負の選択マーカーをコード化するポリヌクレオチドを含む
T細胞による対宿主性移植片病の予防
この発明は、同種異系骨髄移植を受けた患者における、対宿主性移植片病また
はGVHDの予防に関するものである。さらに詳しくは、この発明は、同種異系
T細胞除去骨髄移植に続いて起こる、再発性または慢性白血病に罹患している患
者における対宿主性移植片病の予防であって、負の選択マーカーをコード化する
ポリヌクレオチドを含むT細胞を上記患者に投与した後、対宿主性移植片病が発
現する前に、相互作用剤またはプロドラッグを投与してT細胞、特に対宿主移植
片反応性であるT細胞を殺すことによる予防法に関するものである。
さらに、この発明は、疾病または疾患の処置であって、骨髄切除の後、同種異
系骨髄移植片、例えばT細胞除去骨髄移植片を投与することを含む処置と連係的
に対宿主性移植片病を予防する方法に関するものである。かかる方法では、負の
選択マーカーをコード化するポリヌクレオチドを含むT細胞を同じく患者に投与
し、対宿主性移植片病が発現する前に、相互作用剤またはプロドラッグを患者に
投与することによって、T細胞、特に対宿主移植片反応性であるT細胞を殺す。
発明の背景
血液悪性疾患または白血病、例えば多発性骨髄腫(MM)、慢性骨髄性白血病
(CML)、急性骨髄性白血病(AML)、および急性リンパ芽球性白血病(A
LL)は、毎年何千人ものアメリカ人を襲う。例えば、毎年アメリカ人1000
00人中約4人が多発性骨髄腫にかかる。1993年には新たな症例が1280
0件あった。多発性骨髄腫は、全タイプの悪性疾患のうち1%を僅かに越え、全
血液悪性疾患のうち10%を僅かに越える割合を占めている。(バーロジーら、
JAMA、第268巻、2946−2951頁(1992)、バーロジーら、「
ブラッド」、第73巻、865−879頁(1989))。診断での中央値年齢
は62歳であり、女性よりも男性に多く見られる。(ボーチンら、「トランスプ
ランテーション」、第42巻、29頁(1986)。)患者らは、局所性または
播種性疾患を呈し、血清M−蛋白値は通常3.0g/dlを越え、単純な血清免
疫グロブリンの値は低い。多くの場合尿および骨格病変部にモノクローナル軽
鎖が存在する。形質細胞標識指数、β2ミクログロブリンおよびC−反応性蛋白
(CRP)は、独立した予後因子であることが示された。
同種異系骨髄移植は、選ばれた多発性骨髄腫患者に関する有効な処置様式とし
て登場した(ボーチンら、1986)。268名の患者が異なる試行で同種異系
移植を受けた。(バーロジーら、「セミナーズ・イン・へマトロジー」、第32
巻、31−44頁(1995))。患者の50パーセントは1年以内に死亡し、
約40パーセントは完全応答を達成し、4年プロジェクト化事象−独立生存率は
約35パーセントである。
一つには、ドナー移植片における免疫担当細胞の養子免疫伝達から誘導された
抗腫瘍(すなわち、対骨髄腫性移植片またはGVM)効果があるため、同種異系
骨髄移植は治療に有効であると考えられている。(ゲールら、「ザ・ランセット
」、第2巻、28頁(1984)、ボーチンら、「ネイチャー」、第67巻、7
22頁(1979)。)対宿主性移植片病(GVHD)に伴う抗腫瘍効果は、T
細胞および非T細胞エフェクター細胞集団の両方により仲介されると思われる。
しかしながら、GVHDは、その罹病率および死亡率故に依然として大きな問題
である。
同種異系骨髄移植による早期死亡率を減少させるため、T細胞除去が採られて
いる。T細胞は、密度勾配遠心分離、大豆レクチン凝集およびE−ロゼット形成
、遠心水簸、細胞傷害性薬剤またはコルチコステロイド類、抗T細胞モノクロー
ナル抗体、およびCD34+細胞の正の選択を含む数種の技術のいずれかにより
同種移植片から除去され得る。(チャンプリン、「ジャーナル・オブ・ヘマドセ

