“GRAFT RIAS S ORBIBILE E BIO COMPATIBILE PER L’INNESTO A SEGUITO DI EXERESI DELLA PLACCA DI IPP” "GRAFT RIAS S ORBIBLE AND BIO COMPATIBLE FOR GRAFT FOLLOWING EXERESI OF THE IPP PLATE"
DESCRIZIONE DESCRIPTION
The present invention refers to a graph; resorbable in biocompatible material, such as PGA or PLA, to be used in the urological field, in particular as a dermal graft in the exeresis of the plaque of IPP (induratio penis Plastica) following La Peyronie's disease.
Peyronie's disease, due to a cause not yet well known, concerns the male genital organ and determines a more or less pronounced penile deformity due to the presence of one or more hard fibrous plaques, with a nodular appearance, located above the corpora cavernosa of the penis, in particular at the level of the tunica albuginea (the sheath that covers the corpora cavernosa of the penis and which represents the supporting structure that determines the rigidity of the penis in erection) with irreversible degeneration of the elastic component albuginea.
The area of fibrosis, generically defined "plaque", causes a curvature of the penis towards the diseased side. This disease is associated with severe pain and erectile dysfunction as the disappearance of the elastic fibers that occurs in favor of the growth of the dense fibrous tissue of the plaque constitutes an alteration of the mechanical characteristics of the corpora cavernosa.
When the disease has stabilized (for at least six months) and is such as to compromise sexual function, it is necessary to resort to surgical therapy such as surgery of the albuginea or complete exeresis of the plaque.
The current trend is that of a complete exeresis (removal or removal) of the plaque: in this case there is the consequent replacement of the removed section with a graft or dermal graft consisting of autologous tissue (typical of the subject, such as the dermis of the thigh of the patient) as a tissue is required that undergoes a natural histo-transformation, maintaining the elastic fibers contained in it, to allow the restoration of the organ's functionality: in fact, the production of elastic tissue is not foreseen in the tissue repair processes but only of fibrous tissue since in the cellular matrix there are only fibroblasts and not elastoblasts.
The grafts, also called graft, allow to restore the primitive length to the hiatus involved by the cicatricial retraction tensing the penis while at the same time acting as a support for the patient's autologous tissue that will be formed over time above said graft.
Synthetic grafts in the aforementioned corporopathy technique are not widely used today as their pathophysiological characteristics of engraftment do not allow the reconstruction of a symmetrical and congruous cavernous albuginous envelope.
It is in fact known that their mechanism of engraftment does not provide for vascular inosculation but only for their fibroblastic encapsulation: this would involve a replacement structure of the albuginea devoid of elastic fibers and therefore not extensible, and a fibrous reaction of the erectile tissue below the graft with a tendency to retract.
In cases of severe erectile deficit, another type of surgical technique is used which involves the use of penile prostheses of various types, soft, malleable, hydraulic, in order to straighten and / or lengthen the organ and regain an erection , in combination with self-healing dermal grafts. However, this technique involves a high percentage of infectious complications when using autologous dermal grafts, due to the presence of staphylococci.
Even the use of heterologous (SIS) or synthetic (GORETEX) grafts, while avoiding the aforementioned complications, does not guarantee the achievement of high elasticity due to their tendency to retraction and the quality of tissue reconstruction around them.
Another surgical technique consists of a resection / incision surgery of the plaque, resection of the albuginea and cavernous bodies, and grafting of graphs, preferably autologous of the saphenous vein, between the albuginea and the cavernous bodies. Also in this case, depending on the type of graphs: the above drawbacks may occur.
Therefore it is highly desirable to have grafts available, in particular synthetic ones, which maximize the elasticity characteristics, almost as much as the original tissue, determine a better quality of tissue reconstruction with the creation of elastoblasts in addition to fibroblasts without granulomas, keloids and the like, minimizing the graft retraction reaction, and infectious reactions.
The object of the present invention is to provide a graph for IPP exeresis which overcomes the drawbacks that the known art complains of, in particular to provide a highly elastic biocompatible graph, which presents a reduced formation of the fibrotic capsule around it, even when replacing a extensive IPP plaque, and which can be used in any type of surgical operation without causing infections.
Another object of the present invention is to provide such a graph which is also easily achievable and economical, as well as reliable, functional and absorbable in order to avoid a subsequent intervention for removing the graph.
These purposes are achieved by the absorbable graph according to the invention in biocompatible material consisting of PGA or PLA which has the characteristics of the attached independent claim 1.
Realizzazioni vantaggiose dell’invenzione appaiono dalle rivendicazioni dipendenti. Advantageous embodiments of the invention appear from the dependent claims.
The graph according to the invention for grafting following the exeresis of the IPP plaque (induratio penis Plastica) following Peyronie's disease is made with a tissue deriving from a biocompatible and resorbable polymer chosen between PGA and PLA.
