Abstract
Dispositif pour la fixation d'un ligament à l'os donnant d'emblée un ancrage solide, permettant une rééducation précoce et agressive.Il est constitué d'un implant fémoral et d'un implant tibial. Chaque implant présente un dispositif de double ancrage : au niveau fémoral par le passage en boucle au niveau d'une extrémité (5) et par compression dans le tunnel osseux ; au niveau tibial par compression dans le tunnel osseux et par un appui cortical par l'anneau (13).
Description
La présente invention concerne un dispositif en matériel biorésorbable de
fixation
chirurgicale d'un ligament à l'os. Elle est plus particulièrement destinée à la fixation lors de la reconstruction du ligament croisé antérieur d'un genou.
La fixation traditionnelle par biomatériaux se fait à l'aide d'une vis classique sans tête, dite "vis d'interférence", qui n'offre qu'une fixation modérée au niveau de l'os.
Le dispositif selon l'invention permet de remédier à cet inconvénient. Il comporte en effet un implant fémoral (fig.1), avec un pas de vis en forme d'hélice hélicodale (1) se terminant par une extrémité carrée (2). L'implant est perforé dans toute sa longueur (3) afin de permettre sa mise en place le long d'une broche préalablement introduite.
L'extrémité proximale de l'implant (4) se prolonge par une tige à bout émoussé (5) qui permet l'accrochage des tendons en boucle, lui conférant un ancrage plus résistant. Les spires hélicodales en forme d'hélice offrent une surface de contact importante au niveau de l'os, ce qui permet une meilleure fixation et empêche les mouvements de latéralités. La taille de l'implant est de 4 à 5 cm, à titre indicatif, non limitatif.
1 5 Le deuxième élément comporte un implant tibial (fig. 2) avec une tige centrale (6) en biomatériel résorbable, émoussé à son extrémité (7) et perforé sur toute sa longueur. Elle comporte un pas de vis de forme hélicodale (8) se terminant par un anneau (9) situé à 5 millimètres de son extrémité distale. Cette extrémité distale (10) comporte une fente (11) de 2 centimètres de long, traversant l'implant en son centre. Elle présente également un 20 orifice (12) sur 2 centimètres avec un pas de vis interne permettant l'adaptation d'une vis (fig.3), qui permet, en la serrant, d'obtenir une augmentation du diamètre de l'implant de façon progressive en écartant les deux hémi- valves (10); ceci permet un meilleur ancrage des tendons au niveau de l'os.
La figure 3 comporte également un anneau libre (13) avec un angle aigu au pourtour de la 25 face proximale (14) qui permet de plaquer les tendons au niveau de l'os cortical, lui conférant un double ancrage osseux à son extrémité distale. La longueur tibiale de l'implant assemblé est de 4 à 5 cm, à titre indicatif, mais non limitatif.
Le dispositif, selon l'invention, permet une fixation d'emblée solide d'un ligament à l'os par du matériel biorésorbable. Ce dispositif est plus particulièrement destiné aux 30 ligamentoplasties du genou par tendons droit interne - demi-tendineux (D.I.D.T).

Claims (4)
1. Dispositif de fixation chirurgicale d'un ligament à un os, caractérisé en ce qu'il comporte une tige centrale (6) en matériau biorésorbable perforée sur toute sa longueur, présentant une extrémité proximale émoussée (5, 7) et comportant un pas de vis (1,8), de forme hélicodale permettant un premier ancrage par compression dans le tunnel osseux ainsi qu'un second système d'ancrage (5, 10, 13).
2. Dispositif d'ancrage selon la revendication 1 caractérisé en ce qu'il constitue un implant fémoral et en ce que le second système d'ancrage est formé par l'extrémité émoussée (5) qui fait saillie au-delà du pas de vis et permet l'accrochage en boucle du tendon.
3. Dispositif d'ancrage selon la revendication 2 caractérisé en ce que le pas de vis se termine côté distal par une extrémité carrée (2).

Dispositif d'ancrage selon la revendication 1 caractérisé par le fait qu'il constitue un implant tibial, le pas de vis se terminant côté distal, par un anneau et l'extrémité distale (10) de la tige (6) présentant une fente (12) traversant l'implant en son centre et un orifice permettant l'adaptation d'une vis munie d'un anneau libre (13) qui permet d'obtenir une augmentation du diamètre de l'implant et forme le second système d'ancrage.