ト」、第2巻、327−331頁(1981)、ワルドマンら、「ザ・ランセッ

2)。)骨髄移植片からT細胞を除去するのに現在使用されている技術のうち、
最も有効なのは、ライスナーらの技術(1981)である。この技術は、一貫し
て2.5−3.0 log10のクローン化可能T細胞を除去するもので、まず大豆
レクチンとの分別凝集を用いることにより、T細胞、B細胞、単核球および顆粒
球を含む成熟白血球を除去し、次いでE−ロゼット除去により残留T細胞を除去
する。この技術は、白血病患者に与えられた200を越えるHLA−適合関連移
植片の集団を処置するのに使用されてきた。この集団では、段階II急性GVHD
の発生率は5%であり、段階IIIおよびIV急性GVDHは観察されなかった。(

T細胞除去によりGVDHの発生は減少するが、T細胞除去はまた早期再発、
不完全免疫再構成、移植失敗、およびエプスタイン-バールウイルス関連リンパ
腫の危険度を増加させる。
再発の危険は、慢性骨髄性白血病において最も正確に実証されている。(マー
モントら、「ブラッド」、第78巻、2120−2130(1991)、ゴール
ドマンら、「アナルス・オブ・インターナル・メディシン」、第108巻、80
6−814(1988)。)対白血病性移植片(GVL)効果の直接的証拠は、
追加的化学療法または放射能療法を一切行わず骨髄ドナーからの末梢血単核細胞
注入によるT細胞除去骨髄移植後の再発性慢性骨髄性白血病患者において立証さ
れており、白血病細胞の少なくとも6log10致死がもたらされる。(ドロビス

血単核細胞は上記患者において完全かつ長期軽減を誘導し得るが、上記療法はG

T細胞除去後の同種異系骨髄移植はまた、骨髄移植失敗の発生増加を伴ってい
る。(ビーティら、「ニューイングランド・ジャーナル・オブ・メディシン」、
第315巻、765−771頁(1985)、ヘイルら、「トランスプランテー
ション」、第45巻、753−759頁(1988)、パッターソンら、「ブリ
ティッシュ・ジャーナル・オブ・ヘマトロジー」、第63巻、221−230頁
(1986)。)同種異系骨髄移植に関するネズミモデルでは、コンディショニ
ング治療プログラムにおいて全身照射(TBI)と共に強力な免疫抑制処置を追
加して宿主のT細胞を除去した後のみ永続的移植状態が観察された。(ラピドッ
トら、「ブラッド」、第73巻、2025頁(1989))。ヒト同種移植の場
合、同問題は、コンディショニング治療プログラムにチオテパおよび抗胸腺細胞
グロブリンを加えることにより克服される。
T細胞除去骨髄移植後の免疫不全は、強力なコンディショニング治療プログラ
ムおよび移植片に成熟ドナーT細胞が存在しないことの結果として非常に明白で
ある。それは、ドナーおよび患者間における少量の組織適合性抗原の大きな差異
によりおそらく延長されると思われる。
T細胞除去骨髄移植片における深刻な免疫不全の結果の1つは、エプスタイン
-バールウイルス(EBV)リンパ球増殖性疾患の危険性増加である。(シャピ
ロら、「ブラッド」、第71巻、1234−1243頁(1988)、ズッター
ら、「ブラッド」、第72巻、520−522頁(1988)。)これらのリン
パ球増殖性疾患は、標準的治療形態に対して非応答性であることが多い。メモリ
アル・スローン-ケッタリング骨髄移植グループは、ドナーリンパ球注入で処置
されたリンパ球増殖性疾患を発現している患者7名について報告している。(パ

