Abstract
Description
L'invention concerne une cupule métallique pour un implant cotyloïdien de prothèse totale de hanche, un élément de préhension d'une telle cupule ainsi qu'un ancillaire correspondant. L'invention se rapporte plus particulièrement au domaine des 5 prothèses de hanche à double mobilité. Une telle prothèse comporte classiquement une cupule métallique de forme générale hémisphérique, montée dans la cavité cotyloïdienne d'un patient, à l'intérieur de laquelle un insert en polyéthylène est monté pivotant. Une tige est insérée dans le canal médullaire du fémur, dont une 10 extrémité est équipée d'une tête sphérique montée pivotante dans l'insert. La prothèse bénéficie ainsi d'une double mobilité, c'est-à-dire d'une première mobilité entre la tête fémorale prothétique et l'insert, et d'une seconde mobilité entre l'insert et la cupule. L'utilisation d'une prothèse à double mobilité permet notamment de 15 limiter les risques de luxation pour le patient. Le document FR 2 785 523, au nom de la Demanderesse, décrit une prothèse de hanche à double mobilité dans laquelle la cupule présente une portion hémisphérique prolongée, à partir de son plan équatorial, par une courte portion cylindrique s'étendant sur toute la périphérie de la cupule et 20 délimitant l'entrée de celle-ci. Une telle cupule évite les risques de conflit avec les tissus environnants et assure une bonne répartition des efforts résultant du contact avec l'insert en polyéthylène. Bien que limités, les risques de luxation sont toujours présents. En 25 effet, dans le cas d'un débattement trop important, l'insert peut être sorti de la cupule et provoquer ainsi la luxation. This differs from the embodiment of Figure 2 in that the first hemispherical portion 2 of the cup 1 is extended by a second portion 4 whose internal surface 5 has a frustoconical shape, arranged in the extension of the inner surface of the first part 4. Figures 4 to 7 illustrate a gripping element 15 according to the invention. The latter has the general shape of a disk or a wafer and comprises a first portion 16 in the general shape of a spherical portion, surmounted by a second portion 17 of larger diameter, forming a shoulder intended to abut against the free edge 8 of the second portion 4 of the cup 1. The gripping element 15 has a central opening 18 of generally frustoconical shape which narrowed at the second portion 17 and widened at the first portion 16 , opens radially. The central opening 18 furthermore has three slots 19 each extending radially over a portion of the radius of the gripping element 15. The slots 19 are intended to facilitate the deformation of the gripping element 15.

Celui-ci comporte un manche 21 présentant une première extrémité 22 équipée d'une poignée 23, reliée à une seconde extrémité formant une tête 24 par l'intermédiaire d'une zone médiane courbe 25. La tête 24 comporte une extrémité plane 26 équipée d'un plot de centrage cylindrique 27 s'étendant vers l'extérieur parallèlement à l'axe A de la poignée 23 et d'un plot de forme tronconique 28, écarté du plot de centrage 27 et présentant un épaulement 29 au niveau de son extrémité libre. La partie évasée du plot tronconique 28 est orientée vers l'extérieur, la partie rétrécie étant orientée vers la première extrémité 22 du manche 21. Le plot tronconique 28 comporte une base 29 montée coulissante sur la tête 24, entre une position rentrée et une position sortie du plot cylindrique 28. La base 29 est reliée de manière pivotante à une première 20 extrémité d'une biellette 30, autour d'un axe 31. La seconde extrémité de la biellette est reliée à une poignée 32. La biellette 30 présente une forme courbe similaire à celle du manche 21. Ce dernier comporte un évidement 33 (figure 10) logeant, au moins en partie, la biellette 30. 25 La poignée 32 comporte une zone de préhension courbe et est montée pivotante sur le manche 21, autour d'un axe 34. La poignée 23 est en outre reliée à la seconde extrémité de la biellette 30, autour d'un axe 35, décalé de l'axe 34. L'actionnement de la poignée 32 provoque donc le déplacement de la biellette 30 et, ainsi, le déplacement du plot tronconique 28 30 entre sa position rentrée (figures 8 et 13) et sa position sortie (figures 9, 10 à 12, 14 et 15). Les figures 13 à 15 illustrent le montage et le démontage de la cupule 1 sur l'ancillaire 20, par l'intermédiaire de l'élément de préhension 15. Lors d'un tel montage, en position initiale, le plot tronconique 28 est 35 en position sortie et l'élément de préhension 15 est monté sur le plot tronconique 28. Pour cela, ce dernier est inséré radialement dans l'ouverture centrale 18. L'élément de préhension 15 est ensuite inséré dans la cupule 1, jusqu'à ce que l'épaulement 17 soit en butée contre le bord libre 8 de la cupule 1. La poignée 32 est ensuite actionnée, de manière à ce que le plot tronconique 28 soit rentré et appuie contre la paroi interne de l'ouverture centrale tronconique 18, afin de contraindre et déformer l'élément de préhension 15.
