Outillage de pose d’implant intersomatique, qui comporte un outil (1) de pose dudit implant, comprenant un manche (11) de positionnement dudit implant, un porte-implant (2) et une tige porte-implant (15) de tenue d’une cage (4) de l’implant, ledit manche étant pourvu d’une première extrémité (12) formant poignée et d’une seconde extrémité (13) fixée sur ledit porte-implant, ladite tige porte-implant étant reçue mobile en rotation dans une portion tubulaire (14) dudit manche et munie d’une extrémité distale (16), traversant ledit porte-implant pour s’insérer dans ladite cage, ledit porte-implant étant muni d’un pion (22) d’orientation de ladite cage par rapport audit porte-implant. Description

OUTILLAGE DE POSE D’IMPLANTS DISCAUX INTERVERTEBRAUX
La présente divulgation concerne le domaine des opérations du rachis et en particulier la pose d’implants comportant une cage intersomatique destinée à remplacer un disque intervertébral défectueux.
Une cage intersomatique est un dispositif médical implantable remplissant l’espace intervertébral permettant une fusion entre deux vertèbres adjacentes et venant suppléer la fonction de support mécanique d’un disque défaillant.
La présente divulgation concerne plus particulièrement un dispositif médical permettant la mise en place d’une cage intersomatique lors d’une intervention chirurgicale par voie antérieure pour une défaillance d’un disque intervertébral. Un des buts recherchés est d’optimiser l’abord, le positionnement et la stabilisation de la cage intersomatique. En effet, notamment lors d’une pose d’une cage intersomatique lombaire par voie antérieure, les rapports anatomiques et l’espace de travail limité rendent la mise en place de cette cage complexe et à risque. Le présent dispositif va permettre de simplifier et de sécuriser la procédure en limitant le nombre de manipulations dans un espace restreint à proximité de structures nobles telles que vaisseaux sanguins, tube digestif, nerfs.
Il existe déjà des dispositifs de pose d’implants comportant une cage intersomatique, mais ces outils restent complexes et occupent un espace volumineux.
Le document EP2 945 574 A1 décrit par exemple un dispositif comportant un guide pour une cage intersomatique, sur lequel peut coulisser une plaque de fixation de la cage entre des vertèbres, un guide de perçage pourvu de tubes de guidage pour recevoir successivement un outil de perçage puis des vis et un tournevis. Ce dispositif reste un assemblage nécessitant une place importante dans l’espace de travail et nécessitant des positionnements successifs d’outils.
Problème technique
Actuellement, les dispositifs existants destinés à poser des implants intervertébraux ne permettent pas de sécuriser complètement et de simplifier le geste de mise en place d’une cage, d’une plaque de maintien de la cage et de vis de fixation de la plaque sur les vertèbres, tout en réduisant l’espace de travail nécessaire pour l’opération. La présente invention vise à améliorer cette situation.
Au vu de l’art antérieur la présente invention concerne un implant intersomatique et son outillage de pose pour lequel l’outillage de pose comprend un outil adapté à positionner ensemble une cage intersomatique, une plaque de maintien de cette cage et des vis de maintien de la plaque, sans avoir à changer d’instrument, l’outil étant en outre adapté à assurer la stabilisation de ces éléments implantables dans le même geste chirurgical.
Pour ce faire selon un premier aspect la présente divulgation prévoit un outillage de pose d’implant intersomatique, qui comporte un outil de pose dudit implant, comprenant un manche de positionnement dudit implant, un porte-implant et une tige porte-implant de tenue d’une cage de l’implant, ledit manche étant pourvu d’une première extrémité formant poignée et d’une seconde extrémité fixée sur ledit porte-implant, ladite tige porte-implant étant reçue mobile en rotation dans une portion tubulaire dudit manche et munie d’une extrémité distale, traversant ledit porte-implant pour s’insérer dans ladite cage, ledit porte-implant étant muni d’un pion d’orientation de ladite cage par rapport audit porte-implant.
Les caractéristiques exposées dans les paragraphes suivants correspondent à des modes de réalisation pouvant être mises en œuvre indépendamment les uns des autres ou en combinaison les uns avec les autres.
Selon un mode de réalisation particulier, l’extrémité distale de la tige porte-implant est filetée pour se fixer dans la cage ce qui assure un maintien efficace de la cage sur l’outil pour son implantation.
Le porte-implant peut comporter des logements tubulaires, de réception et de retenue temporaire de vis de fixation de l’implant dans des vertèbres de part et d’autre de la cage, dont les axes réalisent des axes d’insertion desdites vis dans lesdites vertèbres. Ces logements masquent les vis lors de la pose de la cage pour éviter toute blessure du patient et les prépositionnent pour leur implantation.
