The repair of comminuted or pluri-fragmentary fractures of the olecranon is currently relatively difficult to achieve, in the absence of a bone plate giving complete satisfaction from this point of view. Indeed, the different bone fragments are not always perfectly stabilized at the epiphyseal level. The same is true of the bone fragment adjacent to the triceps tendon, whose stabilization in rotation may not be completely ensured. In addition, existing plates are not particularly adaptable to the different conformations of fractures and provisions of fragments that may exist at the epiphyseal level. In addition, the holding of screws placed at the epiphyseal level is uncertain. The present invention aims to remedy all of these disadvantages. For this purpose, the bone plate it concerns comprises an epiphyseal portion, that is to say intended to be placed at the level of the bone epiphysis, which is widened relative to the diaphyseal portion and which has a curved shape seen along the longitudinal axis of the plate, the concavity of this diaphyseal portion being on the face of the plate to be turned to the side of the bone.
The epiphyseal portion of this plate, by its enlarged and curved shape, allows a wide enveloping of the different bone fragments at the epiphyseal level, and, consequently, a perfect stabilization of these fragments. The diaphyseal portion of the plate is, for its part, of conventional type, that is to say relatively narrow and pierced with holes, allowing rigid attachment of the plate to the bone in unfractured locations. Advantageously, the epiphyseal portion is extended, on the opposite side to the diaphyseal portion, by a bent portion terminated by a relatively sharp beak, itself curved towards the diaphyseal portion, this bent portion being such that this beak can be inserted, at the time of placement of the plaque, in the bone fragment adjacent to the triceps tendon. This beak thus allows a maintenance in rotation of this bone fragment. The nozzle preferably has a shape providing a rotational wedging of the same fragment, in particular a triangular shape.
Preferably, said bent portion comprises a hole which passes therethrough. This hole is intended to allow the engagement of the plate on a guide pin previously engaged in the medullary canal of the bone and thus ensure the positioning of said spout substantially at the longitudinal axis of the bone. After placing screws in the other holes of the plate, this pin is removed. The hole thus released can receive a bone anchoring screw. Advantageously, the epiphyseal portion is thinner than the diaphyseal portion, so that it has a thickness allowing it to be slightly deformed. The shape of this epiphyseal portion can thus be adapted to the different situations that may arise, depending on the conformation of the various fractures and the arrangement of the various fragments after repair. In this case, the epiphyseal portion preferably forms two protruding portions on two opposite longitudinal edges of the plate, and comprises screw receiving holes at these projecting portions. The deformation of these protruding parts makes it possible to adjust the orientation of the screws that these holes receive as a function of the conformation of the fractures and the arrangement of the different fragments. Preferably, these screw holes are threaded and receive screws with threaded heads. Thus, once in place, the screws are axially integral with the plate, which ensures the holding in position of these screws relative to the plate. Advantageously, the epiphyseal portion comprises holes for the passage of sutures intended to ensure, where appropriate, the retention in position of small bone fragments.
The invention will be well understood, and other features and advantages thereof will become apparent with reference to the accompanying diagrammatic drawing showing, by way of non-limiting example, a preferred embodiment of the bone repair plate of the present invention. fractures of the olecranon, that it concerns.
Figure 1 is a perspective view; Figure 2 is a side view; Figure 3 is a view from above; Figure 4 is an end view, from the side on the left of Figures 2 and 3, and Figures 5 and 6 are perspective views, after establishment on a bone. Figures 1 to 4 show, from different angles, a bone plate 1 for repairing olecranon fractures. This plate 1 comprises a diaphyseal portion 2, intended to be placed at the level of the bone diaphysis, an epiphyseal portion 3, that is to say intended to be placed at the level of the bone epiphysis, and a Curved portion 4. The assembly is made from the same piece of material, especially metal. The diaphyseal portion 2 is of conventional type, that is to say relatively narrow, rigid and pierced with holes 5, 6, allowing rigid attachment of the plate 1 to the bone in non-fractured locations. In the example shown, this portion 2 comprises three milled holes 5 and a light 6 arranged at the longitudinal median axis of the plate 1. The epiphyseal portion 3 is widened relative to the diaphyseal portion 2, forming two overhanging portions 7 on two opposite longitudinal edges of the plate 1, has a curved shape seen along the longitudinal axis of the plate 1 (Figure 4) and is thinner than the diaphyseal portion 2. The portion 3 comprises four receiving holes 8 of screws, two of which are arranged at the longitudinal central axis of the plate 1 and the other two at the projecting portions 7. These holes 8 are threaded and the corresponding screws are of the threaded head type, so that once in place, they are axially integral with the plate 1. The portion 3 also comprises holes 9 for the passage of sutures intended to ensure, where appropriate, the retention in position of 30 bone fragments of small dimensions. The portion 3 also has a curved shape seen along the longitudinal axis of the plate 1 (see Figure 4), its concavity being on the face of the plate 1 to be turned to the side of the bone. In addition, the portion 3 is thinner than the diaphyseal portion 2, so that it has a thickness allowing it to be slightly deformed.
In particular, the material of the plate 1 being stainless steel, the portion 3 may have a thickness of 1.2 mm and the portion 2 a thickness of 2.5 mm. The curved portion 4 extends the epiphyseal portion 3 on the opposite side to the diaphyseal portion 2. It comprises a base portion located in the extension of the portion 3, a curved portion and a rectilinear end portion, forming an angle of the order of 120 degrees with the base portion. The end portion is terminated by a spout 10 of relatively sharp triangular shape, itself curved on the side of the diaphyseal portion 2, and comprises a hole 11 immediately above the spout 10, passing right through it. In practice, as can be understood with reference to FIGS. 5 and 6, a guide pin is engaged in the medullary canal of the bone and then the plate 1 is slidably engaged on this pin, thanks to the hole 11, up to insertion of the beak in the bone fragment adjacent to the triceps tendon. The hole 11 thus ensures the positioning of the spout 10 substantially at the longitudinal axis of the bone, and the triangular shape of the spout 10 provides a rotational wedging of said fragment. Portions 3 and 2 respectively face the epiphyseal and diaphyseal areas of the bone. Portion 3, by its enlarged and curved shape, allows a wide wrap of the various bone fragments at the epiphyseal level, and therefore a perfect stabilization of these fragments, while the portion 2 allows, by the engagement of screws through holes 5 and 6, a rigid fixation of the plate 1 to the bone in unfractured locations. The portion 3 may, if necessary, be deformed to be adapted to the different situations that may arise, depending on the conformation of the various fractures and the disposition of the various fragments after repair, and in particular to adjust the orientation of the screws that receive the holes 8 The axial connection of these screws to the plate 1, as a result of the screwing of the threaded heads thereof in the holes 8 ensures the maintenance of the screws in position notwithstanding the engagement of the screws at the level of fractured areas. The invention thus provides a bone plate for repairing olecranon fractures having, compared to homologous plates of the prior art, the decisive advantages of allowing perfect stabilization of the various bone fragments at the epiphyseal level, perfect stabilization in rotation of the bone fragment adjoining the triceps tendon, to be adaptable to different fracture conformations and provisions of fragments that may exist at the epiphyseal level, and to ensure the holding of screws placed at the epiphyseal level. It goes without saying that the invention is not limited to the embodiment described above by way of example but that it extends on the contrary to all embodiments covered by the appended claims.