)患者は全てT細胞除去移植片のレシピエントであった(4名関連、3名非関連
)。移植後6カ月以内にリンパ球増殖性疾患が現れた。生検標本は、拡散した大
きな細胞リンパ腫を示した。4つの評価可能標本は、ドナー細胞起源のものであ
ることが見いだされた。EBV DNAは、PCRにより5つの評価可能標本の
うち5つから検出された。患者には、患者体重1kg当たり0.2−1.0×106
CD3+細胞を提供する用量でドナー白血球が与えられた。完全な病理学的およ
び/または臨床応答は、5名の患者全員で観察された。これらの応答は、
注入の8−21日後までに病理学的に実証された。臨床的軽減は14−30日以
内に達成され、存命中の3患者において他の療法を行わないでも持続された。2
名の患者は、それぞれ敗血症および間質性肺炎により注入の8日および16日後
に死亡した。剖検の結果、残留リンパ腫の証拠は全く示されなかった。生存して
いる3患者にGVHDが現れた。

クターによるT細胞の形質導入について開示している。形質導入されたT細胞を
ガンシクロビルで処置すると、80%より大きいこれらの細胞の生長阻害がもた
らされた。ガンシクロビルは、対照(非形質導入)T細胞に対しては効果を示さ
なかった。さらにタイバーフィエンらは、T細胞が単純ヘルペスウイルスチミジ
ンキナーゼ(TK)遺伝子によりエクスビボで形質導入され、次いで造血幹細胞
により患者に投与され得ることを主張している。患者がGVHDを発症した場合
、形質導入T細胞を除去するためにガンシクロビルが患者に投与され得る。
発明の要約
T細胞除去同種異系骨髄移植後に再発性または慢性白血病患者を処置すること
がこの発明の対象である。上記患者には、負の選択マーカーをコード化するポリ
ヌクレオチドを含むように遺伝子操作されたT細胞が与えられる。細胞が治療効
果を提供するのに十分な長さの時間患者に残存した後、相互作用剤またはプロド
ラッグが患者に投与されることにより、遺伝子操作されたT細胞が殺され、GV
HDの発現が予防される。別法として、予め定められた相互作用剤投与時間前に
GVHDが発現した場合、相互作用剤がGVHD発現時点で患者に投与され得る
ことにより、形質導入T細胞を殺すことを通してGVHDが処置される。
疾病または疾患の処置を受けている患者において対宿主性移植片病を予防する
ことがこの発明の別の対象であり、この場合の処置は骨髄切除の後、T細胞除去
骨髄移植片の投与を含む。上記疾病または疾患には、固形腫瘍悪性疾患および後
天性または遺伝性免疫学的または造血性疾患があるが、これらに限定されるわけ
ではない。処置の間、負の選択マーカーをコード化するポリヌクレオチドにより
遺伝子操作されたT細胞を、患者に投与する。細胞が治療効果を与えるのに十分
な長さの時間患者中に残存した後、相互作用剤またはプロドラッグを患者に投与
することにより、遺伝子操作されたT細胞が殺され、対宿主性移植片病が予防さ
れる。
発明の詳細な記載
この発明の一態様によると、宿主にT細胞を投与することにより処置され得る
疾患または疾病、例えば再発性または慢性白血病に関する処置を受けている宿主
における対宿主性移植片病の予防方法が提供される。この方法は、負の選択マー
カーまたは「自殺」遺伝子をコード化するポリヌクレオチドを含むように遺伝子
操作が加えられたT細胞を宿主に投与することを含む。これらの細胞は、宿主に
おいて治療効果を提供するのに有効な量で投与され、かつ有効な長さの時間宿主
中に残存する。T細胞が宿主において治療効果を提供するのに十分な長さの時間
宿主中に残存した後、かつ対宿主性移植片病が発生する前に、相互作用剤または
プロドラッグが宿主に投与される。相互作用剤は、遺伝子操作されたT細胞、特
に対宿主性移植片反応性である、すなわち対宿主性移植片効果を提供し得るT細
胞を殺すことにより、宿主における対宿主性移植片病の発生を予防するのに有効
な量で宿主に投与される。
ここで使用されている「ポリヌクレオチド」の語は、いかなる長さでもよいヌ
クレオチドのポリマー形態を包含し、リボヌクレオチドおよびデオキシリボヌク
