OUTILLAGE POUR LA MISE EN PLACE D'UNE CUPULE D'UNE PROTHESE DE HANCHE ATOOLING FOR THE PLACEMENT OF A CUP OF A HIP PROTHESIS
DOUBLE MOBILITE ET CUPULE D'UNE TELLE PROTHESE DOUBLE MOBILITY AND CUPULE OF SUCH A PROSTHESIS
The present invention relates both to a tool for setting up, replacing or removing the cup of a dual mobility acetabular prosthesis, and such a cup. A tool is known for placing a cup of an acetabular prosthesis, of the type comprising a gripper which is adapted to anchor to the cup and which comprises a central member extending in an axial direction and three anchoring fingers arranged in a star with respect to the central member. Such tooling is known from the patent EP 931 523 in which the three anchoring fingers are articulated, each of these fingers having a tab which acts as a hook and which is pivotally mounted along a pivot axis perpendicular to the axis of rotation. the central member, between a locking position in which it is adapted to engage in a housing made in the cup, and a release position. However, this attachment system does not allow easy manipulation of the cup. The present invention aims to achieve a new tool allows easy manipulation of the cup and a possible repositioning of the latter. According to the invention, in the tooling of the aforementioned type, on the one hand, at least one of the fingers is articulated and comprises a tab which acts as a hook and which is pivotally mounted along a pivot axis perpendicular to the axis of the central member, between a locking position in which it is adapted to engage in a housing made in the cup, and a release position, and, secondly, at least two fingers are fixed 1 and are adapted to anchor to the cup according to a bayonet attachment by rotation about the axis of the central member. According to another aspect of the invention, the dual mobility prosthesis cup is formed, on the one hand, by a hemispherical shell whose inner surface is adapted to receive in pivotal connection the insert of the prosthesis, and whose outer surface has at the equator protuberances for fixation in the bone by wedging as well as housings for the reception of fixed fingers of a gripper by rotation along the axis of the hull and their fixing by bayonet, and on the other hand, by an equatorial flange extending the shell, extending over an angular sector less than 180, and whose outer surface comprises at least one housing adapted to receive a leg of an articulated finger of the gripper by the pivoting of the tab along an axis perpendicular to the axis of the hull. This combination of anchoring means as well on the gripper as on the cup makes it possible to securely and easily anchor these two parts together, not only for the fitting of the prosthesis, but also for its displacement. or its withdrawal. In addition, the presence of an equatorial collar having an angular amplitude of less than 180 makes it possible to minimize the risk of luxation of the hip due to the absence of a collar extending over the entire periphery of the hull. likely to cause a conflict between the femoral stem and the cup, while facilitating the attachment with the tooling. Other features and advantages of the present invention will appear in the description of an embodiment given by way of non-limiting example and illustrated by the attached drawings, in which: FIG. 1 is a view of a cup according to one embodiment, towards the cavity; Figure 2 is a sectional view of the cup according to the line II-II of Figure 1; Figure 3 is a perspective view of the cup according to the embodiment of the invention; Figure 4 is a sectional diagram similar to Figure 2, illustrating the hip connection once the acetabulum and femur prostheses are in place; Figure 5 is a side perspective view of a gripper according to one embodiment, anchored to the cup illustrated in Figure 3; Fig. 6 is a perspective view of the gripper of Fig. 5; Figure 7 is an axial sectional view of the gripper made in a plane passing through a fixed anchoring must; FIG. 8 is a view similar to FIG. 7, made in a plane passing through an articulated finger, here in the locking position; Figure 9 is a view similar to Figure 8, the hinged finger being in the release position; Figure 10 is a perspective view of an impactor-sleeve adapted to cooperate with the gripper illustrated in Figures 5 to 9; Figure 11 is a perspective view of an impaction rod adapted to cooperate with the gripper illustrated in Figures 5 to 9; Figures 12 to 14 illustrate the method of anchoring the gripper to the cup; and FIG. 15 is a perspective view of a tool in use for securing the cup to the bone acetabulum. As can be seen in FIG. 4, a dual mobility acetabular prosthesis comprises a cup 1 which is adapted to be attached to a bony acetabulum 2, and an insert 3 which is rotatably mounted in the hemispherical cavity 4 of the cupule 1 and which is adapted to receive in its internal cavity, a humeral head 5.
