DESCRIZIONE dell'invenzione industriale dal titolo: "Impiego di oATP come farmaco immunodepressivo" DESCRIPTION of the industrial invention entitled: "Use of oATP as an immunodepressive drug"
DESCRIZIONE DESCRIPTION
The present invention refers to a new use of adenosine-5'-triphosphate-2 ', 3'-dialdehyde (hereinafter OATP) as an immunosuppressive drug, as well as to the use of this compound for the preparation of medicaments useful in the treatment therapeutic of conditions that benefit from the administration of active ingredients with immunosuppressive activity, and in particular in the therapeutic treatment of autoimmune diseases and in the anti-rejection treatment of organ transplants.
7:1131-1133, 1979. The molecule oATP is derived from ATP by oxidation of the hydroxyls present in the 2 'and 3' positions of ribose to dialdehydes. Such oxidation can be carried out with a salt of the periodic acid, as described in P. N. Lowe et al., Biochemical Society Transactions, vol. 7: 1131-1133, 1979.
WO02/11737 descrive l'impiego di oATP come principio attivo avente attività antinfiammatoria e analgesica. WO02 / 11737 describes the use of oATP as an active ingredient having anti-inflammatory and analgesic activity.
WO2005 / 077383 describes the use of oATP as a pharmacological agent, useful in the treatment of diseases that require the inhibition of angiogenesis, such as for example atherosclerosis and tumors.
The experimentation on which the present invention is based has shown that oATP has a high immunodepressive activity which allows its use as an anti-rejection drug in organ transplants, as well as as a drug for the therapeutic treatment of autoimmune diseases. In particular, the studies on which the invention is based have shown that oATP is effective in significantly delaying the rejection of transplanted pancreatic cells in diabetic animals.
Furthermore, following the preclinical demonstration of the efficacy of oATP as a drug useful in preventing transplant rejection, the usefulness of the molecule in general in the treatment of autoimmune diseases, such as, for example, systemic lupus erythematosus (SLE), sclerosis, has been verified. multiple disease, scleroderma, Sjogren's and Erdheim-Chester's syndrome, psoriasis, Crohn's disease, Takayasu's arteritis, rheumatoid arthritis, type I and II diabetes, all diseases of autoimmune origin. Therefore, the object of the present invention is the use of oATP in the preparation of medicaments useful in the treatments mentioned above and defined in detail in the following claims.

Generally, the daily dose per patient may be between 1 mg and 1000 mg, preferably between 5 mg and 500 mg.
The new uses of oATP and pharmaceutical compositions containing it are supported by the following evidence:
Esempio 1 Example 1
Prolungamento della sopravvivenza di insule pancreatiche trapiantate in topi diabetici, dopo somministrazione di oATP Prolonging the survival of pancreatic islets transplanted into diabetic mice after administration of oATP
Mice of the C57BL / 6 strain rendered diabetic by administration of streptozotocin (170 mg / kg) were treated with oATP. The diabetic mice were injected with pancreatic islets (containing insulin-producing beta cells) obtained from mice of the BALB / c strain. Isolation of the islets was performed with the method described by Abdi R et al. in Diabetes, August 2002; 51 (8): 2489-95. Briefly, donor pancreases were perfused through the common bile duct with rodent collagenase (Sigma-Aldrich, St. Louis, MO) and digested at 37 ° C for 30 minutes.
The islets were purified on a discontinuous Ficoll gradient (Sigma) and collected with the aid of a stereomicroscope. At least 500 islets were transplanted into recipient mice, placing them below the left kidney capsule.
Blood glucose monitoring was then performed daily, by evaluating the glucose levels of the blood taken from a tail vein of the animals, in order to check the functionality of the islet transplant. Rejection was reported when return to hyperglycemia was shown, i.e. when blood glucose levels were> 250 mg / dl in two subsequent measurements. Furthermore, at the end of the experimentation, the transplanted islets were removed for histopathological examination by removal of the renal capsule, under microscopic guidance.
After the inoculation of islets (time 0) the diabetic mice were treated with intraperitoneal injection of 30 mg / kg (1 mg / mouse) of oATP for 6 days after transplantation: time 0, 2 °, 4 °, 6th, 8th and 10th day. As shown in fig. 1, rejection of transplanted islets was significantly delayed in diabetic mice treated with oATP compared to untreated controls.
2). Similar results were subsequently obtained by administering oATP at a dose of 0.25 mg / mouse (n = 5) (Fig. 2).