Ulteriori caratteristiche dell’invenzione risulteranno più chiare dalla descrizione dettagliata che segue, riferita ad alcune sue forme di realizzazione puramente esemplificative, e quindi non limitative, illustrate nei disegni annessi, in cui: Further features of the invention will become clearer from the detailed description that follows, referring to some of its purely exemplary and therefore non-limiting embodiments, illustrated in the attached drawings, in which:
Fig. la è una vista in pianta dall’alto di una forma di realizzazione del grafi in accordo all’invenzione; Fig. La is a top plan view of an embodiment of the graph according to the invention;
Fig. lb è una vista laterale del graft illustrato in fig. la; Fig. 1b is a side view of the graft illustrated in fig. there;
Figg. 2a-2c sono viste illustranti i possibili posizionamenti del graft di fig. la sull’ un organo genitale maschile rispettivamente in posizione ventrale, dorsale, laterale; Figs. 2a-2c are views illustrating the possible positioning of the graft of fig. the on a male genital organ respectively in the ventral, dorsal, lateral position;
Fig. 3 is a cross-sectional view of a male genital organ presenting the PPI plaque.
Graft 1 (figs. 1a and 1b) is generally flat, rectangular or square in shape, with variable dimensions depending on the possible maximum dimensions of the plate.
A dimensional example of such a graft is 5 cm x 10 cm.
Said graft 1 is in the form of fabric 2 (with holes) and its thickness is not binding for the purposes of the present invention even if an example may consist of a value of around 600 microns.
In the case in which the graft is in PLA, it is preferable that it is in the form of fabric 2 deriving from an ultralight monofilament of PLA.
For PLA we intend to identify all polymers or copolymers based on lactic acid (L-, D-, racemic, or dimer, esters, etc. or their combination). Particularly preferred are polylactic acid fibers (homopolymer) PLA.
This PLA fabric 2 preferably has a grammage comprised between 30 and 160 deniers.
Il termine denari, D, indica il peso del tessuto, dove 1D corrisponde a 9.000 metri di filato di peso 9.000g (P(g) /L (9.000m)), The term denier, D, indicates the weight of the fabric, where 1D corresponds to 9,000 meters of yarn weighing 9,000g (P (g) / L (9,000m)),
When the material of the graft 1 is PGA, said gaft is preferably in the form of a fabric made with an ultralight yarn or monofilament deriving from PGA fibers (polyglycolide or polyglycol acid), preferably homopolymer.
Inoltre detto graft può essere anche dotato di una o più strisce di rinforzo su almeno una delle sue superfici realizzate con lo stesso tipo di tessuto del graft in PGA, Furthermore, said graft can also be equipped with one or more reinforcement strips on at least one of its surfaces made with the same type of fabric as the PGA graft,
The fabric of graph 1 in PGA can be made by weaving said PGA monofilament in various ways, giving rise to a knit fabric, a woven fabric or a non-woven fabric. It is preferable to use a waip knit fabric. Thanks to its mechanical consistency, the present graph can be used alone without the need to be supported thanks to the fact that it is a spun fabric.
When using a PGA monofilament, the fabric 2 preferably has a denier (also called "linear mass density") or weight between 240 and 320 denier and the PGA monofilament has a denier preferably between 120 and 160 denier, where the deniers here refer to the diameter of the monofilament.
When the graph in PGA is made from a yarn of PGA fibers, it is a 75 denier / 30 strand (parallel to each other) multifilament yarn and is warp knitted. This construction allows the graphs to have greater rigidity compared to processing with monofilament.
The processing of graph 1 in tissue form takes place in a controlled contamination environment, in a clean room and with reduced humidity (in the case of PLA). After processing, the graph 1 is placed in a double blister closed with a Tyvek sheet to avoid contamination, and sent to a sterilization cycle with gamma rays. At this point the graph is ready to be used in the operating session,
Graph 1 according to the invention is intended to be placed above the corpora cavernosa 10 (figs. 2a-2c, 3) after removing the IPP plate 11 (fig. 3) and being sutured near the tunica albuginea 12, after making an incision of the dermis 15 (fig. 3) and of the Buck fascia 14 (fig. 3) according to known surgical techniques.
Said graph 1 can be sutured at the incision by suture thread with a diameter of 3/0 or 4/0, in resorbable material. Preferably the thread for suturing this graph is of the same material used to make the fabric of the graph.
The main advantage of a PLA or PGA graph is its reabsorbability during the regeneration of the excised area and therefore its removal is not necessary as is the case for the silicone graph coated with turbostratic carbon. Furthermore, it does not give rise to any risk of infection, and the quality of the re-epithelialization is high.
Furthermore, this PLA or PGA graph does not present any risk of adherence of the fibrotic capsule to the graph; as it is completely absorbable within 3-6 months for PLA and 1-2 months for PGA based on metabolism.
In addition, PGA and PLA have the advantage of resorbing, leaving room for an elastic autologous neotextile like the original one.
Numerous variations and detailed modifications can be made to the present embodiments of the invention, within the reach of a person skilled in the art, however falling within the scope of the invention expressed by the attached claims.
