<P>PROBLEM TO BE SOLVED: To notify an attendant of a medical examination progress state of a medical examinee through a network. <P>SOLUTION: A medical examinee present state notification system has; a means for associating examinee information and attendant link information with the mail address of the attendant, with the need/needlessness of mail notification and with notification/non-notification of each present state information and registering them in a medical institution server 30; a means for registering the progress state of each medical examinee in the medical institution server 30 from each medical institution terminal 20 whenever the progress stage changes and retrieving the need/needlessness of mail notification of the medical examinee; a means for next retrieving the notification/non-notification of each present state information when the need is registered; a means for acquiring the mail address of the attendant and notifying of the attendant of the present state information of the medial examinee when the notification is registered; and a means by which the medical institution server 30 retrieves a corresponding medical examinee on the basis of attendant link information inputted from a user terminal 10, acquires the present state information of the medical examinee and provides the user terminal 10 with the present state information. <P>COPYRIGHT: (C)2004,JPO

【発明の属する技術分野】
本発明は、付添人に対する医療機関受診者の現状通知システム，現状通知方法および現状通知プログラムに関し、特に病院等の医療機関への送迎を必要とする受診者の家族や介護人等の付添人に対して、医療機関内において受診者が受付済，診察待ち，診察中，診察終了，薬待ち，会計終了等のどの状況に現在いるのかをネットワークを介して通知する、付添人に対する医療機関受診者の現状通知システム，現状通知方法および現状通知プログラムに関する。

【従来の技術】
従来、病院等の医療機関の予約後または受付終了後に、自分の診察開始までの診察待ち情報を確認することは可能であった。例えば、特開２００２−８３０４２号公報には自分の現状の診察順番である順番待ち情報を端末からネットワークを介して確認することが、また特開２００１−２２２６２７号公報には自分の診察開始までの待ち時間をネットワークを介して確認することが、それぞれ記載されている。これにより、受診者は医療機関の受付で診察順番を待ち続けなくてもよくなり、医療機関外から診察待ち情報を確認し診察が近くなった時点で医療機関に出向けばよくなるため、待ち時間の有効利用が図れるようになった。

しかし、これらの従来例は診察開始前の診察待ち情報の通知に関するものであり、診察開始後の通知については一切言及されていない。従って、例えば、家族や介護人等の付添人による医療機関への送迎が必要な受診者の場合、受診者を医療機関に送り届け受付を済ませた付添人は、受診者をいつ迎えにくればよいのかがわからないという問題がある。特に、総合病院や大学病院等の大規模な医療機関では診察時間が長くなりがちで診察終了時間の予測が難しい。このため、付添人が受診者の診察終了まで医療機関で待っていたり、診察終了後に受診者が付添人に電話で連絡して迎えにきてもらったり、付添人の予測によりやや早めに出向いて受診者の会計終了まで待つ、等の対応が一般的に行われている。

【発明が解決しようとする課題】
しかし、上述した従来の技術においては、以下に示すような問題がある。

まず、付添人が受診者の診察終了まで医療機関で待つ場合は、付添人が長い間拘束されることになり、他の用事等が殆どできなくなってしまう。

また、診察終了後に受診者が付添人に電話で連絡して迎えにきてもらう場合は、受診者に電話連絡を強いることになるとともに、突然の電話で付添人がすぐに対応できない場合があったり、連絡を受けて医療機関に迎えに行くまでの間受診者を待たせたりすることになる。

また、付添人の予測によりやや早めに出向いて受診者の会計終了まで待つ場合は、受診者の負担はなくなるが、付添人にとって待ち時間が長くなってしまうというリスクがある。

本発明は、以上の問題を解決する付添人に対する医療機関受診者の現状通知システム，現状通知方法および現状通知プログラムを提供することを目的とする。

【課題を解決するための手段】
本発明の第１の付添人に対する医療機関受診者の現状通知システムは、付添人が使用するユーザ端末と、医療機関の各部署に備えられた医療機関端末と、医療機関サーバと、これらを相互に接続するネットワークとを備えた付添人に対する医療機関受診者の現状通知システムであって、
前記医療機関サーバに受診者情報と付添人リンク情報とを対応付けて登録する手段と、各受診者の進捗状況が変わる度に現状情報として前記各医療機関端末から前記医療機関サーバに登録する手段と、前記ユーザ端末から前記医療機関サーバにアクセスし、付添人リンク情報を入力して受診者の現状確認要求を行う手段と、入力された付添人リンク情報により対応する受診者を前記医療機関サーバが検索する手段と、検索された受診者の現状情報を取得して前記ユーザ端末に提供する手段とを有する。

