Abstract
Description
本出願は2001年5月15日に出願された特許文献1の優先権を主張し、その内容は引用により編入する。
細菌およびウイルスに見いだされるような外来抗原に対する免疫応答は、感染を防御し、そして排除する。しかし異常な免疫応答はアレルギー疾患および自己免疫疾患を引き起こす可能性がある。花粉、ホコリダニ、食物抗原およびハチの針のような外来の、時に無害な物質に対する免疫応答は、枯草熱、喘息および全身性アナフィフラキシーのようなアレルギー疾患を生じ得る。膵島抗原および軟骨抗原のような自己抗原に対する免疫応答は、それぞれ糖尿病および関節炎を導き得る。アレルギー疾患の顕著な特徴は、CD4T細胞の活性化およびB細胞によるIgEの高生産である一方、自己免疫疾患の目立った特徴は、CD4T細胞の活性化および炎症性サイトカインの過剰生産である。現行の治療はアレルギーおよび自己免疫疾患の症状の処置に集中してきており、そして疾患の発症および進行を防止するものではない。
IgE生産B細胞および活性化CD4T細胞のような免疫細胞は、アレルギー疾患および自己免疫疾患の病因に中心的な役割を果たす。本発明はアレルギーおよび/または自己免疫疾患を引き起こす免疫細胞を排除または抑制するために、細胞傷害性Tリンパ球(CTL)を利用する。すなわち疾患の発症および進行は本発明の方法により防止され、または中断される。
a)個体からCD8+T細胞を単離し;
b)クラスI MHC分子を有する抗原提示細胞(APC’s)にT細胞エピトープを添加し;
c)CD8+T細胞を抗原を載せたAPC’sと、T細胞エピトープに特異的な前駆体CD8+T細胞の活性化に十分な時間培養し;
d)活性化CD8+T細胞の増殖に必要な成分の存在下で、カルチャー中の活性化CD8+T細胞を増大させ;そして
e)カルチャーからCD8+T細胞を回収する、
ことを含んでなる。
発明の詳細な説明
1つの態様において本発明は、個体を非−腫瘍自己抗原T細胞エピトープで処置する方法を提供し、この方法は;
a.自然に存在する抗原提示細胞(APC)または自然には存在しない抗原提示細胞系(nnAPC)を調製し、ここで該APCまたは該nnAPCはアレルギーおよび/または自己免疫疾患と関係する約15個までの異なるペプチド分子、好ましくは約10個の異なるペプチド−エピトープ分子を提示することができ、同時に各ペプチドは約6〜12個のアミノ酸長であり、そして好ましくは約8〜10個のアミノ酸長であり、そして約10nM〜100μMの濃度範囲であり;
b.該個体または適当な供与体からCD8+細胞を回収し;
c.該CD8+細胞を該APCまたは該nnAPC細胞系で刺激し;
d.該CD8+細胞を、IL−2、IL−7またはCGM、好ましくはIL−2、またはIL−2とIL−7を組み合わせたようなサイトカインを含む培地に加え;
e.該個体または適当な供与体から回収した非懸濁末梢血単球、あるいはCD8−欠乏末梢血単球を、約10〜50μg/mlのペプチドと混合し;
f.該末梢血単球懸濁液を、所望する末梢血単球を除き懸濁液中のすべての成分を殺菌するために必要な、約3,000〜7,000ラドの範囲、好ましくは5,000ラドのような十分な線量のγ−放射線で照射し;
g.接着末梢血単球を単離し;

用語および定義
本明細書で使用する用語「養子免疫療法」とは、疾患または疾患状態を処置するために供与体または自家Tリンパ球の投与を称し、ここで疾患または疾患状態はクラスI HLA分子に通常関連する不十分または未熟な免疫応答をもたらす。養子免疫療法は、感染または形質転換した細胞の排除がCTLsより活性化されることが示された疾患または疾患状態の適切な処置である。例えば疾患または疾患状態には限定するわけではないが黒色腫、前立腺、胸部、結腸−直腸、胃、気管支および首、膵臓、頸部、卵巣、骨、白血病および肺ガンのような癌および/または腫瘍;B型肝炎、C型肝炎、ヒト免疫不全ウイルスのようなウイルス感染;およびマラリア;結核菌およびリステリア モノサイトゲネスのような細菌感染を含む。
1.定めたペプチドを適用した(pulsed)ヒト樹状細胞(DC);
2.リンパ芽球に向けられ、そしてペプチドを適用した末梢血単核細胞(PBMCs);
3.自然なペプチドが酸で剥がされ、そして目的のペプチドが載せられたリンパ芽球状の細胞系(LCL);