レオチドを含む。上記語はまた、1本および2本鎖DNA並びに1本および2本
鎖RNAを包含する。この語はまた、修飾ポリヌクレオチド、例えばメチル化ま
たはキャップポリヌクレオチドも包含する。
負の選択マーカーをコード化するポリヌクレオチドは、T細胞へ形質導入され
る適当な発現ビークル内に含まれ得る。上記発現ビークルには、真核生物ベクタ
ー、原核生物ベクター(例えば細菌性ベクター)およびウイルスベクターがある
が、これらに限定されるわけではない。
別の一態様では、作用物質をコード化するポリヌクレオチド、または作用物質
をコード化するポリヌクレオチドを含む発現ビークルは、リポソーム内に封入さ
れる。
好ましい一態様では、発現ビークルはウイルス性ベクターである。使用され得
るウイルス性ベクターには、DNAウイルスベクター(例えばアデノウイルスベ
クター、アデノ関連ウイルスベクター、ヘルペスウイルスベクターおよびワクシ
ニアウイルスベクター)およびRNAウイルスベクター(例えばレトロウイルス
ベクター)がある。ベクター構築においてRNAウイルスベクターが使用される
とき、負の選択マーカーをコード化するポリヌクレオチドはRN形態を呈する。
ベクター構築においてDNAウイルスベクターが使用されるとき、負の選択マー
カーをコード化するポリヌクレオチドはDNA形態を呈する。
一態様では、ウイルスベクターはレトロウイルスベクターである。使用され得
るレトロウイルスベクターの例には、モロニーネズミ白血病ウイルス、脾臓壊死
ウイルス、およびレトロウイルス、例えばラウス肉腫ウイルス、ハーヴィー肉腫
ウイルス、トリ白血病ウイルス、ヒト免疫不全ウイルス、骨髄増殖性肉腫ウイル
スおよび乳癌ウイルスから誘導されたベクターがあるが、これらに限定されるわ
けではない。ベクターは、好ましくは感染性の複製不全レトロウイルス粒子であ
る。
レトロウイルスベクターは、レトロウイルス性遺伝子を真核生物細胞へ導入す
るのを仲介する作用物質として有用である。レトロウイルスベクターは、一般に
ウイルスの構造遺伝子をコードする配列の大多数を除去し、興味の対象である遺
伝子(複数も可)により置き換える形で構築される。最も多くの場合、構造遺伝
子(すなわち、gag、polおよびenv)は、当業界公知の遺伝子工学技術
を用いてレトロウイルスバックボーンから除去される。
gag、polおよびenv遺伝子を除去することにより得られるベクターバ
ックボーンは、5’LTR、パッケージングシグナル、興味の対象である異種遺
伝子(複数も可)が導入され得る1個またはそれ以上のクローニング部位および
3’LTRから成る。好ましいベクターバックボーンは、G1ベクターバックボ
ーンであり、マクラクリンら、「ヴァイロロジー」、195:1−5(1993
)および1991年7月25日公開の「新規レトロウイルスベクター」に関する
PCT特許出願第WO91/10728号に開示されている。
異種遺伝子(複数も可)が標準技術によってプロウイルスバックボーン中に組
み込まれることにより、レトロウイルスベクターが形成される。レトロウイルス
ベクターの製造技術は、PCT出願WO91/10728並びに次の文献:アー
メンタノら、「ジャーナル・オブ・ヴァイロロジー」、61:1647−165
0(1987)、ベンダーら、「ジャーナル・オブ・ヴァイロロジー」、61:
1639−1646(1987)およびミラーら、「バイオテクニクス」、7:
980−990(1989)に開示されている。最も簡単な構築物は、レトロウ
イルスの構造遺伝子が単一遺伝子により置き換えられ、次いでこれが長末端反復
(LTR)内のウイルス性調節配列の制御下で転写されているものである。複数
の遺伝子を標的細胞中へ導入し得るレトロウイルスベクターもまた構築されてい
る。通常、上記ベクターでは、1遺伝子がウイルスLTRの調節制御下にあり、
第2遺伝子がスプライシングされたメッセージから発現されるかまたはそれ自体
の内部プロモーターの調節下に置かれる。適当なブロモーターには、SV40プ