前記医療機関サーバに受診者情報と付添人のメールアドレス，現状情報毎の通知／非通知とを対応付けて登録する手段と、各受診者の進捗状況が変わる度に現状情報として前記各医療機関端末から前記医療機関サーバに登録する手段と、各受診者の現状が変わる度に該当する受診者の現状情報毎の通知／非通知を検索する手段と、該当現状情報に対して通知の登録がされていた場合に、付添人のメールアドレスを取得して受診者の現状情報をメールにより付添人に通知する手段とを有する。

前記医療機関サーバに受診者情報と付添人リンク情報，付添人のメールアドレス，メール通知の必要／不要，現状情報毎の通知／非通知とを対応付けて登録する手段と、各受診者の進捗状況が変わる度に現状情報として前記各医療機関端末から前記医療機関サーバに登録する手段と、各受診者の現状が変わる度に該当受診者のメール通知の必要／不要を検索する手段と、必要の登録がされていた場合に、次に現状情報毎の通知／非通知を検索する手段と、該当現状情報に対して通知の登録がされていた場合に、付添人のメールアドレスを取得して受診者の現状情報をメールにより付添人に通知する手段と、前記ユーザ端末から前記医療機関サーバにアクセスし、付添人リンク情報を入力して受診者の現状確認要求を行う手段と、入力された付添人リンク情報により対応する受診者を前記医療機関サーバが検索する手段と、検索された受診者の現状情報を取得して前記ユーザ端末に提供する手段とを有する。

受診者情報と付添人リンク情報，付添人のメールアドレス，メール通知の必要／不要，現状情報毎の通知／非通知とを対応付けて登録すると、各受診者の進捗状況が変わる度に前記各医療機関端末から送信される現状情報を登録する手段、各受診者の現状が変わる度に該当受診者のメール通知の必要／不要を検索する手段、必要の登録がされていた場合に、次に現状情報毎の通知／非通知を検索する手段、該当現状情報に対して通知の登録がされていた場合に、付添人のメールアドレスを取得して受診者の現状情報をメールにより付添人に通知する手段、前記ユーザ端末から付添人リンク情報を入力され受診者の現状確認要求を受診したときに、入力された付添人リンク情報により対応する受診者を検索する手段、検索された受診者の現状情報を取得して前記ユーザ端末に提供する手段、としてコンピュータを機能させる。

【発明の実施の形態】
本発明の実施の形態について図面を参照して詳細に説明する。

図１は、本発明の第１の実施の形態の構成を示す図であり、ユーザ端末１０と、医療機関端末２０と、医療機関サーバ３０と、ＬＡＮ等の医療機関内ネットワーク１１０と、ユーザ端末１０と医療機関内ネットワーク１１０とを相互に接続するインターネット等のネットワーク１００とを備えている。

ユーザ端末１０は、医療機関への受診者の送迎等を行う付添人が主に使用する端末であり、インターネットブラウザ機能を備えインターネット接続が可能な、パーソナルコンピュータ等の情報処理装置または携帯電話等の携帯端末である。このユーザ端末１０は、ネットワーク１００を介して医療機関サーバ３０がネットワーク１００上に提供しているホームページに接続する手段と、医療機関サーバ３０から送られる画面情報を受信し表示する手段と、画面上で必要事項を入力する入力手段とを備えている。

医療機関端末２０は、病院等の医療機関の各部署に設置されているパーソナルコンピュータ等の情報処理装置である。医療機関端末２０は、医療機関内ネットワーク１１０を介し医療機関サーバ３０に接続する手段と、医療機関サーバ３０から送られる画面情報を受信し表示する手段と、画面上で各部署の担当者から入力された受診者毎の診察進捗状況（現状情報）を医療機関サーバ３０に登録，更新要求する手段とを備えている。この診察進捗状況（現状情報）の登録は、予め決められている診察進捗状況カテゴリ（受付済、診察待ち、診察中、診察終了、薬待ち、会計終了等）に基づくものとする。

医療機関サーバ３０は、病院等の医療機関に設置されるワークステーション，サーバ等の情報処理装置であり、診察進捗状況管理部３１と、受診者・付添人情報データベース３２と、受診者現状データベース３３とを備えている。

