Abstract
Description
本出願は、同時係属中の、2001年2月14日登録、名称「骨埋め込み装置」なる米国特許出願番号第09/781,847号の一部継続出願である。さらに、この特許出願自体が、同時係属中で、共に2000年11月8日登録の米国特許出願番号第09/708,940号および第09/708,292号の一部継続出願である。前記三件の係属出願の主題全体を引用することにより本明細書に含めることにする。
技術分野
本発明は、患者の脊柱または骨盤の骨に埋め込むための装置を目的とし、特に、埋め込まれた場合、骨内部でのぐらつきや骨からの抜去にたいして抵抗性を持つ装置を目的とする。本発明はまた、隣接脊椎骨の骨体が融合する間、それらの椎体を密着・安定化させるための装置をも目的とする。
Claims (82)
- 患者の脊柱または骨盤の骨にたいする埋め込み装置であって、埋め込まれた場合、骨内部でのぐらつきや、骨からの抜去にたいして抵抗性を示す装置であって、
患者の脊柱または骨盤の骨に密着する台であって、脊柱固定インプラントに接続するための構造を含む台、および、
前記台の回転によって骨の中に埋め込まれる少なくとも1本の螺旋スパイクとを含み、前記少なくとも1本の螺旋スパイクは、前記台から接線方向に突出し長軸の周囲に延び、前記少なくとも1本の螺旋スパイクは、遠位端において、前記台が回転するにつれて骨中に侵入する先端部を有し、
前記少なくとも1本の螺旋スパイクは、埋め込まれると、前記少なくとも1本の螺旋スパイクが前記台から遠ざかって延びるにつれて直径が増す円錐形を有する、
ことを特徴とする装置。 - 前記少なくとも1本の螺旋スパイクは、前記少なくとも1本の螺旋スパイクが第1最大直径を持つ第1条件、および、前記少なくとも1本の螺旋スパイクの少なくとも一部が、前記第1最大直径よりも大きな第2最大直径に拡大する第2条件を有することを特徴とする、請求項1の装置。
- 前記少なくとも1本の螺旋スパイクは、前記第1条件において第1軸長を、前記第2条件において第2軸長を有し、前記第2軸長は前記第1軸長よりも小さいことを特徴とする、請求項2の装置。
- 前記少なくとも1本の螺旋スパイクの少なくとも1部は、指定の転移温度範囲を上回る、また、下回る温度変化にたいして反応する形状記憶合金から形成され、ここに、前記少なくとも1本の螺旋スパイクは、前記少なくとも1本の螺旋スパイクの温度が前記指定の転移温度範囲を下回る場合には前記第1条件となり、前記少なくとも1本の螺旋スパイクは、前記指定の転移温度範囲を越えて加熱されると前記第2条件となり、前記少なくとも1本の螺旋スパイクは、前記第2条件において、骨の中に埋め込まれることを特徴とする、請求項3の装置。
- 前記少なくとも1本の螺旋スパイクは、前記台に接続される近位端において接合部、および、前記接合部と前記先端部との間に延びる中間部をさらに有し、前記中間部と前記先端部の内の少なくとも一方が前記形状記憶合金から形成されることを特徴とする、請求項4の装置。
- 請求項5の装置であって、前記長軸の周囲に延びる一対の螺旋スパイクを含み、前記一対の螺旋スパイクの前記近位端は、互いに180°隔てられていることを特徴とする装置。
- 請求項5の装置であって、前記長軸の周囲に延びる3本の螺旋スパイクを含み、前記3本の螺旋スパイクの前記近位端は、互いに120°隔てられていることを特徴とする装置。
- 前記台は骨に直面する第1面を有し、前記第1面は、骨の外面と密着するために、骨の外面の形と相補的な形を有することを特徴とする、請求項1の装置。
- 前記少なくとも1本の螺旋スパイクは中実断面を有することを特徴とする、請求項1の装置。
- 前記少なくとも1本の螺旋スパイクは管状断面を有することを特徴とする、請求項1の装置。