Esempio 2 Example 2
Prolungamento della sopravvivenza di insule pancreatiche trapiantate, indotto dalla somministrazione contemporanea di oATP e di rapamicina (rapa) Prolonged survival of transplanted pancreatic islets, induced by the simultaneous administration of oATP and rapamycin (turnip)
Using the same experimental model, the effect of coadministration of oATP (1 mg / mouse) and the more active immunosuppressive drug, rapamycin, used at a dose of 0.1 mg / mouse, was investigated. As shown in fig. 3, treatment of diabetic mice with o-ATP + rapamycin induced a highly significant prolongation of pancreatic islet survival when compared to the two single treatments with oATP alone or rapamycin alone. Even better results were obtained with oATP 0.25 mg / mouse and rapamycin at 0.1 mg / mouse.
Questi risultati permettono di preconizzare l'impiego di oATP come sinergizzante dell'azione di farmaci immunosoppressori. Rientra perciò nell'ambito dell'invenzione l'impiego di oATP in terapia combinata con farmaci immunosoppressori, quali rapamicina, mediante somministrazione simultanea, separata o sequenziale; rientrano altresì nell'ambito dell'invenzione formulazioni farmaceutiche contenenti come principi attivi oATP e farmaci immuno soppressori. These results allow us to predict the use of oATP as a synergist of the action of immunosuppressive drugs. Therefore, the use of oATP in combined therapy with immunosuppressive drugs, such as rapamycin, by means of simultaneous, separate or sequential administration, falls within the scope of the invention; pharmaceutical formulations containing as active ingredients oATP and immuno-suppressive drugs also fall within the scope of the invention.
Esempio 3 Example 3
Use of oATP in rheumatoid arthritis Rheumatoid arthritis is an autoimmune disease characterized by major inflammatory and painful manifestations in the joints.
The pre-clinical demonstration of the efficacy of oATP as a drug useful in preventing transplant rejection has led to investigate the possibility of using oATP in rheumatoid arthritis, which in the clinic is treated with anti-rejection drugs, such as cyclosporine.
The experiments were performed using an endothelial cell culture. Endothelial cells (HUVEC = human umbilical vein endothelial cells) were derived from human umbilical cord by collagenase treatment, as described in Ferrero et al., Cancer Res., 15 Jul 1996, 56 (14): 3211-5), and grown in flasks coated with 1% gelatin using 199 medium containing 20% heat inactivated fetal bovine serum, 1% bovine retinal-derived growth factor, 90 μg / ml heparin, 100 IU / ml of penicillin and 100 μg / ml of streptomycin. All experiments were conducted on HUVEC obtained after 1-4 passages. TNF alpha is used at a dose of 200 U / ml. The HUVECs were incubated for 24 hours with oATP.
La sperimentazione eseguita ha permesso di mettere in evidenza i seguenti risultati: A flow cytometric analysis was also performed to evaluate the expression by HUVEC of the adhesion molecule ICAM-1 and of the TNF alpha receptors (TNFR1 and TNFR2). The samples were analyzed at FACScan with the support of a CELLQuest software. The experimentation carried out allowed to highlight the following results:
1) oATP inibisce in vitro la migrazione TNF alfadipendente di PBMC (peripheral blood mononuclear cells) attraverso le HUVEC, come riportato in fig. 1) oATP inhibits in vitro the TNF-dependent migration of PBMC (peripheral blood mononuclear cells) through HUVEC, as reported in Fig.
4; 4;
5). 2) oATP inhibits the ATP-induced up-regulation of the expression of the ICAM1 adhesion molecule on HUVEC, measured by FACS analysis: Δ mean of fluorescence intensity (mean of 4 experiments) = 29 ± 0.3 in HUVEC untreated, 65 ± 0.5 in those treated with ATP, 26 ± 0.1 in those treated with oATP and 35 ± 0.2 in those treated with ATP + oATP. This data indicates that the endothelium, after treatment with oATP, is less suitable for binding (and therefore favoring) the diapedesis of leukocytes; 3) oATP down-regulates the expression of TNF receptor 1 (TNFR1) by HUVECs (Fig. 5).
It was also shown that, in an experimental model of rheumatoid arthritis in mice, able to "mimic" rheumatoid arthritis in humans, the recruitment of immune cells to the site of the autoimmune process was reduced by treatment with oATP. For the purposes of the experiment, arthritis in mice of the C57BL / 6 strain was induced using a mixture of antibodies directed against type II collagen (Arthrogen CIA, i.e. Arthritis-inducing Monoclonal Antibody Cocktail, Chemicon International Inc., Temecula, CA ).
