オリゴヌクレオチド、ビオチン-5'-CTGAACGGTAGCATCTTGAC-3’(Sawady technology、日本、東京)を含む緩衝液200μl中で、45℃で12時間行った。0.01 M MES(pH 6.7)、0.1 M NaCl、および0.001%ポリオキシレン(10)オクチルフェニルエーテル緩衝液を含む緩衝液でチップを洗浄した後、取扱説明書(Affymetrix、米国、サンタクララ)に記載されている通りに、チップをビオチン化抗ストレプトアビジン抗体(Funakoshi、日本、東京、カタログ番号BA0500)と共にインキュベートし、ストレプトアビジンR-フィコエリトリン(Molecular Probes、米国、ユージーン、カタログ番号S-866)で染色し、ハイブリダイゼーションシグナルを増大させた。レーザースキャナ(Affymetrix、米国、サンタクララ)で各ピクセルレベルを収集し、各cDNAの発現レベルおよび信頼性(有/無の判定)を、Affymetrix GeneChip ver. 3.3およびAffymetrix Microarray Suite ver. 4.0ソフトウェアにより算出した。これらの実験から、神経膠腫患者のヒト原発腫瘍における約11,000個の遺伝子発現が決定された。
50のHCC試料および11の非腫瘍(非癌性および前癌)肝臓試料のすべてにおいて、40(Affymetrixによる任意単位)を超えるアベレージディファレンス(average differences)を有する遺伝子を選択した。この手順により、約11,000個の遺伝子から3,559個の遺伝子が得られた。次いでフィッシャー比を決定し(Iizuka, N., Oka, M., Yamada-Okabe, H., Mori, N., Tamesa, T., Okada, T., Takemoto, T., Tangoku, A., Hamada, K., Nakayama, H., Miyamoto, T., Uchimura, S., and Hamamoto, Y. Human prostate cancer and benign prostatic hyperplasia: molecular dissection by gene expression profiling, Cancer Res. 61, 4683-4688(2001))。検定では、検討する2つの段階間で試料標識の順序をランダムに並べ替え、各遺伝子のフィッシャー比を再度算出した。試料標識のこのランダムな順列を1,000回繰り返した。次いで、ランダム化したデータによるフィッシャー比の分布に基づいて、実際のデータから得られたフィッシャー比にPを割り当てた。ランダム化したデータに基づいたフィッシャー比の分布から、ランダム順列検定に合格し得た(P<0.005)全遺伝子を選択した。2つの段階を比較するための全実験において、この手順を実施した。結果として、4.90よりも高いフィッシャー比を有する152個の遺伝子が、L0とL1の間の統計的に有意な識別子であった。同様に、L1とG1を識別する、4.08よりも高いフィッシャー比を有する191個の遺伝子、G1とG2を識別する、1.52よりも高いフィッシャー比を有する54個の遺伝子、およびG2とG3を識別する、1.34よりも高いフィッシャー比を有する40個の遺伝子が同定された。
オリゴヌクレオチドアレイデータを用いて、発癌の過程、すなわち非癌性肝臓(L0)からHCV感染している前癌肝臓(L1)へ、およびL1から高分化型HCC(G1)へ、ならびにHCCの脱分化の過程(G1からG2およびG2からG3)における遺伝子発現の変化を解析した。教師あり学習法の後にランダム順列検定を用いて、L0からL1への移行過程で発現レベルが有意に変化した152個の遺伝子が同定された。152個の遺伝子のうち、67個がこの移行過程で発現が増加し、85個の遺伝子の発現が低下していた。同じ方法で、L1からG1 HCCへの移行過程で発現レベルが有意に変化した191個の遺伝子が同定された。191個の遺伝子のうち、95個がこの移行過程で発現が亢進し、96個で発現が低下した。54個の遺伝子がG1 HCCとG2 HCCの間で差次的に発現されたと見られ、そのうち36個の遺伝子の発現がG1からG2への移行過程で増加し、18個の遺伝子の発現が減少していた。40個の遺伝子がG2 HCCとG3 HCCの間で差次的に発現されることが判明し、そのうち10個の遺伝子の発現がG2からG3への移行過程で増加し、30個の遺伝子の発現が減少していた。