複数の骨の間に延び、かつ、それらの骨を連結するための脊柱固定インプラントとを含み、
前記少なくとも1個のアンカーは、骨に向き合う第1面を有する台を含み、前記台は、前記脊柱固定インプラントに接続されるための構造をさらに有し、
前記少なくとも1個のアンカーは、前記台の回転によって骨に埋め込まれる、少なくとも2本の螺旋スパイクをさらに含み、前記少なくとも2本の螺旋スパイクは互いに隔てられ、前記台において前記第1面から突出し、前記少なくとも2本の螺旋スパイクは長軸周囲に延び、前記少なくとも2本の螺旋スパイクはそれぞれ、遠位端において、前記台が回転するにつれて骨の中に侵入する先端部を有し、
前記少なくとも2本の螺旋スパイクは、埋め込まれると、前記少なくとも2本の螺旋スパイクが前記台から遠ざかって延びるにつれて直径が増す円錐形を有する、
ことを特徴とする装置。 - 請求項12の装置であって、第1骨にたいする埋め込み用の第1アンカー、および、前記第1骨と隔てられた第2骨にたいする埋め込み用の第2アンカーを含むことを特徴とする装置。

前記台の回転につれて、椎体の前記隣接ペアそれぞれの中に埋め込まれる少なくとも1本の螺旋スパイクを含み、前記少なくとも1本の螺旋スパイクは、埋め込まれることによってこれら椎体のそれぞれに密着し、それによってこれら椎体同士を結合し、前記少なくとも1本の螺旋スパイクは前記台から突出し長軸の周囲に延び、前記少なくとも1本の螺旋スパイクは、遠位端において、前記台が回転するにつれて、第1および第2面に侵入し、かつ、椎体の前記隣接ペアの中にネジ込まれる先端部を有し、
前記少なくとも1本の螺旋スパイクは、少なくとも部分的に、これら椎体の融合を促進する物質を受容する内腔を定め、
前記少なくとも1本の螺旋スパイクの少なくとも一部は、指定の転移温度範囲を上回る、また、下回る温度変化にたいして反応する形状記憶合金から形成され、ここに、前記少なくとも1本の螺旋スパイクは、前記少なくとも1本の螺旋スパイクの温度が前記指定の転移温度範囲を下回る場合には第1形態を有し、前記少なくとも1本の螺旋スパイクは、前記指定の転移温度範囲を越えて加熱されると第2形態を有し、前記少なくとも1本の螺旋スパイクは、前記第2形態において骨の中に埋め込まれる、
ことを特徴とする装置。 - 前記少なくとも1本の螺旋スパイクは、埋め込まれると、前記少なくとも1本の螺旋スパイクが前記台から遠ざかって延びるにつれて直径が増す円錐形を有することを特徴とする、請求項76の装置。
- 前記少なくとも1本の螺旋スパイクは、前記第1形態において第1軸長を、前記第2形態において第2軸長を有し、前記第2軸長は前記第1軸長よりも小さいことを特徴とする、請求項77の装置。
- 前記少なくとも1本の螺旋スパイクは、前記第1形態において第1最大直径を有し、かつ、前記少なくとも1本の螺旋スパイクの少なくとも一部は、前記第2形態において第2最大直径に拡大し、ここに、前記第2最大直径は前記第1最大直径よりも大きいことを特徴とする、請求項78の装置。
- 前記少なくとも1本の螺旋スパイクは、前記台に接続される近位端において接合部、および、前記接合部と前記先端部との間に延びる中間部をさらに有し、前記中間部と前記先端部の内の少なくとも一方が前記形状記憶合金から形成されることを特徴とする、請求項79の装置。
- 前記長軸の周囲に延びる一対の螺旋スパイクを含む請求項80の装置であって、前記一対の螺旋スパイクの前記近位端は互いに180°隔てられていることを特徴とする装置。
- 前記長軸の周囲に延びる3本の螺旋スパイクを含む請求項80の装置であって、前記3本の螺旋スパイクの前記近位端は互いに120°隔てられていることを特徴とする装置。
