Abstract
【解決手段】脳内の部分的に閉塞した血管などの病変血管を治療するための自己拡張型ステント及びステントデリバリーシステムを提供する。自己拡張型ステントは、コアワイヤの周りに配置された基端円筒部材、中間円筒部材、及び先端円筒部材を含む細長いコアに取り付けられる。アンカー部材を含むステントが、そのアンカー部材が円筒部材間のギャップ内にインターロックするように、中間円筒部材に取り付けられている。バルーンカテーテルがステントを中間円筒部材に対して圧縮して拘束し、ステントをそのコアワイヤにインターロックするように、バルーンカテーテルのデリバリー内腔内に配置されている。細長いコアワイヤはまた、塞栓細片を捕捉するために捕捉バスケットを含む。

Description
(1)狭窄を治療する方法であって、
デリバリー内腔、先端部分、及びその先端部分に取り付けられた膨張可能なバルーンを有する細長いカテーテルから成るバルーンカテーテルと、前記細長いカテーテルのデリバリー内腔内に配置された、径方向外向きに延びたストッパー部材を有する細長いコア部材と、前記細長いコア部材に取り付けられ、前記ストッパー部材との係合により、前記細長いコア部材が前記デリバリー内腔内を移動する時にそのデリバリー内腔内を移動できる自己拡張型ステントとを用意するステップと、
前記バルーンカテーテルを患者の血管内に挿入するステップと、
前記バルーンカテーテルを前記血管内を進めて、前記バルーンカテーテルを前記血管内の狭窄部にわたって配置するステップと、
前記細長いコア部材に取り付けられた前記自己拡張型ステントを前記細長いカテーテルの前記デリバリー内腔内に挿入し、前記自己拡張型ステント及び前記細長いコア部材を前記デリバリー内腔内を先端側に進めて、前記自己拡張型ステントが前記狭窄部にほぼ整合するようにするステップと、
前記バルーンを膨張させるステップと、
前記細長いカテーテルを基端側に引き戻して、前記自己拡張型ステントを前記血管内で拡張させるステップと、
前記自己拡張型ステントを前記細長いコア部材から外すステップと、
前記バルーンカテーテル及び前記細長いコア部材を前記患者の前記血管から引き抜くステップとを含むことを特徴とする方法。
前記バルーンカテーテルを患者の血管内に挿入するステップと、
前記バルーンカテーテルを前記血管内を先端側に進めて、前記バルーンカテーテルを前記血管内の狭窄部にわたって配置するステップと、
前記細長いコアワイヤに取り付けられた前記自己拡張型ステントを前記細長いカテーテルの前記デリバリー内腔内に挿入し、前記自己拡張型ステント及び前記細長いコアワイヤを前記デリバリー内腔内を先端側に進めて、前記自己拡張型ステントが前記狭窄部にほぼ整合するようにするステップと、
前記バルーンを膨張させるステップと、
前記バルーンを収縮させるステップと、
前記細長いカテーテルを基端側に引き戻して、前記自己拡張型ステントを前記血管内で拡張させるステップと、
前記自己拡張型ステントを前記細長いコアワイヤから外すステップと、
前記細長いカテーテル及び前記細長いコアワイヤを前記患者の前記血管から引き抜くステップとを含むことを特徴とする方法。
ガイドワイヤを用いて前記バルーンカテーテルを患者の血管内に挿入するステップと、
前記ガイドワイヤ及び前記バルーンカテーテルを進めて、前記バルーンカテーテルを前記血管内の狭窄部にわたって配置するステップと、
前記ガイドワイヤを取り出すステップと、
前記細長いコア部材に取り付けられた前記自己拡張型ステントを前記細長いカテーテルの前記デリバリー内腔内に挿入し、前記自己拡張型ステント及び前記細長いコア部材を前記デリバリー内腔内を先端側に進めて、前記自己拡張型ステントが前記狭窄部にほぼ整合するようにするステップと、
前記細長いカテーテルの前記膨張内腔内に流体を注入して前記バルーンを膨張させるステップと、
前記膨張内腔内から前記流体を除去して前記バルーンを収縮させるステップと、
前記細長いカテーテルを基端側に引き戻して、前記自己拡張型ステントを前記血管内で拡張させるステップと、
