Description
Descrizione dell'invenzione industriale avente per titolo:
“USO DELLA PROTEINA UK114 O DI SUOI FRAMMENTI PER IL TRATTAMENTO E LA PREVENZIONE DELLO SHOCK ENDOTOSSICA"
La presente invenzione ha per oggetto l’uso della proteina UK114 o di suoi frammenti per il trattamento e la prevenzione dello shock endotossico.
Lo shock è una sindrome clinica nella quale il dato comune, la perfusione tessutale insufficiente, passa attraverso molteplici manifestazioni cliniche che sono l'ansietà, la confusione, il coma, l'iperventilazione, l'oliguria, l'anuria, l'ipotensione, la vasocostrizione e la vasodilatazione. I sintomi possono essere di tale intensità e gravità da condurre alla morte con una notevole frequenza (Beai AL and Cerra FB. JAMA, 1990, 271: 226-233).
La sepsi è in ordine di frequenza la terza causa di shock dopo l'emorragia e l'infarto del miocardio. In particolare, i batteri gram-negativi sono i maggiori responsabili dello shock settico sebbene importanza possano avere anche le infezioni da funghi, da ricketsie e da virus (Beal AL and Cerra FB. JAMA, 1990, 271: 226-233). Mentre è rara la comparsa di shock endotossico in soggetti sani al di sotto dei 40 anni, sono fattori predisponenti allo sviluppo di tale forma morbosa le manovre endoscopiche, il diabete, le epatopatie croniche, discrasie ematiche e l'uso prolungato di farmaci immunosoppressori quali corticosteroidi e ciclosporina A. Ad oggi, non esiste un approccio terapeutico per il trattamento dello shock endotossico. Questo comporta nei pazienti ospedalizzati un rischio di mortalità molto elevato, specie in caso di infezioni da gram positivi, pari a circa 50-80% (Beal AL and Cena FB. JAMA, 1990, 271: 226-233).
Si è ora trovato che la proteina UK1 14, descritta in WO 96/02567 e in WO 00/63368, fino ad oggi studiata per una serie di applicazioni terapeutiche che comprendono il trattamento di neoplasie (WO 96/02567), malattie autoimmuni (WO 98/11909), patologie indotte da TNF (WO 98/42366), AIDS (WO 98/11137), trattamento e prevenzione del rigetto di organi trapiantati (WO 00/78329), può essere vantaggiosamente utilizzata nel trattamento dello shock endotossico.
Si è inoltre trovato che la stessa attività farmacologica è condivisa da peptidi aventi da 10 a 20 amminoacidi, preferibilmente da 12 a 18 amminoacidi, aventi sequenze corrispondenti alla sequenza 1-20 (N-terminale) e alla sequenza 55-95 della proteina UK1 14 nativa, riportata nella figura allegata.
L’invenzione riguarda pertanto anche tali peptidi e le composizioni farmaceutiche che li contengono.
I peptidi preferiti dell’ invenzione hanno le seguenti sequenze, corrispondenti rispettivamente alle sequenze 1-15, 61-75 e 76-90 della proteina nativa
L’invenzione comprende inoltre i peptidi funzionalmente equivalenti a quelli sopra definiti. Per peptidi funzionalmente equivalenti si intendono peptidi dotati di attività paragonabile a quelli dei peptidi immodificati e che, rispetto alle sequenze sopra citate, presentino sostituzioni conservative di amminoacidi e/o delezioni e/o inserzioni di amminoacidi e/o sostituzioni con amminoacidi corrispondenti della serie D e/o derivatizzati ai gruppi animino, idrossi e tio e/o amminoacidi retroinversi. Tali modifiche sono alla portata di un qualsiasi esperto del settore e possono portare a peptidi più stabili nei confronti dell’idrolisi enzimatica e/o dotati di diverse caratteristiche farmacocinetiche. La preparazione dei peptidi viene effettuata con metodi convenzionali, preferibilmente utilizzando la sintesi in fase solida secondo Merrifield o tecniche analoghe.
