Abstract

The system has a moving cup (3) that slides in smooth articular cavity (7) of an osseous anchoring shell (1) and on polished ball (4) and smooth neck (6) without bump/notch, of a prosthesis (5) with diaphyseal anchoring. An opening (8) of the cup is contracting towards the cups center and is adapted to be conceived with the widened neck (6) in all positions. The osseous anchoring shell (1) is of rigid bio-compatible material having in its internal concave face a smooth articular cavity (7) that is polished hemispherical and is exempt from bumps, of cavities or of relief. The prosthesis (5) comprises at the part opposed to its diaphyseal anchoring part the polished ball (4) and the neck (6) which is smooth without bump or notch and preferably slightly widened, to enable the physiological articular circular movement with soft friction.

Description

II

I / Technical Field and State of the Art: The devices known to date for the realization of an articular bearing to replace the articulations of the upper or lower spherical-type limb are of four types: 1 / Cupules cemented in the cavity bone, intended to receive a ball of prosthesis f. cf. patent application Lannelongue 87 01524 and Gramont 87 10495.

The latter type of implants, although seductive by its principle, is rather disappointing in the medium term because, because of the retention of the ball in the mobile cup, the latter exhibits wear with significant debris creation which can lead to a surgical revision. or even dislocation requiring replacement of implants.

II / Principles and anticipated advantages of the invention: It is important and economical to offer patients implants for replacement of stable upper or lower limb joints, free from dislocation even in the event of high mobility, particularly in circumduction, and of which wear is minimal in order to reduce prosthetic replacements on an older population whose life expectancy is increasing, but for which recovery interventions are heavy in both mortality and morbidity.

The triple articulation object of the invention occurs - on the one hand, for the first articulation, between the outer face of a movable cup biocompatible plastic material and between the inside of a bone anchor shell rigid material biocompatible having a substantially polished hemispherical concavity surface 35 free of asperities, cavities or reliefs that can wear the said movable cup or hinder its free movement, when the latter is urged in rotary displacement by the neck of the prosthesis, - on the other hand, for the second articulation, between the polished articular surface of the ball of a prosthesis and between the inner face of the movable cup whose surface area is preferably greater than a half sphere of diameter similar to that of said ball , and also on the other hand, for the third articulation, between the neck of said prosthesis, characterized in that it is smooth without asperity or notch and pre slightly flared, to allow the wear-free soft friction physiological articular circumduction movement, and between the chamfer at the opening of the movable cup, characterized in that this opening narrows towards the center of said articular cupule mobile having an inner diameter substantially equal to or less than that of the ball, and whose flare is designed to fit perfectly with that of the flared neck of the prosthesis, in all the respective positions of the moving parts 10 when the collar comes in contact with the movable cup and drives it in its race in case of large amplitude deflection.

It is therefore a device which, combining the functionality of systems already described, provides an answer to the problems of wear and fast deterioration and thus allows to treat successfully, without wear, without dislocation, so for 15 longer and with a large range of motion, by its triple articulation, all articular pathologies.
