Abstract
Description
PIECE APTE A ETRE IMPLANTEE A UNE EXTREMITE D'UN OS POUR LA RESTAURATION D'UNE ARTICULATION PIECE THAT CAN BE IMPLANTED AT THE END OF A BONE FOR THE RESTORATION OF AN ARTICULATION
The present invention relates to the parts capable of being implanted at one end of a bone for the restoration of a joint, that this part is sufficient in itself or in combination with another such as for example in the case of a prosthesis joint type ball joint in two parts, a male part and a female part able to cooperate both for example sliding on one another.
The problem that arises in the case of the implantation of such a piece is that of its holding at least until the phenomenon of osteosynthesis has had its effect to permanently maintain, by integration, the piece with the bone. The object of the present invention is to provide such a part which satisfies the aims defined above more satisfactorily than similar parts known from the prior art. More specifically, the present invention relates to a piece adapted to be implanted at one end of a bone for the restoration of a joint, characterized in that it comprises: • a plate defined between two first and second opposite faces , the first face being substantially planar so as to be able to come into contact with a first flat resected surface portion on the end of said bone, a first portion of the peripheral lateral surface of the plate connecting the two said first and second opposite faces being substantially flat and forming, with the first face, a first angle of a first value of less than ninety degrees, this first lateral surface portion of the plate being adapted to come into contact with a second portion of resected surface plane on the end of said bone, said second resected surface portion making, with the first resected surface portion, an angle a value substantially equal to said first value. Other features and advantages of the present invention will become apparent from the following description given with reference to the accompanying drawings for illustrative but not limiting purposes, in which: FIG. 1 is a cross-sectional view of a part according to the invention adapted to be implanted at one end of a bone for the restoration of a joint; and FIG. 2 is a view from above of the part shown in FIG. 1, the section according to FIG. 1 being defined in the plane referenced II in FIG. 2. It is firstly specified that, in the figures, the FIGS. The same references designate the same elements, whatever the figure on which they appear and regardless of the form of representation of these elements.
Similarly, if elements are not specifically referenced in one of the figures, their references can be easily found by referring to the other figure. It is also stated that the figures essentially represent a single embodiment of the object according to the invention, but that there may be other embodiments that meet the definition of this invention. It is furthermore specified that, when, according to the definition of the invention, the object of the invention comprises "at least one" element having a given function, the embodiment described may comprise several of these elements. Conversely, if the embodiment of the object according to the invention as illustrated has several elements of identical function and if, in the description, it is not specified that the object according to this invention must obligatorily comprise a number particular of these elements, the object of the invention may be defined as comprising "at least one" of these elements. Lastly, it should be noted that when, in the present description, an expression defines, by itself, without a specific particular mention concerning it, a set of structural characteristics, these characteristics may be taken, for the definition of the object of the protection requested, when technically possible, either separately, or in full and / or partial combination. With reference to FIGS. 1 and 2, the part according to the invention capable of being implanted at one end of a bone 10 for the restoration of a joint essentially comprises a plate 20 defined between two first and second opposite faces 21, 22, the first face 21 being substantially planar, or equivalent, to be able to come into contact with a first flat resected surface portion 11 on the end of the bone 10, a first portion 25 of the peripheral lateral surface 24 of the plateau connecting the two first and second opposite faces 21, 22 being substantially flat and forming, with the first face 21, a first angle of a first value a, Figure 1, less than ninety degrees, for example seventy-five degrees, this first side surface part 25 of the plate 20 being adapted to come into contact with a second flat resected surface portion 12 on the end of the bone 10, this second resected surface portion 12 making, with the first resected surface portion 11, an angle of a value substantially equal to the first value a. It is pointed out that, although defined as flat, this first face 21 may optionally be curved and / or comprise for example protruding parts such as sawtooth or the like, etc., and that it is its general appearance which must therefore be taken as flat. When the piece is positioned on the first flat resected surface portion 11 of the bone 10, it is therefore already at least partially held by a wedge effect, this wedge being formed between the first and second portions of flat resected surfaces 11 12. However, to complete this maintenance, it is very advantageous that the piece further comprises an anchor blade 30 made of one of the following materials: biocompatible stainless metal, bio-integrable stainless biocompatible metal, biodegradable material, and connecting means 31 for linking this anchor lamella with the first face 21. These connecting means are arranged so that the anchoring lamella 30 forms, with this first face 21, a second non-zero angle such as this lamella is able to plunge into the portion of bone which is limited by the first flat resected surface portion 11 of the bone 10 when the plate 20 rests on this first portion of surface 11.
In addition, for the part according to the invention can maintain itself on the end of the bone 10, it is very advantageous that this second angle, defined between the blade 30 and the first face portion 21 between the strip 30 and the first part of the lateral surface 25, has a value f3 greater than the first value cc and of course less than one hundred and eighty degrees. In this way, the lamella 30 is perfectly maintained by a second wedge effect by the portion of bone 120 located between the lamella 30 and the portion 121 of the first face 21 of the plate 20.
Indeed, with this value p. of this second angle the two planes respectively of the first side surface portion 25 and the strip 30 are not parallel and, with the orientation of the strip 30, the plate 20 is held and can not lift, which could happen if the value f3 of the second angle was equal to or less than the value a of the first angle.
With such a structure, the part according to the invention can be implanted only by a translation which, with reference to FIG. 1, is perpendicular to the plane of this FIG. 1 or anatomically perpendicular to the longitudinal axis 15 of the bone. 10. Consequently, to prevent the implantation of the piece exceeds a limit and for this implantation to become more and more resistant, by wedge effect, as the lamella penetrates into the bone part, it is very advantageous that, as illustrated in the figures, the blade 30 forms, with the edge 125 defined between the first lateral surface portion and the first face 21, a third angle of a non-zero value, FIG. 2, for example from fifteen to twenty degrees. In addition, to facilitate the operative work of the Practitioner, it is very advantageous that the connecting means 31 are constituted by separable elements, so as to be able to adapt the lamella to the type of part to be implanted, especially as to its size, the nature of the material constituting it, etc. These dissociable elements consist, for example, of two male-female parts of a slide or the like, these two male-female slide parts being respectively formed on the lamella 30 and in the plate 10, as schematically illustrated in Figure 1. Such a piece is particularly suitable, but not exclusively, to form a tibial plateau for a uni-compartmental prosthesis of the knee. In this case, the plate 20 has a generally half-moon shape 27, FIG. 2, the edge 26 of the curved part of the half-moon which corresponds to the anterior part of the plate when it is implanted on the proximal end. a tibia having a cutaway 28 to allow the tray to not interfere with the meniscus and prevent it from being damaged by friction on a sharp edge. In this case of application to a tibial plateau, the second face 22 advantageously has a concave shape to be able to receive a convex condylar piece 23 which may be artificial or, in some cases, the original condyle of the end of the femur.
In addition, advantageously, the plate 20 may comprise at least one gripping orifice 29 to be able to extract it after it has been implanted on the end of the bone 10. In FIGS. 1 and 2, only one orifice has been illustrated but it is obvious that there may be several, in different places with axes forming between them zero or non-zero angles, and which can lead to the second face 22 and / or the peripheral side surface 24 of the plateau. It may be provided to deposit, on the plate 20 and / or on the lamella 30, a layer of a bio-integrable product, such as calcium hydroxyapatite, able to come into contact with the bone.