ほとんどの免疫応答関連遺伝子、代謝関連遺伝子、輸送関連遺伝子、タンパク質分解関連遺伝子、および発癌関連遺伝子の発現が、L0からL1への移行過程において増加し、転写関連遺伝子の発現は減少した(表3)。
L1からG1への移行過程において発現が変化した遺伝子には、ほとんどの発癌関連遺伝子、シグナル伝達関連遺伝子、転写関連遺伝子、輸送関連遺伝子、解毒関連遺伝子、および免疫応答関連遺伝子が含まれる(表4)。
G1からG2への移行過程において発現が変化した遺伝子には、IFN関連遺伝子、細胞構造および運動関連遺伝子、転写関連遺伝子、ならびに腫瘍抑制遺伝子が含まれる(表5)。
G2からG3への移行過程において発現が変化した遺伝子には、タンパク質分解関連遺伝子、BCL2関連遺伝子、ならびに代謝およびエネルギー生成関連遺伝子が含まれる(表6)。
L0からL1への移行過程において発現が有意に変化した152個の遺伝子(図1a)、L1からG1への移行過程において発現が有意に変化した191個の遺伝子(図1b)、G1からG2への移行過程において発現が有意に変化した54個の遺伝子(図1c)、およびG2からG3への移行過程において発現が有意に変化した40個の遺伝子(図1d)を、カラーで表示した。これらの遺伝子は、2つの連続した分化段階にある試料を明瞭に識別した。図1e〜hは、各移行期において選択された40個の遺伝子の全試料における発現を示す。L0からL1(図1e)、L1からG1(図1f)、G1からG2(図1g)、およびG2からG3(図1h)への移行過程において発現が有意に変化した遺伝子として選択された40個の遺伝子発現もまた、カラーで表示した。その結果、各移行期の選択された40個の遺伝子は移行前後の試料を明確に識別し得ることが明らかとなった。
各移行期の選択された遺伝子の識別能を検証するため、各移行期で選択された40個の遺伝子を用いて最小距離分類子を作成した。各移行過程において、赤色のバーで示した連続した2つの分化段階にある試料(練習試料)を用いて最小距離分類子を作成し、黒色のバーで示した残りの分化段階にある試料(被験試料)にこれを適用した(図2)。得られた分類子は、被験試料を92%(図2a)、98%(図2b)、84%(図2c)、および100%(図2d)の精度で分類した。
非癌性肝臓(L0)と前癌肝臓(L1)、前癌肝臓(L1)と高分化型HCC(G1)、高分化型HCC(G1) と中分化型HCC(G2)、中分化型HCC(G2) と低分化型HCC(G3)との間で発現が統計学的に有意に異なる遺伝子の発現を、ウェブサイト、http://www.cis.hut.fi/projects/somtoolbox/(Kohonen, 2001)で利用可能なSOMツールボックスを用いてMATLAB R13の方法に従って解析した。非癌性肝臓(L0)と前癌肝臓(L1)、前癌肝臓(L1)と高分化型HCC(G1)、高分化型HCC(G1) と中分化型HCC(G2)、中分化型HCC(G2) と低分化型HCC(G3)との間の各比較には40個の遺伝子を用いた。隣接するセルのベクトルを155次元の遺伝子空間において相互に近接して位置づけ(図3a)、ここで(m, n)はm行目かつn列目に位置するセルを示し、NL-XXはHCV感染していない非癌性肝臓(L0)に由来する試料を示し、IL-XXはHCV感染した前癌肝臓(L1)に由来する試料を示し、G1-XXTは高分化型HCC(G1)に由来する試料を示し、G2-XXTは中分化型HCC(G2)に由来する試料を示し、G3-XXTは低分化型HCC(G3)による試料を示す。マップから、試料がL0、L1、G1、G2、およびG3の順に明らかにS字型曲線を形成することが示された。血管侵襲がないG2試料(青字)はG1試料の近くに位置し、血管侵襲を有するG2試料(赤字)はG3試料の近くに位置した(図3a)。静脈浸潤を有さないG2試料はG1試料の近くに位置し、静脈浸潤を有するG2試料はG3試料の近くに位置した。このように、SOMは、一連の脱分化段階においてG2試料を2つのサブタイプ、すなわち静脈浸潤を有する腫瘍および静脈浸潤を有さない腫瘍に分類した。隣接するクラスター間の距離を、赤が遠距離を示す色によって示した場合、上部領域内の赤色のセルによって、非腫瘍(非癌性および前癌)肝臓とHCC試料が、155次元の遺伝子空間において比較的遠く離れていることが明瞭になった(図3b)。

