Description

Titolo: "ENDOPROTESI D'ANCA PERFEZI
La presente invenzione ha per oggetto un ' endoprotesi d'anca perfezionata.
Sono note endoprotesi d'anca consistenti in un corpo allungato conposto da una parte metaf isiaria, intermedia fra una parte distale ed una parte prossimale.
La parte distale è costituita da uno stelo o infibulo opportunamente rastremato per l' introduzione nel canale midollare .
La estremità dell 'infibulo presenta tagli radiali il cui scopo è quello di modulare la rigidità dello stelo all' interno del canale midollare evitando che insorgano zone di netta differenza di rigidità all'estremità dell' infibulo.
La parte prossimale è costituita da un collare dal quale si estende obliquamente un gambo predisposto per l'accoppiamento conico di una testa sferica destinata ad impegnarsi articolarmente in una sede complementare di un cotile ancorato nell'osso pelvico.
Il collare è assicurato sulla parte metaf isiaria tramite un accoppiamento conico che ne consente l'orientamento rispetto all'asse longitudinale dell ' endoprotesi .
Alle endoprotesi note sono propri alcuni sostanziali inconvenienti che ne compromettono o limitano l'efficacia. Innanzitutto occorre osservare che le endoprotesi note sono dotate esternamente di nervature longitudinali aventi la funzione di realizzare un inpegno antirotazione nel canale midollare . Tuttavia tali nervature creano zone di iperpressione a carico della corticale ossea con conseguente osteolisi reattiva ed indebolimento della stabilità dell ' impianto protesico.
In secondo luogo, durante la marcia, i carichi sulla protesi generano dei momenti che determinano indebite sollecitazioni di conpressione sulla corticale ossea da parte del collare all 'altezza della zona compresa fra il gambo e la parte metafisiaria.
Il carpito tecnico della presente invenzione è ora quello di escogitare un ' endoprotesi d'anca che consenta di ovviare alle citate manchevolezze di quelle attualmente in uso.
Nell'ambito di tale compito costituisce uno scopo della presente invenzione la realizzazione di una endoprotesi d'anca avente una struttura a moduli intercambiabili, tali da consentire una vasta gamma di combinazioni di montaggio adattabili alle diverse esigenze di impianto femorale.
Tale compito e tale scopo vengono ottenuti con un’enctoprotesi d'anca comprendente una porzione metafisiaria che, dal lato distale, è raccordata ad una porzione infibuliforme e, dal lato prossimale, presenta un accoppiamento conico per un collaredal quale si estende obliquamente un gambo di attacco per una testa sferica di articolazione della endoprotesi all'osso pelvico, caratterizzato dal fatto che dette porzioni metafisiarie ed infibuliforme presentano una sezione poligonale e che la detta porzione metafisiaria, al di sotto di detto gambo, si protende oltre la periferia di detto collare, formando un gradino che con detto gambo definisce una rientranza.
Ulteriori caratteristiche della presente invenzione risulteranno maggiormente dalla descrizione che segue di una forma di esecuzione preferita, illustrate a titolo esemplificativo, non limitativo negli uniti disegni incui:
lafig. 1 mostra una vista in sezione longitudinale di una endoprotesi d'ancadi struttura monoblocco;
la fig. 2 èunavista assiale secondo il piano II-.IIdi fig.1;
la fig. 3 è una vista secando il piano di sezione III-III di fig. 1;
la fig. 4 mostra una vista laterale di una endoprotesi d'ancadi tipomodulare ed infine
la fig. 5 mostra una vista esplosa della endoprotesidi fig.4.
Facendo riferimento alle figure 1 e 2, con 1 è indicata una endoprotesi d'anca secondo l'invenzione in oggetto. Tale endoprotesi comprende una porzione 2 metafisiaria, dalla quale si prolunga una porzione infibuliforme (detta in seguito infibulo) di asse A che termina con una punta ogivale 4.
La endoprotesi 1 ha una sezione poligonale ad ottagono, appiattito nel piano di mezzeria P in modo da avere le due facce F1,F2 perpendicolari all'asse maggiore, perpendicolari al piano P.
La porzione metaf isiaria 2 è raccordata all' infibulo 3 in modo da rastremarsi verso la punta ogivale 4.
L'estremità della porzione 2, opposta alla punta 4, presenta un piano 5 dal quale si eleva un codolo tubolare 6 di asse B, conico esternamente. L'asse B è disassato verso la faccia FI rispetto all'asse A.
Sul codolo tubolare 6 è accoppiato un collare 7 dal quale si estende obliquamente secondo l'asse C e formando un angolo ottuso con la parte metaf isiaria 2, un gambo 8 terminante con un tronco di cono 9 per l'applicazione della testa sferica di articolazione dell ' endoprotesi 1 nel cotile ancorato nell'acetabolo dell'osso pelvico.
Attraverso il collare 7 e la boccola 6 è condotta una vite 10 che, avvitata in un foro filettato Ila della parte metaf isiaria 2, permette di bloccare il collare 7 sulla boccola 6 e quindi stabilire l'orientamento angolare del gambo 8 rispetto al piano di mezzeria P. Il collare 7 ha un diametro esterno tale da rimanere interno alla superficie ccnpresa fra l'asse B e la faccia FI. In tal modo la parte metafisiari , sporge, sotto al garrito 8, con un gradino 11 il quale, con il garrito 8, fonia una rientranza 12.
L'endoprotesi descritta presenta una tem a di tagli 13,14,15 talòi che il taglio 13 formi un angolo di 135° con gli altri due tagli 14,15 rispetto all'asse A. Inoltre il taglio 13 giace nel piano P ed è aperto sulla faccia FI, mentre gli altri due 14,15 delimitano un settore 16 comprendente la faccia F2 che rimane integra.
Come si riconosce 11endoprotesi descritta raggiunge perfettamente gli scopi preposti. La sezione ad ottagono appiattito, in particolare della porzione metaf isiaria, conferisce alla protesi 1, la facoltà di potersi adattare alla forma del canale midollare favorendo un intimo legame con l'osso femorale. Nel contempo gli spigoli dell'ottagono, mentre assicurano un valido effetto antirotazione, non danno luogo a taglienti in grado di incidere sul tessuto osseo ed innescare processi degenerativi della stabilità dell'impianto osseo.
Costituisce un particolare vantaggio della presente invenzione il disassamento fra l'asse A dello stelo e l'asse B del collare che riduce il carico sulle corticali.
Un ulteriore vantaggio che favorisce anch'esso migliore distribu zione del carico sulle corticali, è offerrto dalla faccia piana FI che presenta un'anpia area in virtù della ottagonalità della sezione trasversa. La configurazione trasversa ottagonale permette di sfruttare una migliore sollecitazione sulle corticali ottenuta da disassamento nel giusto equilibrio tra sollecitazione sulla corticale mediale e laterale ed il mantenimento del contatto con le corticali anteriore e posteriore tale da rendere stabile l'infibulo alle sollecitazioni in rotazione secondo il piano trasverso. L'accoppiata tra ortogonalità e disassamento permette pertanto di ottenere una migliore ripartizione dei carichi sulle corticali mantenendone pari la stabilità rotatoria.
Da rilevare che la faccia interna FI e la faccia esterna F2, in particolare del settore 16, forniscono una maggiore superficie d'appoggio sulle corticali e quindi una minore pressione. La rientranza 12 consente, durante la marcia, una distribuzione dei carichi ed una consequenziale variazione dei momenti generati dalla componente muscolare di stabilizzazione .
L’endpprotesi descritta è suscettibile di nunerose modifiche e varianti, tutte rientranti nel medesimo concetto inventivo. Le figure 4 e 5 mostrano una soluzione in cui la parte metafisiaria e l'infibulo costituiscono due distinti moduli metafisiario 17 e distale 18 presentanti rispettivamente una cavità conica 19 ed un codolo tubolare 20 complementari fra di loro per consentire l'accoppiamento per mezzo della vite 10 che, dopo aver attraversato il collare 7 ed il modulo metafisiario 17, s'impegna per avvitamento nel foro filettato del modulo distale 18. Per assicurare maggiormente contro la rotazione di un nodulo 17 rispetto all'altro 18, è previsto un dai te 21 che dal bordo inferiore del modulo metafisiario 17 s ' impegna in una tacca 22 del modulo distale 18, alla base del codolo tubolare 20.
Variando opportunamente forma e dimensioni dei moduli 17 e 18 è possibile ottenere un vasto numero di combinazioni in grado di adattarsi alle diverse anatomie femorali.
Nella pratica attuazione dell'invenzione i materiali impiegati possono essere qualsiasi a seconda delle esigenze . Vantaggiosamente 1 ' endoprotesi è realizzata in lega di titanio biocompatibile.