細胞傷害性リンパ球の調製
抗−CD8抗体を用いた陽性選択により白血球除去血サンプルから単離したCD8+細胞を、ヒトクラスI分子(HLA−A2.1)、B7.1、ICAM−1、LFA−3およびB7.2を発現するショウジョウバエ(Drosophila)細胞により提示されるペプチドに関係するIgEおよび/またはCD40Lに対して刺激する。CD8+細胞はIL−2およびIL−7の存在下でペプチドエピトープを添加した自家単球で2回、再刺激する。CTLsはOKT3およびIL−2で非特異的に増大させる。CTL活性は細胞に対して測定し、そしてCD8+T細胞の純度をフローサイトメトリーで評価する。
シュナイダー(Schneider)S2細胞系はキイロショウジョウバエ(Drosophila melanogaster)(オレゴン−R)の卵から公開された手順に従い調製し、そしてアメリカンタイプカルチャーコレクション(CRL 10974)に寄託した。S2細胞は10%ウシ胎児血清を補充した市販のシュナイダーのショウジョウバエ(Drosophila)培地で成長させる。
トランスフェクション用の相補的DNAは以下のように調製した:
HLA−A2.1およびβ−ミクログロブリン:公開された配列に由来するプライマーを使用したK562細胞からの逆転写PCR。
B7.1:公開された配列に由来するプライマーを使用したK562細胞からの逆転写PCR。

ヒトCD8 + 細胞の単離
CD8+細胞は白血球除去血サンプルから、Dynabead(商標)単離法(ダイナル(Dynal))を使用した陽性選択により単離した。抗−ヒトCD8マウスモノクローナル抗体(ヒトガンマグロブリン[Gammagard(商標)]中の50μg/ml)を、1%ヒト血清アルブミン(バクスター−ハイランド(Baxter−Hyland)および0.2%クエン酸Naを補充したダルベッコのPBS中の洗浄した細胞に加える。ゆるやかに混合しながら4℃で45分間インキューベーションした後、細胞を洗浄し、そしてヒツジ抗−マウスIgGでコートしたダイナルの磁気ビーズ(Dynabeads(商標))を含む同じバッファー中に再懸濁する(1:1のビーズ:細胞比)。細胞およびビーズを滅菌した試験管に入れ、そして4℃で45分間ゆるやかに混合する。この時間の終わりに、抗体に結合した細胞は製造元の使用説明(ダイナル)に従いMPC−1(商標)分離機を使用して磁気的に取り出す。CD8細胞−ビーズ複合体の解離は、CD8ペプチド59−70(AAEGLDTQRFSG、配列番号52)の存在下で37℃で45分間インキューベーションすることにより達成する。遊離のビーズは磁気的に除去し、そしてCD8細胞は純度を評価するためにフローサイトメトリーによりカウントし、そして分析する。CD8+細胞の回収は典型的には80%より多い。表1は抗−CD8−抗体を用いた陽性選択により、正常ヒトPBMC調製物からの14の別個のCD8+調製物の細胞組成をまとめる。
初回刺激:トランスフェクトしたショウジョウバエ(Drosophila)S2細胞は10%ウシ胎児血清およびCuSO4を補充したシュナイダーの培地中で(106細胞/ml)、27℃にて24時間インキューベーションする。細胞を回収し、洗浄し、そして100μg/mlのヒトチロシナーゼ369−377(RWJPRI)を含む昆虫X−プレス(press)培地(バイオウィッタカー(BioWhittaker)に再懸濁する。27℃にて3時間インキューベーションした後、S2細胞をCD8+細胞と、1:10の比率で10%自家血清を補充したRPMI培地(ギブコ(Gibco))中にて混合する。細胞混合物を37℃で4日間インキューベーションし、この間、ショウジョウバエ(Drosophila)の細胞は死ぬ。5日目に、IL−2(20U/ml)およびIL−7(30U/ml)を培地交換と共に加え、チロシナーゼ特異的CTL群に増大させる。
