Abstract

The equipment has a thrust piston (a) animated by a spring. A limited rate cannula (c) compensates thrust speed for permitting retraction of a chemotherapy needle. The cannula has an end combined with an interior inversion of a conical section (b) of a passive element of the syringe for permitting a user to avoid the purge of the equipment during its operation. An interior volume of the syringe is slightly greater than aspirated volume during the retraction of the needle by a catheter. The end of the cannula serves as stop for descending and closing the piston at the end of thrust.

Description

On connaît depuis quelques temps déjà les problèmes que posent le retraitFor some time now the problems of withdrawal have been known

In fact, the volume of the needle inside the chamber disappears and is compensated by a suction phenomenon at the end portion of the catheter connected to it; this volume is still artificially increased because the Huber bevel needle, which is an angled needle, has a tendency, by its shape, to cause deformation of the membrane of the implanted chamber during its removal. These two added values are therefore compensated by a suction at the end of the catheter connected to it (Figure 11). The liquid thus absorbed, in fact blood, has a tendency to coagulate with the catheter. inside the catheter during the time or it, still in place is not used for another infusion.

This coagulation can cause partial or total closure of the catheter which is very detrimental to the patient.

This is why we thought of a device allowing the final rinsing during the removal of the needle and not just before this maneuver as do some currently used equipment. The goal is to compensate for the total volume aspirated, see continue somewhat just after the removal of the needle, to be sure that nothing has been sucked during the period of time the withdrawal maneuver. this we propose to protect the following realization: This is a material with an effective volume at least comparable or even slightly higher than that of the total aspirated volume detailed above which would be composed of a diameter cylinder and a useful volume at least necessary to obtain the expected result supplemented by a piston adapted to seal the device. a mini syringe somehow. {Figure 2} This piston would be provided with at least one sighting device visible from the outside when the filling assembly reached its maximum volume. {a 40 Figure 2} It will therefore be a material such as a mini syringe, whose usable internal volume will be slightly greater than the volume generally aspirated, during the withdrawal of the needle, by the implanted catheter connected to a chamber of chemotherapy

The emptying of the device would be obtained by a device such as a spring, {b Figure 2} placed inside the device, and whose longitudinal thrust, once compressed, would be calculated to allow the movement of the piston autonomously. while expelling the contained liquid However, the time required for the practitioner to perform the withdrawal maneuver of the needle, is generally quantified by a time of several seconds.

Also, so that this emptying of the device is not uncontrolled, the regulation of the entire maneuver will be controlled by a cannula of a suitable diameter which, depending on the thrust of the spring of the piston, thus allow flow in the catheter at least the amount of liquid required during at least the estimated time of the maneuvering It is therefore a ratio: return speed of the piston, therefore thrust of the liquid, to the catheter, and flow, controlled by the internal diameter of the cannula of the device, which will provide a drain of the mini syringe for at least the entire time necessary to remove the needle. And it is therefore through this controlled emptying of the mini syringe, connected to the catheter connected to the Huber needle, that the purging, without final aspiration, of the catheter connected to the implantable chamber will be obtained. Thus, the technical characteristics of the piston thrust device will be closely related to the flow of the cannula connected to the cylinder so as to allow the user to have the necessary time to perform the complete movements to be performed for the withdrawal of the needle. This also means that the inside diameter of the cannula will be calculated as a function of the pressure exerted by the piston thrust device to allow the nurse to have the time to perform the complete movements to be performed for the withdrawal. of the chemotherapy needle without terminal aspiration.

On the other hand, this purge material, hereinafter referred to as: positive pressure, may be connected either to a 3-way valve or to another manual device, or even automatic which will allow: filling it in a first, time, sound inert state during the normally performed rinsing time of the catheter, its actual final purge state, during the time spent in removing the needle. However, the various gestures practiced by the operator can not, at the same time be supplemented by an air purge of the equipment.

This is why it seemed useful to provide a complementary device 40 to ensure the purge compensation phenomenon taking into account the dead volume of the syringe at the time of filling. As previously seen, the device will be filled by means of an accessory such as the pre-filled pre-purge syringe, called the large syringe, because of its much larger volume, which will cause a small part of its liquid to be transferred to the positive pressure device, called the mini syringe, by connecting them to each other by a conventional intermediate piece and their usual exit cone called Luer cone {d Figure 2} And so it is the transfer of liquid that will push the plunger of the positive pressure device back to the previously cited mark that will appear on the tail of the plunger when it is filled (Figure 2).

If desired, the nurse, or other caregivers, may wish to delay the onset of this positive pressure immediately after filling the mini-syringe.

This is why it has appeared interesting to provide a device for holding the piston in the position -depositive filling only trigger only at the desired moment by it.

Ce pourrait être, par exemple, une combinaison d'une position légèrement excentrée de l'orifice de guidage de la queue du piston {a Figure 3} associée à une encoche de verrouillage pratiquée dans la queue du piston. {a Figure 4} 15 Lors du glissement de la queue du piston pendant le remplissage, celui-ci se trouvera donc en présence d'une force latérale, et au passage de l'encoche devant l'épaisseur du couvercle de la mini seringue, il reprendra donc sa place centrée et les deux éléments coopérant naturellement, il ne pourra plus alors redescendre sans 20 intervention manuelle de latéralisation de la queue du piston par l'infirmière. { Figure5 } .  This could be, for example, a combination of a slightly off-center position of the piston tail guide hole (Fig. 3) associated with a locking notch in the piston shank. {a Figure 4} During the sliding of the tail of the piston during filling, it will therefore be in the presence of a lateral force, and the passage of the notch in front of the thickness of the lid of the mini syringe, it will resume its place centrally and the two elements cooperating naturally, it can not then down without manual intervention of lateralization of the tail of the piston by the nurse. {Figure5}

This mini-syringe would therefore have an eccentricity of the piston guide orifice so that the piston tail tends to naturally refocus and lock by cooperation during the passage of the notch at the lid of the piston. mini syringe, once it filled because it has a notch in the tail of the piston allowing the latter to ensure the locking so its holding in position once the device filled.

As from this maneuver, it is no longer possible to purge the positive pressure device except to repeat the maneuver several times by tilting it up and down several times until it completely captures the bubble that will be formed at the beginning. It seemed interesting to us to find a way to avoid this maneuver, which consumes precious time for the manipulator. This is why we propose on the one hand to invert the conicity normally practiced {b Figure 4} inside the body of the syringes, so that, if the mini syringe is verticalized, the bubble is automatically placed circumferentially, and on the other hand, to extend the cannula within the positive pressure device (c) so that the volume of the initially formed bubble can not reach the internal end of the cannula regardless of the vertical position, in one direction or the other, or horizontal or any other intermediate angles may bring the air bubble in said injectable position in the transfer tube to the needle of huber implanted in the room. 45 During manufacturing, it is therefore necessary to observe the dead volume filled with air in the positive pressure device and to position the transfer cannula so that the end thereof is higher than the upper level. reached by the bubble inside the device. in the most unfavorable position. {d Figure 4} Which is to say that the positioning of the inner end of the cannula will be determined according to the volume of the bubble remaining in the body of the mini syringe regardless of the position of the latter.

On the other hand the conicity of the interior of the passive element of the syringe is reversed compared to the syringes usually used.

20 25 30 35 40 45  As a result, the end of the canula will serve as a stop for the descent of the piston and completely close at the end of the thrust to prevent the residual air accidentally enters the tubing connected to the chemotherapy needle. body of the mini syringe will have a standard end identical to the ends of other syringes currently in use. so that it can connect tips, fittings, or faucets currently in use. 20 25 30 35 40 45