レトロウイルスベクターは、プラスミド、ウイルスDNAのセグメント、また
はプロウイルスDNAのセグメントの形態であり得る。この発明の場合、好まし
いレトロウイルスベクターはG1TK1SvNaであり、1995年3月9日に
公開された、「ヒト腫瘍細胞の遺伝子形質転換によるヒト腫瘍の処置」と題する
PCT特許出願番号WO95/06486に開示されている。
レトロウイルスベクターは、パッケージング細胞へ導入されることにより、プ
ロデューサー細胞を形成させる。パッケージング細胞がトランスでgag、po
lおよびenv遺伝子を提供することにより、感染性ではあるが複製能欠損の組
換え体レトロウイルスヘレトロウイルスベクターがパッケージングされ得る。ベ
クターは、トランスフェクション、形質導入、リン酸カルシウム沈澱、電気穿孔
、およびリポソーム仲介DNA転移を含む標準遺伝子導入技術によりパッケージ
ング細胞へ導入される。使用され得るパッケージング細胞の例には、PE501
、PA317、Psi−2、Psi−AM、PA12、T19−14X、VT−
19−17−H2、Psi−CRE、Psi−CRIP、GP+E−86、GP
+envAM12、PG13およびDANセルラインがあるが、これらに限定さ
れるわけではない。組換え体レトロウイルスを製造するためのこの発明における
好ましいプロデューサーセルラインは、PA317/G1TK1SvNaと称さ
れるプロデューサーセルラインであり、PCT出願WO95/06486に開示
されている。
負の選択マーカーには、ウイルス性チミジンキナーゼ、例えば単純ヘルペスウ
イルスチミジンキナーゼ、サイトメガロウイルスチミジンキナーゼおよび水痘ウ
イルスチミジンキナーゼ、キサンチン−グアニンホスホリボシルトランスフェラ
ーゼ、およびシトシンデアミナーゼがあるが、これらに限定されるわけではない
。
負の選択マーカーをコード化するポリヌクレオチドを含むレトロウイルスベク
ターを、T細胞へ形質導入する。一般に、約106〜約109、好ましくは約107
〜約108のT細胞がレトロウイルスベクターにより形質導入され、これは約1
05cfu/ml〜約109cfu/ml、好ましくは約2×106cfu/ml〜約1×108cfu/
mlの力価を有する、約2ml〜約500mlのレトロウイルス上清中に含まれ得る。
一旦T細胞が負の選択マーカーをコード化するポリヌクレオチドを含むレトロ
ウイルスベクターへ形質導入されると、T細胞は再発性または慢性白血病を患う
宿主へ投与される。宿主は動物宿主、および特に哺乳類宿主、例えばヒトおよび
ヒト以外の霊長類宿主である。形質導入されたT細胞は、当業界熟練者に知られ
ている手段、例えば脈管内投与、例えば静脈内または動脈内投与、または腹腔内
投与により投与される。一態様において、形質導入されたT細胞はボーラス注入
として投与される。形質導入されたT細胞は、治療効果の提供に有効な量、すな
わち宿主における再発性または慢性白血病の処置に有効な量で投与される。一般
に、形質導入されたT細胞は、約105細胞/kg〜約109細胞/kg、好ましくは
約2×105細胞/kg〜約1×107細胞/kgの量で投与される。
形質導入されたT細胞は、許容し得る医薬用担体、例えば食塩水溶液または水
性緩衝液、例えばリン酸緩衝液、トリス緩衝液、プラズマライトA(バクスター
)または乳酸加リンガー溶液と連係的に投与される。適当な医薬用担体の選択は
、この明細書に含まれる教義から当業界の熟練者には明白なものであると思われ
る。別法として、形質導入されたT細胞は、細胞が宿主に投与されるまで許容し
得る低温保存培地(例えば、リン酸緩衝食塩水、5%DMSOおよびヒトアルブ
ミンを含む培地)中で冷凍され得る。投与前、細胞および培地は解凍され、細胞
および培地は解凍後宿主に投与される。
形質導入されたT細胞が宿主において治療効果をもたらし得るのに十分な長さ
の時間をおいた後、ただし対宿主性移植片病の発現前に、相互作用または化学療
法剤は、形質導入されたT細胞を殺す、特に形質導入された増殖性対宿主性移植
片反応性T細胞、すなわち対宿主性移植片効果を提供し得るT細胞を殺すことに
より、対宿主性移植片病の発現を予防するのに有効な量で宿主に投与される。
好ましくは、対宿主性移植片反応性であるT細胞(従って、上記T細胞は、宿
主細胞のMHCクラスI抗原の認識を通して対宿主性移植片効果を提供し得る)
が増殖相にあり、そしてT細胞の一部、例えば対白血病移植片効果をもたらすか
または抗ウイルス効果をもたらすT細胞の一部が鎮静期または非増殖相にあると
きに、相互作用剤が投与される。相互作用剤またはプロドラッグが上記時点で投
与されると、対宿主性移植片効果をもたらし得る増殖性T細胞の致死を招き、対
白血病性移植片効果または抗ウイルス効果を与えるT細胞の一部を含む非増殖性
T細胞は相互作用剤またはプロドラッグの投与にもかかわらず残存する。すなわ
ち、GVHD誘発に関与するT細胞の大多数は優先的に除去され、対白血病性移
植片効果または抗ウイルス効果をもたらし得る他のT細胞が生き残る。一般に、
相互作用剤は、形質導入されたT細胞の投与後約10日〜約50日、好ましくは
約14日〜約28日の期間をおいて、さらに好ましくは21日目に投与される。
負の選択マーカーがウイルス性チミジンキナーゼ、例えば上述のものである場
合、相互作用剤または化学療法剤またはプロドラッグは、好ましくはヌクレオシ
ド類似体、例えばガンシクロビル、アシクロビル、および1−2−デオキシ−2
−フルオロ−β−D−アラビノフラノシル−5−ヨードウラシル(FIAU)か
ら成る群から選ばれたものである。上記相互作用剤は基質としてのウイルス性チ
ミジンキナーゼにより有効に利用され、すなわち上記相互作用剤は、ウイルス性
チミジンキナーゼを発現し、および特に対宿主性移植片反応性であるT細胞を増
殖させる形質導入T細胞のDNAに致死的に組み込まれることにより、形質導入
T細胞の死を誘発する。
負の選択マーカーがシトシンデアミナーゼである場合、好ましい相互作用剤ま
たはプロドラッグは5−フルオロシトシンである。シトシンデアミナーゼは、5