フィッシャーの直接確率検定、スチューデントのt検定、およびマンホイットニーのU検定を用いて、各分化段階間のバックグラウンドの相違を明らかにした。

Claims (15)
- 癌患者から得られるヒト腫瘍組織の遺伝子および/またはタンパク質の発現レベルまたは発現パターンに基づいて統計学的解析により選択された遺伝子および/またはタンパク質を用いて、腫瘍の分化段階を規定する方法。
- ヒト組織がヒト肝組織である、請求項1記載の方法。
- 腫瘍の分化段階が、非癌性肝臓、前癌肝臓、高分化型肝細胞癌(HCC)、中分化型HCC、および低分化型HCCからなる群より選択される、請求項2記載の方法。
- 遺伝子および/またはタンパク質が、非癌性肝臓と前癌肝臓、前癌肝臓と高分化型肝細胞癌(HCC)、高分化型HCCと中分化型HCC、または中分化型HCCと低分化型HCCとの間で差次的に発現する、請求項3記載の方法。
- 遺伝子および/またはタンパク質の発現レベルまたは発現パターンが、DNAマイクロアレイ、逆転写ポリメラーゼ連鎖反応法、またはタンパク質アレイによって調べられる、請求項1から4のいずれか一項記載の方法。
- フィッシャー比が高い順に遺伝子および/またはタンパク質が選択される、請求項5記載の方法。
- 遺伝子および/またはタンパク質の数が40から100の間である、請求項5または6記載の方法。
- 遺伝子および/またはタンパク質の数が35から45の間である、請求項5または6記載の方法。
- 遺伝子および/またはタンパク質の数が40である、請求項8記載の方法。
- 以下の工程を含む、腫瘍の分化段階を規定する方法:
(a) 非癌性肝臓と前癌肝臓、前癌肝臓と高分化型肝細胞癌(HCC)、高分化型HCCと中分化型HCC、または中分化型HCCと低分化型HCCとの間の比較において、最も高いフィッシャー比を有する遺伝子および/またはタンパク質を選択する工程;ならびに
(b) 遺伝子および/またはタンパク質を用いて、腫瘍の分化段階を規定する工程。 - 以下の工程を含む、腫瘍の分化段階を規定する方法:
(a) 腫瘍の分化段階を規定するための遺伝子および/またはタンパク質の数を決定する工程;
(b) 非癌性肝臓と前癌肝臓、前癌肝臓と高分化型肝細胞癌(HCC)、高分化型HCCと中分化型HCC、または中分化型HCCと低分化型HCCとの間の比較において、最も高いフィッシャー比を有する、工程(a)で決定した数の遺伝子および/またはタンパク質を選択する工程;
(c) 工程(b)で選択した遺伝子および/またはタンパク質のデータを全試料に適用する工程;ならびに
(d) 腫瘍の分化段階を規定する工程。 - 以下の工程を含む、腫瘍の分化段階を規定する方法:
(a) 腫瘍の分化段階を規定するための遺伝子および/またはタンパク質の数を決定する工程;
(b) 非癌性肝臓と前癌肝臓、前癌肝臓と高分化型肝細胞癌(HCC)、高分化型HCCと中分化型HCC、または中分化型HCCと低分化型HCCとの間の比較において、最も高いフィッシャー比を有する、工程(a)で決定した数の遺伝子および/またはタンパク質を選択する工程;
(c) 工程(b)で選択した遺伝子および/またはタンパク質のデータを全試料に適用する工程;
(d) 工程(b)で選択した遺伝子および/またはタンパク質のデータを用いて、最小距離分類子を設計する工程;
(e) 工程(d)で設計した最小距離分類子を全試料に適用する工程;
(f) 工程(b)で選択した全遺伝子および/またはタンパク質のデータを用いて、自己組織化マップを作成する工程;
(g) 工程(f)で作成した自己組織化マップを全試料に適用する工程;ならびに
(h) 腫瘍の分化段階を規定する工程。 - 請求項1から12のいずれか一項の統計学的解析によって選択された遺伝子および/またはタンパク質の発現を調べるためのDNAマイクロアレイ、オリゴヌクレオチドマイクロアレイ、タンパク質アレイ、ノーザンブロッティング、RNaseプロテクションアッセイ法、ウェスタンブロッティング、、および逆転写ポリメラーゼ連鎖反応法の実施に必要なDNAチップ、オリゴヌクレオチドチップ、タンパク質チップ、プローブ、またはプライマーを含む、請求項1から12のいずれか一項記載の方法を実施するためのキット。
- 抗癌剤をスクリーニングするための、請求項1から12のいずれか一項記載の遺伝子および/またはタンパク質の使用。
- 異なる段階にある腫瘍を治療するための、請求項1から12のいずれか一項記載の遺伝子および/またはタンパク質に特異的な抗体の使用。
