Abstract

The brace has a sleeve (1) constituted of a trapezoid piece comprising two lateral sides (4) extending from a large base (3) perpendicular to the lateral sides. The lateral sides are connected by an elastic band (5), and oblique sides (7) extend from the lateral sides to a small base (6). One of the oblique sides comprises a hooking band (8) at proximity of the small base, where the hooking band extends perpendicular to the oblique side. An integrating unit (9) of the oblique side at free end is cooperated with a complementary integrating unit of an external surface of the sleeve.

Description

The present invention relates to the field of orthopedics and more

  particularly that of thumb orthoses for the treatment of pathologies related to the trapezio-metacarpal joint. Pain in the thumb is common and can be disabling at times, with the thumb being needed to grip. These pains are usually either post-traumatic, such as a trapezometacarpal sprain called thumb sprain or due to a degenerative pathology such as rhizarthrosis, tendonopathy De Quervain or similar. In order to relieve these pains, it is well known to use a thumb orthosis to put the thumb column at rest. The thumb orthosis makes it possible to place the thumb in anteposition in which the first commissure is open and the base of the first metacarpal is centered on the trapezium. This is the case, for example, of the French patent application FR 2 822 371 which describes an orthosis of thumb-wrist. This thumb-wrist orthosis consists of a sleeve that can be threaded onto the hand and wrist of a patient. Said sleeve comprises a first means, placed between the thumb and the indexe, in the first commissure, made of a material conforming to the shape of said commissure and adapted to allow on the one hand the closure of said commissure at each urging and of on the other hand the automatic opening of said commissure. The sleeve also comprises firstly a second means placed at the trapezio-metacarpal joint and which consists of a clamping device adapted to exert a recentering force of the bottom of the first metacarpal and thirdly a third means placed on the dorsal surface of the thumb and consisting of a sleeve for receiving a splint, elastic adapted to increase the spreading effect of the material of the first means placed in the first commissure. This type of thumb orthosis has the disadvantage of being made in two parts connected by two seams, a first seam located in the median plane of the first commissure and a second seam located on the outer lateral face of the thumb, which strike the cost of manufacturing this orthosis.

  Moreover, this type of orthosis does not ensure sufficient immobilization of the thumb.

  Finally, given the asymmetry of the hand, the length of the metacarpus being decreasing from outside to inside, this type of orthosis can be adapted to the right hand or to the left hand of a patient. Thus, it is necessary to provide a large number of models according to the size of the hands but also according to the right hand or the left hand, resulting in high costs of manufacturing and inventory management. One of the aims of the invention is therefore to remedy all these drawbacks by proposing a thumb orthosis, inexpensive, advantageously ambidextrous and allowing a quick and easy implementation.

  For this purpose and in accordance with the invention, it is proposed an orthosis of thumb consisting of a sleeve obtained in a flexible material comprising on the one hand a distal opening and a proximal opening for the passage of the fingers of the hand and of on the other hand a sheath adapted to receive at least one splint, remarkable in that said sleeve consists of a generally trapezoidal part comprising a large base, two lateral sides extending from the large base generally perpendicular thereto, said lateral sides being connected by an elastic band, a small base and two oblique sides extending from said lateral sides to the small base, at least one of the oblique sides having close to the small base an attachment band s' extending substantially perpendicular to the oblique edge and provided at its free end with securing means adapted to cooperate with securing means c complementary to the outer face of the sleeve. Said sleeve comprises a strap of contention, on the external face of the sleeve, constituted on the one hand by a compression band integral with one of the lateral sides, and on the other hand by a loop able to receive said strip of contention. Advantageously, the compression band has on its outer face loops adapted to cooperate with hooks positioned on the inner face of said strip at its free end. Incidentally, the orthosis according to the invention comprises a flexible thumb immobilization strip capable of surrounding the thumb of the patient so that said immobilization strip follows the short abductor thumb muscle, the ends of said strip being secured. to sleeve attachment means positioned on either side of the median plane M of said sleeve along the oblique sides. Said immobilization strip consists of a longitudinally elastically deformable strip.

