Abstract
Description
(Vacuum therapy in the treatment of acute suppurative diseases of soft tissues and suppurative wound)」、Davydovら、Vestn. (Pathenogenic mechanism of the effect of vacuum therapy on the course of the wound process)」、Davydovら、Khirurigiia、1990年6月(「1990年6月論文」)、および「化膿性授乳乳腺炎の治療における真空療法(Vacuum therapy in the treatment of suppurative lactation mastitis)」、Davydovら、Vestn.
図示するように、最高の負圧あるいは真空は250mmHg(4.83−PSIG)である。
所望の圧力=圧力設定+/−デルタp(+/−は圧力が増加されているか低下されているかによる)

[0001]
The present invention relates to aggressive wound therapy devices, and more particularly to vacuum wound therapy devices. More particularly, the present invention relates to controlling the vacuum applied by a vacuum trauma treatment device.
[Background Art]
[0002]
Medical professionals, such as nurses or doctors, routinely treat patients with surface trauma of various sizes, shapes, and weights. Active wound therapy, as opposed to passive wound therapy, utilizes the regulation of the environment to actively promote healing. It is known that controlling the topical atmosphere in contact with trauma can increase the progress of healing. Several aggressive trauma treatment strategies are known, including hyperbaric therapy, thermal therapy, and negative pressure therapy.
[0003]
In negative pressure therapy, the trauma is exposed to a lower air pressure than the surrounding air pressure. By applying a negative pressure or vacuum to the wound, exudate including dirt and bacteria is extracted from the wound to further promote healing. Some wound dressings are equipped with a device that applies a vacuum to the wound through a bandage to pull out exudate and promote healing. However, it is known that sudden changes in the level of negative pressure applied to open wounds (chronic wounds) can cause patient discomfort.
[0004]
It is also well known to use trauma dressings to promote wound healing. A vacuum bandage is a bandage that has a lid that seals around the wound, under which vacuum acts on the surface of the wound. The vacuum applied to the surface of the wound promotes healing of the chronic wound. Typically, a suction tube is provided to draw up exudate from the wound, and the suction tube may be used to create a vacuum under the lid. If the lid is a plastic lid, as is common, more comfortable for the patient, a perforated packing may be provided under the lid to provide a vacuum space. Further, it is known that heaters in trauma treatment devices promote healing. The following U.S. patents define vacuum or heat treatment, or vacuum and heat treatment bandages and devices. U.S. Patent Nos. 6,095,992, 6,080,189, 6,071,304, 5,645,081, 5,636,643, 5,358,494, 5,298,015 Nos. 4,969,880, 4,655,754, 4,569,674, 4,382,441, and 4,112,947. All of these references are hereby incorporated by reference for the purpose of disclosing the nature of vacuum or heat treatment of such trauma.
[0005]
For example, as shown in US Patent No. 5,645,081 (hereinafter '081), a method of treating tissue damage is provided by applying negative pressure to the trauma. Negative pressure is applied for a sufficient amount of time and scale to promote tissue migration and facilitate wound closure. In FIG. 1 of the '081 patent, an open cell polyester foam section covering the wound, a plastic hollow tube with one end inserted into the foam section and the other end attached to a vacuum pump, Disclosed are an adhesive sheet overlying the section and tubing that creates a seal that can be applied to the skin surrounding the wound and create a vacuum when the suction pump is activated. The '081 patent further teaches the use of negative pressure at 0.1 to 0.99 atmospheres, wherein the air pressure is generally continuous and the air pressure is reduced only when changing the wound dressing. it can. The '081 patent teaches that air pressure is applied periodically with alternating applied and non-applied cycles. In a preferred embodiment, the air pressure is applied or not applied in 5 minute cycles.
[0006]
The following provisional application of the same applicant as the present applicant is also specifically incorporated herein by reference. No. 09 / 369,113, filed on Aug. 5, 1999, entitled Trauma Healing Device; U.S. Application No. 09/369, filed Nov. 29, 2000, entitled Vacuum Vacuum Therapy and Irrigation Dressing. No. 725,352 (Publication No. US-2002-0065494-A1 published on May 30, 2002) and U.S. Application No. 09 / 725,666, filed on November 29, 2000, entitled Trauma Treatment Device. .
[0007]
Various prior art documents teach the significance of vacuum dressings, or applying a vacuum to the surface of chronic wounds. There are also several Russian documents that demonstrate the effectiveness of vacuum therapy. Examples of such prior art documents each discuss the use of a vacuum applied to the wound to promote healing. They are described in "Vacuum therapy in the treatment of acute suppurative diseases of soft tissues and suppurative wound", Davydov et al., Vestn. Khir., September 1988 ("1988"). September 1998)), "Pathenogenic mechanism of the effect of vacuum therapy on the course of the wound process", Davydov et al., Khirurigiia, June 1990 ( "A June 1990 article"), and "Vacuum therapy in the treatment of suppurative lactation mastitis", Davydov et al., Vestn. Khir., November 1986 ("1986 November paper).
[0008]
A Russian paper distinguishes between drainage of trauma and the use of vacuum treatment for healing, and reports that vacuum therapy can clean and detox the trauma faster than conventional open drainage. In a November 1986 article, a vacuum therapy protocol was used in which a treatment was performed at 0.8-1.0 atm during surgery for 20 minutes, followed by a vacuum of 0.1-0.15 atm for 1.5 to 3 hours a day. Two times. These Russian articles teach that the use of negative pressure promotes healing. Russian papers further teach the use of this vacuum method to reduce the number of wounded microorganisms. A June 1990 article teaches that vacuum therapy provides a significant antimicrobial effect. A June 1990 article describes an increase in blood influx into the area around the trauma, thereby increasing the number of leukocytes reaching the area of inflammation. Furthermore, Russian articles teach the use of vacuum therapy to improve local blood circulation. The September 1988 paper teaches that increasing the flow of blood into the trauma area enhances the repair process. A June 1990 article teaches that vacuum treatment promotes the mobilization of plasma, interstitial fluid, and lymph to the trauma. A June 1990 article reports that in trauma treated with vacuum therapy, connective tissue cells and cellular and non-cellular elements appear twice as fast. Subsequent papers and patents extend the benefits of vacuum therapy. Thus, the prior art teaches the benefits and values of vacuum bandages.
DISCLOSURE OF THE INVENTION
[Problems to be solved by the invention]
[0009]
The devices disclosed herein limit the rate of change of the negative pressure applied to the trauma. The caregiver can change the negative pressure value, and the device controls the rate of change of the negative pressure to reduce patient discomfort.
[Means for Solving the Problems]
[0010]
Thus, the illustrated embodiment provides a negative pressure source, a variable flow orifice, a pressure transducer, a vacuum bandage, a controller, and a caregiver interface. The caregiver interface is configured to allow the caregiver to select a negative pressure setting. The caregiver enters a desired or set value of the negative pressure applied to the trauma through the caregiver interface. The controller monitors the pressure transducer and compares its value to a set value. Based on this comparison, the controller adjusts the variable flow orifice. When the caregiver enters a new set point, the controller limits the input to the variable flow orifice to generate an acceptable rate of change of the negative pressure monitored by the pressure transducer.
[0011]
Further, the illustrated embodiment includes a waste canister operably coupled to a negative pressure source. The canister is coupled to the bandage, and when a vacuum is applied to the canister, a vacuum is also applied to the bandage. In some embodiments, the waste canister is a disposable waste canister.
[0012]
The illustrated embodiment further provides a plurality of valves, a canister, and a vacuum pump. Each valve connects one of the vacuum pumps to the associated waste canister. The controller adjusts each valve to define a vacuum level for each associated canister. A plurality of vacuum regulators are also provided, each coupled to a respective valve. Each regulator is configured to define a maximum vacuum level. Each regulator has an air intake that further supplies air to one of the associated pumps. A plurality of converters are provided. Each transducer is coupled between each valve and an associated waste canister for measuring vacuum.
[0013]
Further features and benefits of the present invention will become apparent to those skilled in the art from a consideration of the following detailed description, which illustrates the best mode of the invention currently understood.