再−刺激:白血球除去血の時点で得た、凍結した自家CD8−欠乏PBMCを解凍し、洗浄し、そして10%自家血清(β2ミクログロフリンの供給源として)および20μg/mlのペプチドエピトープを含むRPMI培地中に、106細胞/mlで再懸濁する。γ−照射(5,000ラド)の後、細胞を37℃で2時間インキューベーションする。非−接着細胞はダルベッコのPBSで洗浄することにより除去する。接着単球は10%自家血清および10μg/mlのペプチドエピトープを含むHepes−緩衝化RPMI培地中で90分間インキューベーションすることによりチロシナーゼエピトープを載せる。上清を除去し、そしてショウジョウバエ(Drosophila)−活性化CD8+細胞懸濁液(10%自家血清を含むRPMI培地中の3x106細胞/ml)を10のCD8+細胞対1の接着単球の比率で加える。37℃で培養の3〜4日後、IL−2(20U/ml)およびIL−7(30U/ml)を、培地交換と共に加え、エピトープ特異的CTL群を選択的に増大させる。
非−特異的拡大:CD8を非特異的に拡大させ、そしてそれらを自家血清、抗−CD3モノクローナル抗体(OKT(商標)3)、IL−2およびγ照射した自家PBMCを補充したRPMI培地中で培養する。
活性および純度に関するアッセイ
CTLアッセイ:エピトープを持つ(標的)細胞を51Cr放出アッセイで標的細胞として使用する。4%ウシ胎児血清、1%HEPESバッファーおよび0.25%ゲンタマイシンを含むRPMI培地中の5x106の標的細胞を、37℃で1時間、0.1mCiの51Crで標識する。細胞を4回洗浄し、そして10%ウシ胎児血清を含むRPMI(ハイクローン(HyClone))中で105細胞/mlに希釈する。96−ウェルのマイクロタイタープレート中で、100μlのエフェクターCTLおよび100μlのペプチドを載せた51Cr−標識標的細胞を100:1、20:1および4:1(エフェクター:標的)の割合で合わせる。K562細胞を20:1(K562)の比率で加えて、ナチュラルキラー細胞のバックグラウンド溶解を減少させる。非−特異的溶解は51Crで標識した細胞を使用して上記のように評価するがエピトープを持たない。51Crの自然な放出および最大放出を測定するための対照を二連で含める。37℃で6時間インキューベーションした後、プレートを遠心し、そして上清をカウントして51Cr放出を測定する。
試験の提案
この実施例は、以下の要因に従い評価されるアレルギー疾患の処置における細胞傷害性T細胞注入の効力を教示する。
1.インビトロ免疫感作後の再注入自家CTLの安全性および寛容(toleration)
2.限界希釈分析を考慮した全身循環における注入CTLの動力学
3.ラジオシンチグラフィーによるCTLの全身配置(disposition)
4.免疫組織学による生検小節の細胞組成(CTL、TH、NK、B細胞);および
5.測定可能な損傷の退行および2カ月間にわたる応答期間
エクスビボで生成した自家CTLでの処置
すべての患者は少なくとも1回の自家CTL注入を受ける。各患者に投与したサイクル数および細胞の用量は表1にまとめる。インビトロで生成した細胞数は、アフェレーシス(aphaeresis)法から単離したPBMC数および各PBMC調製物に存在するCD8+T細胞数のような患者に関連する因子に依存する。インビトロで生成した細胞のすべてが供与体に再注入されるので、各患者に投与される用量は必然的に変動する。患者間の用量を標準化するために、計算された「効力」スコアを各用量について記録する。この値は細胞の総数にペプチドを載せた標的細胞で得られた溶解活性を掛けることにより得られる。患者は各サイクルの終わりの彼らの臨床的状態に基づき、第2、3または4サイクルの処置に入る。単離される自然なCD8+T細胞の総数は、各PBMC調製物中のその割合に依存する。CD8+T細胞の割合は患者間で変動する。エクスビボでのCTLの生成に関する手順は、明細書および上記実施例1に教示されている。
IFNα−2bに応答するクラスIおよび黒色腫関連抗原のアップレギュレーション
抗原特異的なCTLがIgE生産細胞をインビボで溶解する能力を強化する試みでは、低用量のIFNα−2bをCTL注入前に5日間連続して投与し、そしてさらに4週間、週に3回投与する。サイトカインに対するインビボ応答を測定する1つの方法は、連続的な時点で得た生検体を特異的抗体を用いた陽性染色に関して免疫組織化学的分析により評価することである。
エクスビボで生成したCTLの抗原特異性
