Abstract
【解決手段】 デバイスにより実施されるソフトウェアシステムは、頻脈性不整脈の兆候をチェックする手段と、TLRIとDVAIの間の値を計算する手段と、前記値を計算する手段から得られた結果に基づいてPAVを設定する手段と、TLRIをDVAIとPAVの和に等しく設定する手段とを備える。チェックする手段、PAVを設定する手段、およびTLRIを設定する手段は、該ソフトウェアシステムを作動論理により制御する。ソフトウェアシステムは、検出ウィンドウを動作させ、不整脈の兆候の存在または検出を確認した後に必要に応じてPAVを調整する。検出ウィンドウは監視され、検出ウィンドウが必要であるよりも短い場合、特定のペーシングレートに合わせて間隔を調整する。ソフトウェアシステムは、不整脈の兆候があると、ペーシングよりも検出を優先する。
Description
〔背景技術〕
不整脈は、生命を維持する心臓の血液ポンピング作用を妨害する心調律の異常である。不整脈の例には、心室性頻脈および心房性頻脈がある。心室性頻脈は下側の心腔すなわち心室で生じ、心房性頻脈は上側の心腔すなわち心房で生じる。心室性頻脈は、心室内で始まる、3回以上の連続した心室性期外収縮を特徴とする速い心拍である。心室性頻脈は、心臓が体中に十分な血液をポンピングできなくなる可能性があるため、致命的となる可能性がある不整脈である。Medtronic社等の企業は、心室性頻脈が検出された場合に、心室ペーシングパルスを心臓に送出することによって心室性頻脈を上手く処置するために用いることができる埋め込み可能ペースメーカを開発している。
[発明の概要]
本発明は、他の方法では不整脈を検出するためのウィンドウがかなり短くなる、ペーシングレートが高いペーシングの状況、および/または不整脈を検出するための長いウィンドウを維持するために遅いペーシングレートを受け入れなくてはならないペーシングの状況に特に役立つ改良された心臓ペーシング方法を提供する。
Claims (9)
- 検出ウィンドウを監視、制御および調整するように動作する、デバイスにより実施されるソフトウェアシステムであって、
頻脈性不整脈の兆候をチェックする手段と、
TLRIとDVAIの間の値を計算する手段と、
前記値を計算する手段から得られた結果に基づいてPAVを設定する手段と、
TLRIをDVAIとPAVの和に等しく設定する手段と、
を備えること、
前記チェックする手段、前記PAVを設定する手段、および前記TLRIを設定する手段は、該ソフトウェアシステムを作動論理により制御すること、
からなるソフトウェアシステム。 - 前記頻脈性不整脈をチェックする手段は、心室事象を入力する手段を含む請求項1に記載のソフトウェアシステム。
- 前記値を計算する手段は、TLRIとDVAIとの差を計算する手段を含む請求項1に記載のソフトウェアシステム。
- 前記PAVを設定する手段は、TLRIとDVAIとの差を再検討して、該差の値をPAVと比較する手段を含む請求項1に記載のソフトウェアシステム。
- 前記TLRIを設定する手段は、DVAIとPAVとの和をとる手段を含む請求項1に記載のソフトウェアシステム。
- 検出された頻脈性不整脈のエピソードにおいて心房ペーシングの入力を最小化するようにPAVを調整する方法であって、
頻脈性不整脈の可能性を示す兆候を監視すること、
頻脈性不整脈の可能性を示す兆候が確認された場合に、TLRIとDVAIの差をPAVと比較すること、
TLRIとDVAIとの差がPAV未満である場合に、TLRIとDVAIとの差を最小PAVと比較すること、
TLRIとDVAIとの差が最小PAV未満である場合に、PAVを前記最小PAVに設定すること、および
TLRIをDVAIとPAVの和に等しく設定すること、
を含む、PAVを調整する方法。 - 前記TLRIとDVAIとの差をPAVと比較する方法は、該差がPAV以上である場合に、PAVまたはTLRIの変更を指示しないことを含む請求項6に記載のPAVを調整する方法。
- 前記TLRIとDVAIとの差を最小PAVと比較する方法は、該差が最小PAV以上である場合に、PAVをTLRIとDVAIとの差に等しく設定することを含む請求項6に記載のPAVを調整する方法。
- 前記TLRIをDVAIとPAVとの和に等しく設定する方法は、前記PAVを最小PAVに等しく設定することの後に行われる請求項6に記載のPAVを調整する方法。