As can be seen in Figures 1 to 3, the cup 1 is formed of a shell 6 of hemispherical shape, and an equatorial flange 7 extending the shell 6. The shell 6 has an inner face 8 which defines the hemispherical cavity 4 and is, therefore, particularly smooth, and an outer face 9 which has, at the equator, protuberances 10 for fixing the cup 1 in the bone 2 by jamming without using additional attachment organs, such only screws. The collar 7, the presence of which allows easy handling of the cup 1, also comprises an inner face 11, and an outer face 12. The fact that it extends over an angular sector of less than 180 allows, by arranging correctly cup 1 in the bone, increase the range of motion of the femoral stem and consequently reduce the risk of dislocation. In addition, the progressive shape of this flange 7 makes it possible to increase all the amplitude of movement of the femoral stem. As can be seen in Figures 3, 5 and 12 to 14, the cup 1 comprises on its outer face 9,12, two types of anchoring housings 13,14. Each anchoring housing 13 of the first type (here, two housings) is made in the thickness of the shell 6 and is accessible from its outer face 9. Each of these 4 housings 13 is shaped so as to receive a complementary member fixed attachment 15 of a gripper 16 by rotation of the latter along the axis of the shell 6 and to allow the attachment of the gripper 16 according to a bayonet system. For this purpose, each housing 13 is L-shaped, with two branches, a first opening to the free edge 17 of the shell 6 thus permitting the penetration of the fixed complementary fixing member 15 in the housing by translation along the axis of the shell 6, and a second, extending perpendicularly with respect to the first and at a distance from the free edge 17 allowing the penetration of the fixed complementary fixing member 15 pressed to the bottom of the first branch by rotation around the axis of the hull 6. Once this rotation is completed, the fixing by bayonet is performed, and it is no longer possible to separate the gripper 16 by a translation along the axis of the hull 6. This type of anchor has advantage of not requiring an expansion of the gripper 16 during the anchoring operation and therefore considerably limits the conflicts with the bone which would result in a limitation of the mobility of the cup 1 during its gold phase and / or discomfort (or even an impediment) in separating the gripper once the cup is in place. Each anchoring housing 14 of the second type (here, a housing) is made in the thickness of the collar 7 and is accessible from its outer face 12. Each of these housings 14 is shaped so as to receive a complementary member of articulated attachment 18 of the gripper 16 by pivoting a portion of this articulated member 18 along an axis perpendicular to the axis of the shell 6 and to allow the attachment of the gripper 16 5 according to a hook system. For this purpose, each housing 14 is at a distance from the free edge 19 of the flange 7 for the imprisonment of the articulated member 18 when, following its pivoting, it goes into its locking configuration. Once the pivoting is completed, the fixing by hook is carried out, and it is no longer possible to release the complementary fixed fixing members 15 of the housings 13 of the first type by a rotation along the axis of the shell 6. This second type Anchoring thus makes it possible to manipulate the cup in all possible orientations. The fact that this housing 14 of the second type is made out of the shell 6 makes non-confrontational expansion of the gripper 16 during the passage of the articulated attachment complementary member 18 in its release configuration. Thus, this cup 1, with this collar 7 angularly limited, and these two types of housing 13,14 according to their arrangement either in the shell 6 or in the collar 7, allows particularly easy handling of the cup 1 while limiting the dislocation risks inherent in cups with comparable handling facilities. Figures 6 to 9 illustrate a gripper 16 particularly suitable for handling such a cup 1. The gripper 16 which is adapted to be anchored to the cup 1, comprises a main body 20 through which the anchoring to the cup 1 is realized, and a transmission system 21 which is arranged inside the central member and which allows to control the locking movement of the complementary articulated attachment member 18. The main body 20 comprises a central member 22 which extends in an axial direction (here of generally cylindrical shape, of axial symmetry) in which is housed the transmission system 21, and anchoring members 15, 18 which allow the anchoring to the cup 1 and which are formed by at least three anchoring fingers 15,18 (here, three fingers 15,18) arranged in a star with respect to the central member 22. This provision allows to preserve the visual cues which are on the periphery and the bottom of the cup 1 and which help the user for its implantation. The anchoring members comprise at least two fixed fingers (here, two fingers) which are adapted to anchor to the cup 1 according to a bayonet attachment by rotation about the axis of the central member 22. These fingers fixed 15 are the fixed complementary fixing members 15 which are adapted to be housed in the anchoring housings 13 of the first type. Each fixed finger 15 comprises a tab 23 which extends parallel to the axis of the central member 22 and in the thickness of which is formed a cavity 24. The latter is made at a distance from the free end 25 of the tab 23 and it opens to the inner surface of the tab. Each cavity 24 is thus delimited by a bottom wall (limiting the cavity in the thickness of the lug 23) and, perpendicularly to this bottom wall, by a lower wall (separating the cavity 24 from the free end 25 of the tab 23), an upper wall (opposite to the lower wall), and a stop wall (perpendicular to the lower and upper walls). The cavity 24 also comprises an access opening which is disposed opposite the abutment wall and allows cooperation with the anchoring housing 13 of the cup 1. 7 The anchoring members also comprise at least one finger articulated 18 (here, a finger) which is adapted to anchor to the cup 1 according to a hook attachment by pivoting about a pivot axis 26 perpendicular to the axis of the central member 22. This articulated finger 18 is the articulated attachment complementary member 18 which is adapted to be housed in the anchoring housing 14 of the second type. The articulated finger 18 comprises a base 27 which is fixed and has the shape of a fork defining a housing, and a tab 28 which acts as a hook and which is pivotally mounted along the axis of pivoting 26 in the housing defined by the The base 28 is movable between a locking position (see FIG. 8) in which it is adapted to engage in the anchorage housing 14 of the second type, and a release position (see FIG. ) in which the tab 28 can slide along the cup 1 when the gripper 16 is approached or remote and when it is rotated to achieve bayonet fixation. The tab 28 extends parallel to the axis of the central member 22 and comprises, at its free end 29, a rim 30 projecting radially inwardly which is adapted to engage in the anchoring housing 14. The base 27 of the articulated finger 18 and the fixed fingers 25 are integral with the central member 22. As indicated for the cup, the two types of fingers with their separate attachment mechanism (bayonet and hook) allow easy handling of cup 1, without interference with the bone. The conformation of the legs 23,28 allows manipulation of the cup both in support and withdrawal. In addition, the dimensions of the flange 30 relative to the corresponding housing 14, that, once the tab 28 of the articulated finger 18 in the locking position 8, any rotation of the gripper 16 relative to the cup 1, in the sense of an output of the legs 23 of the fixed fingers 15 out of their respective housing 13 is impossible.