The mice were injected with 400 μl of an antibody mixture, containing 10 mg / ml, on day 0. On the following day, 100 μl of a 0.25 mg / ml solution of bacterial lipopolysaccharides were injected into the same mice (Chemicon International Inc. ). After 10 days of inoculation of lipopolysaccharides, induction of arthritis was verified through an evaluation of clinical signs and symptoms.
OATP was then administered, at a dose of 4 mg / kg of body weight, into the peritoneum of the animals. After 6 hours from the administration of the drug, the mice were sacrificed and histological preparations of the paw site of arthritis were prepared, performing a staining with hematoxylin and cosine. The data obtained, shown in fig. 6, which represent a typical sample of the 3 performed, show that the recruitment of inflammatory cells in mice treated with oATP was reduced compared to that of untreated mice.
In view of the results obtained, oATP can be used in the treatment of the etiology of rhemautoid arthritis.
Furthermore, on the basis of the experimentation obtained, the use of oATP in the treatment of Erdheim-Chester disease, which is a systemic form of histiocytosis, not in Langherans cells, is predictable. In this disease, for which there is no pharmacological treatment, the high production of TNFα and other cytochemokines by activated macrophage cells is involved.
Esempio 4 Example 4
This autoimmune disease is also indicated for treatment with oATP.
8). Female mice of the MZB / NZW strain (Jackson Laboratories, Bar Harbor ME), capable of spontaneously developing SLE, were used. Proteinuria and survival levels were evaluated in animals (n = 5) treated with oATP orally, at a dose of 4 mg / kg, once daily, starting from the twentieth week of age of the mice. Survival was observed up to the fortieth week. Proteinuria was studied using special strips for urinary analysis (Bayer, Milan). The results, reported in fig. 7, show that treatment with oATP reduced the increase in proteinuria, facilitating the survival of the animals (Fig. 8).
Esempio 5 Example 5
Utilizzo di oATP nel trattamento della sclerosi multipla Use of oATP in the treatment of multiple sclerosis
The following experimentation refers to the use of oATP in an animal model of acute and chronic experimental autoimmune encephalomyelitis, a model that is used for multiple sclerosis.
A) Activity of oATP in the treatment of sensorimotor deficits in a model of Chronic Experimental Autoimmune Encephalomyelitis (EAE) in the rat This study was conducted to verify whether treatment with oATP can induce the restoration of sensorimotor functions in a model of experimental encephalomyelitis ( EAE) in rats induced by MOG treatment.
This pathology was induced in adult Dark Agouti (DA) female rats weighing 120-170 g by intradermal injection into the base of the tail of 50 µg of recombinant MOG1-125 suspended in PBS and emulsified with CFA (complete Freund's adjuvant) containing M Heat inactivated tuberculosis.
Rats were treated daily with placebo (vehicle) or 0.1 or 1 mg / kg oATP orally starting immediately after MOG inoculation. The animals were placed under daily clinical observation (neuroscoring) to assess the onset and course of EAE.
Clinical investigation revealed a worsening of neurological symptoms starting 2-3 days after MOG injection; peak neurological deficit was reached in the control group on day 13.
Treatment with oATP showed a notable effect in reducing neurological symptoms during the monitoring period compared to vehicle-treated rats.
Animals treated with oATP 0.1 and 1 mg / kg orally showed less disabling symptoms from day 10 until the end of the study; this effect was dose-dependent, and the group treated with oATP 1 mg / kg showed the best neuroprotection profile between day 15 and day 30.
In addition to this, a delay in reaching peak neurological deficit was observed in both oATP-treated groups compared to the vehicle-treated group.
These results confirm that OATP possesses a neuroprotective effect in experimental encephalomyelitis models.
Materiali e metodi Materials and methods
60 Dark Agouti rats (Harlan, The Netherlands) were used in this experiment.
The animals were divided into the following treatment groups:
- 20 ratti trattati giornalmente con oATP (0,1 mg/kg/die per os); - 20 rats treated daily with oATP (0.1 mg / kg / day orally);
- 20 ratti trattati giornalmente con oATP (1 mg/kg/die per os); - 20 rats treated daily with oATP (1 mg / kg / day orally);
- 20 rats treated daily with the vehicle (placebo group).
In all groups, administration began on the same day as the MOG inoculation.
A clinical examination of each animal's neurological condition was performed daily using a rating scale ranging from 0 (no symptoms) to 5 (complete paralysis of all limbs).
Clinical neuroscoring was performed until day 50 (end of study).
