Abstract
Description
A találmány a fehérvérsejt-proliferáció serkentése által csontvelővédő hatás kifejtésére képes gyógyszerkészítmény előállítására, az ilyen gyógyszert tartalmazó gyógyszrészeti készítményre, gyógyszer-készletre, valamint a fehérvérsejt-proliferáció serkentésére és daganatgátló hatás kifejtésére szolgáló eljárásra vonatkozik.FIELD OF THE INVENTION The present invention relates to a pharmaceutical composition capable of exerting a bone marrow protective effect by stimulating white blood cell proliferation, to a pharmaceutical composition comprising a medicament, to a kit, and to a method for stimulating white blood cell proliferation and inhibiting tumor activity.
Emellett számos, részben veszélyes mellékhatás is fellép, ami a gyorsan proliferáló illetve gyorsan megújuló szövetek általános gátlásának a következménye. The treatment of cancer patients is still essentially performed by surgery, irradiation, hormone interventions in endocrine tumors, immune response enhancers and / or cytostatic / cell proliferation inhibitors. However, it has been found that the patient's mental and physical well-being is significantly impaired, especially when exposed to radiation and cytostatic agents. In addition, there are a number of partially dangerous side effects resulting from the general inhibition of rapidly proliferating and rapidly regenerating tissues. Such a system is also known as the hematopoietic system, which is known to be particularly damaged by irradiation and / or cytostatic treatment. Accordingly, leukocytopenia may occur in patients who have received cytostatic or radiation therapy during chemotherapy, which may lead to severe anemia through thrombocytopenia. Therefore, cytostatic drug should not be used in infinitely high doses during chemotherapy. It is an object of the present invention to overcome these disadvantages.
Ismeretes, hogy Escherichia coli-tenyészetekből ··«It is known that cultures of Escherichia coli ·· «
A kivonat a szűrés után nem-pirogén anyagcseretermékeket tartalmaz.- 3 bacteria and a protein-free metabolite filtrate can be recovered. Such a filtrate can be prepared, for example, by culturing E. coli bacteria for 5 days. After killing the bacteria, the metabolites can be removed by filtration. After filtration, the extract contains non-pyrogenic metabolites.
Ilyen terméket Colibiogen néven már gyógyszerként forgalmaz a Laves-Arzneimittel GmbH Ronnenberg/Hannover cég Németországban úgy orális utón, mint injekcióként használható oldat formájában. Ennél például egy injekcióhoz elő- ‘ állított ampulla 4 billió E. coli baktériumból származói anyagcsere-termék 2 ml fehérjementes oldatát tartalmazza.ISuch a product, already known as Colibiogen, is marketed as a medicament by Laves-Arzneimittel GmbH Ronnenberg / Hannover in Germany, both as an oral solution and as a solution for injection. For example, an ampoule for injection contains 2 ml of a protein-free solution of 4 trillion E. coli-derived metabolites.
E gyógyszer indikációi közé tartoznak organikusI r és funkcionális eredetű bélbántalmak, melyeket mindenekelőtt I spasztikus tünetek kisérnek, továbbá Crohn-betegség, fekélyes j bélgyulladás, csalánkiütés, ekcéma, purpura, étel-allergia, ! tüdőasztma, spasztkus légcsőhurut, akne és migrének. Az orálisan használandó ivóoldatok javallatai közé tartoznak a bélmozgás és a bélnyálkahártya-áteresztőképesség zavara, a bélnyálkahártya károsodása, a bélflóra károsodása kemoterápia és antibiotikum terápia alkalmazása után, béllel összefüggő anyagcsere- és felszívódás-zavarok, Salmonella okozta gyomorés bélhurut, csecsemők és kisgyermekek kólibaktérium okozta Járványos bélh^rutja, endogén enzim, idült étvágytalanság és bélgörcs. Az ilyen szürlet elemzésekor lényegében egy ·»« * ·· • « ♦ · ♦ • « · · · ·These pharmaceutical indications include organic functional bowel disease origin and r, which are above all I spastic disorders little worth, and Crohn's disease, ulcerative colitis j, urticaria, eczema, purpura, food allergy,! lung asthma, spastic bronchitis, acne and migraines. Indications for oral use include intestinal motility and gastrointestinal permeability, intestinal mucosal damage, intestinal flora damage following chemotherapy and antibiotic therapy, gastrointestinal intestinal rut, endogenous enzyme, chronic anorexia and intestinal cramps. A szürlet peptidkomponenseket is tartalmaz, melyek lényegében aszparaginból, glutaminsavból, alaninból, glicinből, lizinből, leucinból állnak. Kis mennyiségű zsírsav is kimutatható, főleg palmitin-, sztearinés olajsav. Végül az E. coli kivonat szűrt oldatában kevés- 4 polysaccharide moieties are mainly composed of glucose, galactose and xylose. The filtrate also contains peptide components consisting essentially of asparagine, glutamic acid, alanine, glycine, lysine, leucine. Small amounts of fatty acids can also be detected, mainly palmitic and stearic oleic acids. Finally, there is little in the filtered solution of the E. coli extract
3-hidroxi-mirisztinsav, valamint csekély mennyiségű bakteriális endotoxin is található.3-Hydroxy-myristic acid and small amounts of bacterial endotoxin are also present.