実施例

実施例1
慢性または再発性多発性骨髄腫を患う患者に対する単純ヘルペスウイルスチミ
ジンキナーゼ遺伝子を含むレトロウイルスベクターにより形質導入されたT細胞
の投与、その後のガンシクロビルの投与
ドナーリンパ球の採取
50〜100mlのドナー血液を集め、少なくとも2.5−5×107細胞を回収

G1TK1SvNaによるリンパ球の形質導入
約1×108リンパ球に対し、単純ヘルペスウイルスチミジンキナーゼ(TK
)遺伝子およびネオマイシン耐性遺伝子を含むレトロウイルスベクターGITK
1SvNaにより形質導入する。上記形質導入は、2×108cfu〜1×109cfu
のレトロウイルスベクターを含むウイルス上清100mlをリンパ球に加える
ことにより達成される。レトロウイルスベクターG1TK1SvNaは、199
5年3月9日公開のPCT出願第WO95/06486号にさらに記載されてい
る。新鮮なウイルス上清は、追加量のインターロイキン2およびプロタミンスル
フェートと一緒に合計3日間毎日加えられ、1500単位/mlの最終インターロ

T細胞除去した同種異系骨髄移植を受けた患者に対して、彼らが移植後90日

2日目に測定可能な病状の連続低減化を伴う完全応答または部分応答した患者に
は、病気のプラトーまたは進行が観察されるまで第2のリンパ球注入はしない。
完全応答は、最小限2週間、次の要件:(i)尿および血清中に免疫固定によ
るM−成分が存在しない、(ii)骨髄が免疫染色の結果十分細胞性(すなわち>
20%)であり、形質細胞は3%未満である、(iii)血清カルシウムレベルの
上昇が無い、および(iv)新たな骨病変も既存病変の拡大も無いということを満