La tige porte-implant peut comporter une molette de manœuvre en rotation de la tige porte-implant accessible dans une découpe réalisée dans une partie intermédiaire élargie du manche. Cette molette facilement accessible permet de désolidariser la tige porte-implant de la cage.
Le manche peut comporter une première partie portant la première extrémité formant poignée et une seconde partie tubulaire recevant la tige porte-implant et portant la seconde extrémité du manche, lesdites première partie et seconde partie étant séparées au niveau de la découpe avant insertion de la tige porte-implant dans la seconde partie du manche puis assemblées pour constituer le manche et retenir la tige porte-implant.
L’implant comprenant une plaque, ladite plaque peut se positionner entre la cage et une face avant du porte-implant et être pourvue de trous de passage dudit pion, de l’extrémité distale filetée de la tige porte-implant et des vis, la cage étant elle-même munie d’un trou taraudé de réception de ladite extrémité distale filetée et d’un trou lisse de réception dudit pion d’orientation.
Le vissage de l’extrémité distale filetée de la tige porte-implant dans le trou taraudé de la cage peut maintenir la plaque axialement et angulairement entre la cage et la face avant du porte-implant. La plaque est ainsi maintenue angulairement par la tige porte-implant et le pion.
La présente divulgation concerne en outre à un outillage de préparation de pose d’implant intersomatique pouvant être utilisé avec l’outillage décrit précédemment ou d’autres outillage de pose d’implants et qui comporte un ou plusieurs outils de préparation comportant un ou plusieurs emporte-pièces et un ou plusieurs gabarits d’essai, lesdits gabarits d’essai étant pourvus d’une tige de guidage munie, à une extrémité distale, d’un gabarit de cage et d’un gabarit de plaque, ledit gabarit de plaque étant muni de découpes de passage d’un dit emporte-pièce.
Avantageusement, la tige de guidage constitue un guide à codage anti-rotation pour un porte outil, adapté à coulisser sur la tige de guidage, porteur à une extrémité distale dudit emporte-pièce, et pourvu d’une poignée à une extrémité proximale.
L’emporte-pièce comporte avantageusement des lames adaptées à traverser lesdites découpes lors d’un mouvement coulissant du porte outil sur la tige de guidage, lesdites lames étant conformées pour définir le contour une partie d’os à retirer et pourvues d’une terminaison libre tranchante.
La poignée peut comporter une terminaison libre adaptée à recevoir un impact d’un marteau.
La présente divulgation concerne en outre un implant comportant une cage, des vis et une plaque de rétention de la cage, pour lequel la cage est en forme de coin avec des faces supérieure et inférieure inclinées, une face arrière formant une base du coin et une face avant formant une pointe du coin.
La plaque peut comporter de trous qui forment des passages inclinés pour lesdites vis.
Les vis peuvent comporter un double filetage.
Les trous de la plaque recevant les vis peuvent comporter un épaulement d’appui de têtes de vis et un taraudage de verrouillage des vis dans la plaque.
Les faces supérieure et inférieure de la cage peuvent comporter des découpes de passage des vis pour accroître la compacité de l’implant.
La plaque est avantageusement incurvée selon un arc dont le centre est situé d’un côté opposé à la cage pour respecter la morphologie du patient au niveau des vertèbres.

Claims (15)

  1. Outillage de pose d’implant intersomatique, caractérisé en ce qu’il comporte un outil (1) de pose dudit implant, comprenant un manche (11) de positionnement dudit implant, un porte-implant (2) et une tige porte-implant (15) de tenue d’une cage (4) de l’implant, ledit manche étant pourvu d’une première extrémité (12) formant poignée et d’une seconde extrémité (13) fixée sur ledit porte-implant, ladite tige porte-implant étant reçue mobile en rotation dans une portion tubulaire (14) dudit manche et munie d’une extrémité distale (16), traversant ledit porte-implant pour s’insérer dans ladite cage, ledit porte-implant étant muni d’un pion (22) d’orientation de ladite cage par rapport audit porte-implant.
  2. Outillage de pose d’implant intersomatique selon la revendication 1, pour lequel l’extrémité distale (16) de la tige porte-implant est filetée.
  3. Outillage de pose d’implant intersomatique selon la revendication 1 ou 2, pour lequel le porte-implant comporte des logements tubulaires (24), de réception et de retenue temporaire de vis (6) de fixation de l’implant dans des vertèbres (A, B) de part et d’autre de la cage, dont les axes réalisent des axes d’insertion desdites vis dans lesdites vertèbres.
