Description

The invention relates to a screw for holding two bone fragments. To maintain temporarily two bone fragments from a fracture or osteotomy the time of bone osteogenesis, it is known to maintain both fragments by means of a screw. Conventionally the screw comprises a head and a threaded rod. The head may be flat or conical. For this type of screw, tightening is obtained when the head is in abutment and the screw has made several turns. The surgeon must pay attention to the tightening torque as this may damage the bone. There are known improved screws having at the head a thread with a constant pitch N and in the distal portion another thread with a constant pitch P, the pitch P being different from the pitch N. When the pitch N is less than the pitch P there is a compression effect. There too, the head can be conical. Between the screw head and the distal end, the screw shaft may be devoid of screw thread. The diameter of the screw at this median portion corresponds to the diameter of the drilling performed before installation of the screw. When the head is conical, it sometimes laments a burst of cortical when the screw head comes into contact with the cortex. This results from a crushing of the thread of the tapping made by the passage of the distal end and the need to fDrer a cavity during screwing to fit the conical shape. This poses a problem. The invention proposes to provide a solution. To this end, the invention relates to an osteosynthesis screw comprising a threaded head, a threaded distal end and a cylindrical smooth body connecting the threaded head and the threaded distal end, this screw being characterized in that the head is consisting of two parts juxtaposed, a first conical portion whose diameter decreases while progressing towards the distal end, this prolonging by a second cylindrical portion of constant radius R, the whole of the head having a screw thread or a succession of threads quote. The invention will be better understood from the following description given by way of nonlimiting example with reference to the drawing which shows schematically: FIG. 1: A screw according to the invention

FIG. 2: an axial section along AA of FIG. 1 FIG. 3: An axial section of a variant of a screw Referring to the drawing, it can be seen that to hold two bone fragments A, B resting against one another other, it uses an osteosynthesis screw 1.

Before the establishment of this screw, a drilling is performed. This piercing completely crosses one of the fragments and then enters the other fragment. The screw 1 exerts a force tending to bring the two fragments together. This screw is particularly useful for surgery of the bones of the hand and foot but may be suitable for other applications.

The screw 1 consists of an elongate body comprising at its end so-called proximal a head 2 with an impression 3 to engage a tool. The head 2 has a screw thread. The distal end 4 of the screw also has a screw thread 5. Between the two threaded ends extends a smooth portion 6 whose diameter is substantially that of the diameter of the drilling. It is well understood that the diameter of the threaded ends is greater than the diameter of the bore. According to the invention, the head 2 consists of two juxtaposed portions 2A, 2B, a first conical portion 2A whose diameter decreases progressing towards the distal end4, this prolonging by a second cylindrical portion 2B of constant radius R, head assembly having a screw thread or a succession of screw threads. The screw thread is either continuous at constant pitch or continuous but variable pitch. This variable step is obtained by juxtaposition of net with different pitch. This net can possibly be interrupted. The threads will, in one embodiment, be different. To exert a compressive force, the average pitch of the thread presented by the head will be smaller than the average pitch of the net in the distal portion. The body of the screw may have a channel 10 passing through said body along its longitudinal axis. The screw may be carried by a tool and connected to this tool by a link 11 breakable. It is easy to understand the advantage of this threaded cylindrical portion 2B preceding the conical portion.

Indeed, during tightening, when the screw head will reach the cortex of the first bone fragment, the second portion 2B of the screw head will mesh with the tapping performed beforehand by the passage of the distal end 4 which will center the screw head and thus avoid the bursting of the bone. This interaction of the second part with the tapped hole will not change the tapping when the screw is engaged. Due to the continuity between the first 2A and second part 2B of the head, there is a constant progression of the screw thread in the tapping which limits the risk of bursting of the bone.