診察進捗状況管理部３１は、医療機関内の各部署に設置された医療機関端末２０から入力された受診者毎の診察進捗状況を受診者現状データベース３３に登録，更新する機能と、ユーザ端末１０から受診者の診察進捗状況の確認要求があったときに、受診者・付添人情報データベース３２および受診者現状データベース３３を検索して要求のあった受診者の診察進捗状況を提供する機能とを備えている。

受診者・付添人情報データベース３２には受診者情報と付添人情報とが対応付けられて登録されており、受診者現状データベース３３には受診者毎の最新診察進捗状況（現状情報）が各医療機関端末２０から登録される。

なお、医療機関サーバ３０は、図示していない記憶手段に記憶された現状通知プログラムにより制御され、現状通知プログラムは医療機関サーバ３０の動作を制御する。

次に、本発明の第１の実施の形態の動作について図１〜図４を参照して詳細に説明する。なお、以降の説明では、ネットワーク１００はインターネットであるとする。また本発明では診察券番号は医療機関内で重複のない番号であるとする。

図２（本発明の第１の実施の形態の動作を示すフローチャート）を参照すると、初診受付時に予め受診者・付添人情報の登録を行っておく。この受診者・付添人情報の登録は、受診者の初診受付部署に設置された医療機関端末２０により行われる。初診受付部署担当者は医療機関端末２０から医療機関サーバ３０にアクセスし、「受診者・付添人情報の登録」をメニューから選択する。医療機関サーバ３０の診察進捗状況管理部３１は、図３（本発明の第１の実施の形態における受診者・付添人情報の登録／変更画面の一例を示す図）の（ａ）に示す受診者・付添人情報の登録画面を医療機関端末２０に送信し、初診受付部署担当者は受診者情報（診察券番号、氏名、住所、電話番号等）および付添人情報（例えば、氏名等）を入力し登録ボタンを押下することにより、これらの情報を医療機関サーバ３０に送信し登録を要求する（ステップＡ１）。診察進捗状況管理部３１は、受診者情報と付添人情報とを対応付けて受診者・付添人情報データベース３２に登録する。この登録は変更が無い限り初診時において１回行うだけでよい（ステップＡ２）。

次に、初診でも２回目以降の受診でもよいが、受診者が医療機関を訪れた後の動作について説明する。

受診者は医療機関の診察受付で受診の受付手続きを行う。医療機関受付担当者は受診者の診察受付が終わると医療機関端末２０を用いて診察進捗状況「受付済み」を入力する。このとき、図４（診察進捗状況入力・確認画面の一例を示す図）の（ａ）に示す診察進捗状況入力画面において、診察券番号を入力し検索ボタンをマウス等でクリックすると（診察券番号の入力は診察券磁気カードスキャンやバーコード読み込み等も可能とする）、診察進捗状況管理部３１は診察券番号により受診者・付添人情報データベース３２を検索し、図４（ｂ）に示すように氏名、住所等の受診者情報を表示する。医療機関受付担当者は、図４（ｂ）の画面上で診察進捗状況欄の入力リストから「受付済み」を選択し、登録ボタンをマウス等でクリックする。入力した内容は医療機関サーバ３０に送信され（ステップＡ３）、診察進捗状況管理部３１は診察券番号および受診者現状情報である診察進捗状況「受付済み」を受診者現状データベース３３に登録する（ステップＡ４）
この後、受診者の状況が進捗する度に、ステップＡ３の受付部署の医療機関端末２０と同様な動作で、それぞれの部署にある各医療機関端末２０から医療機関サーバ３０に該当受診者の診察進捗状況の更新要求を行う。例えば、受診者が診察科待合室に到着した時点では「診察待ち」を、医師の診察が開始された時点では「診察中」を、診察が終了した時点では「診察終了」を、薬局における薬の受け取り受付時点では「薬待ち」を、そして、会計窓口における会計終了時点では「会計終了」を、それぞれ入力し医療機関サーバ３０に診察券番号とともに送信する（ステップＡ５）。

医療機関サーバ３０の診察進捗状況管理部３１は、受診した受診者の進捗状況を受診者現状データベース３３に診察券番号に対応付けて登録する。この登録は、医療機関端末２０からの進捗状況の登録要求受診の都度、受信した進捗状況に更新される。通常は、「受付済」→「診察待ち」→「診察中」→「診察終了」→「薬待ち」→「会計終了」というように更新され、受診者の各時点での進捗状況を示すことになる（ステップＡ６）。