  4. Outillage de pose d’implant intersomatique selon la revendication 1, 2 ou 3, pour lequel la tige porte-implant comporte une molette (17) de manœuvre en rotation de la tige porte-implant accessible dans une découpe (141) réalisée dans une partie intermédiaire élargie du manche (11).
  5. Outillage de pose d’implant intersomatique selon la revendication 4, pour lequel le manche comporte une première partie portant la première extrémité formant poignée (12) et une seconde partie tubulaire recevant la tige porte-implant et portant la seconde extrémité (13) du manche, lesdites première partie et seconde partie étant séparées au niveau de la découpe (141) avant insertion de la tige porte-implant dans la seconde partie du manche puis assemblées pour constituer le manche et retenir la tige porte-implant.
  6. Outillage de pose d’implant intersomatique selon l’une quelconque des revendications 1 à 5 caractérisé en ce qu’il comporte un ou plusieurs outils de préparation comportant un ou plusieurs emporte-pièces (58) et un ou plusieurs gabarits d’essai (50) , lesdits gabarits d’essai étant pourvus chacun d’une tige de guidage (51) munie, à une extrémité distale, d’un gabarit de cage (53) et d’un gabarit de plaque (52), ledit gabarit de plaque étant muni de découpes (54, 55) de passage d’un dit emporte-pièce (58).
  7. Outillage de pose d’implant intersomatique selon la revendication 6, pour lequel la tige de guidage (51) constitue un guide à codage anti-rotation pour un porte outil (56), adapté à coulisser sur la tige de guidage, porteur à une extrémité distale dudit emporte-pièce (58), et pourvu d’une poignée (57) à une extrémité proximale.
  8. Outillage de pose d’implant intersomatique selon la revendication 6 ou 7, pour lequel l’emporte-pièce (58) comporte des lames (59, 60) adaptées à traverser lesdites découpes (54, 55) lors d’un mouvement coulissant du porte outil sur la tige de guidage, lesdites lames étant conformées pour définir le contour d’une partie d’os à retirer et pourvues d’une terminaison libre (59a, 60a) tranchante.
  9. Outillage de pose d’implant intersomatique selon la revendication 7, pour lequel la poignée (57) comporte une terminaison libre adaptée à recevoir un impact d’un marteau.
  10. Implant intersomatique implantable au moyen d’un outillage selon l’une quelconque des revendications 1 à 5, comportant une cage (4), des vis (6) et une plaque (3) de rétention de la cage, pour lequel la cage est en forme de coin avec des faces supérieure et inférieure inclinées, une face arrière formant une base du coin et une face avant formant une pointe du coin, ladite plaque se positionnant entre la cage et une face avant (25) du porte-implant dudit outillage et étant pourvue d’au moins un trou (31) de passage dudit pion (22).
  11. Implant intersomatique selon la revendication 10 pour lequel ladite plaque comporte des trous (33) formant des passages inclinés pour lesdites vis.
  12. Implant intersomatique selon la revendication 10 ou 11, pour lequel lesdites vis présentent un double filetage et pour lequel les trous (33) de la plaque comportent un épaulement (332) d’appui de têtes de vis et un taraudage (331) de verrouillage des vis dans la plaque.
  13. Implant intersomatique selon la revendication 10, 11 ou 12 pour lequel les faces supérieure et inférieure de la cage comportent des découpes (43) de passage des vis.
  14. Implant intersomatique selon l’une quelconque des revendications 10 à 13, pour lequel la plaque (3) est incurvée selon un arc dont le centre est situé d’un côté opposé à la cage.
  15. Ensemble outillage de pose d’implant intersomatique selon l’une quelconque des revendications 1 à 9 et implant selon l’une quelconque des revendications 10 à 14 pour lequel ladite plaque (3) se positionne entre la cage et une face avant (25) du porte-implant et est pourvue de trous (31, 32, 33) de passage dudit pion (22), de l’extrémité distale filetée (16) de la tige porte-implant et des vis (6), la cage étant elle-même munie d’un trou taraudé (42) de réception de ladite extrémité distale filetée (16) et d’un trou lisse (41) de réception dudit pion d’orientation et pour lequel le vissage de l’extrémité distale filetée (16) de la tige porte-implant dans le trou taraudé (42) de la cage maintient la plaque (3) axialement et angulairement entre la cage et la face avant (25) du porte-implant.