As illustrated in FIGS. 7 to 9, the transmission system 21 is formed by a threaded transmission shaft 21 which extends along the axis of the central member 22. This transmission shaft 21 cooperates with a threaded orifice made in central member 22, and comprises, on the one hand, connecting members 31 which are adapted to cooperate with an impaction rod 32 by which the user controls the movement of the tab 28 of the articulated finger 18, and, d on the other hand, control members 33 adapted to cooperate, by their translational movement along the axis of the central member 22, with the tab 28 of the articulated finger 18. The control members 33 are formed by a surface The cam surface 33 defines, along this axis, a cavity pivotally receiving an inner head 34 of the articulated finger. The connecting members 31 are formed by a rod of prismatic shape which acts as a male member and which is adapted to be received by a corresponding female member 35 of the impaction rod 32. In order to allow the rotation of the shaft of transmission 21 relative to the main body 20, the central member 22 comprises, at the end of which the connecting members 31 of the transmission shaft 21 project, connecting members 36 which are adapted to cooperate with a handle -impactor 37 by which the user disengages the movement of the two parts 20,21. The connecting members 36 are formed by two lugs 36 which are radially outwardly projecting and diametrically opposed to one another and which act as a male member adapted to be received by a corresponding female member 38 of the handle. impactor 37.
The tooling thus comprises the gripper 16, the impactor sleeve 37 and the impingement rod 32. The impactor handle 37 and the impingement rod 32 extend in a longitudinal direction and comprise, at their first longitudinal end, means 39,40 allowing the user to manipulate them, and at their second longitudinal end (opposite to the first), the female members 35,38 which allow the control of the hinged finger 18. The female members 35 of the shank impaction 32 are formed by an orifice 35 whose cross section corresponds to that of the connecting members 31 of the transmission shaft 21. The female members 38 of the impactor sleeve 37 are formed by two diametrically opposed notches 38 which are adapted to receive the two lugs 36 and which are made in a cylindrical ring adapted to surround the central member 22. In addition, to allow a secure attachment of the impinger handle 37 to prehen However, these indentations are extended by slots 41 which allow cooperation with the lugs 36 in accordance with a bayonet mechanism. Moreover, the impactor sleeve 37 comprises a longitudinal opening opening at its two longitudinal ends and in which the impaction rod 32 is arranged.
La mise en place de la cupule 1 se fait classiquement, sa fixation se faisant par impaction (cf. figure 15). 11 The use of the tooling according to the invention is particularly simple: in a first step, the impactor sleeve 37 is secured to the gripper 16 by introducing the two lugs 36 to the bottom of the two notches 38 and then imposing a rotation to the handle so that these lugs enter the slots 41. Then the impingement rod 32 is inserted into the impactor sleeve 37 until its hole 35 mates with the rod 31 of the transmission shaft 21 After having placed articulated finger 18 in its release position, the fixed fingers 15 are introduced to the bottom of the first branch of the anchor housing of the first type 13 (see Figure 12). Then, the tool is rotated relative to the cup 1 so that the free end 25 of the fixed tabs 23 (located at the right of the cavity 24) is housed in the second branch of the anchor housing of the first type 13 , and that the free end 17 of the cup 11 (located at the right of the second branch) is housed in the cavity 24 of the tab 23 (see Figure 13). Once this bayonet anchorage is achieved, the impingement rod 32 is rotated relative to the gripper handle 37 so as to cause the articulated finger from its release position to its locking position (see Figure 14). The setting up of the cup 1 is done conventionally, its fixation being done by impaction (cf figure 15). 11