Meglepő módon most azt találtuk, hogy a fentébb megjelölt cél, azaz a leukocita-proliferáció stimulálása és ezzel a csontvelővédő hatás - különösen besugárzásnál és/vagy citosztatikus kezelésnél - akkor érhető el, ha legalább egy vagy esetleg több kólibaktérium-anyagcsereterméket alkalmazunk, amelyek E. coli tenyészetekből baktérium- és fehérjementes szürlet formájában nyerhetők ki.Surprisingly, it has now been found that the above stated goal, which is to stimulate leukocyte proliferation and thereby protect the bone marrow, in particular by irradiation and / or cytostatic therapy, can be achieved by using at least one or possibly several coliobacterial metabolites which E. coli cultures can be obtained in bacterial and protein-free filtrates.
Célszerűen mégis több ilyen anyagcsere-terméket, illetve teljes szürletet alkalmazunk. Meglepő módon kitűnt, hogy e szürlet említett anyagcsere-termékei daganatgátló hatással is rendelkeznek.Preferably, however, more such metabolites or whole filtrates are used. It has surprisingly been found that said metabolites of this filtrate also have antitumor activity.
Ezért a találmány tárgyát a fenti uj javallatoknak megfelelő gyógyszerkészítmény előállítása képezi, melynél az E. coli tenyészetekből kinyert baktérium- és fehérjementes szürlet egyik anyagcsere-termékét használjuk fel. Az ilyen eljárás különösen alkalmas rákos betegek citosztatikumokkal történő kemoterápiás kezelésénél. A szer sugárterápiá • · · » · »44 · ·· • 4 · · 4 nál is hatásos. Therefore, the present invention provides a pharmaceutical composition according to the above new indications, which uses a metabolic product of a bacterial and protein-free filtrate obtained from E. coli cultures. Such a method is particularly useful in the treatment of cancer patients with cytostatic chemotherapy. It is also effective for radiation therapy • 44 · ·· • 4 · · 4. Preferably, the medicament of the present invention is administered separately from the cytostatic agent, at the same time or at the same time. It is particularly preferred that the medicament be administered once or repeatedly prior to administration of the cytostatic agent. However, both single administration prior to the start of cytostatic chemotherapy and prior to the administration of certain cytostatic applications have proved to be expedient.
Folyamatos orális adagolásnál is kimutatható a találmány alapját képező meglepő hatás.The patient is usually pretreated with at least one of the pharmaceutical formulations for 2 hours to 5 minutes, preferably 1 to 0.5 hours, prior to administration of the cytostatic agent or irradiation, but preferably both medications are used. Even if pre-treatment is performed 1-2 days before radiation and / or chemotherapy, good results can still be obtained. The surprising effect underlying the invention can also be demonstrated with continuous oral administration.
Surprisingly, it has been found that the protective effect underlying the invention is achievable with all known chemical cytostatic agents. However, the combination with the pharmaceutical composition of the present invention should be selected under standard conditions known to the physician, having regard to the type of tumor. Preferred cytostatic agents for colon and stomach cancers are Fluorouracil, breast cancers are Fluorouracil, Cyclophosphamid, Epirubicin, Methotrexat, and squamous cell cancers are Vepesid and Platinex. Of the above tumor types, the medicament of the present invention is particularly effective in squamous cell carcinoma.
When using a combination of a cytostatic agent and a medicament produced in accordance with the invention, it has surprisingly been found that the reduction in white blood cell count caused by cytostatic treatment is not only completely reversible, but also increases in white blood cell count compared to the baseline. Thus, the medicament according to the invention is particularly suitable for the treatment of tumors that have been previously treated with cytostatic chemotherapy and which otherwise have a very adverse reaction to the treatment. The medicament according to the invention is therefore particularly suitable for metastatic tumors.