すべての患者から生成したCTLは、生検材料が系を樹立するために利用できれば、放出の日にペプチドを載せたT2標的、HLA−A2 IgE生産M−14クローン4細胞系および自家M−14細胞系に対して評価する。細胞の各調製用量は、その細胞溶解活性について評価する。いずれかの各ペプチドのみ、またはすべてのペプチドを同時に提示するペプチドを載せたT2細胞を使用して、各患者について生じるCTL応答の特異性を決定する。内因的に発現した、HLA−A2に結合した抗原を持つ細胞を溶解する能力は、HLA−A2に合った系または自家細胞系を用いて評価する。細胞溶解活性に加えて、抗原−特異性は、特異的なペプチド刺激に応答して作られる細胞内ガンマインターフェロン生産を検出するために確立された方法を用いて評価する。エクスビボのプロトコールの終わりに、生成したCTLはこの方法により評価する。各ペプチドに特異的な細胞の割合を各々記録する。患者に由来する各々の大量のCD8カルチャーの特異的細胞の総数は、そのT細胞群で検出される各ペプチド特異性を加えることにより算出する。特異的細胞の総数における増加が、各連続的処理サイクルで検出される。
アネルギー状態の存在はCTLを生成する能力を排除せず、または臨床的応答を防止しなかった
このプロトコール下で処置された患者のほとんどが、事前に医学的介入を受けている。前処置皮膚試験を行って、7種の共通抗原のパネルに対するアネルギー応答がエクスビボでCTLを生成できないこと、または証明された臨床的応答の防止に相関するかどうかを決定する。エクスビボでCTLを生成する能力は患者の前処置皮膚試験の結果と相関しない。
IgEタンパク質配列に由来する抗原性ペプチドの同定:
マウスIgEの2つの対立遺伝子(IgEaおよびIgEb)はすでに記載された(PO6336)。IgEaおよびIgEbのアミノ酸配列の並列配置により、95%のアミノ酸配列が同一であることが示された。14個のアミノ酸の差異がCH1とCH2間の連結領域に位置し、そして別の5個のアミノ酸の差異がCH3とCH4間の連結領域に位置する。IgEbのアミノ酸配列をLdおよびDbMHCクラスI分子に関する結合モチーフを含む9merのペプチド配列について、http://bimas.dcrt.nih.gov/molbio/hla bind/で利用可能なバイオインフォーマチックス&モレキュラーアナリシスセクション(Bioinformatics & Molecular Analysis Section)のソフトウェアを使用することにより分析した。このプログラムはMHCクラスI分子に対して予測されるる解離のハーフタイム(half time)に基づき有力なノナペプチドの順位をつける。順位付けの分析に基づき、Ld結合モチーフを持つ8個のペプチドおよびDb結合モチーフを持つ5個のペプチドが合成に選択された(表1)。
これらのIgEペプチドがCTL応答を誘導する能力をインビトロで評価した。すでに記載したように、MHCクラスIに加えてB7−1およびICAM−1でトランスフェクトしたショウジョウバエ(Drosophila)細胞は、インビトロで休止している自然なCD8T細胞の活性化において有力な抗原提示細胞(APC)である。休止している自然なCD8T細胞をマウスのリンパ節から精製し、そしてLdまたはDbに加えてB7−1およびICAMでトランスフェクトしたペプチドを載せたたショウジョウバエ(Drosophila)細胞と、サイトカインの不存在下で培養した。IL−2(20単位/ml)を3日目に加え、そしてその後、1日おきに加えた。ペプチドを載せたRMAS(Kb、Db)細胞またはRMAS−Ld細胞に対するCTL活性を9日目に測定した。図1に示すように、IgE44ペプチドにより誘導されるCTLは、IgE44ペプチドを載せたRMAS細胞を特異的に溶解し、標的細胞単独または他のIgEペプチドを載せた標的細胞のいずれもIgE44特異的CTLにより認識されなかった。
抗−IgE特異的CTLによるIgE応答の抑制
IgEペプチドにより誘導されるCTLはインビトロで標的細胞を特異的に殺すので、これらのCTLがIgE応答をインビボで抑制することができれば興味深いと考えた。マウスは大変低い血清IgEを有し、そしてアレルギー応答を自然には生じない。水酸化アルミニウムで沈殿した卵白アルブミンを使用してマウスに抗原特異的IgE応答を誘導した。図3に示すように、OVAに水酸化アルミニウムを加えて用いた2回の免疫感作後、免疫感作したマウス中の全血清IgEおよび卵白特異的IgEの両方は高く、そしてIgEレベルはこれらのマウスをOVAで鼻内追加免疫した後にさらに上昇した。