  Furthermore, the fixing means positioned on either side of the median plane of the sleeve consist of loops adapted to cooperate with hooks positioned at the free ends of said immobilization strip. In addition, the orthosis according to the invention comprises a sleeve extending on the outer face of the sleeve from the middle portion of the small base of the sleeve to the large base of said sleeve, said sleeve extending at a determined distance of the median plane of the sleeve at the level of the large base. Particularly advantageously, the orthosis also comprises a second sleeve extending on the outer face of the sleeve from the middle portion of the small base of the sleeve to the large base of said sleeve, said second sleeve extending at a distance determined from the median plane of the sleeve at the large base so that the sleeves extend symmetrically on either side of the median plane of said sleeve. Said splint consists of a rigid or semi-rigid straight splint. Other advantages and features will become more apparent from the following description of several variant embodiments, given by way of non-limiting examples, of the thumb orthosis according to the invention, from the attached drawings in which: FIG. 1 is a flat top view of the wrist orthosis according to the invention, FIG. 2 is a bottom plan view of the orthosis according to the invention shown in FIG. 1, FIG. 3 is a perspective view of a splint capable of being inserted into the sheath of the orthosis according to the invention; FIG. 4 is a perspective view of a flexible band of the orthosis according to the invention; Figures 5 and 6 are plan views from above and respectively from below the orthosis according to the invention placed on the hand of a patient.

  Referring to Figures 1 and 2, the thumb orthosis comprises a sleeve 1 obtained in a flexible material and consisting of a generally trapezoidal part 2 comprising a large base 3, two lateral sides 4 extending from the large base 3 globally perpendicular to the latter, said lateral sides 4 being connected by an elastic band 5, a small base 6 and two oblique sides 7 extending from said lateral sides 4 to the small base 6, at least one of the oblique sides 7 having close to the small base 6 a fastening strip 8 extending substantially perpendicularly to said oblique edge 7 and provided at its free end with securing means 9 adapted to cooperate with complementary securing means of the outer face of the sleeve 1. Said oblique sides 7 advantageously have a concavity in order to fit the curve of the patient's hand when the orthosis is put in place. The elastic band 5 connecting the two lateral sides 4 consists of a flexible elastic band longitudinally, that is to say parallel to the large base, and having a width substantially equal to the length of the lateral sides 4. The large base 3 of the sleeve 1, generally closed on itself by the elastic band 5, has a length substantially equal to the perimeter of the wrist of the patient, said patient introducing his fingers into the sleeve 1 at said large base 3.

  Furthermore, the small base 6 which is closed on itself when said sleeve is placed on the hand of a patient has a length substantially equal to the perimeter of an inch, the attachment strip 8 allowing the patient to to adapt the sleeve 1 to its morphology. It will be noted, with reference to FIGS. 5 and 6, that when the sleeve 1 is placed on the hand of a patient, the first commissure, that is to say the commissure between the thumb and the index finger, is in the open air. Such a configuration of the sleeve 1 makes it possible to improve the comfort of the wearing of the orthosis by avoiding in particular any unnecessary compression of the commissure and any irritation of the latter. Said securing means 9 of the fastening strip 8 preferably consist of a Velcro (trademark) type hook tape and the complementary securing means preferably consist of the loops of the fabric in which the outer face of the sleeve is obtained. 1.   Furthermore, said sleeve 1 comprises, on its outer face, a strap 10 consisting of a restraining band 11 integral with one of the lateral sides 4 and a suitable loop 12 to receive said compression band 11. The compression band 11 has on its outer face loops adapted to cooperate with hooks 13 Velcro type (registered trademark) positioned on the inner face of said strip 11 at its free end. Thus, when the patient's hand is too small, the elastic band 5 connecting the lateral sides 4 of the sleeve 1 provides a bellows allowing the size of the sleeve to the morphology of the patient and the immobilization of said sleeve 1 on the wrist of the patient is provided by the compression band 11 surrounding the wrist and passing through the loop, the patient adjusting the restraint according to his feeling of comfort. Note that such a sleeve can be adapted to all patient morphologies thus avoiding to provide a large number of models depending on the size of the hands of patients. It is obvious that the sleeve may comprise a plurality of compression straps or any other equivalent means without departing from the scope of the invention. In order to allow resting of the thumb column of the patient providing faster healing of tissues after trauma, the sleeve 1 comprises a sleeve 14 extending on the outer face of the sleeve 1 from the middle portion of the small base 6 from the sleeve 1 to the large base 3 of said sleeve 1, said sleeve 14 extending at a distance d from the median plane M of the sleeve 1 at the level of the large base 3. This sleeve 14 is able to receive a splint 15, removable, straight and rigid or semi-rigid, shown in Figure 3, consisting for example of a flat metal element.

