Abstract
Description
軸方向に末端に配置された2つのねじ山セグメントと、ねじ山なしの中央セグメントとを有する骨ねじが、US5,019,079号公報から公知である。中央セグメントの直径は、遠位のねじ山セグメントの雄ねじの外径にほぼ一致するが、近位のねじ山セグメントの雄ねじのねじ内径よりも大きく、この結果、骨折位置における両方の骨フラグメントの横方向の安定化のために中央セグメントを利用することができる。この骨ねじの構成の不都合な点は、両方の雄ねじが様々なねじピッチを有するので、移植の際に、様々なステップ、すなわち骨フラグメントの整復、骨フラグメントの圧縮、およびねじ頭の沈下を互いに別々に実施できないことである。
本発明による骨ねじは、本質的に、長手軸線に対し同軸におよび骨ねじの末端に配置されたねじ山セグメントを含み、前部もしくは後部ねじ山セグメントのねじピッチSVとSHは等しい。これによって、両方の骨フラグメントの整復および圧縮の実施後に、このために後部ねじ山セグメントが例えば移植器具にねじ込まれ、かつ近位骨フラグメントにまだねじ込まれていない間に、前部ねじ山セグメントのみが遠位骨フラグメントにねじ込まれるのであるが、当該の整復および圧縮の実施後に、同様に後部ねじ山セグメントが近位骨フラグメントに完全に下降されるまで、この際に骨フラグメント位置を互いに変更することなしに、かつ両方の骨フラグメントの圧縮を変更することなしに、骨ねじを骨フラグメントにさらにねじ込み可能であることが達成される。両方のねじ山セグメントは、前部ねじ山セグメントの外径が後部ねじ山セグメントの雄ねじのねじ内径よりも小さいように形成される。

−ねじ頭の沈下
が別々にかつ管理して実施できることにある。
前部ねじ山セグメントの外径よりも大きなねじ内径を有する後部ねじ山セグメントを形成することによって、後部ねじ山セグメントと、骨フラグメント内の前部ねじ山セグメントのためにすでにタップ立てられたねじ山との相互作用を避けることができる。
前部および後部ねじ山セグメントの雄ねじは、セルフタップ式のねじとして形成されることが好ましい。
本発明のさらに有利な実施態様は、従属請求項に特徴づけられている。
本発明および本発明の発展形態について、複数の実施例の部分概略図を参照して以下にさらに詳細に説明する。
両方の骨フラグメント13;14を整復、圧縮および固定するために、最初に、近位の骨フラグメント13を貫通して遠位骨フラグメント14に進入する孔21が設けられ、その直径は、骨ねじ1の前部ねじ山セグメント5の雄ねじ8のねじ内径DKV(図1)に対応する。
骨ねじ1は、ねじ頭が邪魔となるであろう箇所、例えば、関節に近い骨折の場合、舟状骨骨折、小フラグメント等での関節内固定、腱、神経の固定,血管近傍で行われる固定等の場合に使用されることが好ましい。

Claims (6)
- A)長手軸線(2)と、前端(3)と、後端(4)と、前記後端(4)のドライバ(16)を収容するための手段(11)を有し、ならびに
B)ねじ内径DKVと外径DVSとねじピッチSVとを有する雄ねじ(8)を含む、前記長手軸線(2)に対し同軸におよび軸方向に末端に配置された前部ねじ山セグメント(5)と、
C)ねじ内径DKHと外径DHSとねじピッチSHとを有する雄ねじ(9)を含む、前記長手軸線(2)に対し同軸におよび軸方向に末端に配置された後部ねじ山セグメント(7)と、を有する2つの骨フラグメントを連結するための骨ねじ(1)において、
D)前記前部ねじ山セグメント(5)の前記外径DVSが前記後部ねじ山セグメント(7)の前記ねじ内径DKH以下であり、
E)前記前部ねじ山セグメント(5)の前記ねじピッチSVと前記後部ねじ山セグメント(4)のねじピッチSとが等しいことを特徴とする骨ねじ(1)。 - 両方の前記ねじ山セグメント(5;7)の間の前記骨ねじ(1)が、同様に前記長手軸線(2)に対し同軸の外径DMSの中央セグメント(6)を含み、前記外径DMSが前記前部ねじ山セグメント(5)の前記雄ねじ(8)の内径以下であることを特徴とする、請求項1に記載の骨ねじ。
- 請求項1または2に記載の前記骨ねじ(1)によって骨フラグメントを整復、圧縮および固定するための装置であって、
a)同軸の導通中心孔(17)と前端(18)と後端(19)とを含む円筒状または角柱の外科手術用移植器具(15)を含む装置において、
b)前記中心孔(17)が、前記器具(15)の前記前端(18)から深さTまで延在する雌ねじ(20)付きの拡張部分(24)を備え、前記雌ねじが前記骨ねじ(1)の前記後部セグメント(7)の前記雄ねじ(9)に対し相補的であり、
c)前記深さがT>=Lであり、ここで、Lは前記骨ねじ(1)の前記後部ねじ山セグメント(7)の長さであることを特徴とする装置。 - 骨フラグメントを整復、圧縮および/または固定するための方法であって、
A)処理すべき前記骨フラグメント(13;14)に孔(21)を装着するステップと、
B)後部ねじ山セグメント(7)が中心孔(17)内に完全に収容されかつ骨ねじ(1)の後端(4)が移植器具(15)の前記中心孔(17)内の肩部(22)に接触するまで、前記骨ねじ(1)の前記後部ねじ山セグメント(7)を前期移植器具(15)の雌ねじ(20)付きの前記中心孔(17)にねじ込むステップと、
C)前部ねじ山セグメント(5)が前記遠位骨フラグメント(14)にねじ込まれるまで、かつ前記移植器具(15)の前端(18)が前記近位骨フラグメント(13)の表面に位置するまで、前記移植器具(15)から突出する前記骨ねじ(1)の部分を前記孔(21)にねじ込むステップと、
D)両方の前記骨フラグメント(13;14)が必要な寸法に整復されるまで前記移植器具(15)をさらに回転するステップと、
E)前記移植器具(15)が、前記骨ねじ(1)内のドライバ(16)を収容するための手段(11)に導入されるまで、前記移植器具(15)の前記中心孔(17)を通して前期ドライバ(16)を導入ステップと、
F)前記後部ねじ山セグメント(7)が前記移植器具(15)から回して外され、また前記近位骨フラグメント(13)に完全にねじ込まれるまで、前記ドライバ(16)の回転によって前記孔(21)に前記骨ねじ(1)をさらにねじ込むステップと、を含む方法。 - ステップD)で整復を実施した後に、両方の前記骨フラグメント(13;14)の所望の圧縮を達成するまで前記移植器具(15)をさらに回転するステップをさらに含むことを特徴とする、請求項4に記載の方法。
- 前記ドライバ(16)を用いて前記孔(21)に前記骨ねじ(1)をねじ込む間に、前記移植器具(15)の回転に対抗して保持するステップをさらに含むことを特徴とする、請求項4または5に記載の方法。