前記円筒状アンカー部材を前記ギャップから外して、前記自己拡張型ステントを前記細長いコア部材から解放するステップと、
前記バルーンカテーテル及び前記細長いコア部材を前記患者の前記血管から引き抜くステップとを含むことを特徴とする方法。

Claims (9)
- 自己拡張型ステント及びステントデリバリーシステムであって、バルーンカテーテル、細長いコア部材、及び自己拡張型ステントを含み、
前記バルーンカテーテルが、デリバリー内腔、膨張用内腔、先端部、及びその先端部分に取り付けられ、前記膨張用内腔に連通した膨張可能なバルーンを有する細長いカテーテルから成り、
前記細長いコア部材が、前記細長いカテーテルの前記デリバリー内腔内にスライド可能に配置され、その細長いコア部材の周りに配置された基端円筒部材及び先端円筒部材を含み、前記先端円筒部材が、所定の長さを有するギャップを画定するように前記基端円筒部材の先端側に離間して配置されており、
前記自己拡張型ステントが、互いに連結された複数のストラット部材によって形成された複数のセルを含む薄い壁部と前記複数のストラット部材の1つに配置された円筒状アンカー部材とを有する小径の骨格管状部材からなり、前記円筒状アンカー部材が、前記基端円筒部材と前記先端円筒部材のギャップの長さよりも短い長さを有しており、前記円筒状アンカー部材が前記基端円筒部材と前記先端円筒部材の間の前記ギャップ内にインターロックして前記自己拡張型ステントが前記細長いコア部材上に保持されるように、前記自己拡張型ステントが前記円筒部材の一方に取り付けられていることを特徴とする自己拡張型ステント及びステントデリバリーシステム。 - 前記細長いコア部材がワイヤの形態をとることを特徴とする請求項1に記載の自己拡張型ステント及びステントデリバリーシステム。
- 前記骨格管状部材が、前記複数のストラット部材の1つに形成されたスレッドを含み、前記円筒状アンカー部材が、前記スレッドに巻き付けられた、放射線不透過性材料から成る螺旋状に巻かれたコイルの形態をとることを特徴とする請求項2に記載の自己拡張型ステント及びステントデリバリーシステム。
- 前記自己拡張型ステントが、8個の円筒状アンカー部材を含み、前記各円筒状アンカー部材が、前記8個の円筒状アンカー部材のそれぞれに形成されたスレッドに巻き付けられた、放射線不透過性材料から成る螺旋状に巻かれたコイルの形態をとることを特徴とする請求項2に記載の自己拡張型ステント及びステントデリバリーシステム。
- 自己拡張型ステント及びステントデリバリーシステムであって、
デリバリー内腔、先端部分、及びその先端部分に取り付けられた膨張可能なバルーンを有する細長いカテーテルから成るバルーンカテーテルと、
前記細長いカテーテルの前記デリバリー内腔内にスライド可能に配置された、径方向外向きに延びたストッパー部材を有する細長いコア部材と、
前記細長いコア部材に取り付けられ、前記ストッパー部材との係合により、前記細長いコア部材が前記デリバリー内腔内を移動する時にそのデリバリー内腔内を移動できる自己拡張型ステントとを含むことを特徴とする自己拡張型ステント及びステントデリバリーシステム。 - 前記細長いコア部材がワイヤの形態をとることを特徴とする請求項5に記載の自己拡張型ステント及びステントデリバリーシステム。
- 前記バルーンカテーテルが、前記膨張可能なバルーンと連通した膨張用内腔を含むことを特徴とする請求項6に記載の自己拡張型ステント及びステントデリバリーシステム。
- 前記ストッパー部材が、前記細長いコア部材の周りに配置された円筒コイルの形態をとることを特徴とする請求項7に記載の自己拡張型ステント及びステントデリバリーシステム。
- 前記自己拡張型ステントが、互いに連結された複数のストラット部材によって形成された複数のセルを含む薄い壁部と前記複数のストラット部材の1つの周りに配置された円筒状アンカー部材とを有する小径の骨格管状部材から成ることを特徴とする請求項8に記載の自己拡張型ステント及びステントデリバリーシステム。