  In a particularly advantageous manner, the orthosis according to the invention comprises a second sheath 16 extending on the external face of the sleeve 1 from the middle part of the small base 6 of the sleeve 1 to the large base 3 of said sleeve 1, said second sleeve 16 extending at a distance d from the median plane M of the sleeve 1 at the level of the large base 3 so that the sleeves 14 and 16 extend symmetrically on either side of the median plane M of said sleeve 1. In this way the same sleeve 1 can be indifferently used for the right hand or for the left hand, the patient placing the splint 15 in the sheath 14 or 16 corresponding so that said splint extends along the column thumb when the sleeve 1 is threaded onto the patient's hand. It goes without saying that the orthosis according to the invention could comprise two sleeves 14 and 16 extending symmetrically with respect to a plane of symmetry and integral with a sleeve 1 of different shape and structure without departing from the scope of the invention. the invention. In order to allow immobilization of the thumb of the patient's hand, with reference to FIGS. 4 to 6, the orthosis according to the invention advantageously comprises a flexible immobilization band 17 that is elastically deformable longitudinally, the free ends of said band of immobilization 17 comprising fixing means such as hooks 18 of the Velcro (registered trademark) type cooperating with complementary fastening means secured to the outer face of the sleeve 1 positioned on either side of the median plane M of said sleeve 1 along oblique sides 5 such that said immobilization band 17 follows the abductor short muscle of the thumb. In this embodiment, the complementary fastening means consist in the loops of the loops of the fabric in which the outer face of the sleeve 1 is obtained.

  It goes without saying that the complementary fastening means may consist of pieces of loop fabric secured by any appropriate means on the outer face of the sleeve 1. Moreover, it is obvious that the pieces of fabric may be substituted by a single piece extending from one edge to the other oblique sides 4 at the upper ends of the lateral sides 5 without departing from the scope of the invention.

  In addition, the immobilization means of the thumb may consist of any other equivalent means such as a piece of annular compression tissue from which two strips extend to be fixed on the sleeve so that said bands follow the muscle short abductor of the thumb.

  It will be observed that the hook fastening means of the Velcro (registered trademark) type capable of cooperating with complementary fastening means to loops may be substituted by any equivalent means of attachment capable of cooperating with complementary fastening means well known to the Man of the Occupation. Note that the sleeve 1 is advantageously made of a single piece of simple geometry with a reduced number of seam thus facilitating its manufacture. Finally, it goes without saying that the sleeve could be obtained in any type of flexible fabric, and preferably in an antiallergic tissue, and that the examples that have just been given are only particular illustrations that are in no way limiting as to the fields of application of the invention.

Claims (9)

  1 - thumb orthosis consisting of a sleeve (1) obtained in a flexible material comprising on the one hand a distal opening and a proximal opening for the passage of the fingers of the hand and on the other hand a sleeve (14) adapted to receiving at least one splice (15), characterized in that said sleeve (1) consists of a generally trapezoidal part (2) comprising a large base (3), two lateral sides (4) extending from the large base (3) generally perpendicular to the latter, said lateral sides (4) being connected by an elastic band (5), a small base (6) and two oblique sides (7) extending from said lateral sides (4) to at the small base (6), at least one of the oblique sides (7) having near the small base (6) a hooking band (8) extending substantially perpendicular to the oblique edge (7) and provided at its free end securing means (9) able to cooperate with means complementary securing of the outer face of the sleeve (1).