According to the present invention, it has also been found that the antitumor activity described above is surprisingly potentiated by the use of bacterial and protein-free filtrates and ginkgo biloba leaf extracts containing E. coli metabolites. The invention therefore also relates to a pharmaceutical composition which is a compound of formula Ξ. coli is a combination of bacterial and protein-free filtrate and ginkgo biloba leaf extract. The preparation of such extracts is known and commercially available from various manufacturers. For example, Dr. Willmar Schwabe, Karlsruhe, Germany, sells a standardized extract of ginkgo flavon glycoside in the form of drops, drops and infusion ampoules under the trade name Tebonin. Indications for malnutrition and malnutrition are cerebral and peripheral haemorrhages as an indication of this natural substance, and certain symptoms of brain power weakness such as dizziness, tinnitus and, above all, vascular headaches and visual disturbances can be corrected. In vascular-related hearing, the ear improves hearing and speech impairment. In addition, attention and concentration, intellectual performance, anxiety and depression depression, and neurological disorders are also improved.
In accordance with the present invention, it has now been found that the antitumor effect achieved by a combination of cytostatic agents and E. coli metabolites is further enhanced by the addition of a substance derived from ginkgo biloba leaf extract. After the additional administration of such an extract, even tumors and metastases which have not previously responded to any drug or other pharmaceutical form are in remission. An increased effect can also be observed with radiation treatment.
A ··«The invention therefore also relates to a pharmaceutical composition comprising a bacterial and protein-free filtrate of metabolites of E. coli and an extract of ginkgo biloba leaf. - 8 találmány szerinti gyógyszerkészítmény a ginkgo-levél kivonatot előnyösen ginkgo-flavon-glikozidokra standardizált formában tartalmazza. The 8 pharmaceutical compositions of the present invention preferably contain ginkgo leaf extract in a standardized form for ginkgo flavon glycosides. Preferably, the filtrate containing the E. coli metabolites and / or the ginkgo leaf extract is in intravenous administration and is preferably an infusion and / or injection ampoule. Continuous oral administration of ginkgo extract and E. coli filtrate also achieves the effect of the present invention.
Egy különösen előnyös kiviteli formánál a találmány szerinti gyógyszerkészítmény kiegészitőleg térben elválasztott formában tartalmaz citosztatikumot egyidejű, elválasztott vagy t időbelileg szétosztott használathoz. In a particularly preferred embodiment, the pharmaceutical composition of the invention further comprises a cytostatic drug in a spaced-apart form for simultaneous, separated or temporally dispersed use.
A találmány szerinti gyógyszerkészítményt előnyösen a besugárzás és/vagy citosztatikumok alkalmazása előtt előkezelés formájában alkalmazzuk. Előnyösen daganatos betegségek kezelésére, különösen emlőrák, prosztatarák, gyomorrák, nyelőcsőrák, laphámrák, szarkóma, epeutrák, nyelőcsőrák vagy melanoma kezelésére alkalmas.Preferably, the pharmaceutical composition of the invention is administered as a pretreatment prior to irradiation and / or cytostatic administration. Preferably, it is useful for the treatment of cancerous diseases, in particular for the treatment of breast cancer, prostate cancer, gastric cancer, esophageal cancer, squamous cell carcinoma, sarcoma, gallbladder, esophageal cancer or melanoma.
A találmányt a következő példák mutatják be.The invention is illustrated by the following examples.

- 9 Minden egyes Fluorouracil adás előtt naponta 2 ml fehérjementes oldatot alkalmazunk, amely 4 billió E. coli baktérium anyagcsere-termékeit tartalmazza. A kezelést 5 napon át folytatjuk, melynek során a ginkgo-kivonatot és az E. coli szürletet mindenkor a citosztatikum-adás előtt alkalmazzuk intravénás utón. Több beteg kiegészitőleg kis dózisban további citosztatikumokat is kap, éspedig Platinexet, Vepesidet, Cyclophosphamidot, Methotrexatot és Epirubicint. Kitűnt, hogy mind a 18 emlőrákos betegnél a metasztázisok remissziója következett be, noha ezeket a betegeket előzőleg többször, eredménytelenül kezeltük citosztatikus kemoterápiával. Before each administration of Fluorouracil, 2 ml of a protein-free solution containing 4 trillion E. coli metabolites is administered daily. The treatment is continued for 5 days, during which the ginkgo extract and the E. coli filtrate are administered intravenously each time prior to cytostatic administration. Additionally, many patients receive additional low-dose cytostatic drugs, such as Platinex, Vepeside, Cyclophosphamide, Methotrexat and Epirubicin. Remarkably, all 18 breast cancer patients had remission of metastases, although these patients had been previously failed several times with cytostatic chemotherapy.