Risultati Results
Induction of chronic EAE in DA rats produced significant sensorimotor deficits (detected by a dedicated rating scale) with a peak during the 13th day after MOG injection.
Analysis of clinical scores revealed a significant difference between the vehicle group and those treated with oATP (at two different dosages).
9). During the first 10 days of the study starting from EAE induction, all groups showed a similar trend. However, starting from day 13, the clinical score in the vehicle group was significantly higher than in the groups treated with oATP; this difference translates into a considerably higher degree of neurological symptoms (see Fig. 9).
Conclusioni Conclusions
These results suggest that OATP (at doses of 0.1 and 1 mg / kg per os) has a substantial effect in reducing symptoms and promoting restoration of sensorimotor function in a chronic rat model of EAE.
The animals treated with oATP showed a markedly reduced and delayed symptomatology compared to the control group; this effect was shown to be dose-dependent, as the high-dose group showed the best neuroprotection profile between day 15 and day 30 post-injection.
B) Attività dell'oATP nel trattamento dei deficit sensomotori in un modello di Encefalomielite Sperimentale Autoimmune (EAE) acuta nel ratto B) Activity of oATP in the treatment of sensorimotor deficits in a model of acute Experimental Autoimmune Encephalomyelitis (EAE) in the rat
This study was conducted to investigate whether oATP treatment can induce restoration of sensorimotor function in an experimental rat acute encephalomyelitis (EAE) model.
Acute EAE was induced in adult Lewis rats weighing 200-250 g by immunization in the hind leg with 100 µg of MBP (Guinea pig myelin basic protein) suspended in 50 µl of saline and emulsified with 50 µl of CFA ( complete Freund's adjuvant).
The rats were then treated daily orally with placebo (vehicle) or oATP (0.1 or 1 mg / kg) starting 5 days after MBP inoculation; the animals were placed under daily clinical observation (neuroscoring) to assess the onset and course of EAE.
Clinical scoring revealed a worsening of neurological symptoms starting from day 10 post-injection, with a peak in the control group on day 13 and a partial regression of symptoms starting from days 18-19.
Treatment with oATP was able to significantly reduce neurological symptoms during the monitoring period compared to vehicle-treated rats.
Materiali e Metodi Materials and methods
Female Lewis rats (Charles River, Germany) with an age of 10 to 14 weeks and a weight of 200-250 g were used for this experiment.
The animals were divided into the following treatment groups:
- 15 ratti trattati giornalmente con oATP (0,1 mg/kg/die per os); - 15 rats treated daily with oATP (0.1 mg / kg / day orally);
- 15 ratti trattati giornalmente con oATP (1 mg/kg/die per os); - 15 rats treated daily with oATP (1 mg / kg / day orally);
- 15 rats treated daily with the vehicle (placebo group).
In all groups, administration began 5 days after MBP inoculation.
A clinical examination of each animal's neurological condition was performed daily using a rating scale ranging from 0 (no symptoms) to 5 (complete paralysis of all limbs). Clinical neuroscoring was performed until post-injection day 21 (end of study).
Risultati Results
Induction of acute EAE in Lewis rats produced significant sensorimotor deficits (detected by a dedicated rating scale) starting on day 10 after MBP injection, peaking during day 13.
Analysis of clinical scores revealed a significant difference between the vehicle group and those treated with oATP (at two different dosages) in the period between the 12th and the 15th day.
10). The highest mean value of neurological deficit for the vehicle group was equal to 3 (day 13); this value decreased to 2.3 in the 0.1 mg / kg oATP group and to 1.5 in the 1 mg / kg oATP group (see Fig. 10).
Conclusioni Conclusions
These results suggest that OATP (at doses of 0.1 and 1 mg / kg orally) has a substantial effect in reducing symptoms and promoting restoration of sensorimotor function in an acute rat model of EAE.
The animals treated with oATP showed significantly reduced symptoms compared to the control group; this effect is dose dependent, given that the high dose group showed the best neuroprotection profile.
The results obtained also make it possible to anticipate the use of oATP for the treatment of both type I and type II diabetic patients.
Type I diabetes is an autoimmune disease, like rheumatoid arthritis, and the mechanisms underlying the pathogenesis of autoimmunity can be effectively modulated by treatment with oATP, which reduces both diabetes and the activation of immunocompetent cells.
The use of oATP is also useful in diabetic patients who need a pancreatic islet transplant.
Type II diabetes can also benefit from treatment with oATP, since insulitis affecting the pancreas is characterized by the release of many cytochemokines, the production of which can be effectively counteracted by the immunosuppressive effect induced by the administration of oATP.