以上のように受診者が受診している間、付添人は医療機関から自宅等に戻ったり他の場所に外出したりすることができる。そして、いつごろ受診者を医療機関に迎えにいったらよいかを知るために、付添人はユーザ端末１０から医療機関サーバ３０にアクセスし受診者の診察状況の現状を確認することができる。

このためには、各受診者の付添人はユーザ端末１０を用いて医療機関がインターネット１００上に開設しているホームページにアクセスする。これに応答して、医療機関サーバ３０の診察進捗状況管理部３１はユーザ端末１０にメニュー画面情報を送信する。付添人がメニュー画面上で「診察進捗状況確認」を選択すると図４（Ｃ）に示すような画面が表示され、付添人の氏名を入力して検索ボタンを押下する。これにより付添人の氏名が医療機関サーバ３０に送信され。該当する受診者の診察進捗状況の確認要求を行う（ステップＡ７）。

医療機関サーバ３０の診察進捗状況管理部３１は、受診した付添人の氏名により受診者・付添人情報データベース３２を検索し、付添人に対応して登録されている受診者の診察券番号と受診者氏名とを取得する。そして、取得した診察券番号により受診者現状データベース３３を検索し、該当する診察進捗状況を取得する。診察進捗状況管理部３１は、図４（Ｃ）に示すように、この診察進捗状況を受診者氏名とともにユーザ端末１０に送信し（ステップＡ８）、ユーザ端末１０は受診者の診察進捗状況を受診，表示する（ステップＡ９）。

これにより付添人は、受診者が現在診察中なのか薬待ちなのかといった受診者の現状がわかり、あとどの位して迎えに行けばよいのかという目安が立てられる。

なお、以上の説明においては、付添人情報として予め登録された付添人の氏名により受診者との対応を検索し、受診者の診察進捗状況確認を行うとして説明したが、付添人情報は受診者と付添人とを対応付けるリンク情報であるため、付添人の氏名の代わりにニックネームや他の単語等であってもよい。また、付添人情報を登録せず、図４（Ｃ）の診察進捗状況確認画面で付添人氏名の代わりに受診者の診察券番号を入力し、この診察券番号により受診者の診察進捗状況確認を行うようにすることも可能である。

次に、本発明の第２の実施の形態について図面を参照して詳細に説明する。

本実施の形態は、受診者の診察進捗状況を医療機関サーバ３０からユーザ端末１０に電子メールにより自動的に通知するようにした点が、第１の実施の形態と異なる点である。従って、ユーザは受診者の診察進捗状況を知るために随時画面を入力して参照し確認する手間が省ける。図５は本発明の第２の実施の形態の構成を示す図であり、図１の第１の実施の形態の構成と比べると、医療機関サーバ３０にユーザ宛てにメールを送信するためのメール送信部３４が追加されている。また、ユーザ端末１０は医療機関サーバ３０からメールを受信するために、メール受信機能を備えている。

次に、本実施の形態の動作について図５〜図７を参照して説明する。

図６（本発明の第２の実施の形態の動作を示すフローチャート）を参照すると、ステップＢ１，Ｂ２において、初診受付時の受診者・付添人情報の登録時に、付添人情報として付添人氏名（第１の実施の形態）に加えて、付添人のメールアドレス，メール通知の必要／不要およびメール通知を行う受診者状況をさらに追加登録する点が異なっている。図７（本発明の第２の実施の形態における受診者・付添人情報の登録／変更画面の一例を示す図）の（ａ）にこれらの追加部分も示されており、この図においては、受付済，診察中，薬待ち，会計終了の四つの受診者診察進捗状況になった時に付添人にメール通知を行う場合の登録画面が例示されている。

医療機関内の各部署の医療機関端末２０から受診者の診察進捗状況を入力し、医療機関サーバ３０へ登録するステップＢ３，Ｂ４，Ｂ９，Ｂ１０については、第１の実施の形態の図２のステップＡ３，Ａ４，Ａ５，Ａ６にそれぞれ同じであるため説明を省略する。

ステップＢ４に続き、医療機関サーバ３０の診察進捗状況管理部３１は、受診者・付添人情報データベース３２を受診した診察券番号により検索し、該当付添人情報のメール通知の必要／不要を取得する。メール通知不要の場合は処理を終了し、メール通知必要の場合は、次にメール通知を行う受診者進捗状況かを判断する。図７（ａ）に示す付添人の場合は、「受付済」はメール通知を行う受診者進捗状況として登録されているため、ステップＢ７に進む（ステップＢ５，Ｂ６）。