Per i previsti impieghi terapeutici, UK 114 e i peptidi corrispondenti saranno somministrati per via parenterale, in forma di opportune composizioni farmaceutiche, a dosi che saranno facilmente determinate dai clinici sulla base delle caratteristiche farmacocinetiche e tossicologiche della proteina o del peptide utilizzato, oltre che della gravità della patologia e delle condizioni del paziente (peso ed età). I dosaggi saranno in linea di massima compresi tra 0.1 e 10 mg al giorno di UK 114 o di peptide, per via intramuscolare o sottocutanea, eventualmente suddivisi in più somministrazioni.
L’attività farmacologica di UK 114 e dei peptidi dell’invenzione è stata evidenziata nel modello di shock murino indotto da LPS, come riportato nel seguente esempio.
ESEMPIO
Topi Balb/c maschi di 6-8 settimane di vita erano trattati intraperitonealmente con 30 microgrammi di UK114 (in 100 microlitri PBS) 24 ore e 1 ora prima della somministrazione intraperitoneale di 500 microgrammi di LPS (gruppo B). Un gruppo di topi controllo era trattato con 100 microlitri PBS secondo le stesse modalità sperimentali usate per l'UK114 (gruppo A). Un ulteriore gruppo di topi veniva trattato con UK 114 solo 1 ora dopo la somministrazione di LPS (gruppo C). Questo approccio sperimentale è più vicino alla pratica clinica dove è difficile intervenire profilatticamente e si può essere nelle condizioni di dovere trattare un paziente con shock endotossico in atto. La mortalità degli animali in ogni gruppo veniva riportata ogni 12 ore fino al 3° giorno dall'iniezione di LPS.
Come mostrato nella Tabella, la somministrazione intraperitoneale di LPS provocava la morte della maggior parte degli animali controllo (trattati con PBS) entro la fine del periodo di osservazione (72 ore post LPS). Di contro, gli animali trattati profilatticamente con UK114 erano chiaramente protetti dagli effetti letali del LPS, e l'incidenza di mortalità era significativamente più bassa che nel gruppo controllo summenzionato. Questo effetto protettivo, seppure meno marcato, era anche osservabile nei topi trattati "terapeuticamente" con UK1 14 solo 1 ora dopo la somministrazione di LPS. L'effetto protettivo dell'K114 non era semplicemente transitorio perché nessuno degli animali sopravvissuti moriva nel mese di follow-up successivo.
Questi dati dimostrano che la somministrazione di UK114 e in grado non solo di prevenire ma anche di "curare" gli effetti letali dell'iniezione di LPS in un noto modello animale di shock endotossico. I dati ottenuti sono fortemente suggestivi per un importante ruolo dell'UK114 nella prevenzione dello shock endotossico nei soggetti a rischio e nel trattamento di quei soggetti con shock endotossico in atto.

- RIVENDICAZIONI 1. Uso della proteina UK114 o di suoi frammenti per il trattamento e la prevenzione dello shock endotossico.
- 2. Peptidi aventi da 10 a 20 amminoacidi, aventi sequenze corrispondenti alla sequenza 1-20 (N-terminale) e alla sequenza 55-95 della proteina UK1 14 nativa.
- 3. Peptidi secondo la rivendicazione 2 aventi da 12 a 18 amminoacidi.
- 4. Peptidi secondo la rivendicazione 2 o 3 aventi le sequenze corrispondenti alle sequenze 1-15, 61-75 e 76-90 della proteina nativa.
- 5. Peptidi delle rivendicazioni 2-4 modificati da sostituzioni conservative di amminoacidi e/o delezioni e/o inserzioni di amminoacidi e/o sostituzioni con amminoacidi corrispondenti della serie D e/o derivatizzati ai gruppi animino, idrossi e fio e/o amminoacidi retro-inversi.
- 6. Composizioni farmaceutiche comprendenti un peptide delle rivendicazioni 2-5 in miscela con un veicolo opportuno.
