Abstract

1430217 Blood substitute BIOTESTSERUM-INSTITUT GmbH 3 Oct 1973 [3 Oct 1972] 46157/73 Heading ASB Hepatitis-proof, storage-stable, infusible haemoglobin solution which has a stable 2, 3- diphosphoglycerate level is prepared by (a) treating human erythrocytes in dilute solution with beta-propiolactone, (b) washing the product with a neutral or weakly alkaline solution to remove excess beta-propiolactone and its reaction products, (c) haemolysing the erythrocytes by stirring them into distilled water or a physiologically acceptable electrolyte solution, (d) reducing the pH and potassium content of the haemolysate and precipitating the stromalipids by treating the haemolysate with a cation exchange resin in the H<+> form, (e) removing the resin and the precipitated stroma mass by centrifuge, (f) adding substances to regulate isotonicity and pH and optionally ferrohaemoglobin and (g) filtering the product under sterile conditions. The solution used to wash the erythrocytes may be sodium chloride or sodium bicarbonate solution. The cation-exchange resin may be a polystyrene sulphonate. The isotonicity may be adjusted by addition of glucose, sodium acetate or sodium bicarbonate. The pH may be adjusted by addition of sodium hydroxide. The sterile filtration may be through a cellulose asbestos filter or cellulose acetate disc. The solution may be concentrated before its isotonicity is adjusted by freezing it and collecting the haemoglobin-rich melt which runs off the frozen body. The product may be used as a blood substitute, particularly in cases of shock.