Claims (6)

  1. R I V E N D I C A Z I O N I 1. Endoprotesi d'anca comprendente una porzione metafisiaria che, dal lato distale, è raccordata ad una porzione inf ibuliforme e, dal lato prossimale, presenta un accoppiamento conico per un collare dal quale si estende obliquamente un gambo di attacco per una testa sferica di articolazione della endoprotesi all'osso pelvico, caratterizzato dal fatto che dette porzioni metafisiarie ed infibuliforme presentano una sezione poligonale e che la detta porzione metafisiaria, al di sotto di detto gambo, si protende oltre la periferia di detto collare, formando un gradino che con detto gambo definisce una rientranza.
  2. 2. Endoprotesi d'anca secondo la rivendicazione 1 caratterizzata dal fatto che dette porzioni metafisiarie ed inf ibuliforme presentano una sezione ottagonale appiattita sul piano di mezzeria con due facce perpendicolari all'asse maggiore di detta sezione.
  3. 3. Endoprotesi d'anca secondo la rivendicazione 2 caratterizzata dal fatto che detta porzione infibuliforme presenta un'estremità dotata di una tema di tagli definenti un settore avente una faccia perpendicolare all'asse maggiore di detta sezione.
  4. 4. Endoprotesi d'anca secondo una delle rivendicazioni precedenti caratterizzata dal fatto che detta parte metafisiaria e detta parte inf ibuliforme costituiscono due distinti moduli metafisiario e distale presentanti rispettivamente una cavità conica ed un codolo tubolare complementari fra di loro per consentire 1 ' accoppiamento per mezzo di una vite condotta attraverso detto collare e detto modulo metafisiario ed impegnantesi per avvitamento in un foro filettato del detto modulo distale.
  5. 5. Endoprotesi d' anca secondo la rivendicazione 4 caratterizzata dal fatto che, per assicurare maggiormente contro la rotazione un modulo rispetto all'altro, è previsto un dente che dal bordo inferiore del detto modulo metafisiario s'impegna in una tacca del modulo distale.
  6. 6. Endoprotesi d'anca secondo quanto desumibile dalla descrizione che precede e dai disegni allegati.
