Abstract
Description
L'invention concerne un dispositif orthopédique de correction de l'angleThe invention relates to an orthopedic device for correcting the angle
Some lumbar scoliosis originates from an inclination of the fourth or fifth lumbar vertebra, referenced respectively L4 and L5, creating an oblique departure from the vertebral column. This one can be evolutionary and create the formation of an overlying scoliosis. In order to quantify the importance of inclination, the iliolumbar angle is defined. This can be defined by the angle formed by the axis of the lower lumbar spine and the frontal axis of the pelvis, usually represented by the line joining the upper edge of the two iliac crests. This angle is normally of the order of 90. A closure of the angle then corresponds to an oblique departure from the vertebral column. A first type of orthopedic device for correction of the ilio-lumbar angle, intended to prevent the evolution of lumbar scoliosis, is described in the document FR 2 627 690. This device is placed in a strut between the iliac wing. and one of the last two lumbar vertebrae to allow the opening of the iliolumbar angle. This comprises a first hook-shaped piece adapted to the morphology of the patient to bear on or under a lumbar transverse process, a second piece forming a clip intended to be fixed on the iliac wing and a third piece forming a rod. correction to separate the hook and the staple and adjust their distance relative to each other to obtain the verticalization and maintenance in this position of the lumbar spine. Such a device, although having proved its effectiveness in such applications, can cause the displacement of the clip relative to the iliac wing during successive shocks, resulting in pain and decalcification at the level of the fixation of the device on the iliac wing. In addition, the rigidity of such a device makes it difficult for the surgeon to put in place, the rod being in particular not adjustable with respect to the staple. The aim of the invention is to remedy these drawbacks by proposing an ilio-lumbar angle correction device that does not cause pain or decalcification at the zone of attachment of the device on the iliac wing and that can be put in place. in place easily.
To this end, the invention relates to an orthopedic device for correcting the ilio-lumbar angle of the vertebral column of an individual comprising a first hook-shaped piece adapted to the morphology of the patient to come to bear on or under a transverse lumbar process, a second piece forming a clip intended to be fixed on the iliac wing and a third part connecting the first two and forming a correction rod to move the hook and the staple and adjust their distance with respect to the other for obtaining the verticalization and maintenance in this position of the lumbar spine, characterized in that the clip is connected to the correction rod by a ball joint, and in that damping means are furthermore arranged between staple and hook. The ball joint between the staple and the rod allows to freely orient the rod, so as to facilitate its implementation. The damping means serve to dampen the shocks or to compensate for the low deflections that may occur as a result of movements of the patient. Therefore, it avoids the appearance of high stresses or shocks in the fixing of the clip on the iliac bone. According to one characteristic of the invention, the damping means 20 are arranged at the ball joint connection. Advantageously, the ball joint means are made of elastically deformable material. According to another characteristic, the damping means are arranged at the rod. Preferably, the rod comprises, at its end connected to the hook, a zone of frustoconical shape cooperating with a housing of complementary shape formed in the hook. This feature makes it possible to limit the risks of separation of the rod and the corresponding hook by forming a fastening 30 by conical fitting of the rod on the hook. In any case, the invention will be better understood with the aid of the description which follows, with reference to the appended schematic drawing showing, by way of nonlimiting examples, several embodiments of this correction device. Figure 1 is a perspective view showing a correction device according to the invention mounted on the iliac wing; Figure 2 is an enlarged view of the zone of fitting of the rod in the hook; Figure 3 is a view showing a first embodiment of the rod; Figure 4 is a view showing a second embodiment of the rod; Figure 5 is an enlarged view in longitudinal section of the rod and the agglomerator. Figure 1 depicts an ilio-lumbar angle correction device 10 placed in a strut between the iliac wing 1 and a lumbar vertebra (not shown), as is known per se. The correction device comprises a clip 2 having a support 3 fixed to the patient's iliac wing 1 by means of screws 4. This clip further comprises a movable part 5, in ball-and-socket connection with the fixed support 3, and having a central through hole and a tubular zone 6, secured to the movable portion 5, and whose inner surface is threaded. Thus, the connection between the movable portion 5 and the threaded tubular portion 6 on the one hand and the fixed support 3 on the other hand, has three degrees of freedom of rotation. The clip is furthermore equipped with elastic damping means (not shown), arranged between the fixed support 3 and the mobile part 5. The correction device further comprises a rod 7, at least one part 8 of which is threaded. , being inserted into the central hole of the movable part 5 and cooperate, via its threaded portion 8, with the tubular threaded portion 6. The latter 6 then forms a nut so that the rotation of the rod 7 causes the translation thereof. Once connected to the staple 2, the rod 7 is then movable in three degrees of freedom of rotation, is movable in translation by screwing and may have a slight axial movement, due to the presence of the damping means. The rod further comprises complementary damping means 9, shown in FIGS. 3 and 4.
These complementary means 9 may be composed either of a U-shaped part, formed for example at mid-length of the rod 7, as shown in FIG. 3, or of an annular element forming a single piece with the rod 7 and disposed between two longitudinal sections thereof.
The rod 7 further comprises an end 10 of substantially frustoconical shape, turned towards the corresponding vertebra in the mounted position, as appears more particularly in FIG. 2. The correction device further comprises, as is known per se, a hook 11 whose shape is adapted to the morphology of the patient and intended to be supported on or under a lumbar transverse process. This hook 11 comprises a housing 12 of complementary frustoconical shape, the frustoconical end 10 of the rod 7 coming into this housing 12.
FIGS. 5 and 6 show two non-limiting embodiments illustrating the production of the ball joint connection between the movable element 5 and the fixed support 3 of the staple 2. According to a first embodiment shown in FIG. 5, the movable element 5, made of deformable material, is of substantially cylindrical shape and is inserted into a housing of complementary shape formed in the fixed support 3 of the staple 2. According to another embodiment shown in FIG. the mobile support 5 of substantially cylindrical shape is made of deformable material and is inserted into a complementary shaped housing formed in the fixed support 3. The housing has two abutments 13 at the ends, for example in the form of annular shoulders, so that to limit the axial displacement of the movable element 5. The deformation of the movable element 5 then allows the angular displacement of the rod 7 attached thereto. three degrees of freedom of rotation.
In each of the two embodiments shown in FIGS. 5 and 6, the mobile element 5 made of deformable material provides the damping function of the rod 7 at the level of the clip 2, thus constituting the damping means at the level of the ball joint. These damping means could, however, also be made in the form of a spring or resilient lamellae.
As known per se, the axial displacement of the rod relative to the clip adjusts the distance between it and the hook, so as to obtain the verticalization and maintenance in this position of the lumbar spine.
The ball joint between the rod and the clip makes it possible to easily orient the rod so that the correction device can be easily placed on the patient. The damping means further prevent the movement of the clip relative to the iliac wing so that its decalcification and the pain caused by such phenomena is avoided. As goes without saying, the invention is not limited to the embodiments of this system, described above by way of example, but it encompasses all variants. Thus, in particular, the correction device may not comprise a complementary damping element or that the damping elements could be made differently.