The integral term is the result of multiplying the integral gain (derived experimentally, currently set to 0.5) by the running sum of the error signal maintained over all iterations of the control loop. The integral term will not be updated if the
[0061]
The software has been set up, including the derivative term, but the gain has been experimentally selected to be zero and has no effect on the control loop. The teachings of the present disclosure implement any of a variety of methods that can be used to measure the appropriate gain constant for the implementation of the PID control algorithm, and the method will probably provide a value for the differential gain.
[0062]
Mathematically, the described control algorithm is expressed as follows:
When the pressure is 0mmHg, read cal lo as adc310 value at startup
When pressure is 225mmHg, cal hi is set to adc310 value at factory
cal range = cal hi-cal lo
Pressure measurement = ((raw input of adc-cal lo) * 225) / cal range
Delta t = elapsed timer tick after pressure change (due to user, profile mode, alarm, etc.)
Delta p = (7.5 mmHg * Delta t) / 100 ticks per second
Desired pressure = pressure setting +/- delta p (+/- depends on whether pressure is increasing or decreasing)
The desired pressure is suppressed so as not to exceed the pressure setting
Error signal = desired pressure-measured pressure
Proportional term = proportional gain * error signal

Integral sum = integral sum + error signal
Integral term = integral gain * integral sum
Proportional valve output value = startup offset + proportional term + integral term

First, the computer program verifies that the legitimate trauma has been identified as a pressure trauma. If a suitable injury has not been identified, the function sends an error message to the user interface stating that an invalid injury has been identified. If an appropriate lesion is identified, a pressure error signal is calculated from the desired pressure using the latest pressure measurement. The function then determines whether the desired pressure is reachable, i.e. if the highest possible pressure has already been reached and is still lower than the desired pressure, or the lowest possible pressure has been reached and the current pressure is lower than the desired pressure. That is still the case. If either of these conditions exists, the integral term of
